Effects of Early Exercise Education using Video on Exercise Knowledge and Compliance of Elderly Patients Who Undergo Total Knee Replacement Surgery (동영상을 이용한 수술 후 초기 운동교육이 슬관절 전치환술 노인 환자의 운동지식 및 운동이행도에 미치는 효과)

  • Purpose: This study was conducted to identify the effects of early exercise education using videos on exercise knowledge and compliance of elderly patients who undergo total knee replacement surgery. Methods: This is a quasi-experimental study with a nonequivalent control group non-synchronized design. The participants were 70 elderly patients who had total knee arthroplasty in the C university hospital located in G city, Korea. Thirty-five patients were assigned to an experimental group and the other patients were assigned to a control group. After the total knee arthroplasty, the early exercise education accompanied by watching video instruction was conducted on the experimental group at post-operation second and third day for 15 minutes. General exercise education was administered to the control group. The exercise knowledge and compliance of the elderly patients were measured through structured questionnaires. Conclusion: The results of this study indicate that early exercise education using videos has positive effects to improve the exercise knowledge and compliance of elderly patients after total knee arthroplasty. This early exercise education can be used going forward as nursing intervention for elderly patients who undergo total knee replacement surgery.

  • This study was conducted to evaluated the effects adding continuous passive motion(CPM) each day to the entire postoperative program of patients who received a total knee replacement(TKR). A retrospective chart review was completed for 31 patients(12 with bilateral involvement, totaling 44 knees)who received a TKR between 1996 and 1998. The data analysis compared the following variables for 28 patients who received CPM and 16 patients who received no CPM: the length of hospital stay(LOS), the frequency of postoperative complications, the number of post-operative days(PODs) range of motion (ROM). The CPM Group showed significant decreases in the frequency of complication(p<0.05), the LOS(p < 0.05), and No difference was demonstrated in the ROM of the two groups. We concluded that CPM was an effective adjunct to physical therapy care of patients undergoing total knee replacement.

Comparison Analysis of Effectiveness of CPM and PNF Treatments after Total Knee Replacement (슬관절 전치환술 시행 후 CPM과 PNF 치료에 따른 효과 비교)

  • Objective : Compare the results of CPM and PNF in recovery rates when applied as an initial physical therapy. Methods : Randomly selected 19 patients who underwent through total knee replacement surgery. 23 cases were studied and divided into 12 CPM and 11 PNF cases. CPM case was carried out for 60 minutes, a maximum allowed exercise without pain. PNF case was carried out for 20 minutes by a therapist to a patient as 'one on one'. Result : Result showed that PNF treatment was increased significantly at P<0.05 in flexor muscle strength in 8 days after the surgery. Conclusion : The study revealed that in comparison with CPM, PNF was more effective treatment increase of flexor muscle strength in rehabilitation on after total knee replacement surgery.

  • Purpose : The purpose of this article was to review the literature on change of knee position sense after total knee replacement intervention in knee rehabilitation. Methods : This review outlines scientific findings regarding different literature data in knee rehabilitation, and discusses proprioceptive change of knee joints after total knee replacement intervention. Result : Severe pain and diminished joint sensation may precipitate degenerative changes of knees joint, and a strong association between decreased proprioception and function has been identified in elderly patients with knee osteoarthritis. Total knee replacement(TKR) of the knee joint are effective surgical procedures in osteoarthritis patients, resulting in decreased pain, and improved physical function and quality of life in patients. After total knee replacement, joint position sensation is partially recovered, which may improve functional stability of the knee, but aspects of physical functioning are not fully restored to preoperative status. Conclusions : Thus, recovery rate in proprioception after TKR also is considered to be important because it may be a significant risk factor in failure of total knee replacement and knee rehabilitation intervention.

  • Background : This study was identified the effects of a video CD Rehabilitation program on the range of knee angle and activities of daily living in patients with a total knee replacement Method : The data was collected from March 1 to May 31 2009 for the control group and from July 1 to September 31 2009 for the experimental group. The control and experimental groups contained 19 subjects each. A video CD Rehabilitation program configured to 8 minutes in length was shown 8 times every other day. Result : 1. On the day of Hemovac removal, the knee angle and CPM angle in the experimental group increased significantly, but they remained relatively constant on post operation day 21. 2. The activities of daily living were significantly different in the experimental group. Conclusion : 1. Effects of Muscle Energy Technique on Knee Extensor Muscle Strength, Knee Range of Motion, Balance, and Walking Ability in Elderly Women during the Chronic Phase after Total Knee Replacement (슬관절 전치환술 후 만성기 여성 노인의 슬관절 신전근에 근에너지기법이 근력, 관절가동범위, 균형, 보행능력에 미치는 영향)

  • Background: This study aimed to use the muscle energy technique (MET) with total knee replacement (TKR) during the chronic phase in a clinical setting and confirm its effects on the knee extensor strength and ROM, balance, and walking ability. Methods: A total of 20 female patients who underwent TKR 1~4 years ago were assigned to two groups (Control: Q setting exercise+general physical therapy, n=10; Exp: MET+general physical therapy, n=10). Interventions were performed three times a week for 4 weeks. The strength of the knee extensor was evaluated using an aneroid sphygmomanometer, and ROM was evaluated using degrees at the end range on active knee flexion. The main balance outcomes were evaluated using two standard scale (TSS) and timed up and go (TUG) test, whereas the walking ability was evaluated using the 10 meter walk test (10MWT). Results: Analysis showed that both groups had significant increases in strength, ROM, TSS, TUG, and 10MWT. Differences in all variables were significant between the control and Exp groups at the post-intervention evaluation (p<.05). However, no significant difference was observed in strength and TUG. Conclusion: Results of this study demonstrated that MET would help improve the strength, ROM, balance, and walking ability of patients with chronic TKR who want to enhance their abilities and performance in activities of daily living.

Effect of stem design on contact pressure distribution of end-of-stem in revision TKR (슬관절 재전치환술용 경골삽입물 형상이 접촉압력 분포에 미치는 영향)

  • In this study, the effect of stem-end design on contact pressure and stress distribution in revision TKR was investigated using finite element method. The finite element model of tibia, including the cortical bone, the cancellous bone and canal, was developed based on CT images. The stem models with various stem lengths, diameters and frictional coefficients, and press-fit effects were considered. The results showed that the longer stem length, the stronger press-fit, the bigger stem diameter, and the higher frictional coefficient increased both peak contact pressure and the highest Von-Mises stress values. We hypothesized that peak contact pressure and Von-Mises stress distribution around the stem, may be related to the stem end pain. The results of this study will be useful to design the stem endand reduce the end-of-stem pain in revision TKR.

  • Total elbow arthroplasty is a relatively rare procedure compared with total knee or total hip arthroplasty. Total elbow arthroplasty for bony ankylosis is even rarer, and the results are often unsatisfactory. We report a patient who gained good mobility of the elbow after total elbow arthroplasty for the treatment of fractured arthrodesis site with which the patient had lived with for 12 years.

Mid-Term Results of Fixed Bearing Unicompartmental Knee Arthroplasty: Minimum 5-Year Follow-Up (고정형 슬관절 단일 구획 치환술의 중기 추시 결과: 최소 5년 추시)

  • Purpose: To evaluate the clinical and radiological outcomes, and the complications of unicompartmental knee arthroplasty (UKA) using a fixed bearing prosthesis after 5-year follow-up. Materials and Methods: Twenty-six knees (25 patients) that underwent fixed bearing UKA between May 2003 and August 2011 were included. The subjects were 3 males (3 knees) and 22 females (23 knees), and the average age was 63.5 years. The preoperative diagnosis was osteoarthritis (23 knees) and osteonecrosis (3 knees). The mean follow-up duration was 67 months (from 60 to 149 months). The clinical evaluation included pre- and postoperative American knee society knee and function score, and range of motion. The radiology evaluation included standing antero-posterior, lateral view, and fluoroscopic film to analyze the postoperative alignment and osteolysis. Results: The mean American Knee Society knee score and function score were improved from 42.0 and 57.5 to 87.9 and 85.0, respectively (p<0.001). The mean preoperative and postoperative range of motion was $132.9^{\circ}$ and $132.5^{\circ}$, respectively. The mean femorotibial angle were varus $0.5^{\circ}$ preoperatively and valgus $2.2^{\circ}$ postoperatively. A radiolucent line was observed in 2 knees; one knee had a stable implant, while in the other knee, patellofemoral arthritis was identified during UKA. Diffuse pain of the knee joint with tenderness of the medial joint line was identified at the follow-up, so conversion to total knee arthroplasty was recommended. No other complications, such as osteolysis, infections, postoperative stiffness, and dislocation, were encountered. Conclusion: The midterm results of fixed bearing UKA were clinically and radiologically satisfactory.

Deep Vein Thrombosis after Total Knee Replacement in a Patient undergoing Korean Medical Rehabilitation (슬관절 전치환술 후 한방재활치료 중 발생한 심부정맥 혈전증 보고)

  • Objectives The objective of this study is to report the deep vein thrombosis (DVT) after total knee replacement (TKR) in a patient undergoing Korean medical rehabilitation. Methods A female patient who had total knee replacement (TKR) was treated by Korean medical rehabilitation with protocol. After fifteen days treatment, deep vein thrombosis (DVT) was suspected. Therefore, CT venography was performed. Results After Korean medical rehabilitation, pain was decreased. But also occurred deep vein thrombosis (DVT) as a complication. Conclusions We present a case of deep vein thrombosis (DVT) after total knee replacement (TKR) in a patient undergoing Korean medical rehabilitation. There is a limit that this study hasn't sufficient number of cases. Further studies will be needed.
