Ulnar Nerve Subluxation Diagnosed by Dynamic Ultrasonography (동적 초음파를 이용하여 진단한 주관절 척골 신경 아탈구)

  • Musculoskeletal ultrasonography has been used as a convenient and simple tool for diagnosis of various conditions of orthopaedic diseases for many years. Generally magnetic resonance imaging (MRI) is thought to be the best method to search for anatomical structures or variations. However, for dynamic conditions such as dislocation or subluxation of tendons and nerves, MRI is not superior to ultrasonography, especially dynamic ultrasonography. So we present such a patient with an ulnar nerve subluxation at the elbow who has symtoms mimicking cubital tunnel syndrome diagnosed by dynamic ultrasonography and treated successfully by ulnar nerve anterior transposition and think that dynamic ultrasonography is a useful method for diagnosing dynamic condition such as ulnar nerve subluxation mimicking cubital tunnel syndrome.

Arthroscopic Reduction of Subluxed Medial Meniscus using Suture Anchor for Restoration of Hoop Stress - Technical Note - (버팀테응력 회복을 위한 아탈구된 내측 반월상 연골의 Suture anchor를 이용한 정복술 - 술기 보고 -)

  • Purpose: The authors introduce a new technique of arthroscopic reduction of subluxed medial meniscus using suture anchor for the restoration of hoop stress. Operative Technique: Anterolateral, anteromedial, and medial midpatellar arthroscopic portal are used. Arthroscope was inserted through anterolateral portal. Through the scope, we confirmed subluxation of medial meniscus. Transection of menisci including radial and root tear were excluded. We released the anterior horn of medial meniscus through anteromedial and burred the future insertion site of suture anchor. After inserting suture anchor through medial midpatellar portal, we used 90 degree suture hook and no.2 Nylon to retrieve the suture of inserted anchor. We tied the suture by sliding knot-tying method. Weight bearing was limited for 6 weeks postoperatively. Conclusion: Arthroscopic retightening of medial meniscus is less invasive, conserving and progressed method for subluxed meniscus.

Radiological Projection for Diagnosis of Shoulder Subluxation in Patients with Post-Stroke Hemiplegia (편마비 환자에서의 견관절 아탈구 진단에 유용한 촬영법)

  • Purpose : The purpose of this study was to find out useful radiological projection of shoulder subluxation in patients with post-stroke hemiplegia. Methods : A total of 33 patients with post-stroke hemiplegia were included(20 men and 13 women, mean age 62.3 years) and having the subluxed shoulder over one finger breath. The shoulder subluxation was determined as the ratio of the radiographic vertical and horizontal distance. The vertical distance was determined by measuring the distance between the most inferolateral point of the acromion and the central point of the humeral head. The horizontal distance was determined by measuring the distance between the central point of the glenoid fossa and the central point of the humeral head. To measure of the shoulder subluxation, the shoulder AP, axial and transthoracic lateral projections were taken on both affected and unaffected shoulders. We analyzed the difference of subluxation distance by t-test. Results : When patients was in sitting position, the average time of being shoulder subluxation was 123 second. There was significant difference between supine($49.90{\pm}13.6\;mm$) and sitting position($60.72{\pm}16.3\;mm$) in the vertical distance of shoulder anterior-posterior projection. Also, there was significant difference on transthoracic lateral projections, Affected $35.92{\pm}6.2\;mm$, Unaffected $28.76{\pm}5.4\;mm$. But in case of shoulder axial projection(supine position), there was no significant difference (Unaffected and affected was $23.01{\pm}9.0\;mm$, $22.45{\pm}8.2\;mm$ each). Conclusion : Radiological projection of shoulder subluxation has diagnostic value when it goes after check out the process of subluxation through finger breadth test. For this, patients must be in sitting and shoulder neutral position about 2 minutes. In addition, Shoulder anterior-posterior and transthoracic projection were significant to diagnose subluxation. But in axial projection, there wasn't meaningful differences.

  • Posterolateral rotatory instability is the most common pattern of elbow instability especially which is recurrent, and is usually post-traumatic because of inadequate soft tissue healing. The lateral ulnar collateral ligament was reconstructed by using ipsilateral palmaris longus tendon. Functionally good result was obtained. This is a case report illustrating the posterolateral rotatory instability of the elbow and its reconstruction with palmaris longus.

Subluxation of the Long Head Biceps Tendon Diagnosed by Dynamic Ultrasound (동적 초음파를 이용하여 진단한 견관절 상완 이두건 장두의 아탈구)

  • Purpose: To report the usefulness of dynamic ultrasonography in subluxation of the long head of the biceps tendon, which is difficult to detect with static imaging such as plain radiography, static sonography, MRI and in a subtle physical examination. Materials and Methods: Two male patients suffered from subluxation of the long head of the biceps. This difficult diagnosis? was managed by surgery (biceps soft tissue tenodesis) with the aid of dynamic ultrasonography. At final follow up, we evaluated patients' symptoms and functional outcomes using KSS, UCLA and ASES scores. Results: We diagnosed and treated subluxation of the long head of the biceps easily using dynamic ultrasonography. At the final follow up, both patients' symptoms and functional outcomes were improved. There were no significant complications. Conclusion: Dynamic ultrasonography is a useful method in the difficult and subtle diagnosis of subluxation of the long head of the biceps.

A Case Report of Three Yogaktong(腰脚痛) Patients Treated with Acupuncture and Moxibustion Treatment by Analysing Su-point(背兪穴) compared with X-ray measuring (X-선상(線像) 배수혈 진단을 통한 요각통의 침구(鍼灸) 치험 3례(例))

  • Objectives : Yogaktong is the genernal term of lowback and sciatic pain. Simpley this is not for the syndrom or illiness but for the symptoms. In this study, the effect of Acupuncture and Moxibustion Treatment by Analysing Su-point(背兪穴) compared with X-ray measuring for three patients with Yogaktong(腰脚痛) were evaluated. Methods : After treatment with the Acupuncture and Moxibustion Treatment by Analysing Su-point(背兪穴) compared with X-ray measuring for one time, we evaluated the effect by the figure of x-ray measuring and complaints of patients. Results : In three cases, the change in the figure declined dramatically and symptoms of patients got better after one treatment. Conclusions : Acupuncture and Moxibustion Treatment by Analysing Su-point(背兪穴) compared with X-ray measuring was effective. It will be attempted to the patients with it.
