A Study on the usability evaluation of PCM material Taeyeol pillow (PCM 소재 태열베개의 사용성 평가 연구)

  • If congenital fever left untreated, it can cause atopic dermatitis. It is very important to manage the surface temperature of the neonatal's head during sleep. Baby pillows were mostly developed with cotton materials due to their delicate skin, or they were used for head correction due to their flexible skeletons. This paper measured the surface temperature of baby head in a sleep and analyzed the trends to figure out the PCM effect which to prevent the congenital fever. In addition it was performed a usability evaluation from baby's parents to investigate their satisfaction. According to the results, PCM is effective in regulating skin temperature of neonatal's head to prevent the congenital fever.

Clinical Features and the Associated Factors of Staphylococcal Scalded Skin Syndrome during the Recent 10 Years (최근 10년간 포도알균 열상 피부 증후군의 발생 양상과 관련인자 분석)

  • Purpose : Staphylococcal scalded skin syndrome (4S) is uncommon, but reports of 4S are on the increase during the recent years. The purpose of this study is to determine the clinical features and associated factors of 4S during the recent 10 years. Methods : We retrospectively reviewed the medical records and microbiologic results of 63 patients (27 neonates and 36 children) from January 1998 to December 2007. Results : Since 2003, the incidence of 4S has increased. The mean age of the patients was 16.3 months and the gender ratio was 1:1. The clinical types of 4S were 38 cases of the abortive type (60%), 19 cases of the intermediate type (30%) and 6 cases of the generalized type (10%). The culture results were 36 cases of Methicillin resistant S. aureus (MRSA), 4 cases of Methicillin sensitive S. aureus and 17 cases of no growth. The patients were treated with semi-synthetic penicillin. For the 9 patients who had MRSA isolated and who didn't improve with penicillin, they were treated with vancomycin instead of penicillin. All the patients had no complications. 4S abruptly increased in 2005, and especially in neonates, due to an MRSA outbreak at a local nursery room. The associated factors of 4S in neonates were hospitalization (27 cases), including nursery infection in 2005 (18 cases) and dermatitis (1 case). There was an unknown origin for some children, and the suggested factors for their infection were community acquired infection (24 cases), atopic dermatitis (9 cases) and hospitalization (3 cases). Conclusion : 4S has recently been increasing. The major associated factors of 4S are a history of hospitalization, an outbreak in a nursery room, atopic dermatitis and community acquired infection.

  • The Wiskott-Aldrich Syndrome (WAS) is an inherited immunodeficiency caused by a variety of mutations in the gene encoding the WAS protein (WASp). First described in 1937 by Wiskott, the incidence of WAS has so far been estimated at 4 in 106 live births. The Wiskott-Aldrich Syndrome is an X-linked condition characterized by 1) an increased tendency to bleed caused by a reduced number of platelets, 2) recurrent bacterial, viral and fungal infections, and 3) eczema of the skin. The purpose of this report is to present cases highlighting the clinical features of the syndrome and the required considerations in the treatment of patients. The report consists of two particular cases: a 2-year-11-month-old boy seen for a routine oral examination prior to his bone marrow transplantation and a 2-year-6-month-old boy with herpes gingivostomatitis and teeth discoloration.

  • 지구 온난화로 인해 환경파괴, 병원성 세균 감염 등에 의한 각종 질병과 아토피 피부염 등 수없이 많은 요소들에서 우리 몸을 보호하기 위하여 친환경 소재의 용품들이 각광 받고 있다. 이중에서도 키토산과 칼슘알지네이트는 천연재료로써 이미 다른 분야에서 응용되어 사용되고 있으며, 이 두 가지 천연재료를 두 층으로 면섬유에 코팅시킨 CCAC섬유를 제조하였다. CCAC섬유와 키토산이 코팅된 면섬유, 칼슘알지네이트가 코팅된 면섬유, 미처리 면섬유의 총 4가지 섬유에 체액, 증류수, 생리식염수의 각각의 조건에서 흡습량, 흡습시간을 측정하여 비교하고, 수분율과 함수율을 측정하고, 접촉각을 Contact angle system OCA20을 이용하여 측정하였다. CCAC섬유의 키토산 부착 함량을 알아보기 위하여 정량적인 방법으로 add-on율을 이용하여 확인하고, 정성적인 방법으로 원소분석기(Elemental Analyzer, FLASH 1112)를 이용하여 측정하였다. 칼슘알지네이트의 함량 분석은 EDS(EX-250, HORIBA, Japan)를 이용하여 측정하고, 직물의 표면과 단면의 형태는 주사전자현미경(S-4100, Hitachi Co., Japan)으로 ${\times}100$, ${\times}1000$ 배율로 측정하여 단면과 표면 상태를 확인하고, 물리적인 특성은 KES-FB system 을 통하여 확인 하였다.

Change of Antioxidative Activity at Different Harvest Time and Improvement of Atopic Dermatitis Effects for Persimmon Leaf Extract (감잎(柿葉) 추출물의 채취시기에 따른 항산화 활성 변화 및 아토피성 피부염 개선효과)

  • Objectives : The purpose of this study was to investigate the anti-oxidative and anti-atopic dermatitis effects of persimmon leaf extract obtained from Cheongdo-gun, where more than 60% of Korean persimmon is produced. Methods : Anti-oxidative effects of the crude persimmon leaf extract harvested monthly between May and November were determined by in vitro assay using 1,1-diphenyl-2-picrylhydrazyl (DPPH) and superoxide dismutase (SOD)-like reaction. Anti-atopic dermatitis effects of the crude persimmon leaf extract were determined by using collagenase type I inhibition assay and by quantitative assays including serum histamine, prostaglandin E metabolite and leukotriene $B_4$ levels in animal model of atopic dermatitis using Balb/c mice. Results : Persimmon leaf extract harvested in May had higher levels of total phenolic compounds (182.24 mg/g) and flavonoids (23.05 mg/g) than the ones of different month extract. Also, persimmon leaf extract harvested in May showed the most effective extract scavenging activities of DPPH free radical ($13.39{\pm}0.21\;{\mu}g/ml$) and superoxide anion radical ($40.52{\pm}2.32\;{\mu}g/ml$), leading to use the persimmon leaf extract harvested in May for the experiments hereafter. Persimmon leaf extract showed $326.71{\pm}4.6\;{\mu}g/ml$ of 50% inhibitory concentration ($IC_{50}$) for collagenase type I which is responsible for the degradation of extracellular matrix. In addition, persimmon leaf extract application group could decrease serum levels of histamine, prostaglandin E metabolite and leukotriene $B_4$ compared to the negative control in animal model of atopic dermatitis. Especially, persimmon leaf extract showed a significantly decreased serum leukotriene $B_4$ level relative to the levels of histamine and prostaglandin E metabolite. Conclusions : Persimmon leaf extract showed anti-oxidative and anti-atopic dermatitis effects in vitro and in vivo. These results suggest that persimmon leaf extract may have immunoregulatory function for alleviating atopic dermatitis by decreasing collagenase activity and mast cell activation.

2,4-Dinitrochlorobenzene-induced Atopic Dermatitis Like Immune Alteration in Mice (마우스에서 2,4-Dinitrochlorobenzene을 이용한 아토피성 피부염 발현 관련 면역지표치 분석)

  • This study was undertaken to develop a reliable mice model demonstrating similar immunologic phenomena as human atopic dermatitis characterized with predominance of type-2 immune response. BALB/C mice and NC/Nga mice were sensitized twice with $100{\mu}l$ of 1% 2,4-dinitrochlorobenzene (DNCB) or vehicle (acetone : olive oil=4:1 mixture) in a week and challenged twice with $100{\mu}l$ of 0.2% DNCB or the vehicle at the following week. Mice were sacrificed at 19 days following the second DNCB or vehicle challenge for NC/Nga mice and at 28 days following the second DNCB or vehicle challenge for BALB/c mice. Upregulation of plasma 1gE, a hallmark of atopic dermatitis occurrence, was evident in the plasma obtained 4 day after the second DNCB challenge from BALB/c mice (approximately 4-fold) and NC/Nga mice (approximately 6-fold) treated with DNCB in comparison with that of the vehicle treated-control mice, and remain higher $3{\sim}4$ week after the second challenge. Ratio of plasma IgG1 versus IgG2a concentration was significantly higher in the mice treated with DNCB than the control mice, which also implies the skewed type-2 reactivity in vivo. Ratio of interleukin-4 versus interferon gamma produced in the splenic T cell culture supernatants was approximately 3-fold higher in the both strains of mice treated with DNCB than their control mice, respectively. The DNCB-treated mice demonstrated atopic dermatitis-like skin legions characterized with erythma, scaling, and hemorrhage, which was not observed with the control mice. Scratching on face or dorsal area was significantly more frequent (approximately 25-fold) in the DNCB-treated mice than the control at next day of the second DNCB challenge, and scratching frequency remains higher (approximately 4-fold) in the mice treated with DNCB than the control at 14 day following the second DNCB challenge. Overall, the mice model developed through sensitization and challenge with DNCB may be useful for research on atopic dermatitis and development of treatment materials for atopic dermatitis.

Review of Psychiatric Adolescent Inpatient with Dermatologic Consultations (청소년 정신과 입원 환자들의 피부과 자문 의뢰에 관한 행태 분석 및 고찰)

  • Objective:To review the patterns of the dermatologic consultations of psychiatric adolescent inpatient and to explore the relationship between the dermatologic disorders and psychiatric disorders. Methods:We retrospectively studied the data from 22 cases referred by psychiatric adolescent for a dermatologic consultation over 10 years in Daegu Catholic University Medical Center and compared with the data from 108 cases referred by the other department adolescent patients. Results:The mean age of patients was 15.9. The male to female ratio was 1:1.44. The most common psychiatric and dermatologic disorder was major depressive disorder and acne, respectively. The most frequent reason for consultation was to ask for dermatologic disease or condition(54.5%) followed by to perform cosmetic procedure of patients need(40.9%) and to perform dermatologic test(4.6%). Conclusions:More than just a cosmetic disfigurement, dermatologic disorders are associated with a variety of psychopathologic problems that can affect the patient. Increased understanding of biopsychosocial approaches and liaison among psychiatrists and dermatologists could be beneficial.

Complementary and Alternative Medical Therapies in Children and Adolescent with Chronic Disease : Utilizations and Patterns (만성 질환을 가진 소아, 청소년에게 시행되는 보완대체요법의 실태 연구)

  • Purpose : This study describes how often complementary and alternative medical(CAM) therapies, what patterns of CAM therapies utilization, what types of CAM therapies and why CAM therapies are used in children and adolescent with chronic disease. Methods : We interviewed and filled out questionnaires with parents of patients suffering from asthma, atopic dermatitis, congenital myopathy, epilepsy and so on from Mar 2004 to Aug 2004. Results : Two hundred fifty two parents answered questionnaires; 160 cases(63.5 percent) had used CAM therapies. Utilization of CAM therapies according to each chronic disease was as follows : congenital myopathy(100 percent), atopic dermatitis(91.1 percent), asthma(72.5 percent), chronic diarrhea (50.0 percent), epilepsy(17.1 percent) etc. Types of CAM therapy were as follows : dietary supplement (31.1 percent), herb medicine(23.4 percent), massage(14.2 percent), acupuncture(4.6 percent) etc. Sixty four(40.0 percent) case underwent more than two types of CAM therapies simultaneously. One hundred thirty three(83.1 percent) case were asked through non-medical prescription. Ninety nine(61.9 percent) case were recognized positive effect of CAM therapies. Parents' and childrens' ages were influential factors in CAM therapies utilization : the older the parents and the younger the children, the higher the utilization. Conclusion : Although patients receive treatment with western medicine, many of them not only undergo CAM therapies, especially more than two types, but also recognize the positive effects of CAM therapies. As a result, we suggest that it is easy to use of CAM therapies, and the positive effects of CAM therapies are prominent, domestically. Therefore, medical approach and study about the effectiveness and side effects of CAM therapies is necessary to avoid the imprudent use of such therapies.

The Relationship between Allergic Diseases and Functional Gastrointestinal Disorders in Children and Adolescents (소아·청소년의 알레르기 질환 (천식, 알레르기비염, 아토피피부염)과 기능성 위장관 질환과의 관계 (단면 조사 연구))

  • Objectives The purpose of this study is to establish relationship between allergic diseases and functional gastrointestinal disorders in children and adolescents using objective criteria and questionnaires. Methods This study surveyed 237 children and adolescents who visited the department of Pediatrics of ${\bigcirc}{\bigcirc}$ Korean Medicine Hospital from September 23rd, 2013 to December 26th, 2013. The Korean version of International Study of Asthma and Allergies in Childhood (ISAAC) questionnaire and the Korean-translated Questionnaire on Pediatric Gastrointestinal Symptoms-Rome III (QPGS-Rome III) were used. We analyzed the data by using PASW Statistics 18.0 with Chi-square test, Fisher's exact test and Linear by linear association. Results There was no significant difference between prevalence of allergic diseases and functional gastrointestinal disorders. Also, there was no strong relationship between the number of allergic diseases and the functional gastrointestinal disorders. However, some parts showed significant relationships - such as asthma symptoms ever and belch; asthma symptoms last 12 months and belch; allergic rhinitis diagnosis ever and belly aches and abdominal pain around and below the belly button; allergic rhinitis treatment last 12 months and belly aches and abdominal pain around and below the belly button; and atopic dermatitis diagnosis ever and irritable bowel syndrome (respectively; p=0.046, p=0.008, p=0.004, p=0.029, p=0.035). And as the number of allergic diseases increases, the prevalence of functional gastrointestinal disorders are 19.4%, 31.3%, 41.7%, 31.0% respectively. Conclusions Although there was no significant relationship between allergic diseases and functional gastrointestinal disorders, some gastrointestinal symptoms were related to allergic diseases.

A Literature Study about the Correlation between Genetic Studies on Atopic Dermatitis, Sasang Constitution, and Sasang Constitional Study on Atopic Dermatitis (아토피 피부염에서 유전자 연구와 사상체질의학적 접근의 상관성에 관한 문헌적 고찰)

  • Objectives: The objective of this study is to investigate the correlation between genetic studies on atopic dermatitis. sasang constitution, and sasang constitional study on atopic dermatitis. Methods: We retrieved data on PubMed for the papers of genetic study on atopic dermatitis. And for the papers on genetic study of sasang constitution and sasang constitional study of atopic dermatitis, we referred to papers have reported on domestic medical journal and domestic korean medicine journal. And we investigated correlations among the studies. Results: 1. There are two studies on genetic study on atopic dermatitis. One has been performed to find out genes related to atopic dermatitis by case-control study. The other has been to investigate the correlation between atopic dermatitis and the genes their functions were well-known. 2. Gene study on sasang constitution was performed to observe the distribution of the genes which is related with characteristics of sasang constitution, but there was no significant maldistribution of the genes. 3. There was no correlation between genetic study on atopic dermatitis and genetic study on sasang constitution. Conclusion & Discussion: In the Sasang constitutional study of atopic dermatitis, it seems that further studies on genes related to characteristics of skin, character affecting on behavior, and neurobiological difference among Sasang constitution are required as well as studies on distribution of genes related atopic dermatitis.
