Compressive neuropathy in the upper extremity can be clinically diagnosed by careful history taking, physical examination of the involved nerve. Electrodiagnosis for the suspected nerve informs severity of compression of the involved nerve and indicates specific site of the lesion. In the early stage of the disease, non-operative treatment generally cures the symptom, however, if the conservative treatment fails, confirmation of the exact site of the lesion should be preceded before the operation. Recently, ultrasonography, as a supportive tool for the diagnosis of compressive neuropathy has increasing popularity for its ability to find space occupying lesion, anatomical change of the nerve, and the pathologic change in the nerve itself. For the successful treatment of the compressive neuropathy, these various diagnostic tools have to be introduced in the orthopaedic clinic.

This study examined the effect of the application of Theratainment physical factors on the calf blood pressure, ankle angle, and calf circumference. A research sample consisting of normal female university students in their twenties were divided into hot therapy group (n=20), cold therapy group (n=20), and compression therapy group (n=20). By referring to the previous study, 20-minute hot therapy, 15-minute cold therapy, and 30-minute compression therapy with 100mmHg intensity was applied. The therapy was applied to left-side lower limb, followed by the application on the right-side lower limb after an hour of resting time. The calf blood pressure, ankle angle, and calf circumference of each group was assessed. In the study results, hot therapy and compression therapy showed a statistically significant difference on the decrease of blood pressure and increase of ankle angle (p<.05), but cold therapy produced no statistically significant difference. None of the three groups showed a statistically significant difference on the calf circumference. Hence, hot therapy and compression therapy is recommended for reducing calf blood pressure and increasing ankle angle related to the calf. These therapies are expected to be used for solving diverse problems of lower limbs.

Purpose: To evaluate the inter-fractional position and respiratory reproducibility of lung and liver tumors using pressure conserving type(P-type) abdominal compressor in volumetric modulated arc therapy(VMAT). Materials and methods: Six lung cancer patients and three liver cancer patients who underwent VMAT using a P-type abdominal compressor were included in this study. Cone-beam computed tomography(CBCT) images were acquired before each treatment and compared with planning CT images to evaluate the inter-fractional position reproducibility. The position variation was defined as the difference of position shift values between target matching and bone matching. 4-dimensional cone-beam computed tomography(4D CBCT) images were acquired weekly before treatment and compared with planning 4DCT images to evaluate the inter-fractional respiratory reproducibility. The respiratory variation was calculated by the magnitude of excursions by breathing. Results: The mean ± standard deviation(SD) of overall position variation values, 3D vector in the three translational directions were 1.1 ± 1.4 mm and 4.5 ± 2.8 mm for the lung and liver, respectively. The mean ± SD of respiratory variation values were 0.7 ± 3.4 mm (p = 0.195) in the lung and 3.6 ± 2.6 mm (p < 0.05) in the liver. Conclusion: The use of P-type compressor in lung and liver VMAT was effective for stable control of inter-fractional position and respiratory variation by reproduction of abdominal compression. Appropriate PTV margin must be considered in treatment planning, and image guidance before each treatment are required in order to obtain more stable reproducibility
Background: Although sclerotherapy is an established procedure in treating varicose veins, a wide disparity remains in the rates of success. The purpose of this study was to evaluate the effectiveness and complications of sclerotherapy in combination with compression. Material and Method: From December 2000 to September 2001, a combined total of 50 legs in 39 patients with primary varicose veins were treated with sodium tetradecyl sulfate as sclerosant with the empty vein technique. Immediately after the injection, local compression was increased by a cotton wool roll, and additional compression was obtained with class II medical compression hosiery. Patients were evaluated at 5 days, 1 week, 2 weeks, and 6 weeks for degree of improvement and complications. Result: There were 36 women and 3 men ranging in age from 26 to 66 years, with a mean age of 45.3 years. Good sclerosing results were obtained in all patients. Of fifty legs, five had minor complications, two small intravascular clots, two pigmentations, and one compression-related bulla, which needed no treatment. Conclusion: The use of a cotton wool roll for local compression is highly effective, and sclerotherapy in combination with compression is safe and effective in the treatment of varicose veins.
4D-Radiation Therapy is the optimal treatment to track moving organs(tumor) and give the appropriate prescription dose to tumor and low radiation dose to normal tissue surrounding tumor volume. The ABCHES is a 4DRT devices maintaining shallow breathing to patients. It allows the tumor's movement was minimize. Meanwhile, Abdominal compression device is limited the breath compressing abdomen on patients. In this paper we will quantitative analysis the movement of tumor on only ABCHES versus ABCHES with Abdomal compression device and Analysis tumor dose and normal tissue's dose by Dose Volume Histogram on two parts. The result of Comparision ABCHES and ABCHES with Abdominal compression device, SI(Superior-Inferior) direction, AP(Anterior-Posterior) direction and LR(Left-Right) direction was limited 1.0 mm, 0.2 mm, 0.2 mm(average). and also reduction rate of voluume in HPTV was $16{\pm}2%$, and LPTV was $15.8{\pm}0.8%$ under only using ABCHES and ABCHES with compression. The analysis dose volume histogram was more radiation dose in ABCHES and abdominal compression device than only using ABCHES, and less normal tissue-ipsilateral lung, whole lung, kidney-dose in ABCHES and abdominal compression device than only using ABCHES. The overall analysis was ABCHES with abdominal compression better than only using ABCHES method. In hereafter it will be studies that limitation of ABCHES and abdomonal compression device. In other words, patient's discomfort on compression intensity, method of application on patient with inaccurate respiration cycle.
목적 : 한양방 협진을 통한 보존적 입원 치료를 받은 급성 흉요추 압박골절 환자군에서의 압박골절 compression ratio의 급성기 내의 변화를 기록 분석하여 차후 환자군의 예후를 예측하고자 하였다. 방법 : 한양방 협진병원 척추센터에 입원하여 보존치료를 시행한 발병 2주 내의 압박골절 환자 중 2회 이상의 시점에서 압박골절 부위의 측면 X-ray 영상이 있는 환자 16명의 기록을 수집하였다. X-ray상 급성 압박골절이 발생한 추체(n=19)의 anterior wall과 posterior wall의 높이를 입원 1주차, 입원 2주차, 입원 3주차와 최종 영상 확보 시점의 X-ray에서 측정해 wedge fracture에 해당하는 anterior column fracture가 있는 추체(n=14)에 한해 anterior height/posterior height의 공식을 통해 compression ratio를 산출하였다. 2차적 지표로 입원 1주차에서 입원 3주차까지 환자의 VAS(visual analog scale) 통증 척도 점수를 분석하였다. 그 외 연령, 성별, 입원 기간, 총 치료기간, 당뇨, 골다공증, 골밀도, 압박골절 기왕력, 압박골절의 형태의 빈도와 비율을 분석하였다. 결과 : 1. 전방 골주 단독 골절에서의 compression ratio는 입원 1주차에서 3주차, 그리고 최종 영상 확보시점까지 각각 20.92${\pm}$10.42, 25.22${\pm}$10.90, 25.57${\pm}$11.04, 25.07${\pm}$11.94로 기록되었다. 시점간에는 통계적으로 유의한 compression ratio의 차이가 나타나지 않았다. 2. 통증 VAS 점수 평균은 입원 1주차부터 3주차까지 각각 7.44${\pm}$2.07, 4.67${\pm}$1.63, 3.00${\pm}$1.80으로 기록되었다. 시점간 모두 통계적으로 유의한 감소가 있었다(1주차-2주차 p=.003, 1주차-3주차 p<.000 2주차-3주차 p=0.021). 결론 : 본 연구의 협진 입원치료 압박골절의 compression ratio는 입원 1주차 시점에서 최종 follow-up 시점까지 유의한 차이를 보일 만큼의 추가적인 추체 높이 감소를 보이지 않아 향후 좋은 예후를 예상할 수 있다. 통증 VAS 점수는 모든 주별 시점간에서 유의하게 감소되었다.
Stable compression fractures are most often caused by trauma as traffic accidents. Stable compression fractures can refer pain to the mid-back, which is characterized by the pain in the thoracolumbar spinous prosess. recently we experienced a 19 years old man who had lumbago originating from Stable compression fractures and condition was improved through burning acupunture.
배경: 하지 정맥률의 치료법중의 하나인 압박경화요법의 치료효과를 알아보고자 하였다. 대상 및 방법: 영동세브란스병원에서 1997년 8월부터 1999년 8월까지 복대-대퇴정맥판막 부전이 없는 하지정맥류 환자 중 압박경화용법을 시행 받은 94명을 대상으로 하였다. 병역 및 이화학적 검사결과, 압박경화용법의 치료효과와 합병증 등을 살펴보았다. 결과: 환자의 연령은 20대에서부터 70대까지 있었으며, 평균 43.4세였고 성별은 남자가 28명 여자가 66명 이었다, 정맥의 돌출 이외에 다른 증상은 없는 환자가 87명이었으며, 다리가 무거운 증사, 피로, 통증도 각각 1예, 2예, 2예가 있었다. 대상 환자 중에서는 소복재정맥에 이환된 사람이 83예이었다. 50명환자에서 1회의 시술만으로 만족할 만한 결과를 보였으며, 시술 후의 합병증으로 부분적인 열감, 정맥염, 괴사가 있었다. 결론: 복재-대퇴정맥판막 부전이 없는 하지정맥류의 치료에 있어 경화요법은 미용적으로 우수한 효과적인 치료법으로 생각된다.

말초신경은 외상이나 질병 등 여러 가지 원인으로 손상되기 쉬우며, 손상의 정도가 심하거나 치료가 지연되는 경우에는 심각한 기능 소실을 초래할 수 있다. 본 연구에서는 수영이 말초신경손상후 운동기능의 회복과 뇌유인성 신경영양인자 (brain-derived neurotrophic factor, BDNF) mRNA의 발현에 미치는 효과를 알아보기 위하여, 흰쥐 좌골신경에 압박 손상을 가하고 수영을 적용한 후 보행궤적분석 (walking track analysis)과 역전사연쇄반응 (reverse transcription-polymerase chain reaction, RT-PCR)을 실시하였다. 그 결과, 좌골신경 압박손상된 쥐는 특징적인 보행패턴을 나타내어 좌골신경기능지수 (sciatic function index, SFI)가 현저히 낮아졌으며, BDNF mRNA의 발현이 증가하였다. 좌골신경 압박 손상후 수영을 한 쥐에서는 SFI가 현저히 향상되었으며, BDNF mRNA의 발현은 억제되었다. 이러한 결과는 말초신경손상후 수영이 BDNF mRNA의 발현을 조절함으로써 기능 회복을 촉진시키는 효과적인 치료방법이 될 수 있음을 제안하고 있다.

In recent years, according to the increase of the osteoporosis and traffic accidents, the vertebral compression fracture patients are increased, This Study is clinical observation to the patients who be taken compression fracture of the $L1^{\sim}L2$ vertebra bodies with the symptoms of urinary disorder and ileus. She was treated by bed rest, acupuncture and moxibustion, herb medicine, physical therapy. In the observation of this case, I think that conservative treatment which involved acupuncture, moxibustion therapy and herb medicinal treatment is more effective than common nonoperative therapy.
