Comparison of the Effect in ROM Increase and Pain Reduction on Stretching During Ultrasound Treatment in Patients With Frozen Shoulder (동결견 환자의 초음파치료 시 신장여부에 따른 관절가동범위 회복과 통증감소 효과 비교)

  • A single subject experimental design (alternating treatment design) was used to compare the effects of only ultrasound and ultrasound combined with stretching of the joint capsule on the ROM increase and pain reduction in patients with frozen shoulder. Two subjects were included in each group. In the only ultrasound treatment sessions, ultrasound was applied at the pain point of the shoulder joint in supine position. In the ultrasound combined with stretching treatment sessions, ultrasound was applied at the pain point of the shoulder joint positioned in external rotation and abduction in sitting position. Only ultrasound treatment and ultrasound combined with stretching treatment were alternately performed on each patient. Pain and disability was measured by shoulder pain and disability index (SPADI), and range of motion (ROM) was measured by scratch test. The results of this study showed that ultrasound combined with stretching treatment were more effective than only ultrasound treatment in ROM increase and pain reduction. However, disability score was not significantly different.

Effect of Mitochondrial Inhibitor on Lactate Dehydrogenase of Mesocricetus auratus and Bos taurus coreanae (햄스터와 소의 젖산탈수소효소에 대한 미토콘드리아 inhibitor의 영향)

  • The lactate dehydrogenase (EC 1.1.1.27, LDH) inhibitors were isolated from the LDH-free crude mitochondrial fraction of skeletal muscle in Syrian hamster (Mesocricetus auratus) and Korean native cattle (Bos taurus coreanae). The LDH inhibitor in skeletal muscle of M. auratus was successfully isolated by the treatment with 175 mM NaCl and ultrasonic. The LDH inhibitor in skeletal muscle of B. taurus coreanae was highly stable to heat and LDH fu isozyme was largely inhibited by the LDH inhibitor. The molecular weight of inhibitor was 22 kDa. Inhibitor played an important role in the binding of LDH with the mitochondria in tissues of skeletal muscle, kidney and liver except heart.

  • Orthostatic proteinuria is documented as a benign condition and the most common cause of isolated proteinuria. The etiology and pathogenesis of orthostatic proteinuria is not clear yet. Recently there were a few report that nutcracker syndrome seemed to cause orthostatic proteinuria. We experienced a case of a twelve-yeat-old female patient with incidently discovered orthostatic proteinuria who was suspected to have nutcracker phenomenon by doppler sonography. We confirmed this patient as nutcracker syndrome by renal venography.

  • Nutcracker syndrome refers to the compression of the left renal vein between the abdominal aorta and superior mesenteric artery which can lead to renal vein hypertension. Patients can present with hematuria, proteinuria, dysuria or abdominal pain. In diagnosing the nutcracker syndrome, measurement of the peak velocity ratio of aorto-mesenteric border versus renal hilum by Doppler sonography is useful. Currently, there are few reports about the genetic correlation in this syndrome. We report two cases of the nutcracker syndrome found in male siblings with hematuria.

  • An 1-year-old intact male Jindo dog weighing 20 kg was referred with 7-day history of vomiting, anorexia, and lethargy. The dog was diagnosed with chronic kidney disease (CKD) based on clinical signs, urinalysis, serum biochemistry, radiology and abdominal ultrasonography. Ten days of conservative therapy was given, but there were no signs of improvement. The patient was euthanized and necropsy was conducted. Renal histopathology was consistent with juvenile nephropathy and this is the first report of juvenile nephropathy in a Jindo dog.

  • Perinephric abscess is an accumulation of pus in the perinephric space, the area anatomically defined between the kidney and Gerota's fascia. Gram negative organisms are the most prevalent bacterial species found in perinephric abscess. Fever, flank pain, vomiting and abdominal mass are the usual presenting complaints. But with its insidious onset, variable symptoms and rue frequency in children, perinephric abscess has been a major diagnostic problem, leading to delayed diagnosis and inappropriate treatment, which increase the rate of complication and mortality. Clinical diagnosis of perinephric abscess is difficult but must always be considered in children with a febrile septicemic illness. For appropriate treatment, early detection is very important, and either ultrasonography or computed tomography(CT) facilitates the diagnosis and establishment of treatment method. We experienced a case of left perinephric abscess treated by percutaneous drainage in a 1-year 7-month old boy. Review of literature was made briefly.

Prediction of High Grade Vesicoureteral Reflux in Infants Less than 3 Months with Urinary Tract Infection (3개월 미만 요로감염 영아에서 중증 방광 요관 역류의 예측인자)

  • Purpose : Our aim was to investigate the predictive factors for detecting grade III-V vesicoureteral reflux(VUR) in young infants less than 3 months with urinary tract infections (UTI). Methods : Data of infants who underwent ultrasonography and VCUG between January 2004 and September 2007 were reviewed. Age, gender, incidence of bacteremia, C-reactive protein(CRP) and imaging studies were compared between group I(grade III-V VUR) and group II (normal or grade I and II VUR) retrospectively. Sensitivity, specificity, positive and negative predictive values, odds ratio, and likelihood ratio of ultrasonography for high grade VUR were evaluated. Results : Among 54 enrolled infants(41 males, 13 females), 14 infants were group I and 40 infants were group II. In the group I, CRP level was significantly higher(6.11$\pm$5.18 vs. 3.27$\pm$3.45, P=0.025), and there were more ultrasonographic abnormal findings(71.4%, vs. 22.5%, P=0.002) compared with group II. However, ultrasonography was the only significant factor after adjusting with logistic regression(P=0.002). Incidence of bacteremia and abnormal DMSA findings were not significantly different in two groups. Sensitivity, specificity, and odds ratio of ultrasonography was 71.4%, 77.5%, 6.9 respectively. Negative predictive value was 88.6% and negative likelihood ratio was 0.37. Ultrasonography had significant negative likelihood ratio for grade III-V VUR, but missed 4 infants with grade III VUR. Conclusion : We could not find any alternative predictive factors to reduce VCUG in detecting high grade VUR. Therefore, VCUG must be considered in young infants less than 3 months with UTI.

Post-natal Outcome of Fetal Hydronephrosis Detected with Prenatal Ultrasonography (산전 초음파로 진단된 태아 수신증의 생후 경과 관찰)

  • Purpose : Hydronephrosis constitutes a great portion of fetal anomalies screened by prenatal sonogram. The present authors made an attempt to access its natural courses through follow up neonatal hydronephrosis diagnosed by prenatal sonogram. Methods : The study was composed of 23 neonates(36 renal units) who were diagnosed with hydronephrosis through prenatal sonogram screening and confirmed 3-7 days after birth with sonographic evaluation at Kyung Hee University Hospital. The neonates were closely monitored for 12-24 months with renal sonogram, diuretic renogram, intravenous pyelography(IVP) and voiding cystoureterography(VCUG). Results : The underlying diseases were composed of 16 cases(44%) of functional abnormalities, 14 cases(39%) of ureteropelvic junction(UPJ) obstruction, three cases(8%) of vesicoureteral reflux (VUR) and on case each of multicystic dysplastic kidney, UPJ obstruction with ureteral stenosis and ureterovesical junction(UVJ) obstruction with VUR. The degree of hydronephrosis was divided into three classes according to its severity. In 30 renal units with UPJ obstruction and functional abnormalities, 26(87%) showed mild hydronephrosis, while four(13%) were moderate. During the follow up period, six cases(20%) showed natural resolution of hydronephrosis, 15 cases(50%) showed improvement while two cases(7%) were aggravated with improvement only after surgery of the underlying disease. The cases which showed natural resolution were all mild hydronephrosis at diagnosis and the cases which underwent surgery due to continuous aggravation were mild one case and moderate one case. Conclusion : Those with cases of mild hydronephrosis show rapid natural improvement. On the other hand, in some cases, follow up monitoring reveal aggravation of the situation, emphasizing the necessity for thorough follow up for a long period of time.

Renal Anomalies in Children with Turner Syndrome (Turner 증후군 환자에서 신기형에 관한 연구)

  • Purpose : The prevalence of renal anomalies in Turner syndrome(TS) has been reported to vary from 33% to 60%. The purpose of this study was to clarify the true incidence of renal malformations in Korean TS. Methods : We evaluated 33 patients with Turner syndrome diagnosed by karyotype in Inje University Busan Paik hospital and Youngnam University from January 1995. Intravenous pyelography(IVP) was performed on all patients; abdominal ultrasonography and 99mTc-DMSA renal scan were performed on some. Cytogenetic analysis was performed on all patients with peripheral blood lymphocytes. Results : Of the total 33 patients, the karyotype showed 45, X in 18(54.5%) patients, mosaicism in 11(33.3%) patients and structural aberration in 4(12.2%) patients. The overall incidence of renal anomalies was 36.4%. The renal anomalies included four cases of horeshoe kidney, six cases of abnormal renal collecting system one case of single kidney and one case of malrotation. The incidence of renal anomalies in 45, X karotype(44.4%) showed a higher rate than that of mosaicism and structural aberration(26.7%), but there is no statistical significance. Conclusion : The incidence of renal anomalies in Korean TS reveals 36.4%. This rate is similar to other foreign TS studies. We recommend that renal ultrasonography or IVP for investigation of renal anomalies should be done as a screening procedure for the better quality of life in patients with TS.

Flow Velocity of Left Renal Vein in Children with Asymptomatic Hematuria or Proteinuria (무증상 혈뇨 또는 단백뇨 소아의 좌신정맥 혈류 속도)

  • Purpose : Nutcracker syndrome must be considered when hematuria or proteinuria occurs in a healthy child. The purpose of this study is to investigate the prevalence of nutcracker syndrome among children with asymptomatic hematuria or proteinuria, and to obtain the ratios of the peak velocity of the left renal vein between the aortomesenteric portion and the hilar portion in children with asymptomatic hematuna or protelnuria in which nutcracker syndrome was excluded and to observe whether the ratios are affected by sex, age or urinalysis findings. Methods : Using Doppler ultrasonography, we measured the flow velocity and obtained the peak velocity ratios of the left renal vein at the aortornesenteric portion and at the hilar Portion of the left kidney in children with asymptomatic hematuria or proteinuria who visited the Division of Pediatric Nephrology, Severance Hospital from May 2001 to March 2004. Results : Of 304 children with asymptomatic hematuna or proteinuria, 107 children(35.2%) were diagnosed with nutcracker syndrome. For 197 children with asymptomatic hematuria or proteinuria excluding nutcracker syndrome, the mean ratio of the peak velocity was 2.54 $\pm$0.73, which was not affected by sex, age or urinalysis findings. Conclusion : Nutcracker syndrome was the major cause of asymptomatic hematuria or proteinurla In children, comprising 35% of all cases. Doppler ultrasonography was helpful in the screening of nutcracker syndrome and prevention of its complications. For children with asymptomatic hematuria or proteinuria excluding nutcracker syndrome, the peak velocity ratio of the left renal vein did not differ from that of normal children and was not affected by sex, age or urinalysis findings.
