Intraluminal Esophageal Cyst (식도 근육내 낭종 1례)

  • Cystic intrathoracic lesions of foregut origin are now well recognized and account for approximately 10% of lesions presenting as mediastinal tumors. The terminology used to describe mediastinal endodermal cysts has been confused and sometimes ambiguous. The embryological derivation of these lesions has been the cause of much speculation. It Is suggested that these lesions should be classified Into three main categories based on embryology bronchogenic cyst[resulting from a defect of lung budding], Intramural esophageal cyst[true duplication], and enteric cyst[resulting from the split notochord syndrome]. This communication describes a 26 year old man with intramural esophageal cyst who was diagnosed as posterior medlastlnai tumor preoperatively and cured with extirpation of the cyst.

  • Esophageal duplication cysts are quite uncommon benign lesions of the esophagus that have previously been described as occurring only in the thoracic cavity. We experienced a rare case of completely intra-abdominal esophageal duplication cyst in a 10-year-old boy who has suffered from epigastric abdominal pain. Surgical excision is recommended at the time of cyst discovery whether symptoms present or not.

Treatment of Vallecular Cyst Using Laryngopharyngoscope (Laryngopharyngoscope를 이용한 후두개곡낭종의 치료)

  • Cystic lesion which is located in vallecular area is sometimes symptomatic and need surgey. But surgical route to this area is divided external approach into internal one. Internal approach via transoral route with suspension laryngoscope can avoid disadvantages of external approach but conventional laryngoscope is not suitable for this type of procedure. We used Steiner laryngopha-ryngoscope in surgery for vallecular cyst and we could excise cyst easily. Of 20 cases, complete excision was tried in 18 cases and marsupialization in 2 cases. Recurrence was 1 case. We suggest that surgery using laryngopharyngoscope is effective in treating cyst which is located in vallecular area.

Quasi-periodic waveform analysis for diplophonia (이중음성에 대한 음성파형분석)

  • Diplophonia is produced by the voice of two separate tones and produced through quasi-periodic variations in the vocal cord vibration. Diplophonia is generally regarded as a symptom of laryngeal pathology. The difference in the vibratory frequency between the vocal cords can be seen in a tension imbalance and a difference in the level of the vocal folds under the special condition such as incomplete glottal closure. So authors have experienced 19 cases of patient with diplophonia for the unilateral vocal cord paralysis, intracordal cysts and other mass lesions. And we analysed the diplophonic voice with peak variability and noise level for the quasi-periodic waveforms and spectrograms pre-and postoperatively.

Intramural Esophageal Cyst with Ciliated Epithelium -A Report of Case- (식도 벽내에 발생한 식도 낭종)

  • The esophageal cyst result from a wrong cleavage of the primitive gut in the 4 weeks embryo. In embryo and after seperation of the tracheal diverticulum, the esophagus is lined with ciliated cells which are able cover a "cystic duplication". It is often difficult to distinguish between the bronchogenic and the esophageal cyst. Pathological findings showed the presence of a ciliated epithelium without cartilage which was diagnosed as an esophageal cyst. The patient was 21 year old man for evaluation of the cyst in the posterior mediastinum. The cyst was located the intramural esophagus. Microscopically, the cyst was lined with ciliated columnar epithelium and there was no evidence of cartilage. The cyst was confirmed as the intramural esophageal cyst.geal cyst.

  • 호흡곤란에 대한 적절한 대처는 의사로서 숙지하여야 할 가장 중요하고도 기본적인 개념 중의 하나이다. 상부 기도에 발생하는 여러 가지 임상적 상황은 흔히 흡기성 천명을 동반하는 호흡 곤란으로 나타나고, 이에 대해 흔히 기도 삽관이나, 기관절개술 등이 행해 지지만, 그 발생 원인에 대해서 감별점들을 숙지하지 않으면, 원발 질환에 대한 합리적인 치료 뿐만 아니라 응급상황에서 적절하게 기도를 확보하는 데에도 문제가 생길 수 있다. 후두 상부에 일어나는 기도폐쇄의 상황은 비강에서부터 인두와 성문 상부, 식도 입구부에 이르는 해부학적 위치에 발생하는 다양한 질환들에 의한다. 비강과 비인두에는 pyriform aperture stenosis, choanal atresia, lacrymal duct cyst, 또는 teratoma나 encephalocele과 같은 질환이 발생할 수 있다. 구강, 인두부에서는 다양한 종류의 안면골격의 이상이나 설거대증, 또는 설갑상선, 갑상설관낭종, 또는 유피종등이 설기저부에 발생하기도 하고, 흔한 이유로 심한 편도-아데노이드 비대가 심각한 호흡곤란을 일으키기도 한다. 특히 소아에서는 이물의 가능성도 항상 염두에 두어야한다 이와 같이 기도 협착의 위치에 따라서 임상적 표현 양상이 구별될 수 있고, 또 부위에 따라 다양한 질환이 감별되어야 하므로 발생 위치에 따른 각 질환의 이해가 적절한 기도의 관리를 위해서 필수적이다.

Video-assisted Thoracoscopic Surgery for Mediastinal Lesions (종격동 질환의 비디오 흉강경 수술)

  • Recently, video-assisted thoracoscopic surgery for mediastinal lesions has been considered a new effective therapeutic method. From March, 1992 to April, 1997, 33 cases of video assisted thoracoscopic surgery for mediastinal lesions were performed. Gender distribution was 16 males and 17 females. Average age was 42 years old(ranged from 14 to 69). The locations of lesions were anterior mediastinum in 14 cases, middle mediastinum in 5 cases, posterior mediastinum in 11 cases, and superior mediastinum in 3 cases. These included 9 neurilemmomas, 5 benign cystic teratoma, 4 pericardial cysts, 2 ganglioneuroma, 2 thymus, 2 thymic cyst, 1 thymoma, 2 esophageal leiomyomas, 1 dermoid cyst, 1 lipoma, 1 malignant lymphoma, 1 bronchogenic cyst, 1 pericardial effusion, and 1 Boerhaave's disease with empyema. Working window was needed in 6 cases. We converted to open thoracotomy in 6 cases. Reasons of convertion to open thoracotomy were large sized mass(1), severe adhesion(3), and difficult location to approach(2). The average operation time was 116min($\pm$56 min). The average chest tube drainage time was 4.7days. The average hospital stay was 8.7 days. Operative complications were atelectasis(2), empyema with mediastinitis(1), recurrent laryngeal nerve palsy(1), and plenic nerve palsy(1). In conclusion, VATS for mediastinal lesions were performed with shorter operation time and hospital stay, and lesser complications and pain than those of conventional thoracotomy.

A Gastroesophageal Cyst in the Posterior Mediastinum; A Report of Case (후종격동에 발생한 위식도 낭종)

  • The gastroesophageal cyst is rare variety of benign developmental cysts in the mediastinum and it arises from sequestrations of nodules of forgut in the developing embryo.The patient was 23 year old man with complaint of right chest pain. Simple chest X-ray and chest CT scan showed a huge homogeneous cystic mass in the posterior mediastinum. The resected cystic mass showed combining of portion of esophagus and stomach. The cyst was confirmed as gastroesophageal cyst.

  • Among varieties of the mediastinal tumors, benign developmental cysts [Entergenous cysts] occur about 10% of them. From the primitive foregut, tracheobronchial tree and esophagus develop at 3 weeks of its embryonal age, and bronchogenic cyst arises from accessory or supernumerary lung bud. Usually it remains isolated with surrounding structures, and causes no specific symptoms. But few cases of bronchogenic cysts have fistulous communication with esophagus causing compressive symptoms. We report a case of unusual complicated case of bronchogenic cyst with review of literatures.

  • NBCCS is a hereditary disorder characterized by multiple defects of ectodermal and mesodermal structure, which is chiefly affected on skin, jaws & skeleton, CNS, eye and sexual organ. It is well associated with multiple odontogenic keratocyst on the jaws. Thus, we intended to investigate other anomalies presenting on NBCCS in cases of multiple odontogenic keratocyst which were treated in our hospital from 1980 to 1989. Here we reported 6 cases of NBCCS.
