Implant supported fixed prosthesis for complete edentulous maxilla with severe alveolar ridge resorption: A case report (치조골 흡수가 심한 상악 완전 무치악 환자에서 임플란트 고정성 보철물을 이용한 수복 증례)

  • Implant fixed prosthesis for the complete edentulous maxilla provides significant benefits in the aspects of functions and esthetics compared with the conventional denture. Implant supported fixed prosthesis are totally supported by implant, and thus stabilizes the prosthesis to the maximum degree as possible. Also, the improved retention and stability of fixed prosthesis enhance patients' psychological and psychosocial health. This clinical presentation describes a maxillary full arch implant-supported fixed prosthesis in complete maxillary edentulous patient who showed vertical and horizontal alveolar bone resorption in the anterior ridge. To rehabilitate the esthetics and proper lip support, the zirconia framework was fabricated and the pink porcelain was veneered to reproduce the natural gingival tissue. After 9 months of follow up, the restorations were maintained without complications and the patient was satisfied with the restoration both functionally and esthetically.

  • In patients with fully edentulous maxilla, fabrication of implant-supported overdenture can be a viable treatment option, when a minimum of six implants were strategically placed. Among several attachment systems used for implant-supported overdentures, milled-bars prevent rotational movement of denture, thus showing great stability, and have the advantage of splinting multiple implants with each other. In this case report, a milled-bar supported overdenture was fabricated for a patient suffering from condition of fully edentulous maxilla with severe ridge resorption in the anterior residual ridge. Seven implants composed of three different systems were effectively utilized by CAD/CAM customized abutment and cement-retained milled bar.

  • Decrease of occlusal vertical dimension (OVD) due to loss of teeth structure and destruction of the occlusal plane by severely worn dentition may cause cross bite or temporomandibular joint disorder by following change of facial feature or the loss of anterior guidance. Full mouth rehabilitation via an increase of the OVD can be considered to avoid this problem and proper evaluation of patient's OVD is essential. An 80 year old male visited for overall prosthodontic treatment, cross bite due to continuous wear and following decrease of the OVD were observed. We analyzed the existing occlusal relationship using the diagnostic cast, the radiographic evaluation and clinical test, and then proper increase of OVD was selected. The new OVD on diagnostic wax up was placed by the temporary restoration. After 3 months of observation period, final restoration with fixed partial dentures and implant overdenture were made. Throughout the follow-up period of 8 months, the aesthetic and functional improvement can be obtained.

  • Tooth impaction may lead to malocclusion, root resolution, cyst or aberrant changes in adjacent teeth. Clinical and radiographic examinations are used to locate the impaction, and appropriate treatment plans must be made to relocate the impacted tooth. When surgically exposing the impacted tooth, periodontal considerations to conserve maximum amount of soft and hard tissue are used. Oral hygiene instructions are emphasized to maintain sound periodontal health. Securing enough space for the impacted tooth and proper anchorage is important. Proper use of force and mechanics is crucial to prevent such complications as root resolution. Various patterns of orthodontic traction may be employed as situation permits. Most impaction cases can be managed with orthodontic traction to restore function and esthetics, provided that early detection and proper diagnosis and treatment planning are made.

  • Positions, angulation and mesiodistal dimension of lower incisors are important in esthetics, occlusion and post-treatment stability of tower arch. When lower incisor is congenitally missing, problems such as increased overjet and overbite, closing in of adjacent teeth and size/space discrepancies may occur. When creating treatment plans, incisor position and angulation, lip support, anteroposterior skeletal relationship canine-molar relationship, overjet overbite, remaining growth potential, crowding and anterior tooth ratio have to be considered. For an accurate analysis of incisal size discrepancy, diagnostic model set-up may be helpful. The two patients in this presentation both had two lower incisor missing, but the degree of crowding, skeletal relationship, lip support, molar relationship are different and therefore treatment plan was different as well. Long term follow-up may be necessary for stability and retention.

  • The retaining methods of implant prosthesis were classified into a screw-retained and a cement-retained type. A screw-retained prosthesis has many advantages, such as retrievability, preventing residual cement, while their disadvantages include the possibility of screw loosening and fracture, on the contrary advantages of cement-retained prosthesis are relatively low cost, but they are difficult to retrieve. To combine the advantages of both type, screw-cement retained prosthesis (SCRP) type have been introduced. But they still require ideal implant placement. So we introduce fiber post retained prosthesis without residual cement for preventing soft tissue trouble due to excessive cement.

  • Incontinentia pigmenti(IP), so called Block-Sulzberger syndrome is a rare genodermatosis that occurs almost in female infant; usually lethal in males, X-linked dominantly inherited disorder. IP is characterized by abnormalities of mesodermal and ectodermal tissues including eye, tooth, skin, nail, breast and hair as well as neurological deficiencies. Dental problems are congenital missing of teeth, delayed eruption, abnormal crown shape and so on. Here is a case of 6 year-old female with IP. She had congenital missing of primary and permanent teeth, delayed eruption, maxillary deficiency and extra cusps, resulting in unstable occlusion. Systemically, she had a history of operating eyes due to problem of retina and hyperpigmented macules on her trunk and extremities as typical character of IP.

  • Delayed eruption of a maxillary incisor results in midline shift, the space occupied by adjacent teeth and different levels of alveolar height. Extraction or surgical/orthodontic therapy is the most common treatment for a impacted maxillary incisor. Surgical repositioning provides another option for treatment of this problem. The advantages of this approach include immediate esthetic improvement, use of a single and simplified surgical procedure, simple and short orthodontic therapy, a normal gingival margin and the possibility of the developing root adapting to the new position. Autotransplantation of an immature tooth provides for possible adaptation of the developing root apex to the new position. A root with an open apex has good chance of pulp revascularization after transplantation.

Recent trend and surgical management for panfacial fracture (범안면골 골절의 최근 경향 및 수술개념)

  • Panfacial fracture is extremely difficult to manage facial injuries but concomitant injuries and severe complications including facial esthetic and functional problems can make it harder. Thorough evaluation and closed co-work with other specialists is needed when reduction and fixation cannot be achieved quickly. Emergency bony support and soft tissue key suture provide the patients with airway integrity, hard and soft tissue vitality. A systemic treatment plan must be made by 3D CT image. This plan include airway management for surgery, sequence of reduction and fixation, approach method, soft tissue resuspension and reconstruction of lost tissue like inferior orbital wall, zygomaic buttress and soft tissue. From known to unknown structures, accurate reduction and fixation will provide proper occlusion, facial projection, width, hight and function. Consideration about facial retaining ligaments must be given to prevent soft tissue sagging.

Evaluation of the Lip during Smile in Normal Occlusion Adults (정상교합자의 미소시 구순 형태에 관한 연구)

  • This study was performed to mark the criteria of the eathetic smile that was necessary to improve the esthetic problem in oral maxillo-facial region. The facial straight photographs of 62 adults(30 males, 32 females : 19-24 years old) in resting position and during smile were taken. The measurements and proportion of lip-teeth relationship during smile were statistically analyzed with photogrammetry. The following results wow obtained : 1. In the evaluation of the change of lips, smile line ratio was 0.93, buccal corridor ratio was 0.63, and smile symmetry ratio was 0.96. 2. The width of mouth during smile was 1.31 times of the width inthe resting position and 0.48 times of face width. 3. The upper lip height during smile was 0.69 times of the height in the resting position and the lower lip height during smile was 0.93 times of the height in the resting position. 4. The mean exposed lenght of upper central incisor was 9.96mm. Maxillary incisor exposure was significantly correlated with the upper lip change ratio, mouth width change ratio, and buccal corridor ratio.
