대상포진 후 신경통은 통증치료실에서 당면하는 가장 치료하기 어려운 질환중의 하나로서 현재까지 교감신경 차단, 경피적 전기자극, 약물요법 등 여러 방법이 소개되어 있으나 어느것이나 만족스럽지 못하다. 저자는 상기의 모든 방법을 동원하여 통증의 제거에 실패한 60세의 여자환자에 지주막하로 45 ml의 냉식염수를 주입하여 폐부종, cauda equina syndrome 등을 포함한 심한 합병증을 경험하였기에 문헌과 함께 고찰해 보고자 한다.

Objective : This study aims to explain the effect of Buja-tang on Postherpetic Neuralgia with Chronic Fatigue Syndrome which a Soeum-byeong patient has. Methods : This is a case report on a female patient who has been suffering from Postherpetic Neuralgia with Chronic Fatigue Syndrome. According to Diagnostic system based on Shanghanlun provisions, Buja-tang was provided for one month. Results : After a series of Buja-tang treatments, the patient's symptoms and result improved. Conclusions : This case study show an effectiveness of using Buja-tang according to Diagnostic system based on Shanghanlun provisions on Postherpetic Neuralgia with Chronic Fatigue Syndrome.

Objectives : The purpose of this study is to report the effect of Traditional Korean Medical treatment on Postherpetic Neuralgia. Methods : Five patients suffering from postherpetic neuralgia were treated with Warm Needling, Bee venom Acupuncture, Cupping Therapy, Sipjeondaebo-tang. To evaluate the results of this treatment, we used Visual Analogue Scale(VAS), Pain Rating Scale(PRS), Quality of Sleep(QSP). Results : After treatment, VAS, PRS, QSP and clinical symptoms of 5 patients were improved remarkably. Conclusions : According to the results, These findings suggest that Traditional Korean Medical treatment can be effective for the treatment of postherpetic neuralgia.

Trigeminal neuralgia is nerve systematic disease appearing in the distribution scope of trigeminal nerve. It's characterized by extreme pain accompanying with a repeated and simultaneous fit from several seconds to 1-2 minutes. The oriental medical name of trigeminal neuralgia is generally Dootong, Doopoong, Myuntong, Pyundootong, Pyundoopoong, and Myuntong is the nearest in Occidental medicine. So I study for general prescription of Trigeminal neuralgia and point out Galgunhaegui-tang(葛根解肌湯). Trigeminal neuralgia patients were given Galgunhaegui-tang. After taking Galgunhaegui-tang, the patients's Trigeminal neuralgia was improved.

아세트아미노펜(파라세타몰)은 p-aminophenol 유도체로서 (그림 1) 두통, 치통, 신경통 등의 통증에 널리 사용되는 해열진통제인데 아스피린과 같은 정도의 해열 진통 효과를 나타내며, 이것은 중추신경계의 체온조절 중추에 작용하여 피부혈관을 확장함으로써 열의 확산을 증가시키는 해열작용과 시상 및 대뇌피질에의 통각역치를 높여 진통작용을 하는 것으로 추정 된다. 아세트아미노펜은 백색의 결정 또는 결성성 가루로 물에 조금 녹고 메탄올 또는 에탄올에 잘 녹으며 수산화나트륨 시액에 녹고 에텔에는 매우 녹기 어렵다 (표1). 대한약전에서는 정제가, 미국약전에는 캅셀제, 좌제, 경구현탁액제, 발포성 건조시럽, 정제 등이 수재되어 있고, 세계 각국에서 OTC 제품으로 1정당 160mg의 츄잉정까지 판매되고 있다. 그러나, 시판되고 있는 정제등은 붕해되어 용출되는데 오랜시간이 소요되어 대한약전에는 약 30분간에 80%이상의 용출기준이 설정되어 있으며, 독특한 쓴맛 때문에 microencapsulation 한 제피세립을 사용하고 있으나 역시 1 정당 300mg 이상의 확산정이나 속용정은 존재하지 않는다. 이것을 개선하기 위하여 붕해속도가 빠르고 특히 진통효과가 빠르며 물없이 구강내에서 간편히 녹여 복용하거나 또는 씹어서 또는 물과 함께 복용할 수 도 있는 $\ulcorner$알카펜$\lrcorner$ 속용정을 개발하게 되었다 (그림2).

Ossification of the ligamentum flavum(OLF) is not infrequent in the cervical and lumbar regions but is very rare in the thoracic spine. We reported a 62-year-old women with left thoracic intercostal neuralgia due to an OLF. Physical and neurological examination were normal. MRI showed an OLF with compression of the thecal sac in the posterolateral aspect of the T9-T10 level. Laminectomy and removal of the ligament resulted in marked clinical improvement. OLF is known to cause thoracic radiculomyelopathy, but presentation with intercostal neuralgia only is very rare.

Trigeminal neuralgia is a unique disease entity to be dealt with at pain clinic due to it's severe pain attack. A 33-year old male patient had severe pain on left cheek area usually initiating from first upper molar tooth area for three years. We successfully treated this patient with maxillary nerve block using pure alcohol by a lateral approach. Three months after maxillary block the patient is still pain free.

A 67 year old male with postherpetic neuralgia complained of right anterior chest throbbing pain and also allodynia. We performed thoracic epidural block with 0.25% bupivacaine and 1% lidocaine. Oral carbamazepine and imipramine were also administred concomitantly. One month later, patient complained of fatigue, anorexia, pruritis, jaundice, and dark urine. Liver function test values were significantly elevated then. Therefore we removed the epidural catheter and ceased all medication. Liver function test values decreased rapidly after discontinuation of medication. Hepatitis might have developed as result of drugs administered and could have been prevented if the patient had been monitored with regular clinical laboratory follow up. The purpose of this case report is emphasize the importance of early regular laboratory test to detect any possible side effect that may occur by administration of drugs during treatment of chronic pain.

Anlgesic agents against visceral pain typically rely on a noxious chemical irritation of the peritoneum, e. g., acetic acid and phenylquinone writhing test. While useful, this type of assay depends upon an acute inflammation and the release of local alogens. Further, ethical and scientific constraints prevent repeated assessments in a single animal, thereby compounding the difficulty of assessing tolerance development to analgesic agents. To overcome these constraints, Colburn et al. developed a model for mechanical visceral pain model (VPM) based on a repeatable and reversible duodenal distention in the rat. A chronic indwelling intraduodenal balloon catheter is well tolerated and upon inflation produces a writhing response graded in proportion to distention. This response is inhibited by morphine in a dose dependent manner. We found that a model for visceral pain was thought to be a great value.

We experienced two cases of malignancy diagnosed during the treatment of postherpetic neuralgia. One case was a lung cancer and the other case was a pancreatic cancer. Generally, herpes zoster frequently occurred in the patients who have immunosupressive disease, diabetes mellitus, malignancy and tuberculosis, etc. It is necessary to rule out malignancy in the patients who have herpes zoster, especially those patients whth severe eruptive cluster, persisting to the therapy or multifocal pain. So, we must carefully observe and follow up the patients to find out malignancy as well as to diminish the pain.
