The use of implant-assisted removable partial denture in the partially edentulous maxilla with a few unilateral remaining teeth and implant overdenture in the mandible: A case report (상악 편측 소수 잔존치에서 임플란트 융합 국소의치와 하악 임플란트 피개의치의 수복 증례)

  • Successful cases of the implant-assisted removable partial denture and implant overdentures are reported in which a few implants are additionally placed to secure the maintenance, support, and stability of the denture when there are a few residual teeth. When the lateral force applied to the tooth abutment and implant surveyed crown is minimized, the horizontal and rotational movement of the denture is significantly reduced which is an effective method that can improve the address in patients who complain of reduced retention and stability of their dentures. In this case, a small number of implants were placed to fabricate an implant-assisted removable partial denture with implant surveyed crown in the maxilla and implant overdenture with Locator® attachment in the mandible to improve the retention, stability, and support of the dentures. The patient was satisfied with both functional and aesthetic aspects after the final dentures were delivered.

Fabrication of complete denture using 3D printing: a case report (3D 프린팅을 이용한 양악 총의치 제작 증례)

  • Recently with the advance in digital dentistry, the fabrication of dentures using computer-aided design and computer-aided manufacturing (CAD-CAM) is on the rise. The denture designed through a CAD software can be produced in a 3-dimensional manufacturing process. This process includes a subtractive processing method such as milling and an additive processing method such as 3D printing and in which it can be applied efficiently in more complex structures. In this case, complete dentures were fabricated using Stereolithography (SLA)-based 3D printing to shorten the production time and interval of visits in patient with physical disabilities due to cerebral infarction. For definitive impression, the existing interim denture was digitally replicated and used as an individual tray. The definitive impression obtained with polyvinyl siloxane impression material was including information about the inclination and length of the maxillary anterior teeth, vertical dimension, and centric relation. In addition, facial scan data with interim denture was obtained so that it can be used as a reference in determination of the occlusal plane and in arrangement of artificial teeth during laboratory work. Artificial teeth were arranged through a CAD program, and a gingival festooning was performed. The definitive dentures were printed by SLA-based 3D printer using a FDA-approved liquid photocurable resin. The denture showed adequate retention, support, and stability, and results were satisfied functionally and aesthetically.

A study of Korean Norm about tooth size and ratio in Korean adults with normal occlusion (한국성인 정상교합자의 치아크기와 비율에 관한 연구)

  • The purpose of this study was to measure the average tooth size of Korean with normal occlusion. According to the study, the average tooth ratios between the upper and lower teeth which could assure the proper ovebite, overjet and good interdigitation were calculated. The normal occluson sample of this study consisted of 43 Korean male adults and 51 Korean female adults. Among them, 22 Korean male adults and 51 Korean female adults were from KAO(Korean Association of Orthodontists), 21 Korean male adults were from Department of Orthodontics, College of Dentistry, Yonsei University. The results from this study were as follows : 1 The average tooth size of Korean Norm classified by male and female was measured. 2. The average tooth size of Korean male adults with normal occlusion was significantly larger than that of Korean female adults except upper and lower first molars. (p<0.05) 3. The tooth ratio which could Predict the proper overbite and overjet in anterior teeth and proper occlusion in posterior teeth was calculated. 1) Sum of inciosrs = 4:2.97 2) Neff's anterior coefficient = 1.22 3) Bolton's anterior ratio = 78.29%, overall ratio = 91.14% 4. A positive correlation was observed between the sum of lower anterior incisors and the sum of unilateral canine and premolars In each jaw. Based this correlation, the regression equation was made which could Predict the sum of unerupted unilateral canine and premolars in mired dentition. 1) Sum of unilateral unerupted upper canine and premolars' width = 10.435018 + 0.513346 ${\times}$ (sum or lower 4 incisors' width) 2) Sum of unilateral unerupted lower canine and premolars' width = 9.654002 + 0.502565 ${\times}$ (sum of lower 4 incisors' width)

Evaluation of shade guide using digital shade analysis system (색조 선택 시스템을 이용한 shade guide의 색조 분석)

  • Statement of problem: There are two methods of color choice for the esthetic restoration. One is visual shade matching which draws a comparison between shade guide and teeth in dentist's own eye and the other is using a digital shade analysis system recently introduced. Although the visual shade matching has a lot of problems, decision of color by this visual shade matching and the ways of expression for the decided color are still applicable to clinical dentistry. Purpose: This study is designed to investigate shade guides used in the dental clinics and laboratories have the same value using ShadeEye-$NCC^{(R)}$ dental chroma meter (Shofu Inc., Kyoto, Japan) using shade guide are evaluated. Material and methods: At the first experiment, eight Vita Lumin Vacuum shade guides (Vident Inc., California, USA) were collected from the dental clinics. A1 and B1 shade tabs are chosen and the colors are analyzed five times each in both tooth and porcelain modes by digital shade analysis system, ShadeEye-$NCC^{(R)}$. In the second experiment, twelve Vita shade guides using practically in the dental clinics and laboratories were collected and also A1 and B1 shade tabs are chosen and the colors of A1 and B1 are analyzed one time each in both tooth and porcelain modes by ShadeEye-$NCC^{(R)}$. Results and conclusion: There were significant differences among eight shade guides in terms of shade (chroma), value and hue in both of A1 and B1 (P<.05). Shade guides using in present both dental clinics and laboratories did not show significant differences, except A1 in the porcelain mode, it showed significant differences (P<.05) in the shade even though the shade tab has the same name.

WEAR AND CHEMICAL DEGRADATION OF ESTHETIC RESTORATIVE MATERIALS (차세대 레진 개발을 위한 광중합형 복합레진의 화학적 분해와 마모에 관한 연구)

  • The aim of this study was to evaluate the resistance to degradation and to compare the wear resistance characteristics of four esthetic restorative materials in an alkaline solution(0.1N NaOH). The brands studied were MetafilCX(Sun medical, Japan) Solitaire 2(Heraeus Kulzer, USA), Composan LCM(Promedica, Germany), DenFil(Vericom, Korea). The results were as follows: 1. The mass loss of each brand was 0.74~7.94% and highest value in Metafil($7.94{\pm}0.39%$). 2. The sequence of the degree of degradation layer depth was in descending order by Metafil, Solitaire 2, DenFil, Composan LCM. There were significant differences between Metafil and the others(p<0.05). 3. The sequence of the Si loss was in descending order by Metafil, Solitaire 2, Composan LCM, DenFil. There were significant differences among the materials(p<0.05). 4. On SEM, destruction of bonding between matrix and filler and on CLSM, the depth of degradation layer of specimen surface was observed. 5. The sequence of maximum wear depth was in descending order by Metafil, Solitaire 2, Composan LCM and DenFil. There were significant differences among the materials(p<0.05). 6. The correlation coefficient between Si loss and degradation layer depth (r=0.491, p<0.05) and Vicker's hardness number and maximum wear depth (r=-0.942, p<0.05) were relatively high. These results indicate that hydrolytic degradation and wear may consider as a evaluation factors of composite resins.

The effects of mandibular setback osteotomy on the oropharyngeal airway space in mandibular prognathic patients (하악전돌 환자에서 하악골 후퇴수술이 기도공간에 미치는 영향)

  • As a result of surgical orthodontic treatment of mandibular prognathism, changes take place in the skeletal and soft orofacial components. Although some investigators had stated that permanent reduction of airway space was observed after mandibular setback surgery, it was not clear that this permanent reduction was sustained during long-term follow-up. The purpose of this study was to assess the changes in oropharyngeal airway space and soft tissue orofacial component following the mandibular setback surgery and during the follow-up period. The correlation between the changes of the oropharyngeal airway space and the changes of other soft tissue orofacial component was also assessed. The findings of this study were as follows ; 1. The oropharyngeal airway space area decreased following mandibular setback surgery for mandibular prognathism and continued to decrease during the follow-up period(p<0.05). 2. The pharyngeal depth at Xi point level and the 2nd cervical vertebra point level decreased after the surgery and remained during the follow-up period(p<0.05). The decrease of these pharyngeal depth was correlated with the decrease of oropharyngeal airway space area(p<0.01). 3. The decrease of pharyngeal depth at the 3rd and 4th cervical vertebra point level was not significant after the surgery and during the follow up period. 4. The hyoid bone moved downward after the surgery(p<0.05), but returned to its original position during the follow-up period. 5. The length & height of tongue and the Position of epiglottis base did not change significantly(p>0.05). 6. The soft palate was displaced posteriorly after the surgery and remained to its changed position during the follow-up period(p<0.05) due to posterior displacement of tongue. The changes of soft palate were significantly correlated with the decrease of oropharyngeal airway space area(p<0.01). 7. The narrowing of oropharyngeal airway space was due to the posterior displacement of tongue above the level of epiglottis tip. The posterior displacement of tongue following mandibular setback osteotomy remained during the follow-up period.

Changes of root length and crestal bone height before and after the orthodontic treatment in nail biting patients (손톱 깨물기 습관을 가진 아동의 교정 치료 시 전치부 치근 길이와 치조골 높이의 변화)

  • Although the purpose of orthodontic treatment is to increase the function and esthetics of the jaws along with increasing stability, there are many side effects during the treatment itself, such as root resorption and alveolar bone resorption. Such resorption of the apical root Is unpredictable, and may even proceed into the dentin layer. Once the process has begun, it is irreversible. By evaluating the effect of many oral habits, especially that of nail biting, in correlation with the root and the periodontal tissues, the appropriate biomechanics for orthodontic treatment can be taken into consideration, along with the possibility of root resorption and alveolar bone loss during orthodontic treatment, and any legal problems that might occur. Among the male and female patients of the ages $10\~15$ without skeletal deformity, 63 were chosen as the experiment group with known nail biting habits at time of examination, and within the same age group without nail biting habits as the control. After the orthodontic treatment, number of the experiment group was 31 and the control group was 22. The periapical radiographies of anterior teeth were taken and the assesment of the root length and alveolar bone level were taken before(T1) and after(T2) the orthodontic treatment. The results from this study were as follows : 1. Before the orthodontic treatment, average crown-to-root ratio of the experimental group showed noticeably high values in 4 maxillary incisors and mandibular right central incisor. 2. Before the orthodontic treatment, comparing the root length, maxillary and mandibular right central incisors and both mandibular incisors had a smaller value in the experimental group. 3. Before the orthodontic treatment, comparing and evaluating the alveolar bone loss measured from the cemento-enamel junction to the alveolar bone crest, some crestal bone of the experiment group showed greater loss than the control. 4. After the orthodontic treatment, there was shortening of the root length and loss of the crestal bone in both groups. 5. After the orthodontic treatment, the changes of C/R ratio and the shortening of root length were significantly high in the experimental group. 6. After the orthodontic treatment, the level of alveolar crestal bone showed greater loss in the experimental group.

The study on the shear bond strength of resin and porcelain to Titanium (티타늄에 대한 레진과 도재의 결합 강도에 관한 연구)

  • Statement of problem: Recently, titanium has become popular as superstructure material in implant dentistry because titanium superstructure can be easily milled by means of computer-aided design and manufacture (CAD/CAM) technique. But retention form such as nail head or bead cannot be cut as a result of technical limitation of CAD/CAM milling and bond strength between titanium and porcelain is not as strong as that of conventional gold or metal alloy. Purpose: The objective of this study was to evaluate the shear bond strength of three different materials: heat curing resin, composite resin, porcelain which were bonded to grade II commercially pure Titanium (CP-Ti). Material and methods: Thirty seven CP-Ti discs with 9 mm diameter, 10 mm height were divided into three groups and were bonded with heat curing resin (Lucitone 199), indirect composite resin (Sinfony), and porcelain (Triceram) which were mounted in a former with 7 mm diameter and 1 mm height. Samples were thermocycled for 1000 cycles at between $5-55^{\circ}C$. Shear bond strength (MPa) was measured with Instron Universal Testing Machine with cross head speed of 1 mm/min. The failure pattern was observed at the fractured surface and divided into adhesive, cohesive, and combination failure. The data were analyzed by one-way ANOVA and Scheffe's multiple range test (${\alpha}=0.05$). Results: Lucitone 199 ($17.82{\pm}5.13\;MPa$) showed the highest shear bond strength, followed by Triceram ($12.97{\pm}2.11\;MPa$), and Sinfony ($6.00{\pm}1.31\;MPa$). Most of the failure patterns in Lucitone 199 and Sinfony group were adhesive failure, whereas those in Triceram group were combination failure. Conclusion: Heat curing resin formed the strongest bond to titanium which is used as a CAD/CAM milling block. But the bond strength is still low compared with the bond utilizing mechanical interlocking and there are many adhesive failures which suggest that more studies to enhance bond strength are needed.

A study on the smile of Korean Youth compared with twenty years ago (20년전과 비교한 한국청년의 미소 변화에 관한 연구)

  • Purpose: Teeth are generally exposed when people smiling. Moreover, the exposed teeth and soft tissue when smiling becomes an important guideline for esthetically prosthetic restoration. This research is to compare and find out differences of Korean young men's smile living in presence and twenty years ago. Materials and methods: Subjects, 100 young men (50 male and 50 female), were required about several aspects; normally developed physical condition, no psychological or genetic disorders, a fine face with no loss of teeth, no experience in orthodontic or prosthetic treatment, relatively normal occlusion, aged between 20-29. The photos of the subject at rest position and front face when fully smiled were taken three times. 100 photos (50 male and 50 female) were chosen at random from the 240 university students' smile photos taken by Yoon and his colleagues in 1991. By Hulsey's method of measuring smile, several factors; the change of upper lip curvature, the change of the relation between the upper lip and teeth, parallelism between Mx. incisor and lower lip, contact relation between Mx. incisor and lower lip and teeth displayed in a smile, were measured and analyzed. Meanwhile, ten dentists assessed aesthetic evaluation about men and women's smile for twice and recorded and compared smile score. The -test (P<.05) was used to compare the measured value. The difference of smile score was analyzed by t-test (P<.05). Results: The smile score calculated in 2011 (60.22) was higher than that of 1991 (52.80). Among five measurement categories, the noticeable difference was distinguished from two factors; the change of upper lip curvature and contact relation between Mx. incisor and lower lip. Conclusion: The Korean young men's smile has been considerably improved for twenty years. And it is found that the change of upper lip curvature plays an important role, that is, the smile formed with an ascended labial commissure has been increased significantly.

A COMPARATIVE STUDY OF WATER ABSORPTION AND RELATED CHANGES OF COMPRESSIVE STRENGTH IN THREE RESTORATIVE MATERIALS (치과용 수복재의 수분흡수도와 압축강도의 변화에 관한 연구)

  • The aesthetic restorative materials are widely used in recent dentistry, showed not only the different quality between each component but also showed that the environment of the material has important effect on the physical properties of the material. Especially, when the restoratives are restored in the mouth, and since saliva is existing, the restoratives are always exposed to water and affected by the saliva. The purpose of this study was to research and compare the change of the degree of water sorption after water immersion of three types of the resin-based materials and the compressive strength, and observe the relation between the change of the water absorbing degree and the compressive strength. Z-100(3M, U.S.A) as a composite resin, F-2000(3M, U.S.A) as a compomer and Vitremer Restoratives(3M, U.S.A) as a resin-modified GIC were used, and each specimen was made to measure the water sorption and to evaluate the compressive strength. The specimens for measurement of the water sorption and the compressive strength were divided into 4 groups(1 day, 2 weeks, 4 weeks, and 8weeks). Each specimen was filled in the 30cc vial with 20cc of distilled water during the fixed amount of period in $37^{\circ}C$. The water sorption is decided by dividing the difference of weight before and after the immersion by the volume, and the compressive strength was measured by using the instron after the immersion. The following results were obtained ; 1. The more the water sorption increased, the more the immersion period of three restorative materials was long. And the most of water sorption was obtained during the first 2 weeks(P<0.001). 2. The water sorption of resin-modified GIC was higher than composite resin and compomer. 3. The more the compressive strength decreased, the more the immersion period of three restorative materials increased(composite resin and compomer: P<0.001, resin-modified GIC: P<0.05). Especially, the amount of the reduction in compressive strength of the composite resin was the highest. 4. The more the water sorption of all materials increased, the more the compressive strength decreased(P<0.05).
