The prevalence of overuse syndrome in the lower extremity including chronic compartment syndrome and stress fracture is increasing with popularity of sports activities. Chronic compartment syndrome is defined as elevation of the interstitial pressure during exertional activities in a closed osseofascial compartment that results in microvascular compromise and operative procedures can be necessary if conservative treatments fail. Stress fracture can be classified as fatigue and insufficiency fracture; stress fracture occurs by repeated strain under abnormal conditions from the patient's activity whereas insufficiency fracture does by those from a process intrinsic to the bone. Most stress fractures occur in the lower extremity, most commonly in the tibial region. Fatigue fractures begin in athletes with the change in their training programs. The radiographic findings are usually diagnostic or at least strongly suggestive and MRI has proven to be a beneficial diagnostic tool for difficult diagnostic cases. Fatigue fractures are treated with a decrease in activity, but surgical procedure may be necessary in those in anterior cortex of the tibial diaphysis.
Nonunion of the hallucal sesamoid usually occurs after stress fracture. In case of persistent disabling symptoms after non-operative treatment, surgery can be an option. We report a rare case of combined stress fractures of the medial bipartite sesamoid and the lateral sesamoid after overuse. Nonunion of the medial sesamoid developed in spite of cast immobilization, and it was eventually healed with curettage and bone grafting.
Operative treatment including a bone graft and an internal fixation was done in a fifteen-year old wrestler with transverse olecranon stress fracture. At 6 weeks after the operation, he could start active muscle strengthening exercise and returned to the previous level of exercise at 6 monthes after surgery. Conservative treatment including resting and muscle strengthening exercise was performed in an eighteen-year old baseball player with oblique olecranon stress fracture. At the follow-up of three months, he could start staged throwing exercise without pain or tenderness. He returned the previous level of throwing following strengthening exercise for 6 months.
Isolated stress fracture of the first rib is rare, and repeated muscular pulling and fatigue of bone is thought to be responsible for this fracture. The diagnosis can be made by taking a thorough history and performing chest roentgenography, computed tomography or magnetic resonance imaging. Conservative treatment generally cures this condition. We report here on a case of exercise-induced isolated stress fracture of the first rib in a non-athlete college student and we review the related articles.
Fractures of isolated spinous processes of cervical and thoracic vertebrae are called as Clay shoveler's fracture. Such fractures related to sports activities are quite rare. In this report, a case of 27-year-old male with isolated spinous process fracture of C7 during golf swing is reported. The patient was prescribed on muscle relaxant and analgesic therapy. Cervical immobilization was maintained for four weeks with a cervical collar. The patient gradually recovered and was asymptomatic. He started playing golf twice a week after 5 months from the onset of pain.
The present study evaluated the systemic oxidative stress status during major orthopedic surgery in dogs. Sixteen dogs presented with various orthopedic diseases involving fractures or luxation of limbs. All patients underwent orthopedic surgery for treatment of fractures or luxation of limbs. A significant increase in the plasma total oxidant status (TOS) and oxidative stress index (OSI) levels in dogs after surgery was observed. Plasma total antioxidant status (TAS) levels were significantly decreased in dogs after surgery. The results of this study suggested that association or relationship in serum between TOS or TAS levels and redox imbalance were caused by surgical trauma in orthopedic disease conditions.
This study was conducted to identify the relationship between depression and subjective/objective health status, and to examine predicting factors on depression in the elderly in Korea. This study was a secondary analysis using the data of Korea National Health and Nutrition Examination Survey(VI-1) 2007. A total of 939 data from the subjects ≥60 years who completed health-related survey were used for analysis. Data were analyzed using SAS (version 9.1) PC program. Depression was identified in the 20.3% of the older subjects. Multiple logistic regression analysis showed that women (OR=2.04), senior high school graduation (OR=0.27) and lowermiddle household income (OR=2.83) were significant associating factors(p<0.05). After adjustment for socio-demographic factors, hypertension (OR=1.93) and asthma (OR=3.32) as objective health status, and stress (OR=7.27), limited activity in daily living due to fracture or joint injury (OR=6.59) and poor self-rate health (OR=1.64) as subjective health status were found as factors predicting depression in the elderly(p<0.05). According to the type of health status, the subjects who had chronic disease or perceived poor physical health were 5.94 times more likely to have disposition to depression than the subjects who had no chronic disease or perceived good physical health (p=0.001). These findings suggest that preventive education and intervention focus on preventing and managing chronic diseases such as hypertension, asthma, fracture and joint injury should be needed to decrease depression in the elderly.
This study recognizes it as a social problem that can be prevented by basic research on safety security design for the elderly, and aims to clearly analyze risk factors for injury. For this study, raw data from the 6th Korean National Health and Nutrition Survey were employed and analyzed. Their social characteristics were classified with standards including gender, education, EuroQoL(human locomotion, normal activity and pain uncomfortableness), the quality of life(PHQ-(1)(2)(4)), recognition on stress, activity restriction status and associated causes(bone fracture and joint injury). Their epidemiological traits were chronic diseases, 'diabetes and hip-joint pain', and health-behavioral traits were 'smoking, breakfast frequency and high-level physical activity status.' In conclusion, it can be found that damage risk of elderly rose in case of female, lower education, inconvenience or limitation, pain or discomfort in locomotion and daily activities, higher recognition of stress, lower interest in work, feeling of depression, despair, and weariness, restriction in activity caused by bone fracture and joint injury, having diabetes and hip-joint pain, smoking, low frequency in having breakfast and high-level physical activities. Based on this study, we envision that an effective solution for injury risk factors caused by the organic relation can be reached in the near future.
도계장에서는 항상 비품이 발생한다. 골절, 피부병변, 착색이상 등 여러 가지 상태의 계육이 비품으로 분류되어 큰피해를 입고 있다. 가까운 예로 미국의 경우 이에 대한 피해액이 연간 5천만 달러로 추산되고 있다. 국내에서는 이러한 문제 때문에 육계 담당 부서에서는 항상 비품에 대해서 스트레스를 받고 있다. 하지만 이러한 문제들이 정말 스트레스가 되는 원인은 사육 중에 발견되기보다는 도계 후에 발견되며, 이에 따라상품가치가 현저히 감소하고 추가적인 작업이 요구되기 때문일 것이다. 대부분의 회사들이 이 문제를 접하게 되면, 주로 곰팡이 독소 흡착제를 사용하는 것이 유일한 대책인 것으로 생각한다. 이런 경우엔 약품에 대한 효과는 잘 모르겠지만, 필자의 느낌으로는 일정한 시간이 흘러 자연스럽게(?) 해결되는 과정을 걷는 듯 하며, 관련 부서에서는 이 약제의 효과에 대해 판단하기보다는 일단 직면한 문제가 해결되었
다고 결과론적으로 만족하는 듯 하다. 필자는 이 지면을 통해 그 동안의 관점과는 다른 측면에서 이런 문제의 원인과 그 대책에 대해 간략히 논하고자한다. 도계장에서는 항상 여러 가지 비품들이 발생한다. 이 모든 것들을 논하기에는 너무 범위가 넓기 때문에 여기서는 독자들의 이해를 위해 비품들 중에서 등 부위의 상처와 복부의 상처(배꼽 닭)에 대해 국한하도록 하겠다. 아마도 이 부분외의 다른 비품들은 대부분 그 원인을 쉽게 추정할 수 있기 때문이다. 먼저, 독자들에게 질문하고 싶은 것이 있다. 이러한 비품들의 직접적인 원인은 무엇인가? 앞서 얘기한 것과 같이 각 회사들의 대책으로 이용되는 곰팡이 독소 흡착제를 보면, 곰팡이 독소가 가장 직접적인 원인으로 생각할 수도 있다. 하지만 결론적으로 직접적인 원인은 외상이다. 즉, 외부의 물리적인 작용에 의한 상처인 것이다. 이런 측면에서 볼 때 가장 직접적인 원인은 외상이며, 이 외에 깃털발육 이상(혹은 저하), 바닥상태 불량 등과 같은 사양관리 이상과 면역 억제성 질환 및 지혈능력의 저하 등이 직접적인 원인으로 인한 문제들을 더욱 악화시키는 간접적인 원인으로 작용한다.
