The Relationship between Bone Bruise in MR-Imaging and the Degrees of Acute Anterior Cruciate Ligament Injury (급성 전방십자인대 손상 시 자기공명영상에서 나타나는 골멍과 전방십자인대 손상 정도의 관계)

  • Purpose: The aim of this retrospective study was to evaluate the relationship between appearance and size of bone bruise and severity of anterior cruciate ligament (ACL) injury with magnetic resonance imaging (MRI), furthermore, to identify the mechanism of ACL injury by analyzing the footprint of bone bruise. Materials and Methods: Sixty-five subjects who was taken an MRI within 6 weeks after acute ACL injury were studied. All MRI showed ACL injury and associated bone bruises. Bone bruises were analyzed using Costa-Paz classification based on appearance and location and also scored using Kornaat bone bruise grading system based on size and location. The severity of the ACL injury was graded using a 4-point system (ie, grade 0-3) with oblique coronal MRI. Results: There was statistically significant correlation between Costa-Paz classification of bone bruises in lateral femoral condyle and grade of ACL injury. There were more type 2 lesions in partial tear group (grade 1, 2), however, type 3 lesions of Costa-Paz classification in complete tear group (P=0.037). Significant positive correlations were observed between Kornaat total bone bruise score and grade of ACL injury (P=0.014). Most common ACL injury mechanism was pivot shift injury. Other mechanisms were clip valgus injury, phantom foot injury, and hyperextension injury. Conclusion: It is highly suggested that appearance and size of bone bruise is related to severity of ACL injury after acute ACL injury. The foot print of bone bruise provides valuable clues to identify ACL injury mechanism.

Anterior Cruciate Ligament Segmentation in Knee MRI with Locally-aligned Probabilistic Atlas and Iterative Graph Cuts (무릎 자기공명영상에서 지역적 확률 아틀라스 정렬 및 반복적 그래프 컷을 이용한 전방십자인대 분할)

  • Segmentation of the anterior cruciate ligament (ACL) in knee MRI remains a challenging task due to its inhomogeneous signal intensity and low contrast with surrounding soft tissues. In this paper, we propose a multi-atlas-based segmentation of the ACL in knee MRI with locally-aligned probabilistic atlas (PA) in an iterative graph cuts framework. First, a novel PA generation method is proposed with global and local multi-atlas alignment by means of rigid registration. Second, with the generated PA, segmentation of the ACL is performed by maximum-aposteriori (MAP) estimation and then by graph cuts. Third, refinement of ACL segmentation is performed by improving shape prior through mask-based PA generation and iterative graph cuts. Experiments were performed with a Dice similarity coefficients of 75.0%, an average surface distance of 1.7 pixels, and a root mean squared distance of 2.7 pixels, which increased accuracy by 12.8%, 22.7%, and 22.9%, respectively, from the graph cuts with patient-specific shape constraints.

  • The concept of double-bundle ACL reconstruction was introduced recently to restore the anatomical and biomechanical functions of the native ACL. According to anatomical and biomechanical studies, the separate reconstruction of anteromedial and posterolateral bundle expect to increase the overall postoperative stability and clinical results compared to single-bundle ACL reconstruction. But there is still a lack of available clinical outcome studies with sufficient follow-up to demonstrate the real advantages of double-bundle ACL reconstruction. The purpose of this article is to review the evidence to support double-bundle technique and to address controversies existing over the usefulness of this technique.

Reconstruction of Anterior Cruciate Ligament with Fresh-frozen Achilles Allograft (신선 동결 동종 아킬레스건을 이용한 전방 십자인대 재건술)

  • Purpose: We analyzed the clinical results of ACL(anterior cruciate ligament) reconstruction of chronic anterior cruciate ligament injuries with a fresh frozen Achilles allograft, retrospectively. Methods: Thirty-eight patients(average age 32.4 years) who had an chronic injury of ACL reconstructed with an Achilles allograft between January 1999 and February 2001 were included. The mean follow-up was 19.2(12-36) months. The clinical evaluation was done by range of motion, Lachman and pivot-shift test, KT-1000 arthrometer, Lysholm knee score, and the modified Feagin score. Results: All patients recovered full range of motion of affected knees. With use of Lachman and pivot-shift test data, thirty seven(97.4%) were functional. When comparing KT-1000, we found mean maximum side-to-side differences less than 2.1 mm. According to Lysholm knee score, mean postoperative score was 90.4. Thirty four(89.5%) had good to excellent results in modified Feagin score. Conclusion: Results of ACL reconstruction using fresh frozen Achilles allografts were comparable to autografts. ACL reconstruction with Achilles allograft could be a reasonable altemative to autografts.

Differences in Functional Recovery according to Exercise Rehabilitation after Posterior Cruciate Ligament with or without Posterolateral Complex Reconstruction (뒤십자인대 및 뒤가쪽 복합체 동반 수술 후 재활운동에 따른 기능회복 차이)

  • This study aimed to compare functional recovery after rehabilitation exercise between isolated PCL reconstruction and combined PLC reconstruction. Patients were divided into two groups: those who had isolated PCL reconstruction (n = 16) and those who had combined PLC reconstruction (n = 16). We assessed knee joint ligament laxity, subjective questionnaires, and isokinetic muscle function before, after 12, and 24 weeks of a rehabilitation exercise program. In both groups, there were significant differences in knee joint laxity (p = 0.048), IKDC subject score (p < 0.001), Lysholm knee (p < 0.001), Tegner activity scale (p = 0.027), and isokinetic muscle deficit (p = 0.040) by estimated period. However, no significant difference between groups was observed (p > 0.05). These results suggest that rehabilitation exercise after isolated PCL and combined PLC reconstruction influenced structural, subjective, functional recovery positively.

  • 후방십자인대 및 후외측인대 재건술 후 결과는 많은 발전이 있었지만, evidence medicine의 측면에서 재활 방법에 관한 전향적 연구는 없다. 현재까지의 재활 방법은 다양하게 소개되었지만 전방십자인대 재건술처럼 가속 재활을 하는 것은 바람직스럽지 않다. 후방십자인대, 후외측인대 그리고 다발성 인대 재건술 후 재활은 수술 방법과 환자의 재건된 인대의 상태, 하지 정렬 등에 따라서 개별화하여 점진적으로 진행하여야 할 것이고, 재활기간 동안 의사의 세심한 관찰이 필요하다.

  • The purpose of This study was to compare the effect of Convergence-Based russian current(RC) and transcutaneous electrical nerve stimulation(TENS) on pain, muscle strength, function of knee who had a surgical anterior cruciate ligament reconstruction(ACLR). A total of 40 Participants divided into 2 groups, with ACLR, were assigned to russian current group(RCG) or transcutaneous electrical nerve stimulation group(TENSG). Both groups were measured by varieties of tests: visual analog scale(VAS), knee flexion & extension muscle strength, knee injury and osteoarthritis outcome score(KOOS), korean lower extremity functional scale(LEFS). Both groups has shown significant changes within the measurements of VAS, Knee Flexion & Extension muscle strength, KOOS and LEFS. The RCG had more significant increase within knee extension muscle strength, LEFS compared to TENSG. By studying this research, both interventions were effective to ACLR patients, moreover, RC was more effective for knee muscle strength and lower extremity function than the TENS.

The Efficacy of Conjugated Linoleic Acid and Carprofen in Progression of Early Stage of Experimentally Induced Osteoarthritis in Dogs (개에서 Conjugated Linoleic Acid와 Carprofen이 실험적 골관절염 초기 진행에 미치는 효능)

  • This study was performed to compare early stage changes following the administrations of Conjugated Linoleic Acid (CLA), Carprofen and their combinations in the experimental canine cranial cruciate ligament rupture. Twenty five mongrel dogs were divided into five groups; Groups 1, 2, and 3 received a sectioning the cranial cruciate ligament (CCL) of the right stifle joint, and were administered CLA (250 mg/kg/day/orally), carprofen (4.4 mg/kg/day/orally), and their combinations for 4 weeks beginning 4 weeks postsurgery, respectively. Group 4 received sectioning CCL and no treatment. Group 5 was composed of unoperated normal dogs. The macroscopic observation of cartilage erosions on both the condyles and plateaus were evaluated, and the severity of the cartilage lesions and synovial inflammation was examined histologically at eight weeks after surgery. Histological examinations including hematoxylin and eosin stain, standard toluidine blue method, PAS technique and Masson trichrome technique, hematologic and radiographic evaluation were perfonned after experimental surgery. Slight yellowish discoloration of the surface was found in some of the experimental dogs. However macroscopic findings showed no significant differences among the groups. In radiographic findings, cranial displacement of the proximal tibia relative to the femoral condyles in all groups was observed but no significant differences among the groups was seen. This study showed that oral administration of CLA, carprofen and their combinations revealed no early stage change in the canine stifle joint following experimental rupture of the cranial cruciate ligament.

The change of the Mechanoreceptor of Posterior Cruciate Ligament after Posterior Cruciate Ligament Injury in the Rabbit (토끼에서 후방십자인대 손상 후 후방십자인대의 기계적 수용기의 변화)

  • Purpose : To assess the change in the number of mechanoreceptors of posterior cruciate ligament(PCL) after transecting posterior cruciate ligament of the rabbit. Materials and Methods : We selected 10 skeletally-matured New Zealand white rabbits and one of the two posterior cruciate ligaments was transected and the other underwent sham-operation on each rabbit. Afterward, PCLs with synovium were obtained at femoral attach area in both knees in 20 weeks postoperatively, but in one of the cases, the transected PCL did not heal and had to be excluded from this study. Consequently, histologic examination of the ligament for the remaining 9 cases was done after being prepared with modified gold chloride stain. Results : The study showed a little bit more decrease in the number of mechanoreceptors in the transected group compared with that in the sham-operated group, but the number was statistically negligible. Conclusions : It is considered that the knee joint with injured PCL may still maintain proprioception since the decrease in the number of mechanoreceptors is not considerable, but further study on biomechanical role and function of mechanoreceptor of healed PCL is required.
