렘수면 행동장애 유사 수면장애에 대한 고찰 (A Review on Sleep Disorders Similar to REM Sleep Behavior Disorder)

  • 렘수면 행동장애는 렘수면에서 과도한 근전도 활동과 관련된 불쾌한 꿈과 격렬한 행동이 특징인 사건수면이다. 이는 특발성으로 나타날 수 있고, 다른 신경학적 또는 의학적 상태에 이차적으로 나타날 수 있는 등 여러 이질적인 기저 질환이 있을 수 있다. 특발성으로 나타나는 렘수면 행동장애는 대부분 시누클레인 병증으로 인한 신경퇴행성 질환이 나타날 가능성을 의미하므로, 정확한 진단이 예후를 예측하는데 중요하다. 렘수면 행동장애의 진단을 위해서는 수면다원검사에서 나타나는 무긴장증이 없는 렘수면이 필수적이다. 노인 환자에서 수면 중 꿈을 꾸면서 행동을 보이는 질환으로는 폐쇄성 수면 무호흡증, 외상 관련 수면 장애, 수면 중 격렬한 주기적인 다리 운동 등이 알려져 있다. 렘수면 행동장애를 모방할 수 있는 다른 수면장애가 동반되어 나타날 수 있음을 고려하여 신중한 병력 청취와 수면다원검사 등을 이용하여 수면장애의 감별 진단 및 치료가 중요하겠다.

보기제통탕(補氣除痛湯)이 Streptozotocin으로 유발된 당뇨병성 말초신경병증 동물 모델에 미치는 영향 (Effects of Bogijetong-Tang on Diabetic-peripheral Neuropathy Induced by Streptozotocin in the Mouse)

  • Objectives: Previous reports have shown that Bogijetong-Tang (BJT) is effective in peripheral neuropathy induced by taxol and crush injury. In this study, we researched the effects of BJT on diabetic neuropathy induced by STZ in the mouse. Methods: We performed both in vitro and in vivo experiments to verify the effects of BJT on diabetic neuropathy induced by STZ in mice. Changes in axonal recovery were observed with immunofluorescence staining using NF-200, Hoechst33258, $S100{\beta}$, caspase 3 and anti-cdc2. Proliferation and degeneration of Schwann cells were investigated by immunofluorescence staining and western blot analyses. Results: BJT showed considerable effects on neurite outgrowth and axonal regeneration in diabetic neuropathy. BJT contributed to the creation of NF-200, GAP-43, Cdc2, phospho-vimentin, ${\beta}1$, active ${\beta}1$, ${\beta}3$ integrin, phospho-Erk1/2 protein. Conclusions: Through this study, we found that BJT is effective for enhanced axonal regeneration via dynamic regulation of regeneration-associated proteins. Therefore, BJT had a pharmaceutical property enhancing recovery of peripheral nerves induced by diabetic neuropathy and could be a candidate for drug development after more research.

항암제 유발 말초신경병증 한의 치험 2례 (Case reports: Effect of Korean Medicine Including Pharmacopuncture on Chemotherapy Induced Peripheral Neuropathy)

  • Objective : Peripheral neuropathy refers to the symptoms caused by damage to peripheral fibers, inflammation and degeneration. This study reports the effects of moxibustion, electric moxibustion, and pharmacopuncture including GeonChil(Rhus verniciflua stokes, 乾漆)and Whalhyul(活血) pharmacopuncture on patients with peripheral neuropathy induced by anti-cancer drugs and chemotherapy. Method : We administered moxibustion, electric moxibustion, GeonChil and Whalhyul pharmacopuncture to two patients who showed peripheral neuropathy induced by anti-cancer drugs and chemotherapy. The symtoms were evaluated using Visual Analog Scale (VAS) and chemotherapy induced peripheral neuropathy assessment tool (CIPNAT). Results : Following observations were made after treatments. Case 1 : After nine procedures, the score of VAS was decreased. Feeling of cold and numbness were improved, and as rotation movement of ankle was also possible, gait disturbance were improved. Case 2 : After, seven procedures symptoms of both shoulder pain were improved, and the symptoms of peripheral neuropathy were eliminated. Conclusion : We found the possibility of symptom improvement after moxibustion, electric moxibustion, and pharmacopuncture treatment on peripheral neuropathy caused by anti-cancer drugs. Clinical studies of pilot study and control settings will need to be carried out later.

흉곽 출구 포착성 신경 병증의 외과적 치료 (Surgery for Entrapments of the Thoracic Outlet)

  • Surgical treatment was performed on the 39 cases out of 76 cases of entrapments of the thoracic outlet. The remaining 36 cases of entrapments were treated by conservatively. The operated cases were categorized as follows. They were 34 cases of scalenus anticus syndrome, 1 of cervical rib syndrome, 2 of costoclavicular syndrome, and 2 of hyperabduction syndrome. 1. Scalenus anticus syndrome : Anterior scalenotomy was performed by simple sectioning of the attachment to the first rib. 2. Cervical rib syndrome : Complete decompressive resection of cervical rib sometimes required both anterior and posterior approaches to avoid over-retraction of the brachial plexus. 3. Costoclavicular syndrome : Partial decompressive claviculectomy was undergone instead of conventional total claviculectomy. 4. Hyperabduction syndrome : The resection of coracoid process was performed as well as conventional tenotomy of pectoralis minor muscle to insure free up-and-down moving of neurovascular bundle at the time of hyperabduction. Every diagnostic maneuver was tested at the time of operation to observe whether or not neurovascular decompression including restoration of radial pulse was sufficient. Despite of the postoperative vascular restoration was inmediate, neurogenic symptoms were improved slowly. Because this entity is essentially chronic nerve injnry, its recovery needed a couple of months or several. Although improvement was slow, ultimate results were definite. Complication was not observed.

빈소산가미방(檳蘇散加味方)으로 호전된 당뇨병성 말초신경병증 치험1례 (One Case of Diabetic Peripheral Polyneuropathy Improved by Binsosan-gamibang)

  • Objectives : This study it designed to evaluate the effects of an oriental medicine therapy, namely Binsosan-gamibang, on diabetic peripheral polyneuropathy. Methods : The clinical data was analyzed on a patient with diabetic peripheral polyneuropathy due to subyeolongsung(濕熱壅盛), gihyeoloeche(氣血瘀滯) whose main symptoms were pain and numbness in both legs. The patient was admitted at the internal medicine department of Dae-Gu Hanny University Dae-Gu Oriental Medicine Hospital on December 22, 2003, and was treated with herbal medicine(Binsosan-gamibang), acupuncture and physical therapy. Results : After treatment, improvement in pain and numbness in both legs was seen in the subsection of the pain rating score(PRS) and the visual analogue scale(VAS). Conclusions : This study suggests that Binsosan-gamibang is significantly effective in treatment of diabetic peripheral polyneuropathy.

HIV/AIDS 연관 말초신경병증에의 침 치료 활용가능성에 대한 고찰 (Review of the Application of Acupuncture for the HIV-associated Distal Sensory Neuropathy)

  • Objectives: To discuss the application of the acupuncture therapy for the HIV-associated distal sensory neuropathy. Methods: We searched papers using KTKP, Oasis, CNKI, PubMed, Science Direct, CINAHL, J-STAGE, and CiNii with the keywords "HIV", "Peripheral neuropathy" and "acupuncture". The search range included randomized controlled trials (RCTs) and clinical case series. Reviews and animal experiments were not included. Not matched with inclusion criteria were excluded. Results: A total 675 studies were found. 670 were excluded by scanning titles and abstracts and finally 5 articles were selected. One of the five articles noted that acupuncture was not effective in HIV-SN, but two articles reanalyzing the former, pointed out the statistical errors and noted acupuncture significantly reduced pain, attrition and mortality. Other RCT and the other case series concluded acupuncture had a beneficial effect in HIV-SN. Conclusions: Acupuncture seems to be effective to improve HIV-SN symptoms, however further large-scale RCTs are warranted.

전자침술로 호전된 유방암 환자의 항암화학요법 유발 말초신경병증 증례보고 (Electroacupuncture for the Treatment of the Chemotherapy-induced Peripheral Neuropathy in Breast Cancer Patient: A Case Report)

  • Objectives : The purpose of this study is to report the effect of electroacupuncture on chemotherapy induced peripheral neuropathy in breast cancer patient. Methods : The patient is a female with chemotherapy induced peripheral neuropathy (CIPN) who was diagnosed with breast cancer and suffering from paresthesia of extremities after chemotherapy of docetaxel. The patient was treated with electroacupuncture (15 minutes per one time) twice a day for 14 days. The clinical outcomes were measured by Patient Neurotoxicity Questionnaire (PNQ) grade and visual analogue scale (VAS). Results : In this case, PNQ Item 1 (numbness) grade was improved from D to C, Item 2 (weakness) grade was from C to B. The score of upper limbs VAS was decreased from 7 to 2, lower limbs was from 7 to 5 respectively. Edema of both extremities was also improved. Conclusions : This case study suggests that electroacupuncture therapy may have significant effects of CIPN in breast cancer patient.

유방암 수술 후 발생한 림프부종과 말초신경병증에 대한 도침술과 정맥자락술 집중치료 증례보고 (Acupotomy and venesection in Upper Limb Lymphedema and Peripheral neuropathy following Breast Cancer Surgery)

  • Purpose: In order to estimate clinical effects of acupotomy and venesection in a patient with peripheral neuropathy and upper limb lymphedema following breast cancer surgery. Methods: From 17th August, 2009 to 29th August 2009, 1 female patient with peripheral neuropathy and upper limb lymphedema following breast cancer surgery was treated with general oriental medicine therapy(acupuncture, moxibustion, cupping, physical therapy, herbal medication) and acupotomy with venesection. Results: The patient's chief complaints- Lt hand numbness, Lt arm edema, Lt. wrist flexion limitation - were notably improved. Conclusions : This study demonstrates that oriental medical treatment with acupotomy and venesection therapy has significant effect in improving symptoms of peripheral neuropathy and upper limb lymphedema following breast cancer surgery, as though we had not wide experience in this treatment, more research is needed.

항암제 유발 말초신경병증환자와 유기산검사 마커와의 상관성 연구 (Correlation Analysis of Organic Acid Comprehensive Profile Markers with Chemotherapy Induced Peripheral Neuropathy in Cancer Patients)

  • Objectives: The purpose of this study is to evaluate the urinary organic acid comprehensive profile for chemotherapy induced peripheral neuropathy (CIPN). Methods: Participants are 66 patients with CIPN who had symptom (Visual analog scale ${\geq}30mm$, Eastern Cooperative Oncology Group ${\leq}2$). Participants were tested with organic acid comprehensive profile markers. Results: Positive Correlation was observed in the neurotransmitter metabolism markers, N-methyl-D-aspartate (NMDA) modulators markers, detoxification markers, energy production markers, amino acid metabolism markers, and intestinal dysbiosis markers. Especially, all the neurotransmitter metabolism markers were showed positive rate of 44%. In addition, neuro-endo-immune was associated with energy metabolism (mitochondrial dysfunction) in CIPN of cancer patient. especially detoxification, intestinal bacterial hyperplasia, vitamin deficiency (folate, complex B group, vitamin C). Conclusions: Significant urinary organic acid comprehensive profile results were obtained in cancer patients who induced peripheral neuropathy by chemotherapy.

신경뿌리병증을 동반한 요추 추간판탈출증 환자의 봉약침 후관절 심부시술 효과에 대한 증례 보고 (A Case Report of Bee Venom Pharmacopuncture Therapy at Facet Joint for the Two Patients with Herniated Intervertebral Disc of Lumbar Spine)

  • The purpose of this study is to research and report the effect of Bee Venom pharmacopuncture therapy at facet joint for the two patients who was diagnosed as herniated intervertebral disc (HIVD) of lumbar spine. Two patients were hospitalized and treated with Complex Korean Medical treatment such as acupuncture, herbal medicine and Essential bee venom (EBV) pharmacopuncture in Dongsuwon hospital of oriental medicine. We measured their pain and function of daily life by numerical rating scale (NRS), straight leg rasing test (SLRT), Roland-morris disability questionnaire (RMDQ). As the result, we found the improvement of gait disturbance, functional disorder, and pain. This study suggested that Bee Venom pharmacopuncture therapy at facet joint is considered to conservative treatment for HIVD of lumbar spine. But further clinical studies are needed to clarify the effect of Bee Venom pharmacopuncture therapy at facet joint for the two patients with HIVD of lumbar spine.
