Validation of Onen's Alternative Grading System for Congenital Hydronephrosis (선천성 수신증을 위한 Onen 등급 분류법의 임상적인 타당성)

  • Purpose: The purpose of this study was to compare the validity of Onen's alternative grading system (AGS) with that of the APDRP and SFU grading systems in patients with isolated and complicated congenital hydronephrosis. Methods: We retrospectively reviewed the medical records of 153 patients (204 renal units) diagnosed with congenital hydronephrosis between January 2002 and December 2011. We classified patients into 2 groups; isolated or complicated hydronephrosis. All renal units were graded according to anterior-posterior diameter of renal pelvis (APDRP), Society for Fetus Urology (SFU) and Onen's grading systems. We analyzed the prognosis of hydronephrosis, according to each grading system, at 2 years of age. Results: There were 152 renal units with isolated hydronephrosis and 52 renal units with complicated hydronephrosis. The isolated hydronephrosis group had a lower grade according to Onen's AGS, and showed more frequent spontaneous remission by 2 years of age. There was more frequent obstruction (P =0.000) and surgical treatment (P =0.000) of units with high-grade hydronephrosis according to Onen's AGS. In the complicated hydronephrosis group the frequencies of spontaneous remission (P =0.015) and renal dysfunction (P =0.013) were significantly higher than those in high-grade hydronephrosis, as indicated by Onen's AGS. There were no significant differences in clinical outcomes among the highest grade groups, according to the 3 systems, in either isolated or complicated hydronephrosis. Conclusion: Onen's AGS reflects the prognosis of hydronephrosis as well as other grading systems in those with isolated hydronephrosis. It was better predictor of renal dysfunction in those with complicated hydronephrosis. However, Onen's AGS was not superior to the other grading systems in terms of predicting prognosis, especially in high-grade hydronephrosis.

Natural History of Unilateral Ureteropelvic Junction Obstruction : Five Cases of High Grade Hydronephrosis (신생아 편측 신우요관이행부 폐색의 자연경과 : 중증 수신증 5례)

  • Purpose: Unilateral ureteropelvic junction obstruction with high grade of hydronephrosis an obstructed diuretic renogram but good differential renal function were studied to evaluate the natural history of untreated obstruction. Materials and Methods: During the last three years, we encountered 18 kidneys with hydronephrosis detected on prenatal ultrasound. In all these patients other urologic abnormalities were excluded by voiding cystourethrography and other radiological studies. Diuretic renography was initially performed at an age ranging from 2 weeks to 4 weeks. Five had high degree of hydronephrosis and obstructed diuretic renogram despite good relative renal function. Periodic urine examination, ultrasonography and diuretic renogram were done at regular intervals. Results: We have followed 5 neonates with unilateral hydronephrosis and suspected ureteropelvic junction obstruction with noroperative treatment for 15 months. During followup, percentage and relative renal function were maintained in all patients and hydronephrosis improved. Obstructive patterns on diuretic renogram were variable. Conclusions: These findings help to define the natural history of untreated good functioning hydronephrotic kidneys. Many newborn kidneys with severe hydronephrosis are not obstructed despite even high grade of hydronephrosis and obstructed diuretic renogram.

Effects of Cooling on Repeated Muscle Contractions and Tendon Structures in Human (냉각이 반복된 근수축과 사람의 건 구조에 미치는 영향)

  • Purpose: This study compared the effects of non-cold and cold conditions on the viscoelastic properties of tendon structures in vivo. Methods: Seven male subjects perfomed plantar flesion exercise with maximal isokinetic voluntary contraction, which consisted of muscle contraction for 6 see and relaxation for 60 secs, 10 times for 1 set, Totally 10 sets were repeated. Before and after each task, the elongation of the tendon and aponeurosis of the medial gastrocnemius muscle (MG) was directly measured by ultrasonography. (The relationship between the estimated tendon force and tendon elongation.) Tendon cross-sectional area and ankle joint moment arm were obtained from magnetic resonance imaging (MRI). The tendon force was calculated from the joint moments and the tendon moment arm and stress was obtained by dividing force by cross-sectional areas (CSA). The strain was measured from the displacements normalized to tendon length. Results: After cooling, the tendon force was larger in cold than non-cold. The value of the tendon stiffness of MVC were significantly higher under the cold condition than under the non-cold condition. The maximal strain and stress of $7.4{\pm}0.7%$ and $36.4{\pm}1.8$ MPa in non-cold and $7.8{\pm}8.5%,\;31.8{\pm}1.1$ MPa in cold (P<0.05). Conclusion: This study shows for the first time that the muscle endurance in cooling increases the stiffness and Young's modulus of human tendons. The improvement in muscle endurance with cooling was directly related to muscle and tendon.

  • A 14-year-old, castrated male mixed-breed dog weighing 9 kg was presented for hematuria and dysuria. Abdominal ultrasound showed unilateral hydronephrosis and hydroureter with focal thickening of the ureteral wall. Surgical exploration revealed an intraluminal mass arising from the proximal left ureter. Mass resection was performed. Histopathology of the ureteral mass was consistent with a papillary transitional cell carcinoma. The patient recovered well post-operatively, but was diagnosed with another tumor three months later, this time in the right kidney. Fine-needle aspiration cytology of the renal mass revealed an epithelial cell tumor with mesenchymal features.

Successful Renal Autotransplantation for the Treatment of Severe Renovascular Hypertension in a 14-year-old Boy (신 자가이식을 통한 14세 소년에서 발생한 심한 신성 고혈압의 성공적인 치료)

  • Percutaneous transluminal renal angioplasty (PTRA) is the current treatment of choice for renal artery revascularization, but renal autotransplantation has been an alternative treatment for complex cases. Here we report a 14-year-old boy with severe hypertension successfully treated with PTRA and renal autotransplantation. Doppler ultrasonography and computed tomography (CT) angiography revealed slight narrowing in the right renal artery ostium and complete obstruction in the left renal artery ostium with multiple collaterals. PTRA with stent insertion was performed for the treatment of the right renal artery, but it was impossible for the left renal artery due to the total obstruction. Therefore, left nephrectomy for autotransplantation was done with the peritoneal approach and the left kidney was autotransplanted to the ipsilateral iliac fossa. Postoperatively, Doppler ultrasonography and mercapto-acetyl-triglycine (MAG-3) renogram were performed, which showed normal renal artery blood flow and kidney function. Blood pressure was normalized and anti-hypertensive drugs were gradually tapered. Fibromuscular dysplasia was suspected to be responsible for the renal artery stenosis based on clinical aspects. In conclusion, renal autotransplantation is also a good treatment option for children with severe renovascular hypertension when endovascular treatment has failed or is not possible.

The Neonatal Follow up and Correlative Analysis of Fetal Hydronephrosis (산전 수신증의 생후 추적 관찰)

  • Purpose: To determine the various prenatal factors related to the prenatal hydronephrosis diagnosed on prenatal ultrasonography. We also attempted to correlate the prenatal and neonatal renal pelvic anteroposterior diameter with the outcome in infancy Methods: Between 1985 and 1997. We retrospectively reviewed 105 renal unit (75 patient) with fetal hydronphrosis persisting postnatally. Investigation consisted of renal ultrasonography, voiding cystourethrography, diurectic renogram, and DMSA scan. Results: The 75 patient assesed had the following underlying cause: UPJ(Ureteropelvic juction) obstruction(52%). multicystic dysplastic kidney(10%). UVJ (Ureterovesicai juction) obstruction (10%) and no underlying cause (25%). Of theses cases 36 cases (40 renal unit) underwent operation, while 28 cases (50 renal unit) resoled spontaneously. 12% of mild hydornephrosis deteriorated. whereas 50% of modrate hydrophrosis and 81% of severe hydronephrosis required surgical correction. Attempting to find the renal unit that were at risk for deterioration. our study showed that urinary tract infection group and calyceal blunting group had a predictive role. Conclusion: It is necessary to follow up after birth dilatation of caylx or urinary tract infection are present. Early operation is considered when prenatal pelvic AP diameter greater is than 22 mm and postnatal diameter greater than 17 mm. This may make it possible to prevent further progression of renal damage and prompt treatment of asymptomatic hydronephrosis before complications occur.

The Analysis of Ultrasonography on Hamstring after Static and PNF Stretching Protocol (정적 신장과 고유수용성 신경근 촉진(PNF)신장기법 적용 후 슬괵근에 대한 진단 초음파를 통한 분석)

  • Objective : The purpose of this study was to analyze ultrasonography and 90-90 SLR test on hamstring after both groups were applied static stretching exercise and PNF stretching exercise. Methods : For this experimental study, total 20 healthy university students, who were divided into static stretching exercise group and PNF stretching group. Each group was assessed before and after stretching(0.5 minute, 1 minute, 1.5 minutes, 2 minutes, 2.5 minutes, 3 minutes, 4 minutes, 5 minutes, 6 minutes, 7 minutes, 8 minutes, 9 minutes, 10 minutes). Results : Total score of 90-90 SLR test was showed a significant difference between pre and post. Hamstring diameter was gradually increased to 2.5 minutes and 3 minutes, where after it was gradually decreased in static stretching exercise group. Hamstring circumference had regular interval scores before and after on PNF stretching exercise group. Conclusion : There was a different change on hamstring diameter and circumference.

  • Ureteropelvic junction obstruction and duplicated collecting system are common urologic anomalies in upper urinary system, but they seldom occur in combination. These obstruction can occur in both partial or complete duplicated collecting system. Complete duplication of the ureter may be asymptomatic or recognized when complications develop as a result of reflux into the lower pole ureter or obstruction of the upper pole with an ectopic ureterocele. It is difficult to choose an optimal therapy due to the high variability in function, degree of obsturction, damage and potential fir regeneration in growing kidney. We report our experience of successful surgical interventions in the ureteropelvic junction obstruction of the lower pole with complete duplicated collecting system.

  • By using ultrasonic waves, we obtained conclusion from the experiment for measuring joint axial force of high tension bolt. The conclusion is followed. : From the high tension bolts used at turbine of Thermoelectric Power Plant, we obtained the equation of calculating joint axial force that is ${\sigma}=\frac{{\Delta}BPD}{j{\times}{\Sigma}{\delta}}$. By using IBM PC, which is inputed by the equation for calculating joint axial force of high tension bolts, we got joint axial force of high tension bolts form beam path of ultrasonic waves. Furrther, we can identify that constitution of plant safety inspection system is possible.

  • The authors carried out an experiment to investigate the echo fluctuations from ocean bottom due to ship's motion. The bottom echoes was continuously measured, by using a 50 kHz Echo sounder on board of the ship being at anchor under the sea condition of 15 knots in wind velocity and approximately 2 meters in wave height, to extract the information about the pulse stretching and the ship's motion from the first return and the second return. A data acquisition system was used to record digitally the envelope of the echoes, and the analysis was applied to the echo data collected from the continental shelf in the South China Sea. The results obtained can be summarized as follows: 1. The equivalent pulse width of the second return echoes from ocean bottom was 2.4 times longer than that of the first return echoes. 2. The echo peak values of the first return fluctuated markedly than that of the second return and was shown to be extremely sensitive to small change in ship's motion. 3. Energy target strength and peak target strength of the sandy-mud bottom were -13.4 dB and -14.6 dB, respectively.
