Surgical Treatment of Esophageal Adenocarcinoma in Barrett's Esophagus - A case report - (바렛 식도에 발생한 식도 선암종의 수술적 치료 - 1예 보고 -)

  • Barrett's esophagus is precancerous lesion of esophageal adenocarcinoma, but this has been rarely reported in Korea. A 81-year-old man with esophageal adenocarcinoma was admitted to our hospital, and we performed a distal esophagectomy and end-to-end esophagogastrostomy. The microscopic examination of the resected tissue revealed the intestinal metaplasia with goblet cells around the esophageal adenocarcinoma, which indicates this was a Barrett's esophagus. We report here on this case along with a review of the relevant literature.

  • Adenoid cystic carcinoma (ACC) commonly originates in the major salivary glands and respiratory tract, but it is extremely rare to find ACC in the esophagus. ACC of the esophagus is clinopathologically different from the salivary gland variant. It shows more aggressive malignant behavior and a very poor prognosis. We report here on the surgical and clinopathologic findings of ACC of the esophagus in a 65-year-old man, and we also include a review of the relevant medical literature.

  • Most laryngeal carcinomas are squamous cell carcinomas. Adenocarcinoma of the larynx is rare, its incidence has been reported less than 1% of all laryngeal neoplasms. Adenocarcinoma arises from glandular structures of the larynx and usually arises from the supraglottis. Compared to other carcinoma, deep biopsy and aggressive treatment is manditory. We report a case of adenocarcinoma of the larynx with review of current literatures.

  • Epithelial-myoepithelial carcinoma(EMC) is a rare low grade malignant salivary gland neoplasm that most commonly occurs in the parotid gland but can also arise in minor salivary glands. It is a distinct neoplasm characterized by tubular and solid growth patterns and a dual cell population including an inner larger of cuboidal to columnar epithelial cells which are peripherally bounded by a layer of myoepithelial cells with distinctly clear cytoplasm. The clinical course is characterized by a high incidence of local recurrence and not infrequent distant metastasis. The differential diagnosis included acinic cell tumor, adenoid cystic carcinoma, mucoepidermoid carcinoma, sebaceous carcinoma and pleomorphic adenoma. We report a case of EMC of left parotid gland in a 55-year-old woman.

  • Primary tumor arises infrequently in the parotid gland and generally, only about 20 to 40 percent of which prove to be malignant. They are characterized by histopathologic diversity, slow tumor growth, significant proportion of patients who have received previous treatment elsewhere. We have reviewed retrospectively 101 cases of parotid gland tumors which were treated for the recent eight years (1985-1992), Non-neoplastic tumor-like lesions were all excluded.

  • A 4-year-old female beagle with progressive exophthalmos and which had a neoplastic mass with diameter of 1.4 cm in the left lower ocular adnexa. Histologically, the mass was composed of hyper-plastic lobules and tubular structures separated by fibrous septum. The well differentiated sebaceous gland forming various sized lobules, and infiltration of mast cells and mononuclear inflammatory cells were observed. Apical decapitation secretion of these tubular structures with basophilic materials in their lumen showed mild sebaceous gland metaplasia. Immunohistochemical studying, cell groups were positive in ${\alpha}-SMA$ and vimentin. The primary tumor was diagnosed as adenocarcinoma originated from moll gland and meibomian gland of the eyelid, and the infiltrating intraocular neoplasm was diagnosed as a malignant mixed tumor.

  • Primary retroperitoneal mucinous cystadenocarcinoma is a very rare malignancy, and little is known concerning its pathogenesis, optimal treatment, and prognosis. A 29-year-old pregnant woman (21 weeks) presented with abdominal discomfort. CA 19-9, CA 125, and CEA were normal. Abdominal CT scanning revealed a $19{\times}15{\times}13cm$ retroperitoneal tumor. Exploratory laparotomy and tumor excision were performed. Mucinous retroperitoneal implants were removed as completely as possible. Histologically, the tumor showed focal areas of capsular invasion, but free resection margins. The uterus and both ovaries were normal in appearance. No adjuvant therapy was pursued. Six months later, peritoneal and bilateral ovarian metastases were discovered. Hence, we report the details of this case of primary retroperitoneal mucinous cystadenocarcinoma and present a review of the literature.

Cytologic Findings of Epithelial - Myoepithelial Carcinoma of the Salivary Gland - A Cese Report - (타액선 상피-근상피세포암종의 세포학적 소견 - 1예 보고 -)

  • The report of aspiration cytologic findings of epithelial-myoepithelial carcinoma(EMC) in the salivary gland is extremely rare. We present a case of fine needle aspiration cytology(FNAC) from EMC in the right submandibular gland of a 46 years old male patient. Neck CT scan revealed a confined lesion in the submandibular gland without enlargement of the regional lymph node. FNAC from the tumor showed several three-dimensional cellular clusters with admixed normal acinar cells. They frequently formed blanching tubular structures composed of two type of cells; darker cells haying eosinophilic scanty cytoplasm with round dense nuclei and clear cells having abudant pale cytoplasm with vesicular nuclei at the periphery of clusters. The tumor cells of both types did not show pleomorphism or mitoses. The resected submandibular gland showed an ill-defined whitish firm tumor, measuring $2{\times}1.5{\times}2cm$. The histology revealed an infiltrative tumor showing characteristic two cell types in a duct-like arrangement surrounded by thin basement menbrane. An inner layer of darker cells and outer layer of clear cells were postive for cytokeratin in the former and S-100 protein in the taller on the immunohistochemical stain.

Perianal Adenocarcinoma in Dog (개에서의 항문주위선 샘암종)

  • A 12.6-year-old, male Shitzu was diagnosed with perianal adenocarcinoma. The presented mass was brown to black, $4{\times}3{\times}3cm$ in size, and yellowish on cut section. Microscopic findings revealed that the mass composed of variable sized clusters of hepatoid cells with inconspicuous distinct. The tumor cells were polyhedral and pyknotic and exhibited high mitotic activity. Tumor cells intermingled with basaloid cells and primitive cells invaded the adjacent normal tissues. Basaloid cells exhibited positive immunoreactivity for Epidermal growth factor receptor (EGFR), human epidermal growth factor receptor (HER-2/neu), matrix metallopnateinase 9 (MMP-9), and perianal adenocarcinoma, protein kinase C alpha (PKC ${\alpha}$). Generally, tumors of the perianal gland are common and benign 4.5 times more often than carcinoma in the dog, particularly in males. In the present report, we examined histopathological and immunohistochemical characteristics of a rare perianal adenocarcinoma in association with proteins involved tumor metastasis and adenocarcinoma development.
