  • 위기관지 누공은 매우 드문 질환으로 대부분의 경우 외상, 상부위장관 수술, 횡경막하 농양, 양성 위궤양, 신생물 등 이 원인으로 생각된다. 식도암의 Ivor Lewis 수술후 2년과 3년에 양성 위궤양으로 인해 발생한 재발성 위기관지 누공 1예를 문헌고찰과 함께 보고하는 바이다.

  • 후두암의 치료는 병변의 위치, 크기 등에 따라 치료방법이 다양하고 그 예후 또한 상이하다. 일반적으로 초기 후두암에서는 수술이나 방사선 요법이 비슷한 치료결과를 보이고, 진행된 후두암에서는 수술적 치료가 더 좋은 예후를 나타내는 것으로 알려져 있으나 발성장애를 초래하는 경우가 대부분이다. 저자들은 1986년부터 1990년까지 5년간 강남성모병원에서 후두암치료를 받았던 환자 81명을 대상으로 치료방법에 따른 음성보존 성적을 분석하였다. 1. T1 성대암 환자 18명중 16명 (88.9%)에서 방사선치료 또는 수술로써 음성을 보존할 수 있었다. 2. T2 후두암 환자 11명중 6명은 수술후 음성보존이 가능하였다. 3. 진행암환자 (T3, T4) 50명중 33명에 대해 수술을 시행하였고 이중 16명(48.5%)은 후두부분 또는 근전적출술 및 기관-식도 누공술에 의해 음성기능을 보존할 수 있었다. 4. 진행암환자에서 유도화학요법과 방사선치료의 병용요법만을 받은 17명 모두 6개월 이내에 재발하였고, 수술 혹은 수술과 방사선 치료를 병용한 33명중 27명이 최소한 6개월 이상 평균 2년 이상의 무병상태였다. 5. 후두부분 또는 근전적출술은 총 81명중 28명에서 시행되었고 그중 5명 (17.9%)에서 재발하였다.

  • Phlegmonous esophagitis is a disorder in which bacterial infection occurs in the submucosal and muscular layers of the esophagus. This malady is very rare and it is usually associated with high mortality. A 69-year-old male was admitted with chest pain and fever he'd experienced for 7 days. The chest computerized tomography scan revealed mediastinal widening, circumferential esophageal thickening, an air shadow along the esophagus and right pleural effusion. Drainage and debridement of the mediastinum and primary repair of the perforated esophageal muscular layer through a right thoracotomy was done immediately. Further surgical treatment was not performed. He had a good oral intake without dysphagia or esophageal leakage at discharge.

Congenital Tracheoesophageal Fistula in an Adult (성인에서 발견된 선천성 기관 식도루)

  • Congenital esophago-respiratory fistulae in adults have rarely been reported. Moreover, most of those are the cases of bronchoesophageal fistulae, that is to say esophago-lower respiratory fistulae. We experienced case of a congenital tracheoesophageal fistula in an adult, not a bronchoesophageal fistula. At our hospital, a 20-year-old male with recurrent episodes of a paroxysmal(especially postprandial) cough, respiratory infection and relative growth retardation had been diagnosed by using esophagography and esphagoscopy as having a congenital tracheoesophageal fistula with a concomittant esophageal diverticulum. The surgical correction was done successfully. We are excited to report a case of a congenital tracheoesophageal fistula in an adult, which is believed to be the first case of its kind in Korea.

Surgery of Esophageal Cancer with Metastasis to Distant Abdominal Lymph Nodes(M1LYN) (원격 복부 림프절의 전이(M1LYN)를 동반한 식도암의 수술)

  • From June 1987 to December 1994, 372 patients underwent operation for resection of esophageal cancer, and 48 patients with metastasis to distant abdominal lymph nodes were analyzed.. The primary tumors were located predominantly in lower thoracic esophagus(n=29). The location of involved lymph nodes were celiac L/N(n=45), common hepatic L/N(n=4), paraaortic L/N(n=l), and retropancreatic L/N(n=l). Most tumors penetrated the esophageal wall(T3,T4, n=43), metastased to regional L/N(N1, n=41), but a few tumors were limited to the esophageal wall(T1,T2, n=5), metastased to distant abdominal L/N without metastasis to regional L/N(NO, n=7). Resectability rate was 87.5%(42/48), and complete resection was possible in 31 patients(64.6%). The most frequent cause of incomplete resection and unresectability was unresectable T4 lesions(n=8), extranodal invasion(n=7). Overall operative mortality and morbidity was 4.2%, 22.9%, and resection mortality was 4.8%. Adjuvant therapy was given to 27 patients, and postoperative follow-up was possible in all patients(median follow-up, 32 months). The 1 year and 3 year survival for resection group was 54.0%, 18.1%(median, 386 days) including operative deaths. Our results suggest that resection of the esophageal cancer with metastasis to distant abdominal lymph nodes(M1LYN) can be done with acceptable mortality and morbidity, and may playa role in long-term survival in carefully selected patients because prognosis is dismal in unresectable esophageal cancer. We recommend that lymph nodes around celiac axis be dissected thoroughly for exact staging and possible prolongation of survival, and multimodality therapy as necessary because most patients with M1(LYN) esophageal cancer do poorly with only primay surgical treatment.

Long Term Survival after the Resection of Esophageal Cancer (식도암 절제수술 후 장기 성적)

  • Background: Despite the recent promising efforts to improve survival in patients with esophageal carcinoma, the long term survival results of patients with esophageal cancer have seldom been reported in Korea. To establish standard control for future studies, we re trospectively analyzed the surgical treatment results of the esophageal cancer patients managed in our department at Seoul National University Hospital. Material and Method: From January 1984 to December 1996, 734 patients were diagnosed with esophageal cancer. Among them, 253 patients underwent surgery in our department. We retrospectively analyzed the operative results and long term survival rates of these patients. Result: The majority of patients(237) had squamous cell histology and only 6 patients had adenocarcinoma. The final TNM stage grouping for these patients was based on the 1988 revised American Joint Commitee on Cancer classification. Twenty one patients were surgically classified as stage I, 109 as stage II, and 107 as stage III. C respiratory failure in 8, sepsis in 1, hepatic failure in 1, bleeding in 1 and unknown etiology in the remaining 3. The actuarial survival of 222 patients in whom the curative resection was accomplished at 1-, 2-, 3- and 5 years was 74.7${\pm}$3.1%, 46.5${\pm}$3.7%, 32.3${\pm}$3.7%, and 19.9${\pm}$3.3%, respectively. CONCLUSION: The poor long term survival rates suggest that an alternative treatment method such as intensive combined modality therapy should be developed for the management of esophageal cancer.

Surgical Treatment of Bronchoesophageal Fistula in Adult (성인 식도 기관지루의 외과적 치료)

  • Bronchoesophageal fistula is a rare clinical entity whether congenital or acquired in adult. We experinced 8 cases of bronchoesophageal fistula and performed surgical correction from 1991 to 1994. Of the 8 patients, 5 patients were male and three were female aging from 21 to 61 years(mean 44.12$\pm$14.62 years). Seven of 8 patients had congenital bronchesophageal fistula and the other one had acquired bronchoesophageal fistula. According to the classification of Braimbridge and Keith, 4 cases were belonged to type I and 3 cases were type II . The diagnosis was confirmed by esophagogram in six patients, by bronchoscopy and bronchogram in two patients, and in one patient, the fistula was discovered i cidentally during operation. All patients received astulectomy and concomitant procedures were applied as follows ; 4 diverticulectomy, 4 right lower lobectomy, 1 bilobectomy, 1 left lower lobectomy and 1 wedge resection of left lower lobe. All but one patient were discharged without any complication and have been in good condition.
