A Study on Sandbag Management, vital sign, Pain and Hemorrhage after Kidney Biopsy in Children with Nephrotic Syndrome (신장증 환아의 신생검후 모래주머니 적용시간에 따른 활력징후, 통증 및 출혈에 관한 연구)

  • Purpose: The purpose of this study is to investigate correlations in vital sign changes, the severity of pain, signs of complications, and the duration of sandbag management in order to suggest a standardized practice related to sandbag management in children with Nephrotic Syndrome(NS). Method: From October 2000 to May 2001, seventy children with NS who underwent kidney biopsy were interviewed at one hospital in Seoul Korea, and participated in this study. Result: 1) The average sandbag applying time after kidney biopsy was 18.1 hours. 2) Systolic blood pressure and respiration increased until 15 minutes after kidney biopsy, after then, they decreased signifi- cantly (systolic BP, p= .006; respiration, p= .029). However, no significant changes were noted in diastolic blood pressure and pulse. 3) Pain was reported minimal for 1 hour after kidney biopsy. The severity of pain increased until 12 hours after the procedure, then, decreased significantly(p= .0001). 4) Reported complications were hematuria (74.7%) and abnormal sonogram (32.9%). No apparent bleeding on the biopsy region was reported in any children. Conclusion: From these findings, it is possible to change the protocols of the duration of absolute bed rest time and sandbag application management shortly after kidney biopsy. But it is needed to study the fit protocols for kidney biopsy. Several implications in nursing practice are suggested. 1) Replicated studies for more participants are needed. 2) Further research on the effect of sandbag application after kidney biopsy is required. 3) The best duration of sandbag application management after kidney biopsy need to be investigated.

  • A salivary stone on mouth floor was removed in a 10-year-old girl with cerebral palsy and epilepsy. If left untreated, sialolithiasis can lead to inflammation of whole salivary gland with severe pain and inflammation that spread to other side of salivary gland causing damaging of teeth, abscess, and nerve disorders. Special needs patients have difficulty to express their pain or discomfort. To overcome communication problem and offer appropriate treatment in proper time, a careful and comprehensive clinical examination including soft tissue and tooth should be prioritized in special needs patients visiting the dental clinic.

A survey on the nonpharmacologic nursing intervention for children in pain (통증 환아를 위한 비약물적 간호 중재 방법 조사)

  • This study was done to understand nonpharmacologic pain management for pediateric patients and nurses' knowledge and attitudes toward it. The aim of this study was that which method did the patient's use according to the nurses' age, and how did they effectively use these methods in their field. The subjects of this study were 77 nurses working in the Pediatric unit in the Kyung Medical Center from September 2 to 15, 1999 using questionnaire form. The results of this study were as follows : 1. We divided the subjects into four groups : Younger than one year old, 1-6 years, 6-12 years, 12-18 years group. In the group younger one year old, most of the nurses participating in this study used speaking in soft quiet tones, supportive touch, toys, pacifiers. In the group of 1-6 years, they used speaking in soft quiet tones, toys, distracting attention, story talking, and visual stimulus. In the group of 6-12 years. they used pop-up books, providing information, cold therapy, speaking in soft quiet tones, supportive touch. In the group of 12-19 years, most of them used providing information, controling respiration and supportive touch. 2. The effective nursing intervention used in their field are speaking in soft quiet tones, pacifiers and nesting with blanket in the group of younger than one year old. Un the group of 1-6 years old, speaking in soft quiet tones, toys, and supportive touch were effective method in the control of nonp-harmacologic pain management. In the group of 6-12 years old, story talking, supportive touch, and speaking in soft quiet tones were effective method and in the group of 12-18 years old, providing information, cold therapy and supportive touch were effectively used to control nonpharmacologic pain management. 3. To compare the general characteristics and non-pharmacologic pain nursing intervention, in the group of younger than one year, touching stimuli is widely used. In the groups of 1-6, and 6-12 years old, visual and audio method were widely used. In the group of 12-18 years old, sensitive intervention were used as well as education, information and guided imagery. In conclusion, there was no significant difference in nurses' demographic characteristics, child's age and nonpharmacologic pain management. There was significant difference only in the nurses working area, that is nurses working in the surgical department used more audio-visual-tactile pain management methods than medical department.

Pain Relieving Effect of Intraoral Sucrose Replacement in Neonates (신생아의 통증완화를 위한 자당(sucrose) 경구투여 효과)

  • The ability of neonates to perceive and react to pain, has been acknowledged recently. Recent researches have been shown that even short term pain can have lasting negative effects. We know that most of the anatomical pathway and neurotrans-mitter function necessary for pain perception are fully or nearly fully developed in the neonatal period. Many people are still reluctant to believe that pain felt by neonates may be as severe as that felt by older children or adults yet. The objective of the study is to assess and compare the analgesic effects of orally administered sucrose and pacifiers. And to determine the synergistic analgesic effects of sucrose and pacifiers. The tools for this study is Lawrence's NIPS (neonatal infant pain scale) with behavioural pain responses and index for pain physiological reponses as heart rate, respiratory rate and degree of SaO₂. The participants are 96 healthy infants of neonatal age 1-7days and randomised to receive heel prick before 2minutes to blood sampling and physiological test in Nursery of K Medical University Hospital on May-July, 2000. The experimental group assigned to one of three treatment groups : no treatment; a pacifier; placebo(2ml 25% sucrose). Collected data were analyzed with the SAS program using X²-test, ANOVA and Duncan's multiple range test as post hoc. The results were as follows. 1) Pain behaviour responses: The pain score of placebo group(2ml 25% sucrose) is lower than no treatment group and pacifier group significantly(P=.000). Placebo group is different from no treatment and pacifier group with Duncan's multiple range test. 2) Pain physiologic responses (1) heart rate: The heart rate of placebo group(2ml 25% sucrose) is different from that of no treatment group and pacifier group significantly(P=.000). The heart rate change of placebo group is less than the other groups. (2) respiratory rate: The respiratory rate of placebo group(2ml 25% sucrose) is different from that of no treatment group and pacifier group insignificantly (P=.2340). But, the respiratory rate of placebo group is lower than the other groups. (3) SaO₂: The SaO₂ score of placebo group(2ml 25% sucrose) is different from that of no treatment group and pacifier group insignificantly (P=.3265). But, the change of SaO₂ score of placebo group is less than the other groups. In conclusion, the sucrose placebo showed pain relief effect in behavior responses and less physiological responses. Accordingly, the sucrose placebo should be applied nursing intervention for simple pain management as heel prick.

Current Research Trends in Hospice-Related Domestic Nursing Research Theses (1998~2017) (호스피스 관련 국내 간호연구 논문 동향(1998~2017))

  • Purpose: In this study, the hospice nursing research trend in 1998~2017 was investigated by analyzing research articles on hospice nursing. Methods: Literature search was performed with keywords of 'nursing' and 'hospice on an academic research information service. Also reviewed were all articles published in the Korean Journal of Nursing Science and its 8 subcategory journals, the Korean Journal of Hospice and Palliative Care and the Korean Hospice Association from 1998 through 2017, except thesis papers. Results: In 2013~2017, 79 articles (31.0%) were relevant, up over 7% from 61 articles (23.9%) in 2008~2012. The most studied field was psychology (92 papers, 36.2%) in the Korean Journal of Hospice and Palliative Care. Enrolled in most papers were patients with end-stage cancer (75, 29.5%), which is overwhelmingly high. Most studies used quantitative methods (183, 72.0%). Recognition was the most studied theme (62, 24.4%), and 16 of them focused on recognition of death (6.3%). Intervention studies totaled 34 (13.4%), and most of them (7, 2.8%) were conducted for palliative education. Conclusion: Most hospice nursing studies were quantitative studies, conducted with patients, and hospice nursing intervention programs were similar to each other. Thus, more studies with hospice patients and their families are needed. And more experimental and qualitative studies are needed to build an evidence-based nursing study environment. Considering how most studies examined psychological factors, physiological factors such as pain, a major issue in hospice care, should be analyzed in experimental studies to construct a pain intervention program for hospice patients.

Trends in Research on Hospice Care in Korea (호스피스 간호 관련 국내 연구 동향)

  • Purpose: This study was conducted to analyze a trend in studies of hospice care in Korea and to suggest future research directions. Methods: This study analyzed a total of 183 studies related to hospice care which were found at http://www.riss.kr. Results: Of the 183 studies, 62 (33.9%) were for thesis studies for a degree, 121 (66.1%) for academic journals. There were 159 (87.8%) quantitative studies, 18 (8.9%) qualitative studies, and 6 (3.3%) Q-Method studies. The most frequently used study design was a descriptive study. The majority of study participants were terminal cancer patients, patients' family members and health care personnel. The thesis articles were searched by using key words as follows: health, nursing, environment, children's hospice, and others. Conclusion: The number of studies of hospice care has increased, and research key words varied. Furthermore, more experimental research is needed on nursing intervention such as pain relief, alternative therapies and hospice care for children.

A study on the utilization of complementary and alternative medicine for elementary children (학동기 소아에서 보완대체요법의 이용실태)

  • Purpose: Recently, complementary and alternative medicine (CAM) has been increasingly used in children. Studies have shown that 34% of adults and 11% of children use CAM in the USA and Canada. The purpose of this study was to investigate the prevalence and patterns of CAM use in elementary children in Korea. Methods:From July to August 2007, parents of elementary children completed a questionnaire survey at Gwang-ju. In all, 794 questionnaires were analyzed. Results:Of the 794 respondents, 278 answered that their pupils (35%) had experienced CAM. The following types of CAM therapy were used: herbal medicine, 62.5% dietary supplements, 31.2% vitamins, 30.2% and acupuncture, 11.1%. CAM therapies were used for the following diseases: nutritional deficiency, 33.3% atopic dermatitis, 31.3% arthralgia, 31.3% allergic rhinitis, 28.8% obesity, 26.3% and asthma. The following were the motives to use CAM: prevention of diseases (33.5%), dissatisfaction with modern medicine (21.2%), and complementary therapy to modern medicine (20.5%). People gained information about CAM through neighbors (65%) and mass media (21%). Moreover, 83 parents (30%) were satisfied with CAM because of its effectiveness. Conclusion:Many parents have advocated the use of CAM in their children. However, most of them used CAM without any prescription or adequate knowledge. Further studies are required to determine the efficacy of CAM.

Study of Oral Health-related Quality of Life Index for Primary School Oral Health Program (초등학교 구강보건사업을 위한 아동의 구강건강관련 삶의 질 연구)

  • The purpose of this study was to collect base-data for evaluation of primary school oral health program. The data was Obtained from 648 children of four primary schools in Gwangiu city, Gyeonggi-Do Korea, during 2 month (March to May, 2006). The survey was performed by a face-to-face interview questionnaire consisting of OIDP. We analyzed knowledge, attitude, action and DMFT, oral health knowledge, the state satisfaction, no dental treatment need according to being or not being of the score using SPSS 12.0. ODIP score measure was based on scoring method for OIDP. The results were as follows. 1. In knowledge, attitude, behavior of oral hygiene with regard to OIDP score, he group with OIDP score is higher than the group without that and in case of knowledge and behavior, the group with OIDP score has a significant difference from the group without (p < 0.05). 2. In case of the oral hygiene cognition, a situation satisfaction, a no dental treatment need based on OIDP score, the group without OIDP score has a desirable andsignificant difference from the group with OIDP score (p < 0.05). 3. In case of DMFT based on OIDP score, the group with has a higher DMFT than the group without and has an significant difference from the group without (p < 0.05).

The Relationship between Recurrent Abdominal Pain and Depressive Trends in School-Aged Children (학동기 소아에서 반복성 복통과 우울 성향과의 관계)

  • Purpose: Recurrent abdominal pain (RAP) is common, affecting between 7% and 25% of school-aged children and young adult. This study was designed to assess the relationship between RAP and depressive trends in school-aged children. Methods: In 1999, We surveyed 1,181 elementary school children (566 boys and 615 girls) whose ages ranged from 9 to 12 years. The degree of depression was compared between the group with RAP and normal control group without RAP according to Korean Children's Depression Inventory (CDI) of Kovacs and Beck. Results: The number of the group with RAP was significantly higher in female (25.7%) than in male (18.9%). The most common site of abdominal pain was periumbilical. The mean depression score in the group with RAP was higher than that of the control group. The depression score was significantly higher in the group with RAP who has frequently pain and additional symptoms. The depression score was significantly higher in the group with RAP who prefered to play alone, did not enjoy going to school and had few friends than in the group with RAP who had a lot of friends, had active and meticulous characters. The depression scores were not significantly different according to the duration of abdominal pain, the onset time of pain, and the day of pain in the group with RAP. Conclusion: The depression score of the group with RAP was higher than that of normal group. So, there is relatively a specific association between childhood RAP and depression in school aged children. It is good for physicians to consider psychological support in the treatment of childhood RAP.

A Study on the Influence of Long-Term Intake of Fermented Milk on Oral Ecology in Preschool Children (유산균 발효유의 장기 섭취가 미취학 아동의 구강 생태에 미치는 영향에 관한 연구)

  • As fermented milk has been regarded as health food, the consumption of fermented milk has been increasing significantly these days. But there is not sufficient information on the effect of fermented milk on oral health. We have investigated the effect of long-term intake of t\fermented milk on saliva in preschool children, Sixty-four healthy, unmedicated preschool children were included in this study and were divided into control, fermented milk, and milk groups. The experimental period was 210days. We investigated the number of slaivary S. mutans, lactobacilli, and Candida, salivary pH, viscosity, and the concentration of salivary IgA at the beginning of the experiment. We examined these parameters at 10, 60, 110, 160, and 210 days after. The obtained results were as follows : 1. There were increases in the numbers of slaivary S. mutans in all groups through the experimental period. But, there was no consistent and significant difference among groups. 2. There was no significant change in the number of salivary lactobacilli in the fermented milk group throughout the experimental period. 3. The numbers of salivary Candida in the fermented milk and milk groups showed less fluctuation than that of the control group through the experimental period. 4. There was no decrease of salivary pH in all groups through the experimental period. 5. There were increases in the values of salivary viscosity in all groups. But, there was no consistent and significant difference among groups. 6. There were no consistent and significant difference in the concentration of salivary IgA in all groups through the experimental period. 7. There were increases of dmfs rate in all groups. But, there were no significant difference within each groups.
