Current Knowledge on Fatigue in Advanced Cancer Patients (진행된 암환자에서 피로의 최신지견)

  • 진행된 암환자에서 피로는 70% 이상이 경험하게 되는 흔한 증상이며 삶의 질을 저하시키는 주된 요인이다. 피로에 대한 선별검사는 "피곤하거나 지친감이 있나요?"라는 단순한 질문으로 가능하며 이미 개발된 평가도구를 사용하여 평가할 수 있다. 동반 질환이나 피로의 원인을 알기 위한 병력청취와 검사실 검사가 필요할 수 있으며 결과에 따라 가역적 인자를 치료하는 과정이 우선되어야 한다. 적절한 약물치료를 시행하여 증상을 호전시킬 수 있으며 이중 methylphenidate는 암 연관 피로를 호전시키고 마약성 진통제에 의한 진정에 효과가 입증되었다. 환자의 여명에 따라 부신 피질 호르몬제도 사용할 수 있고, 운동 및 환자의 교육과 영양관리 또한 중요하다.

Factors Influencing the quality of life in thyroid cancer patients after thyroidectomy (갑상선암 수술환자의 삶의 질에 영향을 미치는 요인)

  • The purpose of this study was to examine the affection factors of quality of life in thyroid cancer patients after thyroidectomy. The subject of this study were 125 patients with thyroid cancer who were receiving operation and outpatient medical examination in general hospital in Gwangju city. Data were analysed using descriptive statistics, t-test, one-way ANOVA, Sheffe test, Pearson correlation analysis and stepwise multiple regression analysis using SPSS 21.0 program. The significant factors influencing quality of life were depression(${\beta}=-0.39$, p<.001), spiritual fatigue(${\beta}=-0.30$, p<.001) and physical fatigue(${\beta}=-0.24$, p<.001), which explained 57.2% of the quality of life. Therefore, it is necessary to develop nursing intervention program reducing fatigue and depression for thyroid cancer patients after thyroidectomy.

The Influence of Cancer Fatigue, Quality of Sleep, and Environmental Sleep Disturbance on Comfort in Hospitalized Cancer Patients (입원한 암환자의 암피로, 수면의 질 및 환경적 수면장애가 안위에 미치는 영향)

  • This study is a descriptive research study to identify the relationship between cancer fatigue, quality of sleep, environmental sleep disturbance, and comfort perceived by hospitalized cancer patients. Data were collected with structured questionnaires from 113 cancer patients from a university hospital in J city, Gyengnam, from September 17 to November 5, 2019, and analyzed using SPSS 21.0, using independent t-test, one-way ANOVA, Scheffé test, Pearson's correlation coefficient, and stepwise multiple regression. The subjects' Comfort is significantly different depending on education level, Cancerous type, Radiation treatment status, Usual exercise status. The result showed that comfort was negatively correlated with cancer fatigue(r=-.609, p<.001), quality of sleep(r=-.478, p<.001), and environmental sleep disturbance(r=-.297, p=.001). The variables that had a significant effect on comfort were cancer fatigue(β=-.42, p<.001), subjective quality of sleep(β=-.30, p=.001), and cancer type(β=-.18, p<.015), and the explanatory power was 46.1%(F=27.24, p<.001). Based on these results, it is necessary to develop a program to improve the quality of sleep and to reduce the cancer fatigue by cooperating with medical and nursing staff in multidisciplinary ways to enhance the comfort of hospitalized cancer patients.

  • Purpose: The purpose of this study was to identify the relationship between fatigue and quality of life (QOL) of Korean cancer inpatients. Methods: The data were collected from May to August 2007. Study subjects were recruited at D, Y and A university hospitals in Daegu and Kyungpook, Korea. The research instruments utilized in this study were fatigue (FACT-F) and quality of life (FACT-G) in Korean version 4. Data were analyzed with descriptive statistics, t-test, ANOVA, and Pearson correlation using SPSS Win 12.0 program. Results: Fatigue of subjects showed a significant difference according to the type of treatment, change in weight, performance status, exercise, and sleep. The QOL of subjects showed a significant difference according to the purpose of treatment, change in weight, performance status, exercise, and sleep. The mean score of fatigue was 22.48 and the mean score of QOL was 55.52. The fatigue was negatively related to QOL, physical well-being, emotional well-being, and functional well-being. Conclusion: The results suggest the needs for intervention in order to reduce fatigue and to improve QOL of cancer inpatients.

An Exploration of the Experience of Compassion Fatigue in Nurses Working with Oncology Patients (암환자를 돌보는 병동 간호사의 공감피로 경험 탐색)

  • This study aimed to understand experiences of compassion fatigue reported by nurses providing services to oncology patients and families. We conducted in-depth interviews with 8 nurses currently working in oncology. Three main themes and 13 subthemes were extracted from the thematic analysis suggested by Braun and Clarke. Participants experienced compassion fatigue while feeling the sufferings transferred from patients and caregivers. The stress resulted from unique nature of oncology work added to their compassion fatigue. Participants became running low on empathy towards oncology patients and caregivers because of external work environments and negative perceptions on compassion. The results of the study inform health care professionals about increasing awareness of compassion fatigue of nurses working with oncology patients and families and the importance of providing interventions to promote their individual and professional quality of life.

Factors Related to Fatigue in Cancer Patients Receiving Chemotherapy (항암 화학요법 환자의 피로 관련 요인)

  • Purpose: The purpose of this study was to investigate the degree of fatigue and its related factors in cancer patients during chemotherapy. Methods: The subjects of this study consisted of 90 patients over 20 years old who were receiving chemotherapy at the injection room of the o.p.d. and ward admission care unit in a University hospital located in Gwang-ju city and data were collected from August 8th to October 2nd, 2002. Collected data were analysed using SPSS v 10.0. to obtain summary statistics for the descriptive analysis, t-test, ANOVA, pearson correlation, and multiple regression. Results: 1. Fatigue of the subjects was significantly correlated with physical distress score. and 6 items of subscale those were nausea, vomiting, anorexia, pain, and immobility, showed statistically significant correlation. 2. Fatigue of the subjects showed statistically significant differences according to a nap satisfaction. Fatigue of the subjects was significantly correlated with mood state, Also, all 5 items of subscale, which are those were anxiety, confusion, depression, energy, and anger showed statistically significant correlations. 3. Fatigue of the subjects showed statistically significant differences according to metastasis, chemotherapy cycle, post operation existence, post radiation therapy existence. There were significant negative correlation between fatigue and hematocrit and fatigue and weight change. There was no significant correlation between fatigue and spiritual well-being state. With the result to multiple regression, Immobility, Anorexia, Anger explained fatigue by, pain, and immobility showed statistically significant correlation.

Construct Validity of the Revised Piper Fatigue Scale in Korean Women with Breast Cancer (Revised Piper Fatigue Scale의 구성타당도 : 한국여성 유방암 환자를 중심으로)

  • Revised Piper Fatigue Scale (Piper et al., 1998)은 미국에서 개발된 암환자의 피로를 측정하는 자가보고서 형식의 도구이다. 이 도구는 총 22문항으로 네 개의 구성요소로 이루어져 있으며, 미국인 유방암 환자를 대상으로 신뢰도와 타당도가 검증되었다. 본 연구의 목적은 한국 유방암 환자를 대상으로 Revised Piper Fatigue Scale의 구성타당도를 재평가하기 위함이다. 총 122명의 유방암 환자가 번역-역번역 과정을 거쳐 한국어로 번역된 Revised Piper Fatigue Scale에 응답을 하였고 그 자료는 요인분석 (principal axis analysis with oblique rotation)에 의해 분석되었다. 요인분석 결과, 총 19문항으로 이루어진 네 개의 구성요소가 도출되었다. 총문항의 Cronbach's alpha는 .93이었고, 도출된 네 요인들의 alpha는 .84에서 .91이었다. 한국어로 번역된 총 19문항의 Revised Piper Fatigue Scale(revised PFS-K)은 한국인 유방암 환자에게도 적용될 수 있는 신뢰도와 타당도가 검증된 도구라 할 수 있다. 앞으로 다양한 한국인 암환자를 대상으로 계속해서 도구 검증이 이루어지기를 제언한다.

  • 스멀스멀 아지랑이 피듯 나른한 오월, 몸이 피곤하고 의욕이 없는 증상은 누구나 한 번쯤 겪어 보았을 것이다. 일반적으로 과로에 의한 피로는 충분한 휴식과 영양을 공급해주면 회복된다. 그러나 휴식을 취해도 호전되지 않고 피로가 6개월 이상 지속된다면 '만성피로'로 의사의 진찰을 받아보아야 한다. 왜냐하면, 피로의 원인이 당뇨, 암, 갑상선질환, 고혈압, 우울증 등에 있을 수 있기 때문이다. 실제로 원인 모를 피곤함으로 병원을 찾은 환자 중 종종 당뇨 진단을 받기도 한다. 그러면 당뇨병환자는 왜 기력이 떨어지고 피곤해하는지 당뇨와 피로의 상관관계에 대해 알아보도록 하자.

Structural Analysis on Control Arm of Automobile under Nonuniform Fatigue Load (불규칙 피로 하중을 받는 자동차의 컨트롤 암의 구조 해석)

  • This study analyzes structural stress and fatigue on control arm of automobile under nonuniform load. Maximum equivalent stress at bolt part is shown with 419.1MPa and the corner is deformed with maximum displacement of 1.1628mm. Among 3 cases of nonuniform fatigue loads applying on control arm, 'SAE bracket history' with the severest change of load becomes most unstable but 'Sample history' becomes most stable. In case of 'Sample history' with the average stress of $-10^5MPa$ to $10^5MPa$ and the amplitude stress of 0 MPa to $10^5MPa$, the possibility of maximum damage becomes 3%. This stress state can be shown with 6 times more than the damage possibility of 'SAE bracket history' or 'SAE transmission'. Safety and durability on automobile can be effectively improved by applying the fatigue analysis result on control arm.

  • This study was conducted to understand the meaning and nature of the compassion fatigue experiences among nurses working in oncology wards. The participants in this study were 11 nurses who worked for at least 6 months in oncology wards. The data were collected using in-depth interviews and analyzed using the phenomenological method by Giorgi. Six categories of the meaning of compassion fatigue were derived as follows; physically difficulty, emotion is control of patient's condition, collapsing in front of cancer patients, reminding of patient-related memory, finding a changed attitude, and searching for how to recover.
