  • Destruction of normal temporomandibular joint architechture may produce serious functional and cosmetic deficiencies. The literature is well documented as to the etiology and pathogenesis of temporomandibular joints. Numorous surgical procedure have been advocated for temporomandibular joint ankylosis from condylectomy to arthroplasty, cartilage transplant, metallic prosthesis, interpositional implant. These were to able reconstruct the normal mandibular function, and any even procedure could obtain the satisfactory results. In this paper, we reviewed young adult patient with TMJ ankylosis and facial asymmetry who was treated with metallic condylar prosthesis and orthognathic surgery.

A Study of Sound Analysis of Temporomandibular Joint (악관절 잡음에 관한 연구)

  • 저자는 서울대학병원 치과진료부 구강진단과를내원한 두 개하악장애 환자 중, 악관절음이 있음을 호소하는 환자 106명을 대상으로 SonoPAK을 이용하여 악관절음을 분석하고 Transcranial TMJ Series와 파노라마상 판독결과를 종합하여 다음과 같은 결론을 얻었다. 1. SonoPAK에 기록된 210회의 발생 악관절음 중 왕복성 관절음은 44건, 88회 발생하였고, 개구시 관절음은 74회, 그리고 폐구시 관절음은 48회 발생하였으며, 개구시에 관절음이 더 빈번히 나타났다. 2. 왕복성 악관절음이 있다고 인정되는 혼자군에서, 개구시 평균 최대 단위는 18.6(%)$\pm$15.6(%), 폐구시 평균 최대 진폭은 16.6(%)$\pm$17.4(%) 이었으며, 개폐구간 진폭의 차이에는 유의성이 없었다. (p>0.05). 3. 악관절음을 미약한 소리, 중간소리, 큰소리로 분류할 수 있었으며 각각 110, 79,21회의 발생을 기록하였다. 4. (>300Hz)/(<300Hz)의 비율이 0인 경우는 34회 였고, 300Hz 이상의 주파수가 관찰된 경우는 176회였다. 5. Transcranial TMJ projection series 와 파노라마상의 판독 결과 하악과두의 골 변화가 인정된 환자군에서 SonoPAK 기록의 평균 주파수 비율((>300Hz)/(<300Hz))은 0.387$\pm$0.284이었고 골 변화가 인지되지는 않았으나 주파수 비율이 0.01 이상인 환자군의 평균은 0.286$\pm$0.227이었다.

A STUDY OF $Na^+,K^+$-ATPase IN THE RAT SUBMANDIBULAR GLAND TUMOR INDUCED BY DMBA (백서의 DMBA 유도 악하선종양에서의 $Na^+,K^+$-ATPase에 대한 연구)

  • In attempts to evaluate carcinogenic chemical effect on $Na^+,K^+$-ATPase activity in the rat submandibular gland tumor induced by DMBA, pellet from DMBA powder was inserted into the right submandibular gland. Both right and left submandibular gland were excised and weighed following the period of experiment. Excised glands were observed microscopically and estimated biochemically. The results were obtained as follows. 1. Swelling and nodular mass in the right submandibular gland region ould be found at 11th week post-implantation with DMBA. 2. The weight and size of the right submandibular gland was markedly increased following the period of the experiment. 3. Epithelial dysplasia and invasive epidermoid carcinoma could be observed at 7th and 11th week after implantation of DMBA, respectively. 4. The rate of tumor induction in the right submandibular gland was about 76% at 17th week following implantation of DMBA. 5. DMBA caused markedly depressed $Na^+,K^+$-ATPase activity as well as the activity ratio in the rgiht submandibular gland following the period of experiment.

Argument Structure of Leibniz's Theodicy (라이프니츠 변신론의 논증 구조)

  • This study aims to reconstruct Leibniz's theodicy. Theodicy is to defense of the highest wisdom of the creator against the charge which reason brings against it for whatever is the evil in the world. For this defense, Leibniz created his own new kind of concepts: the principle of sufficient reason, the principle of perfection, the best of all possible worlds, moral necessity. Leibniz's theodicy is developed as following. Most good and wisest God created this world freely by moral necessity. God's will was to choose the goods antecedently. But God's will could not create goods only. For God's final purpose is to create the best. For this reason, it happens that the evils may come about by concomitance, and as a result of other greater goods. Therefore the evils are necessary in the world. And evil consists in imperfection. Man has free will as God. Freedom, according to Leibniz, consists in intelligence, which involves a clear knowledge of the object of deliberation. Man has freedom, but man's freedom is imperfect. Evil is originated in man's imperfect freedom.

  • We present here a draft genome sequence of Bifidobacterium dentium strain ATCC 15424, originally isolated from pleural fluid of an empyema patient. The genome is 2,625,535 bp in length and has a GC content of 58.5%. The genome includes 2,154 protein-coding genes, 4 rRNAs, and 55 tRNAs. Unlike other B. dentium strains isolated from human dental caries, ATCC 15424 carries 247 strain-specific genes, including prophage remnants and type III/IV secretion system proteins, N-acetylmuramoyl-L-alanine amidase, and PRTRC system protein E. The sequence information will contribute to understanding of the natural variation of B. dentium as well as the genome diversity within the bacterial species.

Patients who Visit to Dental Clinic Utilization Behavior, Dental Fear and Oral and Maxillofacial Pain (치과내원 환자의 이용행태 및 공포와 구강악안면 통증)

  • This study was conducted to know oral maxillofacial symptoms accroding to dental fear and dental use behavior. 334 persons Older than junior high school students in Daegu, South Korea who visited to dental clinic were targeted From March 1.2008 to March 15, 2008. Statistics tool was SPSS 12.0 for Windows. General characteristics and response degree according to oral maxillofacial symptoms was analyzed by frequency study. Fear and difference according to oral maxillofacial symptoms we re conducted by T-test and One-Way ANOVA. Correlation analysis was used to verify relation between fear and oral maxillofacial symptoms. Results are followings. 1. Women felt fear higher than men did. Persons who were older felt fear higher and these were statistically related. 2. Extraoral external was 62.9%, hightest on the symptoms response degree according to oral maxillofacial symptoms. Feeling back side of the head on the strain and neck was stiff on the items were high. 3. Oral maxillofacial symptoms were high in the case when they had regular inspection, experience of scaling, experience of anesthesia and visited to the clinic often. And. Two cases(having regular inspection and visiting to the clinic often) were statistically related. Conclusion: Each pan of oral maxillofacial symptoms in the high fear group according to oral maxillofacial symptoms and it was all statistically related. And, fear and each oral maxillofacial pains were positively related and it was statistically related.

The structural change in the hyoid bone and upper airway after orthognathic surgery for skeletal class III anterior open bite patients using 3-dimensional computed tomography (3D-CT를 이용한 골격성 III급 개방교합자의 악교정 수술 전, 후 설골 및 상기도의 변화)

  • Objective: The purpose of this study was to investigate the structural changes of the hyoid bone and upper airway after orthognathic surgery for skeletal class III anterior open bite patients, and make comparisons with normal occlusion. Methods: Pre- and post-operative computed tomography (CT) examinations were performed on 12 skeletal class III anterior open bite patients who were treated with mandibular setback osteotomy. Using the V-works $4.0^{TM}$ program, 3-dimensional images of the total skull, mandible, hyoid bone, and upper airway were evaluated. Results: In the Class III open bite group, the hyoid bone were all positioned anteriorly, compared to the Normal group (p < 0.05). The angle between the hyoid plane and mandibular plane in the Class III openbite group before surgery was greater than in the Normal group (p < 0.05), and the difference increased after surgery (p < 0.01). In the Class III openbite group, the volume of the upper airway decreased after surgery (p < 0.001) and the volume of the upper airway was smaller than the Normal group before and after surgery (p < 0.001). Conclusions: The narrow upper airway space in skeletal Class III openbite patients decreased after mandibular setback osteotomy. This may affect the post-surgical stability.

  • The purpose of this study was to evaluate the characteristics of craniofacial skeleton on orthognathic surgical cases with skeletal Class III malocclusion. For this study, 74 students at the dental college of Chosun University volunteered as a normal occlusion group. They had well-balanced faces and good occlusions with acceptable Class I molar relationship. They had not received orthodontic treatment and had no signs or symptoms of temporomandibular joint dysfunction. 45 malocclusion patients enrolled for orthognathic surgical treatment with skeletal Class III malocclusion at the Department of Orthodontics, College of Dentistry, Chosun University. On the basis of this study. the results of this study were as follows: 1. Skeletal Class III malocclusion was largely due to the overgrowth of mandible in man and the undergrowth of maxilla in woman. 2. The mandible was antero-inferiorly overgrown by large MP-HP angle and large genial angle in orthognathic surgical cases with skeletal Class III malocclusion. And also, upper incisors were severely labioversioned, but on the other hand lower incisors were linguoversioned. 3. In female, lower-third facial height was characteristically shortened in comparison with middle-third facial height and also, lower facial throat angle was small in male.

The Association between Temporomandibular Joint Disorders and Lumbar Diseases in Adults (성인의 악관절 증상과 요추질환의 관련성)

  • The purpose of this study was to examine the association between the symptoms of temporomandibular joint disorders and lumbar diseases in adults when the prevalence rate of osteoarthritis is increasing and to help develop health policies that can improve oral health and health in general. The study used representative data from the 5th Korea National Health and Nutrition Examination Survey phase 3 (2012). In total, we analyzed the data of 3,017 individuals aged over 50 years who participated in the health-related survey and underwent radiography of the lumbar joints. PASW statistics ver. 18.0 was used for analysis. This study revealed the following results: 16.1% experienced at least one symptom of temporomandibular joint disorders within the recent single year, 20.6% experienced lower back pain in the recent three months, and 30.6% had lumbar osteoarthritis revealed using radiography of the lumbar joints. Symptoms of temporomandibular joint disorders, lower back pain, and lumbar osteoarthritis were correlated with each other; the respondents who experienced symptoms of temporomandibular joint disorders had 1.70 times (95% confidence interval [CI], 1.30~2.22) higher prevalence of lower back pain and 1.20 times (95% CI, 0.86~1.68) higher prevalence of lumbar osteoarthritis than in those with no such difficulties. The results of this study confirmed that the symptoms of temporomandibular joint disorders affected lumbar diseases in adulthood. Therefore, adequate treatment and prevention of the symptoms of temporomandibular joint disorders in adults is expected to make crucial contributions to decreases in the prevalence rate of lumbar diseases and an improvement in the quality of life.

  • A morphological and positional classification was undertaken in the transcranial and transorbital views of 174 traceable films which had been selected from those of 274 patients and previous tracing was performed. For the positional analysis, the author adopted the method which is not influenced by TMJ remodelling, and for the morphological classification, made progress the general method by adding indefinite type to four basic types. The aims of this sutudy consists in performing positional and morphological analysis of condylar head and in its morphological and positional variation in symptomatic patients.
