Major type of facial asymmetry results from facial deformity and needs surgical correction. To diagnose facial asymmetry and set a treatment plane for patients, setting a sagittal reference plane is crucial. The purpose of this study is comparison of measurements of sagittal soft tissue reference point to three different sagittal planes. The subjects are 25 of asymmetry patients (M:15, F:10) and 19 of normal people. There are differences in point Gnathion and Stomion. Most of measurement of sagittal reference points showed within 1 mm difference from sagittal reference plane. Deviation of point Pronasale in Sa1 plane revealed significant difference among 3 reference planes. The deviation of Gnathion was proportional to the deviation of Stomion in all 3 reference planes.

임플랜트 보철 환자의 하악 운동에 관한 연구를 위해 완전 고정성 보철 치료를 한 8명의 환자와 상하악 총의치를 장착한 8명의 환자, 그리고 5명의 자연치아 피검자를 대상으로 Sirognathograph(Siemens, Benshiem, Germany)와 BioPAK program(BioResearch, Milwaukee, U.S.A.)으로 한계운동과 당근시편에 대한 편측 저작운동에 대한 검사를 시행하였다. 편측 저작 운동은 전두면에서의 저작운동의 높이와 폭, 전두면에서의 저작각도, 시상면에서의 절치기준점의 최대 이동거리, 시상면에서의 저작운동의 전후방 이동거리, 최대개구속도, 최대폐구속도의 기준점으로 각 저작주기의 성적을 평균처리 하여 다음과 같은 결론을 얻었다. 1. 전두면에서의 저작운동의 높이는 자연치, 총의치, 임플랜트 군간에 유의성있는 차이가 없었다(p>0.05). 2. 전두면에서의 저작운동의 폭은 임플랜트 군이 자연치, 총의치 군에 비해 유의성있게 작은 수치를 보였다(p<0.05). 3. 전두면에서의 저작각도 분석시 임플랜트 군과 자연치 군, 임플랜트 군과 총의치 군간에 유의성있는 차이가 없었다(p>0.05). 4. 시상면에서의 절치기준점의 최대 이동거리 분석시 임플랜트 군과 자연치 군, 임플랜트 군과 총의치군간에 유의성 있는 차이가 없었다(p>0.05). 5. 시상면에서의 전후방이동거리는 자연치 군이 임플랜트 군, 총의치 군에 비해 유의성 있게 크게 나타났다(p<0.05). 6. 최대개구속도는 자연치 군이 총의치 군과 임플랜트 군에 비해 유의성 있게 작은 수치를 보였다(p<0.05). 7. 최대폐구속도는 각 군간에 유의성 있는 차이가 없었다(p>0.05). 8. 저작운동은 개인간의 차이가 커서 명확한 결론을 유추하기가 어려웠다.

The purpose of this study was to investigate the eruption pattern of the mandibular first molar in sagittal, frontal and horizontal views using the cone beam CT scanning. CT images were obtained from healthy 83 children (42 boys, 41 girls) between 3 to 10 years of age with a normal dentition according to Nolla stage. 1. In the frontal and horizontal view, the intermolar width decreased continuously with stage and slightly increased at the last stage. 2. In the sagittal and frontal view, eruption distances from occlusal plane were observed the largest change between stage 5 and 7. 3. In the horizontal and sagittal view, mandibular first molar from distal surface of primary second molar moved distally between stage 4 and 6. 4. In the sagittal view, angle from occlusal plane to mesio-distal axis increased between stage 4 and 8. 5. In the frontal view, angle from occlusal plane to bucco-lingual axis increased continuously during all stage. 6. In the horizontal view, angle from midsagittal plane to long axis increased between stage 5 and 8.

The three-dimensional (3D) changes of bone, soft tissue and the ratio of soft tissue to bony movement was investigated in 8 skeletal Class III patients treated by mandibular setback surgery. CT scans of each patient at pre- and post-operative states were taken. Each scan was segmented by a threshold value and registered to a universal three-dimensional coordinate system, consisting of an FH plane, a mid-sagittal plane, and a coronal plane defined by PNS. In the study, the grid parallel to the coronal plane was proposed for the comparison of the changes. The bone or soft tissue was intersected by the projected line from each point on the gird. The coordinate values of intersected point were measured and compared between the pre- and post-operative models. The facial surface changes after setback surgery occurred not only in the mandible, but also in the mouth corner region. The soft tissue changes of the mandibular area were measured relatively by the proportional ratios to the bone changes. The ratios at the mid-sagittal plane were $77\~102\%(p<0.05)$. The ratios at all other sagittal planes had similar patterns to the mid-sagittal plane, but with decreased values. And, the changes in the maxillary region were calculated as a ratio, relative to the movement of a point representing a mandibular movement. When B point was used as a representative point, the ratios were $14\~29\%$, and when Pog was used, the ratios were $17\~37\%(9<0.05)$. In case of the 83rd point of the grid, the ratios were $11\~22\%(p<0.05)$.
Objective: The purpose of this study is to compare landmark position between cephalometric radiography and midsagittal plane projected images from 3 dimensional (3D) CT. Methods: Cephalometric radiographs and CT scans were taken from 20 patients for treatment of mandibular prognathism. After selection of land-marks, CT images were projected to the midsagittal plane and magnified to 110% according to the magnifying power of radiographs. These 2 images were superimposed with frontal and occipital bone. Common coordinate system was established on the base of FH plane. The coordinate value of each landmark was compared by paired t test and mean and standard deviation of difference was calculated. Results: The difference was from $-0.14{\pm}0.65$ to $-2.12{\pm}2.89\;mm$ in X axis, from $0.34{\pm}0.78$ to $-2.36{\pm}2.55\;mm$ ($6.79{\pm}3.04\;mm$) in Y axis. There was no significant difference only 9 in X axis, and 7 in Y axis out of 20 landmarks. This might be caused by error from the difference of head positioning, by masking the subtle end structures, identification error from the superimposition and error from the different definition.
This study was performed to investigate the reproducibility of the horizontal and midsagittal planes, and to suggest a stable coordinate system for three-dimensional (3D) cephalometric analysis. Eighteen CT scans were taken and the coordinate system was established using 7 reference points marked by a volume model, with no more than 4 points on the same plane. The 3D landmarks were selected on V works (Cybermed Inc., Seoul, Korea), then exported to V surgery (Cybermed Inc., Seoul, Korea) to calculate the coordinate values. All the landmarks were taken twice with a lapse of 2 weeks. The horizontal and midsagittal planes were constructed and its reproducibility was evaluated. There was no significant difference in the reproducibility of the horizontal reference planes, But, FH planes were more reproducible than other horizontal planes. FH planes showed no difference between the planes constructed with 3 out of 4 points. The angle of intersection made by 2 FH planes, composed of both Po and one Or showed less than $1^{\circ}$ difference. This was identical when 2 FH planes were composed of both Or and one Po. But, the latter cases showed a significantly smaller error. The reproducibility of the midsagittal plane was reliable with an error range of 0.61 to $1.93^{\circ}$ except for 5 establishments (FMS-Nc, Na-Rh, Na-ANS, Rh-ANS, and FR-PNS). The 3D coordinate system may be constructed with 3 planes; the horizontal plane constructed by both Po and right Or; the midsagittal plane perpendicular to the horizontal plane, including the midpoint of the Foramen Spinosum and Nc; and the coronal plane perpendicular to the horizontal and midsagittal planes, including point clinoidale, or sella, or PNS.
Total body irradiation is operated to irradicate malignant cells of bone marrow of patients to be treated with bone marrow transplantation. Field size of a linear accelerator or cobalt teletherapy unit with normal geometry for routine technique is too small to cover whole body of a patient. So, any special method to cover patient whole body must be developed. Because such environments as room conditions and machine design are not universal, some characteristic method of TBI for each hospital could be developed. At Seoul National University Hospital, at present, only a cobalt unit is available for TBI because source head of the unit could be tilted. When the head is tilted outward by 90$^{\circ}$, beam direction is horizontal and perpendicular to opposite wall. Then, the distance from cobalt source to the wall was 319 cm. Provided that the distance from the wall to midsagittal plane of a patient is 40cm, nominal field size at the plane(SCD 279cm) is 122cm$\times$122cm but field size by measurement of exposure profile was 130cm$\times$129cm and vertical profile was not symmetric. That field size is large enough to cover total body of a patient when he rests on a couch in a squatting posture. Assuming that average lateral width of patients is 30cm, percent depth dose for SSD 264cm and nominal field size 115.5cm$\times$115.5cm was measured with a plane-parallel chamber in a polystyrene phantom and was linear over depth range 10~20cm. An anthropomorphic phantom of size 25cm wide and 30cm deep. Depth of dose maximum, surface dose and depth of 50% dose were 0.3cm, 82% and 16.9cm, respectively. A dose profile on beam axis for two opposing beams was uniform within 10% for mid-depth dose. Tissue phantom ratio with reference depth 15cm for maximum field size at SCD 279cm was measured in a small polystyrene phantom and was linear over depth range 10~20cm. An anthropomorphic phantom with TLD chips inserted in holes on the largest coronal plane was bilaterally irradiated by 15 minute in each direction by cobalt beam aixs in line with the cross line of the coronal plane and contact surface of sections No. 27 and 28. When doses were normalized with dose at mid-depth on beam axis, doses in head/neck, abdomen and lower lung region were close to reference dose within $\pm$ 10% but doses in upper lung, shoulder and pelvis region were lower than 10% from reference dose. Particulaly, doses in shoulder region were lower than 30%. On this result, the conclusion such that under a geometric condition for TBI with cobalt beam as SNUH radiotherapy departement, compensators for head/neck and lung shielding are not required but boost irradiation to shoulder is required could be induced.

Purpose: The purpose of this study was to analyze the sagittal root position of maxillary anterior teeth and report the frequency of each classification in Korean for immediate implant placement. Materials and Methods: A retrospective review of cone-beam computed tomography (cone-beam CT) images was conducted on 120 patients (60 male and 60 female) who fulfilled the inclusion criteria. After reorientation of the axis, cone-beam CT images were evaluated and the relationship of the sagittal root position (SRP) of the maxillary anterior teeth to its associated osseous housing was recorded. Class I, II, and III were classified respectively when the root was positioned on the labial, central, and palatal aspect of the alveolar bone. Class IV was the position that at least two thirds of the root is engaging both the labial and palatal cortical plates. Then, the angulation of the root axis and the alveolar bone axis was measured. Descriptive statistics and Kruskal-Wallis test were used to compare the angulation according to the root position and SRP class. Results: The frequency distribution of sagittal root position of maxillary anterior teeth indicated that 81.1%, 10.3%, 1.9%, and 6.7% were classified as Class I, II, III, and IV, respectively. The sagittal angulation at approximately 77.5% of central incisor, lateral incisor, and canine was < 20 degrees, but the angle at more than 42.7% of canine was ≥ 20 degrees. Within the class, the angulation was statistically significantly greater in Class I (16.19) compared to Class II (8.72) and Class III (9.93), and smaller in Class IV (3.79). Conclusion: Within the limitation of this study, a majority of the maxillary anterior roots were positioned close to the buccal cortical plate. However, some roots have very thin alveolar bone and sagittal angulation larger than 30 degrees. Therefore, cone-beam CT analyses of the sagittal root position and the sagittal angulation are recommended for the selection of the appropriate dental implant treatment approach.

Purpose: Our goal for this study was to prospectively evaluate the functional & structural outcomes, by means of CT arthroscopy, of arthroscopic double-row fixation for treating rotator cuff tear. We also attempted to determine the variants that affect the functional & structural outcomes. Materials and Methods: Twenty seven consecutive patients underwent arthroscopic rotator cuff repair with double-row fixation. The average age at the time of the operation was fifty six years. The preoperative and postoperative examinations consisted of determining the Constant score, the score for the visual analogue scale for pain, the UCLA score, the American Shoulder and Elbow Surgeons (ASES) score, as well as a full physical examination of the shoulder. Preoperative MR arthrography was used to evaluate the integrity and atrophy of the rotator cuff. We measured the intraoperative tear size in the sagittal and coronal planes. Postoperative CT arthrography was used at one year postoperatively to evaluate the integrity and atrophy of the repaired tendons and muscles. Results: Preoperative MR arthrography revealed an average 29.22 mm tear size in the sagittal plane and an average 22.72 mm tear size in the coronal plane. Twelve cases of supraspinatus muscle atrophy and two cases of infraspinatus atrophy were observed on the preoperative MR arthrography. The average clinical outcome scores all significantly improved at the time of follow-up. At a mean of one year postoperatively, CT arthrography revealed 48.1% of the shoulders had healed, 11.1% showed incomplete healing and 40.7% showed retear of the repaired tendon. Conclusion: Arthroscopic double-row repair can result in improved clinical outcomes and good patient satisfaction. However, the problems about how to enhance healing of the repaired tendon still remain.
The purpose of this experimental study was to evaluate aㅜd compare maxillary arch dimensional and positional changes between first and second premolar extraction groups. The Calorific Machine was used to illustrate tooth movement in three dimensions. The experimental teeth except the first or second premolars were embedded in artifical alveolar bone. The extraction space was closed using arch wires with bull loops into which 15 degree gable bends were placed. Before and after space closure, radiographs were taken in the sagittal and occlusal directions using occlusal films. The results showed greater mean maxillary incisor retraction and less anchorage loss in the maxillary first premolar extraction group than in the maxillary second premolar extraction group. Mesiopalatal rotation of anchor teeth was greater after extraction of a maxillary second premolar than a maxillary first premolar (P<.001).
