  • Purpose: Hand washing has received renewed emphasis in recent years as Swine flu epidemic threaten health of public. This study aimed to describe measures used to prevent new types of influenza (NTI); describe hand washing frequency; and analyze the relationship of hand washing to locus of control, uncertainty, and state anxiety. Methods: One hundred thirty three adults participate in the questionnaire survey in which participants indicated measures they used to prevent NTI, frequency of handwashing for the prevention of NTI. They also responded to questions related to locus of control, uncertainty and state anxiety. Data were analyzed using descriptive statistics, Pearson correlation and multiple regression. Results: A majority (66.9%) of the participants indicated that handwashing was an important measure to prevent NTI. Importance of hand-washing was related the highest among the measures they used and it was significantly related to participants who had high score in internal locus of control. Other significant factor to the hand-washing behavior was state anxiety, which explained 6.9% of the frequency. Conclusion: According to the results of this study, majority of participants indicated that handwashing was one of the most important measures to prevent NTI. Nurses need to continue educating public the importance of handwashing and pay attention to personal characteristics such as internal locus of control and anxiety to promote hand-washing.

  • Purpose: In this study influenza A (H1N1) infection control performance and stress in nurses were surveyed, to identify factors influencing stress for the nurses, and to provide basic materials for promoting infectious disease control by nurses. Method: The participants were 447 nurses who had worked at isolation clinics and/or isolation rooms in hub hospitals in the Honam region during the period of the outbreak of 2009 pandemic influenza A (H1N1) virus. Data were collected in February 2010 using self-report questionnaires. Results: The performance of participants from clinics was high for 'helping patients wearing masks', but low for 'maintaining distance over 1 meter among waiting patients'. The performance of participants from hospital rooms was high for 'putting a surgical mask on a patient going out of the room', but low for 'keeping patients' family out of the room'. The participants' stress was higher in those younger and less experienced nurses, those working at a hospital with 500-999 beds, those working at both isolation clinics and rooms, and those working at a clinic longer. Conclusion: In order to lower nurses' stress from working at isolation clinics/rooms for infectious diseases, we may need to deploy experienced nurses and limit their working days to five or less.

  • Since its identification in April 2009, a swine-origin H1N1 influenza A virus (S-OIV) which is a reassortment of gene segments from both North American triple-reassortant and Eurasian swine influenza has been widely spread among humans in unexpected rapidity. To date, each gene segment of the 2009 influenza A (H1N1) outbreak viruses have shown high (99.9%) neucleotide sequence identity. As of July 6, 94,512 people have been infected in 122 countries, of whom 429 have died with an overall case-fatality rate of <0.5%. Most confirmed cases of S-OIV infection have been characterized by self-limited, uncomplicated febrile respiratory illness and 38% of cases have also included vomiting or diarrhea. Standard plus droplet precautions should be adhered to at all times. Tests on S-OIV have indicated that current new H1N1 viruses are sensitive to neuraminidase inhibitors (oseltamivir). However, current less virulent S-OIV may evolve into a pathogenic strain or acquire antiviral resistance, potentially with more severe clinical consequences. Efforts to control these outbreaks would be based on our understanding of novel S-OIV and previous influenza pandemics.

  • The clinical picture in severe cases of pandemic (H1N1) 2009 influenza is markedly different from the disease pattern seen during the epidemics of seasonal influenza as many of those affected were previously healthy young people. Current predictions estimate that during a pandemic wave, 12~30% of the population will develop clinical influenza (compared with 5~15% for seasonal influenza) with 4% of those patients requiring hospital admissions and one in five requiring critical care. Until July 6, 94,512 people have been infected in 122 countries, of whom 429 have died with an overall case-fatality rate of <0.5%. Most of the confirmed cases of S-OIV (Swine-Origin Influenza A Virus) infection have been characterized by a self-limited, uncomplicated febrile respiratory illness and 38% of the cases have also included vomiting or diarrhea. Efforts to control these outbreaks are based on our understanding of novel S-OIV (Swine-Origin Influenza A Virus) and the previous influenza pandemics. So, this review covers the experience with S-OIV (Swine-Origin Influenza A Virus) for the admission and background data and the clinical presentation, diagnosis and treatment of H1N1 in pediatric patient with S-OIV (Swine-Origin Influenza A Virus) at YUMC, 2009.

  • Influenza A viruses periodicall y cause worldwide epidemics, or pandemics, with high rates of illness and death. A pandemic can occur at any time, with the potential to cause serious illness, death and social and economic disruption throughout the world. Historic evidence suggests that pandemics occurred three to four times per century. In the last century there were three influenza pandemics. The circumstances still exist for a new influenza virus with pandemic potential to emerge an d spread. The unpredictability of the timing of the next pandemic is underlined by the occurrence of several large outbreaks of highly pathogenic avian influenza since the early 1980s. In 1999, the World Health Organization published the Influenza pandemic plan. The role of WHO and guidelines for national and regional planning. And in 2005, WHO revised the global influenza preparedness plan for new national measures before and during pandemics. This document outlines briefly the Korean Centers for Disease Control's plan for responding to an influenza pandemic. According to the new pandemic phases of WHO, we set up the 4 national levels of preparedness and made guidelines for preventing and control the epidemics in each phase. And also we described the future plans to antiviral stockpiles and pandemic vaccine development.
