Radiation Treatment for Malignant Small Cell Tumor of the Thoracopulmonary Region (Primitive Pluripotent Histogenesis and Differential Diagnosis - A Case Report and Review of Literatures -) (흉폐부에서 발생한 악성소세포 종양의 방사선치료)

  • Malignant small round cell tumor (SRCT) of the thoracopulmonary region appears to originate in the soft tissues of the chest wall or the peripheral lung. A differential diagnosis of poorly differentiated small round cell tumors which include Ewing's sarcoma of bone and soft tissue, embryonal rhabdomyosarcoma, Askin tumor, neuroblastoma, peripheral neuroectodermal tumor, small cell osteogenic sarcoma and Iymphoma are often difficult by light microscopy alone. In recent, by the extensive studies electron microscopic examination, histochemical study, immune-chemical study, cytogenetics and gene analysis, these tumors may be derived from the primitive and pluripotential cells, differentiating into mesenchymal, epithelial and neural features in variable proportions. Treatment for SRCT of thoracopulmonary region is not determined because of massive involvement of the lung, pleura or soft tissues of the chest wall resulted in a dismal outcome despite aggressive surgery, irradiation and chemotherapy.

Primary Neuroblastoma Uptake in $^{99m}Technetium$ Methylene Diphosphonate Bone Scan (2 Cases) ($^{99m}Technetium$ Methylene Diphosphonate Bone Scan에서 원발병소가 조영된 신경아세포종 2예의 치험)

  • Although the exact mechanism is not clearly understood yet, there were examples of the uptake of radiopharmaceuticals to the soft tissue lesions with $^{99m}Technetium$ methylene diphosphonate bone scan. Recently, we experienced two cases of neuroblastoma of which primary sites were imaged with $^{99m}Tc-MDP$ bone scan preoperatively and could make the diagnosis. So, we report here that $^{99m}Tc-MDP$ bone scan is the reasonable method not only to find out the bone metastasis, but also to diagnose the neuroblastoma preoperatively.

  • 대기오염물질이면서 동시에 생체내 화학반응의 산물이기도 한 nitric oxide(NO)는 그 생체내 분포가 광범위하고 생리적 역할이 다양하여, 최근의 생명과학 분야에서 가장 크게 주목받는 몇가지 연구대상 중 하나이다. 세포에서의 NO 산생은 nitric oxide synthase (NOS)에 의해 촉매되는데, 이들은 brain form (bNOS, neuronal; nNOS, NOS I), inducible form (iNOS), 및 endothelial form(eNOS)로 구분되는데, 이중 bNOS(nNOS)와 eNOS는 inducible form에 대비되는 constitutive form(cNOS)에 해당하므로 각각 ncNOS 와 ecNOS로도 불리운다. NOS는 아미노산인 L-arginine을 산소와 결합시켜 L-citrulline으로 변환시키면서 NO를 유리하고, 이 NO는 세포내의 guanylate cyclase를 활성화하여 cyclic GMP를 생산하거나, superoxide(O2-) 및 수소이온과 차례로 결합하여 반응성이 매우 높은 수산화기(-OH)를 발생시켜 세포독작용을 유발하기도 한다. 정상상태에서 뇌혈관내피세포의 ecNOS로 부터 유리된 NO는 혈관을 확장시켜 신경세포에 대한 산소공급을 원활히 유지해 주지만, 순환장애를 일으켰을 때는 뇌조직내의 iNOS로부터 대량의 NO가 유출되어 신경세포의 손상을 가져온다. 호흡기에서는 NO가 기도평활근을 이완시키고 폐혈류를 개선하므로, 미숙아나 성인의 호흡장애시에 소량의 NO를 흡입시키면 oxygenation을 호전시킬 수 있다. 그러나 대기오염이나 흡연 등으로 대량의 NO를 흡입할 경우 치명적인 폐부종이나 methemoglobin혈종을 일으킬 수 있다. 순환계에서는 cNOS가 혈관을 확장시켜 조직의 혈류를 유지하는데 일익을 담당한다. 세균내 독소(lipopolysaccharide; LPS)나 각종 명역조절물질들이 혈관내피세포와 혈관평활근세포로 부터 과다한 NO를 유리시키면 혈압이 급격히 떨어져 순환허탈상태에 빠지게 된다. 심장에서는 관상혈관 내피세포의 eNOS가 심근의 혈류를 유지해 주지만 허혈이나 세균내독소 또는 면역조절물질 등에 의하여 심근세포나 침윤된 대식세포의 iNOS로 부터 과량의 NO가 유리되면 심근세포의 손상이 초래된다. 신장에서는 내피세포의 cNOS에 의하여 사구체여과가 조절되고 있는데, 세균내독소나 면역 조절물질 등에 의하여 사구체관막세포(mesangial cell)등의 iNOS로 부터 과량의 NO가 유리되면 신조직과 사구체의 손상을 초래한다. 위와 같이 대부분의 장기에서 ecNOS는 조직의 혈류를 유지하는 역할을 하며, iNOS는 애초 세균 등 침입자에 대한 세포독작용이 그 존재 목적이라고 풀이할 수 있겠으나 일종의 부작용으로 자체조직의 손상을 초래하게 되는 것으로 본다. 따라서 NO와 관련된 각종 병변의 치료를 위해서는 NOS의 비선택성 억제제인 arginine 유도체 보다는 iNOS에 대한 선택적 억제제인 S-methylisothiourea(SMT), aminoethylisothiourea(AETU), aminoguanidine (AMG), agmatine, L-canavanine, transforming growth factor b1(TGF-b1) 등의 사용을 검토해 보는 것이 타당할 것으로 사료된다.

  • 최근 경제수준 향상과 소자녀 가치관의 확립, 그리고 전국민 의료보험 실시 등으로 인하여 모자보건 대상자의 대부분은 민간 의료시설의 전문인력으로부터 서비스를 제공받게 되었고, 모자보건 수준도 급격히 향상, 1992년 시설분만율의 경우, 99%에 도달하였다. 이렇듯 의료시설 이용의 증가와 의료기술의 발전에도 불구하고, 영아사망율 및 모성사망율이 최근 몇년동안 같은 수준에 머무르고 있음은, 보다 질적인 관리측면으로 사업의 방향이 전환되어야 함을 의미하는데 이는 곧 공공성을 띠고 있는 모자보건사업을 국가가 관리하여야 할 필요성을 더욱 크게 한다. 공공부문에서는 취약대상을 위하여 민간 전문인력과의 유기적인 연계체계를 마련하여 계속적인 관리를 제공할 수 있도록 하고, 보건교육 강화를 위한 관련 홍보물(모자보건수첩 활용, 모유수유 권장, 제왕절개수술 지양 등)을 제작하며 신경아세포종 검사 등과 같은 새로운 예방사업 개발에 중점을 두어야 할 것이다. 또한 영유아관리는 저체중아 및 장애아에 대한 추구관리서비스까지 확대되어야 할 것이다. 현 우리나라 주산기구급이송체계는 응급의료체계내에서 이루어지고 있다고 볼 수 있는데 주산기관리를 위한 의료여건이 성숙되어 있지 못하고 있는데다(이 시기의 집중관리를 통하여 사망 및 장애아 예방이 가능) 관련 제도마저 취약하여 민간의료부문에서는 영아사망 및 모성사망을 낮추기 위해서는 이 부문에 대한 노력이 집중되어야 할 것이다. 첫째, 주산기학, 신생아학 전문인력의 훈련제도 확립파 주산기 관리시설의 지역적 적정분배(분만 2,000건에 1개 시설마련), 둘째, 집중적인 인력과 고가장비가 투입되는 주산기 의료활동 강화를 위한 관련 의료제도의 수정 및 보완, 세째, 질적관리가 매우 중시되는 고위험 신생아의 집중관리를 위한 '표준 의료관리지침서' 마련, 네째, 동 시설 및 관리에 준하여 주산기 의료시설에 대한 감독 및 감시기능 강화를 위한 제도적 장치가 마련되어야 할 것이다.

Multisystemic Aspergillosis with Granulomas in Layer Chickens (산란계에서의 전신 다발성 육아종성 Aspergillosis 발생)

  • A case of aspergillosis in 39-day-old layer chickens having a history of gradual emaciation and subsequently death with nervous signs such as torticollis and lack of equilibrium was documented. Based on the results from serology and polymerase chain reaction (PCR) test, this flock was not affected with known viral or bacterial diseases. On postmortem examination of the affected birds, multiple white to yellow nodules measuring 1~5 mm in diameter were observed in the lungs, cerebrum, liver and kidney. Microscopically, these nodules were identified as granulomatous lesions characterized by mixed population of multinucleated giant cells and lymphocytes. By periodic acid-schiff staining and nucleotide sequencing analysis, Aspergillus flavus with characteristic septate and branched hyphae were identified in the granuloma of lung and cerebrum. This case was a chronic and multisystemic aspergillosis specialized to central nervous system caused by Aspergillus flavus infection in the layer flocks.

An Intradural Extramedullary Hemangioblastoma of Distal Spinal Cord - A Case Report - (척수 원위부에 발생한 경막내수막외 혈관아세포종 - 증례보고 -)

  • It has been known that hemangioblastoma of spinal cord occurs about 4% of all the spinal tumors. The authors present a rare case of intradural extramedullary(IDEM) hemangioblastoma of distal spinal cord in 41-year-old male patient. This IDEM mass at the level of conus medullaris showed iso-signal intensities on T1-weighted image(T1-WI) and high-signal intensities on T2-WI, and was enhanced homogeneously on MRI. At surgery, T12- L1 total laminectomy and enbloc mass removal were performed. This IDEM hemangioblastoma was confirmed by histopathologic findings. For less postoperative complication in IDEM tumors, we suggest that precise preoperative evaluation and complete excision via proper surgical approach to distal conus should be reemphasized.

Anti-neuronal Injury Effect of Evodiae Fructus Water Extract in Sodium Cyanide-induced SK-N-SH Cell Lines (Sodium Cyanide로 유도된 신경아세포종 세포주에서 오수유의 신경상해 보호효과)

  • Objectives : This study investigated effect of Evodiae fructus water extract (EVOR) on apoptotic cell death induced by NaCN in SK-N-SH neuroblastoma cell lines. NaCN stimulates glutamate release which can activate glutamate receptors to initiate excitotoxic processes. This study examines the role of EVOR in mediating NaCN-induced cytotoxicity. Methods & Results : Cytotoxicity was assessed by measuring lactate dehydrogenase (LDH) in the culture media. NaCN(0.1mM) produced cytotoxicity following 12hrs of incubation. NaCN-induced cytotoxicity was partially blocked by EVOR. The treatment of EVOR in simultaneous exposure of cultures to NaCN provided complete protection against cytotoxicity. NaCN-induced cytotoxicity was found to inhibit DNA fragmentation, repaired by cell cycle and simultaneous exposure to NaCN, regenerated with neurite outgrowh by EVOR. These results indicate thaf damage by NaCN in neumnal cell cultures was repaired by EVOR, whereas NaCN-induced cytotoxicity is blocked Primarily by activation of anti-apoptosis. Conclusions : These results suggest that EVOR may be beneficial for the treatment of dementia and other degenerative problems of the central nervous system.

The Effect of Bee Venom & melittin Herbal acupuncture solution to Apoptosis in NG pretreated neuroblastoma cell line (Bee Venom 및 Melittin 약침액(藥鍼液)이 신경마비물질(神經麻痺物質)을 처치(處置)한 신경아세포종(神經芽細胞腫)의 세포사(細胞死)에 미치는 영향(影響))

  • Objective : This study is designed to investigate the effects of bee venom and melittin on cell death in neuroblastoma cell line after pretreatment with NG(nerve growth inhibitory substance) Methods : It was evaluated by using MTT assay, morphological method, DNA fragmenation, flow cytometry, immunocytochemistry analysis, RT-PCR and Western blot. Results : The MTT assay demonstrated that neuroblastoma cell viability was significantly inhibited dose-dependently by treatment with bee venom and melittin after pretreatment with NG in comparison awith control. The morphological study and fow cytometry demonstrated that neuroblastoma cell showed apoptosis. DNA fragmenation showed DNA ladder below 1 Kbp. Immunocytochemistry assay demonstrated that Fos and MAPK were down-regulated. RT-PCR analysis demonstrated that Fos and MAPK was down-regulated. Western blot demonstrated that Fos and MAPK were down-regulated from $1{\mu}g/ml$ bee venom in neuroblastoma cell pretreated with NG. Conclusion : These result suggests that bee venom and melittin after NG treatment have significant anti-cancer effect and further study is needed in vivo.

Bilateral Adrenal Neuroblastoma (양측성 부신 신경아세포종)

  • Neuroblastoma is the most common solid malignancy in childhood. However, a neuroblastoma presenting as bilateral and multifocal is very rare. A male newborn weighing 2.7 kg was born by normal vaginal delivery at 39 weeks' gestation to a 27-year-old mother who had a normal pregnancy. He was in good condition at birth but presented palpable masses in the both upper abdomen and both side of the neck. Ultrasound examination showed the lesions to be $3{\times}3cm$ sized calcified mass in right suprarenal area and $5{\times}3cm$ sized homogenous mass in left suprarenal area. The abdominal mass was also examined by computed tomography with similar findings. With the impression of bilateral neuroblastoma or metastatic spread, the laparotomy was performed on the 13th day of life. Frozen section of biopsy of the left neck mass was obtained, which showed neuroblastoma. Intraoperative findings revealed bilateral adrenal masses which were distinct and anatomically separate. Bilateral adrenalectomy was accomplished and the initial postoperative course was uneventful. The patient was discharged for hopeless and expired at age 45 days. We present bilateral adrenal neuroblastoma considered to be simultaneous occurrence rather than metastases from one site to another.

  • Alzheimer's disease(AD) is a geriatric dementia that is widespread in old age. In the near future AD will be the biggest problem in public health service. Although a variety of oriental prescriptions in study Jeonmaedan have been traditionally utilized for the treatment of AD, their pharmacological effects and action mechanisms have not yet fully elucidated. It has been widely believed that A${\beta}$ peptide devided from APP causes apoptotic neurotoxicity in AD brain. However, recent evidence suggests that CTl05(carboxy terminal 105 amino acid peptide fragment of APP) may be an important factor causing neurotoxicity in AD. In addition, AD is one of brain degeneration disease. So we studied on herbal medicine that have a relation of brain degeneration. In Oriental Medicine, Jeonmaedan has been used for disease in relation to brain degeneration. As the result of this study, in Jeonmaedan the apoptosis in the nervous system is inhibited, the repair against the degerneration of SK-N-SH cell lines by CT105 expression is promoted. So Jeonmaedan may be beneficial for the treatment of AD.
