Impacts of Fatigue, Pain, Anxiety, and Depression on the Quality of Life in Patients with Breast Cancer (유방암 환자의 피로, 통증, 불안, 우울이 삶의 질에 미치는 영향)

  • Purpose: This study was conducted to identify the impact of fatigue, pain, anxiety and depression on the quality of life (QOL) among breast cancer patients. Methods: This study was conducted from July 12th to July 20th, 2010. One hundred and fifty breast cancer patients were recruited from D city in Korea. The instruments used in this study were the fatigue, pain, anxiety, depression and the quality of life scales for patients with breast cancer. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients and multiple regression with the SPSS/WIN 12.0 program. Results: The quality of life for cancer patients showed a significant relationship with fatigue, pain, anxiety and depression. The significant factors influencing quality of life were fatigue, pain, anxiety and depression that explained 65.6% of the variance. Conclusion: Patients with breast cancer experienced fatigue, pain, anxiety and depression which led to a negative effect on quality of life. The results suggest that intervention programs to reduce fatigue, pain, anxiety and depression could improve the quality of life for breast cancer patients.

Effect of Auricular Acupressure on Fatigue and Depression in Patients Undergoing Chemotherapy for Breast Cancer (이압요법이 항암화학요법을 받은 유방암 환자의 피로 및 우울에 미치는 효과)

  • Objectives: This study aimed to investigate the effects of auricular acupressure on fatigue and depression among patients undergoing chemotherapy for breast cancer. Methods: A nonequivalent control group nonsynchronized design was employed. Data were collected from December 2017 to May 2018, in the cancer center at a hospital located in B city. The experimental group received auricular acupressure four times a day, for 5 days, followed by a 2-day rest period. The control group received routine nursing care. The data were analyzed with independent t-tests, using the SPSS 21.0 program. Results: The level of fatigue and depression was lower in the experimental group as compared to that in the control group(t=5.29, p<.001; t=2.21, p=.038, respectively). However, depression scores increased post-intervention in the experimental group, as compared to the pre-intervention scores. Conclusions: The findings confirmed that auricular acupressure decreased fatigue however, depression scores increased post-intervention in the experimental group, as compared to the pre-intervention.

Analysis of Prognositic Factors and Long-tenn Survival According to the Pattern of Lymph Node Metastasis in Surgically Resected N2 Non-Small Cell Lung Cancer(NSCLC) (N2 병기의 비소세포성 폐암 수술 후 림프절 전이 양태에 따른 생존율 비교 및 예후 인자 분석)

  • Background : Current studies on multimodal strategy for N2 non-small cell lung cancer are being high interest, have drawn much attention. N2 lung cancer, however, is composed of is divided into several sub groups with that have different prognoses. The prognostic factors still remain controversial. Methods : Between January 1990 and June 1999, 180 patients with N2 lung cancer who underwent surgical resection were investigated, excluding 10 of these for surgical mortality. All patients underwent mediastinal lymph node dissection. 20 clinicopathologic factors were investigated by univariable and multivariable analyses to identify significant prognostic factors among resected N2 disease. Results : The overall 5-year survival rate was 20.6%. Multivariable analyses among overall patients revealed 3 significant prognostic factors : Age, Histologic type, Vascular invasion. Based on the result, 49 patients with both age more than 60 and pathologic Non-squamous cell showed a 5-year survival of 5.0%, whereas 37 patients with neither of the factors showed a 5-year survival of 56.6%(p<0.001). And 12 patients with both vascular invasion and pathologic Non-squamous cell showed a 5-year survival of 11.9%, whereas 67 patients with neither of the factors showed a 5-year survival of 33.6%(p=0.01). Conclusion : The prognosis of surgically resected N2 disease varies according to the 3 significant prognosis factors. Tumor size may be an additional influencing factor in the prognosis of N2 disease.

  • Purpose: Cachexia is a complex metabolic syndrome associated with wasting of skeletal muscle which contributes to nearly one-third of all cancer deaths. Cachexia lowers the frequency of response to chemotherapy and radiation and ultimately can impact survival as well as quality of life during treatment. NF-kappa B is one of the most important molecular mediators of cachexia. In this study, therefore, possible candidates for inhibitors of NF-kappa B were searched. Methods: Amino acids that regulate cellular redox potential by adjusting the level of NAD/NADH ratio, such as aspartate, pyruvate, and isocitrate were selected. Results: Pyruvate effectively inhibited luciferase activity in TNF-stimulated 293T cells transfect with an NF-kB dependent luciferase reporter vector. Pyruvate also showed protective effect on muscle atrophy of differentiated C2C12 myocyte induced by TNF/IFN. Conclusion: We might be able to develop the nutritional management strategy for cancer cachexia patients with pyruvate supplementation.

  • Objective: A Case Study of a Patient with Cancer Fatigue Syndrome and Cancer Metastases who Received Ortho-Cellular Nutrition Therapy (OCNT) Methods: The patient is a Korean woman aged 61 years. 13 years ago, she was diagnosed with stage 1 breast cancer and underwent surgery, chemotherapy, and radiation therapy. owever, 13 years later, the disease spread to the lungs and brain. Results: Following nutritional therapy, cancer fatigue syndrome improved. Conclusion: Nutritional therapy can be beneficial for cancer fatigue syndrome, a condition that remains undiagnosed and untreated.

Comparison of Setup Deviations for Two Thermoplastic Immobilization Masks in Glottis Cancer (성문암 세기변조방사선치료에서 두 가지 열가소성 마스크에 대한 환자위치잡이 오차 평가)

  • The purpose of this study was compare to the patient setup deviation of two different type thermoplastic immobilization masks for glottis cancer in the intensity-modulated radiation therapy (IMRT). A total of 16 glottis cancer cases were divided into two groups based on applied mask type: standard or alternative group. The mean error (M), three-dimensional setup displacement error (3D-error), systematic error (${\Sigma}$), random error (${\sigma}$) were calculated for each group, and also analyzed setup margin (mm). The 3D-errors were $5.2{\pm}1.3mm$ and $5.9{\pm}0.7mm$ for the standard and alternative groups, respectively; the alternative group was 13.6% higher than the standard group. The systematic errors in the roll angle and the x, y, z directions were $0.8^{\circ}$, 1.7 mm, 1.0 mm, and 1.5 mm in the alternative group and $0.8^{\circ}$, 1.1 mm, 1.8 mm, and 2.0 mm in the alternative group. The random errors in the x, y, z directions were 10.9%, 1.7%, and 23.1% lower in the alternative group than in the standard group. However, absolute rotational angle (i.e., roll) in the alternative group was 12.4% higher than in the standard group. For calculated setup margin, the alternative group in x direction was 31.8% lower than in standard group. In contrast, the y and z direction were 52.6% and 21.6% higher than in the standard group. Although using a modified thermoplastic immobilization mask could be affect patient setup deviation in terms of numerical results, various point of view for an immobilization masks has need to research in terms of clinic issue.

Diagnostic Usefulness of Tc-99m Tetrofosmin Scintimnmmography in Patients with Primary Breast Cancer (유방암 환자에서 Tc-99m Tetrofosmin Scintimammography의 진단적 유용성)

  • Purpose: The purpose of this study was to evaluate the diagnostic usefulness of scintimammography per-formed with Tc-99m tetrofosmin in the detection of primary breast cancer Materials and Methods: Sixty-one patients underwent Tc-99m tetrofosmin scintimammography, plain-film mammography and ultrasonography. After intravenous injection of Tc-99m tetrofosmin (740 MBq), prone lateral and anterior scintimammograms were obtained. Scintimammogram was visually interpreted as positive, probably positive, probably negative and negative for malignancy. The tumor to background count ratio (T/B) was measured at 5 minutes and 1 hour. Plain-film mammogram was interpreted as one of 5 categories. Final diagnosis was achieved by surgical histology (58/61) or fine needle aspiration (3/61). Of 61 patients, 44 had cancer and 17 had benign lesion. Tumor size of malignant and benign lesions on ultrasonogram were $2.51{\pm}1.30cm$ (range 1-8 cm), $2.50{\pm}1.35cm$ (range 0.96-6 cm), respectively. Results: The sensitivity of plain-film mammography was 88.6%, specificity 58.8%, positive predictive value 84.7%, and negative predictive value 66.7% The sensitivity of Tc-99m tetrofosmin scintimammography was 90.9%; specificity, 88.2%; positive predictive value, 94.9%, negative predictive value, 18.9%. Of 25 patients with indeterminate degree of suspicion for malignancy on plain-film mammogram, 23 were correctly diagnosed by scintimammography. The T/B at 5 minutes and 1 hour were $3.78{\pm}2.21$, $3.25{\pm}1.80$ respectively. The T/B was decreased significantly at 1 hour (p<0.001). Conclusion: Tc-99m tetrofosmin scintimammography was useful dia-gnostic procedure in the detection of primary breast cancer, especially in patients with indeterminate degree of suspicion for malignancy on plain-film mammogram.

Effects of Lifestyle Intervention on Fatigue, Nutritional Status and Quality of Life in Patients with Gynecologic Cancer (생활습관중재가 부인암 환자의 피로와 영양상태 및 삶의 질에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effect of lifestyle intervention on the development of fatigue, nutritional status and quality of life of patients with gynecologic cancer. Methods: A nonequivalent control group quasi-experimental design was used. Participants were 49 patients with gynecologic cancer. They were assigned to the experiment group (n=24) or the control group (n=25). The lifestyle intervention for this study consisted of physical activity, nutritional education, telephone call counseling, health counseling, monitoring for lifestyle, and affective support based on Cox's Interaction Model of Client Health Behavior and was implemented for six weeks. Results: Significant group differences were found for fatigue (p =.037), nutritional status (p =.034) and social/family well-being (p =.035) in these patients with gynecologic cancer. Conclusion: Results indicate that this lifestyle intervention is effective in lessening fatigue, and improving nutritional status and social/family well-being. Therefore, nurses in hospitals should develop strategies to expand and provide lifestyle interventions for patients with cancer.

Mediation Effect of Hope between Fatigue and Psychosoical Adjustment in Women with Breast Cancer (유방암환자의 피로와 심리사회적 적응에 대한 희망의 매개 (mediation) 작용)

  • The purpose of the present study is to identify the mediation effect of hope between fatigue and psychosocial adjustment in women with breast cancer. The framework for this study was guided by concepts and propositions derived from the theoretical and empirical literature on fatigue, hope and adjustment. The design of this study is a descriptive correlation study using a cross-sectional design. One hundred and twenty two outpatients with early breast cancer, receiving post-surgical radiation therapy or chemotherapy, were selected from three major medical centers in Seoul, Korea. A packet including PABCF (Psychosoical Adjustment to Breast Cancer Factor), revised RPFS (Revised Piper Fatigue Scale), HHI (Herth Hope Index), and self-addressed return envelope was given to the participants at seven to eight weeks post surgery. The questionnaires were to be completed at home and returned to the researcher by mail. The obtained data were analyzed using three regression equations guided by Baron and Kenny (1986); first, hope was regressed on fatigue; second, psychosocial adjustment was regressed on fatigue; and third, psychosocial adjustment was regressed on fatigue and hope, simultaneously. In the first equation, fatigue explained 4% of the variance in hope. In the second equation, fatigue explained 47% of the variance in psychosocial adjustment. In the last equation, hope and fatigue significantly explained the variance in psychosocial adjustment. Therefore, all conditions for the test of mediation effect of hope were satisfied. For the test of the mediation effect, the beta coefficients of fatigue on psychosocial adjustment on the second and third regression equations were compared. The beta coefficients were decreased from .69 (p < .001) on the second regression equation to .63 (p < .001) on the third regression equation. Thus, the hypothesis of this study was supported. As a result of this study, the negative Influence of fatigue on psychosocial adjustment is dampened through the mediator effect of hope in women with breast cancer. Therefore, when planning care for the adverse effect of fatigue on psychosocial adjustment, oncology nurses should consider hope as a mediator between fatigue and psychosocial adjustment to breast cancer.

The Effect of BeHaS (Be Happy and Strong) Program on Self Esteem, Fatigue and Anxiety in Postoperative Care Patients with Breast Cancer (유방암 수술 환자의 추후관리를 위한 베하스(BeHaS) 프로그램이 자아존중감, 피로와 불안에 미치는 영향)

  • Purpose: The purpose of the study was to identify effects of the BeHaS program on self esteem, fatigue and anxiety in postoperative care patients with breast cancer who had surgery with, chemotherapy, radiotherapy, with or without current hormone therapy. Methods: This study was a 2-group quasi-experimental research study with a pre and post test design. Fifty-four patients with breast cancer were assigned to the experimental (n=29) or control group (n=25). The BeHaS program which strengthens self esteem through support over a 90-minute period, consists of theme activity (30 minutes), education (15 minutes), group support (15 minutes) and exercise (30 minutes). The experimental group participated in the program once a week for 10 weeks, but the control group was not involved. Data were gathered from October to December, 2008 using a questionnaire with measures of self esteem, fatigue and anxiety. Data were analyzed using t-test with SPSS Win 12.0 to identify differences between the groups. Results: Self esteem was significantly increased (p= .001) and fatigue significantly decreased (p= .013) in the experimental group. But there was no significant difference in anxiety (p= .868). Conclusion: These results suggest that the BeHaS program for patients with breast cancer had beneficial effects on self esteem and fatigue.
