Effect of the Radius of Curvature on the Contact Pressure Applied to the Endplate of the Sliding Core in an Artificial Intervertebral Disc (인공추간판 슬라이딩 코어의 곡률반경 변화가 종판의 접촉압력에 미치는 영향)

  • The treatments for spinal canal stenosis are radicular cyst removal, spine fusion, and implantation of an artificial intervertebral disc. Artificial intervertebral discs have been most widely used since the mid-2000s. The study of artificial intervertebral discs has been focused on the analysis of the axial rotation, lateral bending, the degrees of freedom of the disc, and flexion-extension of the vertebral body. The issue of fatigue failure years after the surgery has arisen as a new problem. Hence, study of artificial intervertebral discs must be focused on the fatigue failure properties and increased durability of the sliding core. A finite element model based on an in the artificial intervertebral disc (SB Charit$\acute{e}$ III) was produced, and the influence of the radius of curvature and the change in the coefficient of friction of the sliding core on the von-Mises stress and contact pressure was evaluated. Based on the results, new artificial intervertebral disc models (Models-I, -II, and -III) were proposed, and the fatigue failure behavior of the sliding core after a certain period of time was compared with the results for SB Charit$\acute{e}$ III.

Characteristic Analysis of Flexibility and Muscle Strength according to Exercise using Lumbar Strengthen Exercise Instrument (요추강화 운동기기의 훈련을 통한 유연성 및 근력 특성 분석)

  • In this paper, We investigated the characteristic analysis of flexibility and muscle strength for exercise to verify capacity in rehabilitation exercise of lumbar using lumbar strengthen exercise instrument. We have experiment in 20th years man and woman who are 20 subject with no medical history, we divided subjects into control group with no exercise and training group with lumbar strengthen exercise. We used Hi-Spine(Medicalscience.korea) also, provided exercise 40 minute a day, three days a week and progressed total four weeks. Moreover in our experiment, subjects exercised four postural position as lay down, sit, stand and stretch each ten minute. We measured trunk extension backward, trunk flexion forward, evaluation of based physical fitness and lumbar joint torque. The reults have shown that there more improved all for flexibility, based physical fitness and lumbar joint torque in training group than control group. We indicated that by rotating 3-D axis movement flatform of exercise instrument, muscle spindle in subject have been stimulated and these rotation direction and angle caused muscle tonus and contraction that makes muscle, flexibility and based physical fitness improve more. Our study can be used rehabilitation exercise program to aged people and patient with lumbar injury.

Human-Computer Interface using sEMG according to the Number of Electrodes (전극 개수에 따른 근전도 기반 휴먼-컴퓨터 인터페이스의 정확도에 대한 연구)

  • NUI (Natural User Interface) system interprets the user's natural movement or the signals from human body to the machine. sEMG (surface electromyogram) can be observed when there is any effort in muscle even without actual movement, which is impossible with camera and accelerometer based NUI system. In sEMG based movement recognition system, the minimal number of electrodes is preferred to minimize the inconvenience. We analyzed the decrease in recognition accuracy as decreasing the number of electrodes. For the four kinds of movement intention without movement, extension (up), flexion (down), abduction (right), and adduction (left), the multilayer perceptron classifier was used with the features of RMS (Root Mean Square) from sEMG. The classification accuracy was 91.9% in four channels, 87.0% in three channels, and 78.9% in two channels. To increase the accuracy in two channels of sEMG, RMSs from previous time epoch (50-200 ms) were used in addition. With the RMSs from 150 ms, the accuracy was increased from 78.9% to 83.6%. The decrease in accuracy with minimal number of electrodes could be compensated partly by utilizing more features in previous RMSs.

An Experimental Study on the Effect of Supporting Vest for Musculoskeletal Disorders in Low Back (허리 근골격계질환 예방 보조 조끼의 효과에 대한 실험적 연구)

  • With the aging population, the prevalence of musculoskeletal disorders in the elderly continues to increase. In addition, musculoskeletal disorders are the third most common social and economic loss among occupational illnesses in Workers' compensation insurance benefits as of 2015 standard and are also common among workers. In this study, a vest that can assist in the prevention and treatment of musculoskeletal disorders of the waist was produced, and it is intended to confirm the presence or absence of the effect through experiments. Diseases of the waist are closely related to core muscles. Therefore, the produced vest was made in a form that supports the core muscles. Before and after wearing the vest for effectiveness verification, the While performing 10-times of the Mackenzie exercises, a waist extension exercise, collected the EMG data of the internal oblique, Rectus abdominis, and Erector spinae, which correspond to the core muscles. The collected data were analyzed through the electromyography analysis program RM-3 to obtain RMS values for 10-times measurements of each muscle and to compare the average values. After comparing the data, to verify the significance, the technical statistical value of the measurement result was calculated and the average difference was verified through PASW ver18.0, And the paired t-test is performed on the experimental results to perform statistical processing. and Statistical processing was performed by setting the significance level to α=.05.

Debridement Arthroplasty using Posteromedial Approach in Elbow Joint with Pain and Bony Limitation of Motion (동통과 골성 운동 제한이 있는 주관절에서 후내측 도달법을 이용한 변연 절제 관절 성형술)

  • Purpose: This study evaluated the clinical outcomes of debridement arthroplasty using the posteromedial approach in elbow joints with pain and bony limitation of motion. Materials and Methods: This study involved 16 elbows in 16 patients with pain and bony limitation of motion, which were treated by debridement arthroplasty using the posteromedial approach from March 2005 to March 2008. The mean follow up period was 27.6 (13~52) months. The clinical outcomes were analyzed using the Visual Analogue Scale(VAS) for pain scale, the preoperative and postoperative range of motion and the Mayo Elbow Performance Scores(MEPS). Results: The VAS was decreased significantly from a preoperative mean of 4.5 to a postoperative mean 1.1 (p<0.001). The average arc of motion improved significantly from $61.6 (0~90)^{\circ}$ preoperatively to $109.4 (80-120)^{\circ}$ postoperatively (p<0.001). The MEPS also improved significantly from 59.4 to 85.6 postoperatively (p<0.001). There were no complications, such as hematoma and elbow instability. Conclusion: Debridement arthroplasty using the posteromedial approach is a useful surgical procedure in the elbow joint with pain and bony limitation of motion, where all compartments can be debrided, the ulnar nerve can be manipulated easily and damage to the medial collateral ligament can be minimized.

Total Elbow Arthroplasty after Failed Surgical Treatment for Elbow Fracture or Dislocation (주관절 골절 혹은 탈구의 실패한 수술적 치료 후 시행한 주관절 전 치환술의 결과)

  • Purpose: The current study reports the clinical results of total elbow arthroplasties (TEA) which were Performed on patients with poor clinical and radiological results after initial surgeries for elbow fractures or dislocations. Materials and Methods: The clinical outcomes of twelve consecutive patients who underwent TEA after failed surgeries for elbow fractures or dislocations from january, 1995 to December, 2005 were evaluated. The initial diagnoses were distal humeral fractures in 8 cases and fracture-dislocations in 4 cases. The Mean period from the initial operations to the TEAs was 12 months. The mean folloow up period after TEA was 43 months. Results: The mean range of motion, in terms of active extension, activeflexion, supination, and pronation, improved from $14.2^{\circ}$, $96.7^{\circ}$, $50.8^{\circ}$ and $53.3^{\circ}$ to $5.4^{\circ}$, $122.1^{\circ}$, $63.3^{\circ}$ and $67.5^{\circ}$, respectively (p<0.05). RAdiolucent lines were found in 3 cases, which were 1 case of type 3, and 2 cases of type 4. All three loosening cases underwent revision TEAs. The mean postoperative Mayo elvow performance score was 79 point. There were 6 cases of excellent, 2 cases of good, and 4 cases in poor. Conclusion: Good clinical results were obtained after TEA performed in failed surgeries for elbow fractures of dislocations.

Muscle Fatigue Assessment using Hilbert-Huang Transform and an Autoregressive Model during Repetitive Maximum Isokinetic Knee Extensions (슬관절의 등속성 최대 반복 신전시 Hilbert-Huang 변환과 AR 모델을 이용한 근피로 평가)

  • In the working population, muscle fatigue and musculoskeletal discomfort are common, which, in the case of insufficient recovery may lead to musculoskeletal pain. Workers suffering from musculoskeletal pains need to be rehabilitated for recovery. Isokinetic testing has been used in physical strengthening, rehabilitation and post-operative orthopedic surgery. Frequency analysis of electromyography (EMG) signals using the mean frequency (MNF) has been widely used to characterize muscle fatigue. During isokinetic contractions, EMG signals present strong nonstationarities. Hilbert-Haung transform (HHT) and autoregressive (AR) model have been known more suitable than Fourier or wavelet transform for nonstationary signals. Moreover, several analyses have been performed within each active phase during isokinetic contractions. Thus, the aims of this study were i) to determine which one was better suitable for the analysis of MNF between HHT and AR model during repetitive maximum isokinetic extensions and ii) to investigate whether the analysis could be repeated for sequential fixed epoch lengths. Seven healthy volunteers (five males and two females) performed isokinetic knee extensions at $60^{\circ}/s$ and $240^{\circ}/s$ until 50% of the maximum peak torque was reached. Surface EMG signals were recorded from the rectus femoris of the right thigh. An algorithm detecting the onset and offset of EMG signals was applied to extract each active phase of the muscle. Following the results, slopes from the least-square error linear regression of MNF values showed that muscle fatigue of all subjects occurred. The AR model is better suited than HHT for estimating MNF from nonstationary EMG signals during isokinetic knee extensions. Moreover, the linear regression can be extracted from MNF values calculated by sequential fixed epoch lengths (p> 0.0I).

Ultrasonographic Diagnosis of the Patellar Clunk Syndrome after Posterior Stabilized Total Knee Arthroplasty - A Case Report - (후방 안정형 인공 관절 전치환술 후 슬개골 덜컹 증후군의 초음파적 진단 - 증례 보고 -)

  • The patellar clunk syndrome is one of the patellofemoral complication, caused by formation of the fibrous nodule at the suprapatellar region after total knee arthroplasty. The symptom involves painful catching, crepitus and clunk during knee extension. It has been mainly but not exclusively associated with the posterior stabilized total knee system. The fibrous nodule is entrapped in the femoral intercondylar notch of the femoral component during flexion and as the knee is extended, it displaces back to the trochlear groove abruptly and the typical symptoms occur. The risk of developing this complication is primarily related to the design of the femoral component and higher incidence was noted with earlier designs of posterior stabilized knee prosthesis. Modifications have been made to the femoral component to optimize the kinematics of the patellofemoral joint and thereby reduced the incidence of patellar clunk syndrome but did not eliminate the problem completely. Clinical examination is the gold standard of diagnosis and imaging study has been used as a possible adjunct to diagnosis. Especially ultrasonography is an imaging modality, which can be easily performed to detect the fibrous nodule on the quadriceps tendon. We report a case of patellar clunk syndrome which was diagnosed with ultrasonography.

Effect of Backward Walking Exercise on ROM, VAS score and Proprioception in Anterior Cruciate Ligament Reconstruction Patients (전방십자인대 재건술 환자의 후방보행 재활운동이 관절의 가동범위 및 통증점수, 고유수용성감각에 미치는 영향)

  • This study examined the effects of 4-weeks of backward walking exercise on ROM, VAS (Visual Analogue Scale) score and proprioception in anterior cruciate reconstruction patients. Fourteen subjects were randomly assigned to either a backward walking group (BWG, n=7) or a control group (CON, n=7) and then participated in rehabilitation program for 4 weeks. The ROM of the flexion significantly decreased in both groups (BWG, p<0.001 vs CON, p<0.01), while ROM of extension significantly decreased only in the BWG (p<.05) post exercise, but no significant difference was observed between groups. Additionally, VAS score decreased significantly in both groups (P<0.001) and BWG was lower than CON post exercise (p<0.001). Finally, proprioception decreased significantly in both groups (BWG, p<0.001 vs CON, p<0.05), but did not differ significantly between groups. These results suggest that the 4 week backward walking rehabilitation program improves ROM, VAS score, and proprioception and has positive effects on functional recovery of anterior cruciate ligament reconstruction patients.

Kinematical Analysis of Side Kick Motion in Taekwon Aerobics (태권에어로빅스 옆차기동작의 운동학적 분석)

  • The purpose of this study was to analyze kinematic variables during Side kick motion in Taekwon Aerobics. The subjects of this study were the 7 skilled and 7 unskilled female college students. A QTM and an Auto Track were used to acquire raw data. The sampling rates camera was 100 Hz. The parameters were calculated and analyzed with Visual3D and SPSS 12.0. The results were as following; 1. In the elapsed time, there was no significance difference statically between a skilled and unskilled group. 2. In the cases of knee angle, there was significant difference statically at Maximum Knee Flexion2(p=0.046, F=4.925). 3. In the cases of knee angular velocity, there was significant difference statically at Maximum Knee Flexion1(p=0.031, F=5.940). 4. In the flexion/extension of hip angle, there was significant difference statically at Maximum Knee Flexion2(p=0.012, F=8.668). 5. In the abduction/adduction of hip angular velocity, there was significant difference statically at Minimum Knee Flexion (p=0.019, F=7.324). 6. In the external rotation/internal rotation of hip angular velocity, there was significant difference statically Minimum Knee Flexion(p=0.005, F=11.87).
