Implant Placement in Growing Adolescents : a Literature Review (성장기 소아 청소년 환자에서의 치과 임플란트 식립)

  • Although there are a number of studies on the treatment planning and the prognosis of dental implants, limited studies provide the information on the implant placement on growing adolescents. The aim of this review is to discuss the use of dental implants in growing patients and the impact of skeletal and dentoalveolar growth on the long-term stability of implants. The general information regarding skeletal growth of maxilla and mandible would be briefly reviewed and the general treatment options would be discussed.

FACIAL ASYMMETRY OF UNILATERAL CLEFT LIP AND PALATE PATIENTS (편측성 순구개열자의 안면비대칭에 관한 연구)

  • Craniofacial asymmetry was analyzed in 39 patients with complete unilateral cleft lip and palate(UCLP). The samples are devided into three groups nine below 9 years, twenty three from 9 years 1 month to 14 years and seven over 14 years group. Seventeen measurements were obtained from the tracing of PA X-ray cephalometric headfilms to evaluate the asymmetric characteristics and changes accdording to aging in UCLP. The obtained results were as follows. 1. Facial asymmetry in UCLP is variable(1.22-3.47 $mm/^{\circ}$) and the length from midsagitta1 reference line to maxillary 1st molar, to upper central incisor and the length of mandibular ramus showed significant asymmetry 2. Nasal septum and anterior nasal spine were deviated In the cleft side and the lower border of nasal cavity was 1ower in cleft side. 3. The deviation of nasal septum was continued significantly till after 14 years old.

THE LONGITUDINAL GROWTH CHANGES OF CRANIOFACIAL STRUCTURE IN KOREAN ADULT (DURING THE AGE OF 24 TO 31) (성인 두개안면골의 성장변화(24세에서 31세까지))

  • Recently, according to the Increase of adult patient, it is neccessory to understand the growth changes of adult after cessation of active pubertal growth in clinical orthodontics. The purpose of this study was to investigate the growth changes of craniofacial structure after active growth period(adult) in order to use as reference in clinical orthodontics. Authors followed the 40 sample(male 25, female 15) from 24 to 31 years of age. By analysing the serial cephalograms, authors could get the following findings. $\cdot$ The mandible rotated clockwise in female, but not in male, and no incremental growth change in both genders. $\cdot$ The anterior facial height and lower anterior facial height were increased in both genders, the increase of lower anterior facial height exceed the posterior facial height increase in female. $\cdot$ The cranial base was stable throughout observation period. $\cdot$ The upper incisors uprighted slightly in female. $\cdot$ There were great the individual variation in the growth change of craniofacial structure in adult.

Angular Growth Changes of the Mandible (각계측에 의한 하악골의 성장변화에 관한 연구)

  • 320 cephalometric roentgenograms of 25 boys and 15 girls which was longitudinally taken from the age 6.5 to 13.5 have been analyzed to access the angular growth changes of the mandible. The statistical analysis of the results allowed the following conclusions. 1. Gonial angle was reduced with ageing. 2. Chin angle was reduced with ageing. 3. Occlusal plane to mandibular plane angle was relatively stable. 4. Lower incisor to occlusal plane angle was reduced and showed the greatest variability.

  • Three dimensional analysis of malocclusion and craniofacial deformation is essential for the successful orthodontic treatment. But the orthodontists are not familiar with diagnosis and treatment plane based on lateral cephalometric analysis. Since orthodontists do not posses a sufficient knowledge in standard value of posteroanterior cephalometric anaysis and of clinical importance for transverse jaw growth. In this study male(n=130) and female(n=171) aged from 6 to 16 and diagnosed as Class I malocclusion were selected to analysis width of cranium, maxilla and mandible on the posteroanterior cephalogram. The changes as a function of chronologic age and cervical vertebrae maturity index(CVXI) were examined. The Proper regression model was selected by sex with polynominal regression models and method of variable selection. Mean of each measurements and 70% confidence interval of individual measurement according to age was assesed and a graphs were made. Results are as follows :1. All the measurements for the width are gradually incresed as increase in chronologic age and CVMI. From the total amount of change between age 6 and 16, there is a tendency that mandibular width is broader than maxillary width and the width of male is broader than female. 2. There is no statistically significant sexual difference in Mx-Mn difference, Mx-Mn width differential, Mx/Mn ratio according to age and CVMI. 3. Mean of each measurement and 70% confidence interval of individual measurement according to age and sex were assessed and graphs were made for maxillary width, mandibular width, Mx-Mn difference, Mx/Mn ratio. 4. The width of maxilla and mandible in Korean children are broader than Western children during growth period.

  • [$Bj\ddot{o}rk$] and Skieller concluded after following facial growth patterns using implants in upper and lower jaw bones that stable structures, which are not influenced by the growth do exist and so these must be the reference landmarks in the process of superimposition. In spite of such facts, for the last 40 years since the discovery of stable structures, most orthodontists have preferred best fit superimposition method to structural method. Cases such as Angle's Class II division 2 malocclusion that show characteristic forward and upward growth or one under long period of post-treatment observation demonstrate that distinct differences exist between the two methods of superimposition. Today, ethical concerns prohibit further growth studies that use implants, md so there is no choice but to use stable structural superimposition method based on $Bj\ddot{o}rk's$ data. Thus, to encourage clinical use of stable structural superimposition method, logic for the use of stable structural superimposition method will be demonstrated, and its technical methods of application will be introduced step by step.

  • Transverse skeletal deficiency is a common clincal problem associated with narrow basal and dentoalveolar bone. The clinical characteristics of transverse deficiency presents with anterior crowding and posterior buccal crossbite. Orthodontic expansion, using lip bumper and functional devices, was recommanded for younger ages. However, expansion of lower anterior area in older Patients is unstable and tends to relapse toward the original dimension. Distraction osteogenesis is a unique form of clincal tissue engineering and biologic process of new bone formation between bone segments that are gradually separately by incremental traction. Distraction osteogenesis was considered that great potential for correcting transverse mandibular deficiencies. In this Paper, a case of treated transverse deficiency patients with distraction osteogenesis using tooth-borne and tooth & bone-borne distractor is presented.

  • The purpose of this study was to assess the early effect of FR III on the growing patients with anterior cross-bite. The lateral cephalograms and models were obtained from 7 patients at the time of pretreatment and correction of anterior cross-bite. The results were as follows: 1. A slight tendency of rotation toward anterosuperior direction and the growth to anterior direction were shown in maxilla. 2. There were a little change of mandibular vertical position and increase in lower facial height although some variations existed. 3. The bodily or labial tipping movement was shown in maxillary incisors. 4. The lingual tipping of mandibular incisors was shown in all cases. 5. Maxillary arch width increased while mandibular arch width usually changed a little although some variations existed. But it was difficult to summary in a word because variable responses were noted according to a wide variety of skeletal type, growth, and malocclusion.

  • Chemotherapy and radiotherapy proved conservative and effective in treating tumors. However, both the cancer therapies will also have aberrant effects on developing maxillofacial and dental organs of children. The purpose of this report is to describe the clinical cases of Disturbances of maxillofacial and dental development after Cancer therapy. The first case reported a 7-year-old female patient diagnosed at age 2 years with bilateral retinoblastoma, receiving chemotherapy and radiotherapy. She had agenesis of premolar, microdontia, short tapered teeth in lower anterior area and generalized root stunting. The second case presented a 12-year-old female patient treated with chemotherapy and radiotherapy for neuroblastoma in her early childhood. She presented with a class III malocclusion on a skeletal III base due to maxillary retrognathism. Contemporay oncology had improved survival of children with malignant disease. It will be needed prevention of these side effect after cancer therapy to improve the quality of life.

THE EFFECT OF RETRACTIVE FORCE ON JAW GROWTH IN GROWING RABBITS (하악골 후방 견인력이 가토의 악골 성장에 미치는 영향에 관한 실험적 연구)

  • The purpose of this study was to investigate the effect of jaw growth in a growing rabbits, when they are subjected to refractive force and after removal of refractive force. The experimental animals were Oryctolagus couniculus, male rabbits of 4 weeks of age. The mandible is retracted with 200gm in force of each side to the posterior and superior direction for 14 hours a day. Then rabbits were used as control group. First experimental group received refractive force for 4 weeks. Second experimental group received for 8 weeks. Third experimental group received for 12 weeks. True lateral films and dorso-ventral films were taken before wearing appliances, 4 weeks, 8 weeks, 12 weeks and 16 weeks after wearing appliance. The changes of rabbits jaw growth were observed radiographically. The findings were as follows : 1. Mandibular refractive force decreased total mandibular length, mandibular condylar length and angular length. 2. Mandibular refractive force increased nasal height, condylar width and angular width. 3. Mandibular refractive force decreased mandibular lenght growth but increased mandibular width growth. 4. There is no phenomena of catch-up growth after refractive force removal.
