Leisure Programs Participation motivation and Leisure Life Satisfaction of the Institutional Elderly in Seoul (무료양로시설 거주 노인의 여가프로그램 참여동기 및 여가생활만족도 - 서울특별시를 중심 으로 -)

  • This study investigated the motivation of the institutional elderly to participate in the leisure programs the participation motivation and their leisure life satisfaction. A survey questionnaire was administered to 265 elderly in an institution for the elderly. The data was analyzed using frequency analysis, factor analysis, and regression analysis. Major findings are as follows: First, the motivation to participate in leisure programs on the part of the institutionalized elderly tended to be more external than internal. Second, regression analysis indicated that the main internal factors that affected participation were health condition, interaction with roommate, gender. education level, and interaction with institution's staff. The major external factors that affected the participation were health condition, interaction with colleagues, gender, and education level. Finally, the results of the regression analysis indicated that the main internal factors that affected the leisure life satisfaction were, intrinsic motivation to participate in leisure programs, interaction with their roommate, gender, and education level.

Residential Satisfaction of No-Charge Welfare Facility for the Elderly Integrated with Community Facility (지역시설복합 무료노인복지시설의 거주만족도)

  • The purpose of this study is to assess the residential satisfaction of no-charge welfare facility for the elderly, and to examine the effect of the integration with pay-training institute on residential satisfaction. The data were collected from staffs and fifty one residents of welfare facility by interview and questionnaire. The statistical analysis was conducted to identify the residential satisfaction. A half of the elderly were aged over seventy five years old, eighty four percents of them were female, and seventy seven percents of them were non-educated. Forty two percents of the elderly considered themselves to be unhealthy, but the ADL data reveals that they are relatively healthy. The most of residents are highly satisfied with whole welfare facility. But, the level of satisfaction on the bathroom, the storing space, the cooking table and the benches were relatively low. These dissatisfactions can be solved by the expanding of the storing space, setting up of a common washing room and electric cooking stove, and the increasing of the number of benches. Also, it was recommended for the elderly to set up the Ondol-style rest room with the western style rest room. The level of satisfaction on the attached training institute with no-charge welfare facility for the elderly were high. This has positive effect on residential satisfaction of the elderly. Therefore, the community integrated welfare facility can be the alternative plan for the enhancement of residential satisfaction and quality of elderly life.

Effects of Swallowing Training Program for Nursing Home Residents with Stroke (요양시설 거주 뇌졸중 노인을 위한 연하 훈련 프로그램의 효과)

  • Purpose: This study aimed to verify the effects of a swallowing training program on swallowing function and depression for nursing home residents with dysphagia after stroke. Methods: This is a quasi-experimental study with non-equivalent control group pre-post test design. The program (oro-facial muscle strengthening exercises, swallowing exercises, expiratory muscle strengthening exercises, and brain stimulation exercises) applied to the experimental group three times per week for eight weeks, 40-45 minutes for each intervention. The final data from 42 people (21 experimental and 21 control) were analyzed by SPSS/WIN 25.0 using descriptive statistics. 𝜒2 test, t-test, Wilcoxon rank sum test, and Friedman test. Results: The experimental group was significantly improved than control group in oro-facial muscle strength, swallowing symptoms (Z=-2.22, p=.026), and oral intake function level (Z=-2.00, p=.046). However, there was no significant difference between two groups in depression. Conclusion: This study is meaningful in that it reorganized and mediated a swallowing training program as a safe, non-invasive exercise that nurses can implement in a nursing facility with limited medical personnel and it could be easily followed by older adults.

Factors Affecting Satisfaction with Medical Service of the Elderly with Disabilities : Focusing on Health Characteristics and Medical Service Characteristic (장애노인의 의료서비스 만족도 영향 요인 : 건강 특성과 의료서비스 특성을 중심으로)

  • The purpose of this study is to identify the satisfaction with medical services of the disabled elderly who have the highest need for medical services. For this purpose, the effect of health characteristics and medical service characteristics of the disabled elderly on medical service satisfaction was verified. The subjects of analysis were 3,323 persons with disabilities aged 65 or older who were taken from the national survey of people with disabilities in 2017. For statistical analysis, descriptive analysis, correlation analysis, and regression analysis were performed using the SPSS 26.0 program. The results of the study showed as follows. As a result of the regression analysis, gender (β= -.045, p<.05) and residence status (β= -.048, p<.05) among the demographic characteristics as control variables had a statistically significant effect on the level of medical service satisfaction. Among the health characteristics, IADL (β=-.044, p<.05) had a statistically significant effect on medical service satisfaction level. In the case of medical service characteristics, satisfaction with medical facilities and equipment (β = .290, p< .001), medical staff's level of understanding of disability (β = .404, p< .001), health-related service use (β = .182, p<.05) had a statistically significant effect on the level of medical service satisfaction. Based on the results, practical alternatives to ensure health equity in the community medical system were suggested in the discussion to enhance the health management and self-determination capabilities of the disabled elderly.

Oral health status of long-term care facility residents (노인요양시설 거주자의 구강건강상태)

  • Objectives: The purpose of this study was to examine the practice of oral hygiene behaviors and oral health status of long-term care facility residents and to analyze the factors related to salivary hemoglobin level which can predict active periodontal disease. Methods: From 30th October 2015 to 7th January 2016, a questionnaire was provided to 63 participants and their dental plaque and saliva samples were collected to assess the levels of salivary hemoglobin and dental plaque acidogenicity. In order to analyze the factors related to salivary hemoglobin level, multiple linear regression analysis was performed. Results: Toothbrushing was most frequently performed by the participants themselves (98.4%) and toothbrushing was performed after eating breakfast (81.3%). 68.8% of participants reported brushing their tongue. 35.9% of participants perceived having bad teeth, and 87.5% had high dental caries activity. The percentages of participants with hyposalivation and ${\geq}0.20{\mu}g/ml$ salivary hemoglobin level were 45.3% and 59.4%, respectively. The salivary hemoglobin level was significantly higher in the group in which stimulated salivary flow rate was ${\leq}0.70ml/min$, dental plaque acidogenicity was superior, and perceived having bad teeth (p<0.05). There was also a tendency for the salivary hemoglobin level to increase with age (p<0.05). Conclusions: Oral health status of the long-term care facility residents was still not improved, and the characteristics of salivary volume and dental plaque were important factors affecting salivary hemoglobin level. Therefore, it is necessary to operate an oral hygiene intervention program by oral health professionals in such facilities in order to provide residents with effective oral care aligned with their respective needs. Furthermore, it is necessary for caregivers to complete mandatory oral health education to improve the oral hygiene status of the long-term care facility residents.

Effects of Tai Chi Exercise on Physical Fitness, Bone Mineral Density, and Fracture Risk in Institutionalized Elderly (저강도 타이치 운동이 시설거주노인의 체력, 골밀도 및 골절위험도에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of Tai Chi exercise on flexibility, balance, walking ability, muscle strength, bone mineral density, and fracture risk in institutionalized elders. Method: A quasi-experimental pretest-posttest design with a nonequivalent comparison group was utilized, and 53 older adults living in one institution were recruited and assigned to one of two groups, experimental group (18) or comparison (20). Both groups completed posttest measures at 6 months. There was a 31% rate of dropouts. Tai Chi exercise was provided twice a week for 24 weeks. Outcome measurements were conducted by a physiotherapist at a university hospital health promotion center who did not know the group assignment. Results: At 6 months, the experimental group had significantly greater grip strength(t=2.12, p=.04), back muscle strength (t=2.42, p=.02), balance (t=5.31, p<001), and flexibility (t=3.57, p<.001). They also showed significantly greater bone mineral density of lumbar spine and femur, and reduced fracture risk. Conclusion: Tai Chi exercise was safely and effectively used with institutionalized elders for 6 months and significantly improved physical fitness, bone mineral density along with a reduction in fracture risk. Whether Tai Chi exercise would lead to prevention of fall episodes and fall related fractures in this population will require further study.

Interventions Using Technologies for Older Adults in Long-term Care Facilities: A Systematic Review (장기요양시설 거주 노인 대상 테크놀로지 활용 중재에 관한 체계적 문헌고찰)

  • Purpose: Although innovative interventions using technologies have been introduced in long-term care settings, available evidence is still anecdotal. The purpose of this study is to investigate and synthesize the outcomes of interventions using technologies delivered to nursing home residents. Methods: Published clinical trials were identified through PubMed, CINHAL, Cochrane and PsycINFO databases and manually hand-searching. Eligible studies were articles published between 1997-2016 in English or Korean with a randomized controlled trial or quasi-experimental design in which interventions using technologies were delivered to nursing home residents. Results: A total of 20 studies were selected for this review. Types of interventions using technologies were classified into the electronic documentation technology (n=1), the clinical decision support system (n=1), the safety technology (n=1), the health and wellness technology (n=10), and the social connectedness technology (n=7). Overall resident outcomes indicated that interventions using technologies improved behavioral symptoms and psycho-social outcomes, but mixed results were shown in the aspects of physical function, cognitive function, social relationship and quality of service. Conclusion: This review demonstrates that incorporating technologies into nursing home care have positive effects on residents' psycho-social outcomes and behavioral symptoms. To disseminate the effectiveness of interventions using technologies, further research is needed to determine what mechanisms underlying such relationships exist.

The Study on Regional Linkage Characteristics of Elderly Facilities in Aichi Prefecture, Japan (일본 노인시설에서 나타나는 지역연계특성에 관한 연구 - 아이치현(愛知県)지역 노인시설을 중심으로 -)

  • Purpose: While Japan undergoes super aged society, Japanese Elderly Facilities are trying to keep elderly residents from social isolation and expand regional interaction. Methods: This study analyzes regional linkage characteristics through 8 cases of Aichi-Elderly Facilities which realize regional interaction and symbiosis(life together) between various generations by visiting. Result: 1) Most researched facilities except SO provide and share opportunity for interaction with community and residents through space for regional interaction including cafe and various programs and interactive settings. 2) The facility which seeks regional linkage through Mixed-use of facilities can devide into (1) Mixed-use of facilities easy to found, (2) Addition of housing function (NM, SM, DM) and (3) Mixed-use with different facilities(HY, GM) 3) The characteristics of interaction and symbiosis between generations are (1) to adopt concept of life together to have interaction possibility with young generation and children: most studied facilities except SO (2) to establish elderly residence into facility to have interaction(FE, SM, GM) or place family room or single room into elderly residence (NM, DM, BN) (3) to disperse small facilities into community (FE) or facility or community realizing symbiosis through various facility arrangement (SM, GM). 4) Therefore, this study can categorize (1) Program network, (2) Temporary interaction, (3) Symbiosis residence, (4) Symbiosis community according to characteristics which regional linkage has. Implication: Regional Linkage is an important concept to improve social interaction in community-based facilities. It is a thought-provoking concept to Korean elderly facilities because Korean facilities are still far from a city and in isolated environment.

A Qualitative Study on Coping strategies of Older adults with Depression: Focused on the Experience of Coping with Depression in Older Adults Living in Seoul (노인의 우울 대처 전략에 관한 질적 연구: 서울시 거주 도시 노인의 우울 대처 경험을 중심으로)

  • The purpose of this study is to explore qualitatively what kind of coping strategy the Korean older adults use when they are depressed and why. Participants were users, older than 60, of an elderly welfare center in Seoul, and answers of 34 respondents who experienced depression were included in the analysis. The collected data were analyzed through content analysis. The results of the analysis showed that strategies to cope with depression used by participants were composed of 6 domains and 11 sub-domains: health behavior (medical approach / exercise and diet), family and social contact (social interaction / going out and going on a trip / communication with family), Religious activities (Religious activities), lifelong education (hobbies / educational activities), productive activities (labor), health risk behaviors (drinking and gambling / resignation). The depression coping strategies of the older adults and their characteristics are as follows. First, older adults used diverse problem-focused coping strategies to cope with depression. Second, older adults considered various coping strategies together and used them simultaneously. Third, the facility space for older adults functions as a shelter. Fourth, although there were cases where medical approach was used, the intention to reuse was very low. Through this study, the following suggestions were made to help older adults cope with depression more successfully. First, access to various coping strategies should be promoted. Second, more places where older adults can spend their time comfortably need to be provided. Third, resistance to mental health care should be resolved.

Dietary Habits and Behaviors of College Students in the Northern Gyeonggi-do Region (경기 북부 지역 일부 대학생의 식습관과 식행동 조사)

  • This study was conducted to assess the dietary habits and behaviors of college students in the Northern Gyeonggi-do Region(Republic of Korea). To accomplish this, a survey was conducted to investigate the preference, intake frequency and menus of breakfast, lunch, dinner and snacks by male and female students. Most male and female students had irregular breakfasts with the main reasons for skipping a meal 'not enough time' (47.3, 45.5%) and 'irritating' (19.0, 21.9%), respectively. The principal breakfast menu for both was 'rice and side dishes'. The lunch time for male and female students was regular, and the lunch cost made up most of their spending money. The snack time of male and female students was mostly 'between lunch and dinner'. The snack menus of male and female students were 'cookies' (25.8, 19.0%), 'drinks' (18.0, 14.4%), and 'breads' (16.9, 13.2%), respectively. For male students, the reasons of snack intake were 'hunger' (46.1%), 'boredom' (15.7%), and 'habitual' (9.0%). For female students, the reasons were 'hunger' (28.2%), 'habitual' (22.4%), 'boredom' (15.5%). Half of the college students had regular delivery foods for reasons of 'hygiene', 'taste', and 'nutrition' in the order and in consideration of being the elderly. The facility foodservice was also used half of the college students for the reasons of being 'economic' (30.9, 22.6%), and 'liberated from preparing meals' (21.4, 23.8%), respectively. Both male and female students were found to have less knowledge of the facility foodservice in the elderly. Based on these results, greater efforts should be made to provide meaningful information regarding the dietary habits and behaviors of college students, especially, when elderly.
