Cardiac Surgery for the Patient with VVI Unipolar Pacing System -One Case- (VVI 단극유도의 심박조율치를 가진 환자의 개심술 -1례 치험-)

  • A 56-year-old female with a permanent unipolar pacing(VVI) system underwent elective coronary bypass surgery and mitral valve replacement. Because the unipolar sensing is subject to considerable nonmyocardial electrical noise, the pacing function may be disturbed by the use of electrocautery. Temporary atrial and ventricular bipolar epicardial leads and external generator were used for maintaining A-V sequential pacing during the use of electrocautery. Before aortic cross-clamp was released after cardiac operation, regular cardiac rhythm could spontaneously be resumed with an integrated cardioplegic strategy, avoiding the use of defibrillator

Thirteen-year Experience of Permanent Epicardial Pacing in Children (소아연령군에서의 영구 심외막 심박 조율 13년 경험)

  • Background: We investigated the longevity, thresholds of epicardial pacemaker and causes of reoperation in the pediatric patients who underwent epicardial pacemaker implantation performed during the last 13 years Material and Method: 121 operations were performed in 83 patients from January 1989 to July 2002. We analyzed the stimulation threshold, resistance, R-wave and P-wave, and sensitivity of pacemaker lead at initial implantation. Longevity and causes of reoperations were investigated. Result At implantation, epicardial ventricular mean stimula-tion threshold was 1.2$\pm$0.1 (0.1∼5) mV, mean resistance was 519.1$\pm$18.1 (319∼778) Ohm, and mean R-wave sensitivity was 8.9$\pm$0.7 (4∼20) mV, and mean P wave sensivity was 2.5$\pm$0.7 (0.4∼12) mV. The mean longe-vity of pacemaker generator was 64.7$\pm$3.7 (2∼196) months. The reoperation free rate was 94.6% for 1 year, 93.6% for 2 years, 80.8% for 5years, 63.7% for 7 years, and 45.5% for 10 years. The causes of reoperation were battery waste in 26 cases and lead malfunction in 9 cases. There was no postoperative death related to pacemaker malfunction. Conclusion: in the childrens, average longevity of epicardial pacemaker was within accep-table range. 19.1% of the patients required pacemaker related reoperation. However, recent developments, including steroid eluting lead, 6.7% of the patients required pacemaker related reoperation, look promising in expansion of pacemaker life span.

  • When any part of the pacemaker system is infected, all pacemaker hardware should be removed, because reinfection rates of 51% to 77% have been noted in patients whose infected pacemaker system has been only partially explanted. The removal of infected leads during cardiopulmonary bypass prevents mechanical injury as well as the spread of infection and vegetation. We report one case of endocarditis by staphylococcus aureus with infected transvenous pacing lead which was migrated into the heart from the left subclavian vein. We removed the migrated pacing lead and vegetation under cardiopulmonary bypass.

  • Bleeding due to cardiac perforation by endocardial pacemaker lead is a rare complication. We report one case of left hemothorax due to right ventricular perforation after the insertion of permanent transvenous pacemaker. Operative finding showed a pacing lead penetrating right ventricle, pericardium, and left pleura sequentially, but there was no evidence of hemopericardium.

  • In this paper, we developed a pacemaker that can increase the efficacy of a left ventricular assist device (LVAD) and increase the survival rate for patients suffering end-stage heart failure. Because LVAD patients can experience arrhythmia, the pacemaker incorporated into the LVAD has the important role of sustaining sufficient blood circulation during arrhythmia. The electrode of the pacemaker is located at the apex of the left ventricle, where the VAD's inlet cannula is connected. This is efficient placement, in that the electrode can transmit electrical stimulation directly to the Purkinje fibers of the myocardium. The pacemaker can change the stimulation rate from 0 bpm to 191.4 bpm when a button is pressed on the external control module, and the pacemaker normally stimulates the heart at 60 bpm with 0.25 J of energy. We performed animal experiments to evaluate the performance and reliability of the combination of the LVAD and pacemaker. At pacemaker stimulation rates of 86.4 bpm, 100.2 bpm, 126.6 bpm, we recorded the ECGs, aortic pressures, and flow rates to analyze the heart loads.

  • The most important indication for the removal of the lead-electrode system is the presence of an infection. When an infection occurs, the entire pacemaker system including the impluse generator and lead-electrode system should be removed. The entrapped electrode can be removed by a continous traction, by the use of forceps, snares or baskets, by the use of a locking stylet or a dilator sheath, and by an operation. We report a case that underwent a removal of an entrapped transvenous pacemaker electrode by the use of snaring technique.

  • Pacemaker lead-related infective endocarditis is an uncommon, but serious complication. We report a case of a 45-year-old man who had symptom of intermittent high fever and rupture of sinus Valsalva that developed after a redo aortic valve replacement and transvenous permanent pacemaker implantation. Positive blood cultures of streptococcus viridans and transesophageal echocardiography showing a large mobile vegetation on pacemaker lead and tricuspid valve lead to the diagnosis of pacemaker lead-related infective endocarditis. Initial antibiotic therapy followed by surgical extraction of the pacemaker lead and wide debridement of infective tissues including multiple vegetations was required. Postoperative antibiotic therapy was continued for 4 weeks. The postoperative course has been uneventful. The patient is totally asymptomatic and is doing well up to now.

Effect of Providing Information on Anxiety, Knowledge and Compliance of Patients with a Permanent Pacemaker (정보제공이 영구형 심박조율기 이식환자의 불안, 지식 및 치료지시이행에 미치는 효과)

  • Purpose: This study was undertaken to identify the effect of providing information on anxiety, knowledge and compliance in permanent pacemaker patients. Method: A quasi experimental design with non-equivalent control group and non-synchronized design was used. The subjects of this study were 50 patients who had received permanent pacemaker implantation at a university hospital in Seoul. They were divided into an experimental group of 22 patients who received education and a control group of 28 patients. The education was composed of group education(twice) and individualized reinforcement education(once) using an education booklet. Results: Anxiety decreased in the experimental group. Knowledge significantly increased in the experimental group compared to that in the control group. Compliance significantly increased in the experimental group. Conclusion: It can be concluded that providing information is effective for reducing anxiety, increasing knowledge and improving compliance of the permanent pacemaker patients.
