  • 최근 한국인의 주요 사망원인 중 하나로 부정맥이 부각되고 있다. 심방조기수축(PAC:Premature Atrial Contraction)은 심방이 동방결절의 명령이 있기 전에 수축해 버리는 것이다. 심방조기수축은 일시적으로 유발하였다 사라지곤 할 수 있기 때문에 심한 증상이 없다면 생명에 위협을 가하진 않지만 반대의 경우에는 위험할 수 있다. 따라서 비정상적인 심장 박동이 발생하면 이를 검출하여 조기에 부정맥을 진단할 수 있는 방법이 필요하다. 이를 위해 대상의 ECG 신호로부터 QRS패턴에 해당하는 특징들을 추출하였고 특징들을 이용하여 심방조기수축 파형을 분류한다. 오류 역전파 기반으로 특징들을 훈련하며 가중치와 바이어스값을 구한뒤 이를 이용하여 정상파형과 심방조기수축 파형을 분류한다.

Effects of Inspiratory Muscle Training on Diaphragm Thickness, Pulmonary Function, and Chest Expansion in Chronic Stroke Patients (흡기근 저항훈련이 만성 뇌졸중 환자의 횡격막 두께와 폐기능에 미치는 효과)

  • 연구목적: 본 연구는 만성 뇌졸중 환자를 대상으로 흡기근 저항훈련이 횡격막 두께와 폐기능 및 흉곽 확장에 미치는 효과를 알아보고자 수행하였다. 연구방법: 연구 대상자는 총 29명(남자 17명, 여자 12명)으로 흡기근 저항훈련군(15명)과 대조군(14명)으로 분류하였다. 모든 대상자는 6개월 이상된 만성 뇌졸중 환자로 일반적인 신경발달치료를 받고 있으며, 같은 기간 동안 흡기근 저항훈련군에 역치부하 흡기근육 단련기(threshold IMT device)를 제공하고 주 3회${\times}$1회 20분씩 6주간 시행하였다. 마비측과 비마비측 횡격막 두께측정을 위해 초음파의 7.5MHz linear probe를 사용하여 최대 흡기시(Tdi.con)와 휴식시(Tdi.rel)의 두께를 측정하고 수축률(TR)을 계산하였다. 또한 폐 활량계를 사용하여 노력성 폐활량을 측정하였으며, 줄자를 사용하여 흉곽 확장을 측정하였다. 연구결과: 6주간 중재 후 흡기근 저항훈련군에서 최대흡기시 횡격막 두께(Tdi.con)와 수축률(TR)은 유의한 증가를 보였다(p<.05). 1초간 노력성 호기량 ($FEV_1$)과 최대 호기 속도(PEF)도 유의한 증가를 보였으나(p<.05), 노력성 폐활량(FVC)과 1초간 노력성 호기량의 노력성 폐활량에 대한 비($FEV_1$/FVC), 흉곽 확장은 유의한 증가는 보이지 않았다(p>.05). 결론: 본 연구는 만성뇌졸중 환자를 대상으로 흡기근 저항훈련의 적용이 횡격막의 수축력과 폐기능 및 흉곽 확장력을 향상시켜 호흡근의 협응력을 증가시키고, 비활동성으로 인해 감소된 운동내성을 증가하게 함으로써 향후 재활에서 만성 뇌졸중 환자에게 2차적인 기능향상에 도움을 줄수 있을 것으로 보여진다.

EMG Power Spectrum Analysis of the Medial Gastrocnemius and the Soleus Under the Fatigued Condition (피로조건하에서의 내측 장딴지근과 가자미근의 근전도 스펙트럼 분석)

  • 연구목적 이 연구의 목적은 피로유발수축후 장딴지근과 가자미근의 근전도 power spectrum 중앙경항치를 비교 검사하는 것이다. 실험대상 열 여섯명의 자원자(남자 10, 여자 6)를 대상으로 하였다. 남자 대상자의 연령범위는 25세에서 33세(평균 $28.6{\pm}2.5$)였고 여자 대상자의 연령범위는 26세에서 31세($27.8{\pm}1.9$)였다. 실험방법 실험대상자는 피로유발수축전후 50% 최대등척성근수축을 수행하였다. 자료수집과 분석을 위해 LabVIEW 소프트웨어를 사용하였다. 결과 t-검정결과 내측 장딴지근에서는 피로유발수축후 주파수의 중위값과 평균이 통계적으로 유의하게 감소하였으나 가자미근에서는 통계적으로 유의한 차이가 없었다. t-검정결과 두 근육간 주파수 감소의 차이는 통계적으로 유의한 차이가 있었다. 토의 및 결론 이 결과들은 피로유발수축후 근전도 power spectrum 중앙경향치가 저주파대역으로 감소함을 나타내었다. 이 연구는 중앙주파수와 평균주파수가 피로지수로서의 신뢰도와 타당도가 뛰어나며 객관적 측정과 훈련효과 평가로 사용할수 있음을 보였다.

Effects of Isometric Contraction Training by Electrostimulation on Type I and II Hindlimb Muscles in Cerebral Ischemia Model Rats (전기자극을 이용한 등척성 수축훈련이 뇌허혈 유발 쥐의 환측 Type I, II 근육에 미치는 영향)

  • Purpose: The purpose of this study was to examine the effects of cerebral ischemia on Type I(soleus) and Type II(plantaris, gastrocnemius) muscles, and to determine the effects of isometric contraction training by electro- stimulation on Type I and II muscles in cerebral ischemia model rats. Method: Twenty-five male Sprague-Dawley rats were randomly divided into four groups: ST(stroke), STES(stroke+electrostimulation), SH(sham) and SHES (sham+electrostimulation). The ST and STES groups received a transient right middle cerebral artery occlusion operation. The SH and SHES groups received a sham operation. The STES and SHES groups had daily isometric contraction training by electrostimulation(100Hz, 45mA, 7.5V) on hindlimb muscles for 7days. Result: Plantaris and gastrocenmius muscle weight, myofibrillar protein contents of soleus and gastrocnemius, and the muscle fiber cross-sectional area of gastrocnemius in the ST group significantly decreased compared with the SH group. Soleus, plantaris, gastrocnemius muscle weight, myofibrillar protein contents of soleus and gastrocnemius, and the Type I muscle fiber cross-sectional area of soleus and the Type II muscle fiber cross-sectional area of gastrocnemius in the STES group significantly increased compared with the 57 group. Conclusion: Hindlimb muscle atrophy occurs after acute stroke and isometric contraction training by electrostimulation during early stages of a stroke attenuates muscle atrophy of Type I and Type II muscles.

The purpose of this study was to find out that combined concentric with eccentric resistance training program using Thera-Band was more effective than only concentric resistance training program on shoulder rotation torque max and peak power. 30 female students were randomly assigned to 3 equal group; concentric and eccentric, concentric, non training group. Subjects were tested in concentric and eccentric torque max and peak power of internal and external rotation using a CON-TERX isokinetic dynamometer. Subjects in training group participated in their regular five times a week for 4 weeks. After exercise, in concentric and eccentric training group, concentric and eccentric torque max, and peak power of internal rotator at 60 deg/sec were increased significantly. concentric peak power at 120 deg/sec were increased significantly. In concentric training group, only concentric peak power of internal rotator at 60 and 120 deg/sec was increased. In conclusion, we found out that combined concentric with eccentric resistance training program using Thera-Band was more effective than only concentric resistance training program on shoulder rotation torque max and peak power.

Improvement effect of Functional Myopia by Using of Vision Training Device(OTUS) (Vision Training Device(OTUS)적용에 따른 기능성 근시의 개선 효과)

  • This study is about the development of ICT-based wearable devices for vision recovery that can cause functional myopia improvement through accommodation training. Vision Training Device(OTUS) is a head mount type wearable device, which naturally stimulates the contraction and relaxation of the ciliary muscles of eye. Users can conduct customized vision training based on personal vision information stored through the device. In the experiment, the effects of improvement of the symptoms by the accommodation training were compared and analysed for the two groups (16 comparative group and 16 accommodation training group) after causing functional myopia. The result showed the functional myopia improved average 0.44D±0.35 (p<0.05) at the accommodation training group compared to the comparative group. This study proved the effectiveness of vision training device(OTUS) on functional myopia, but further clinical trials are judged necessary to prove the possibility of long-term control of the functional myopia.

Effect of Exercise Training on Aging Atrophyin Rat Skeletal Muscle III. Effect of Short Term Exercise Training for Senile Rat (흰쥐 골격근의 노화성 위축에 대한 운동훈련의 영향 III.노화 흰쥐에 적용한 단기간의 운동훈련의 영향)

  • The present study was designed to examine effect of short term treadmill and weight-training on aging arophy in the rat skeletal muscle. Male rats of 24 months old were used. Each groups included control, treadmill and weight-training for 4 weeks by using treadmill apparatus and body press apparatus. The histo and cytochemical, ultrastructural and stereological changes in senile skeletal muscles of the rat were observed in the present study. During the training period the body weight and muscular weight in all groups remained constant. The volume density of muscle fiber type IIC and IIB were increased, that of type IIA was decreased, but type I remained constant in treadmill-training group. In weight-training rat, the density of type IIA and IIB were increased, both those of type IIC was decreased. But, all changes of muscle fiber type is not significant. Senile control group some usual formation of mild contraction band, liposuscin pigment and muscular splitting were observed. After treadmill-training, histological and ultrastructural changes occurred in the muscle fiber, such as irregularity of the sarcolemma, interfibrillar vacuolization, longitudinal splitting, and widened I-bond. After weight-training, the changes occurred in the trained muscle fiber, such as appearances of many lysosomes and autophagic vacuoles, severe contraction band, and breakup of myofibrils. Histo and cytochemical studies showed that the activities of succinic dehydrogenase and acid phosphatase remained constant, activities of $Mg^{++}$-ATPase decrease with training. Stereological changes were not observed in the volume and numerical density of all subject component, but the surface density of mitochondrial inner membrane was increased with treadmill-training. These experimental results suggested that endurance training during short-term may result in the adaptible response in senile skeletal muscles. On the other side, weight-training is bad for senile skeletal muscle.

  • Rehabilitation of upper limb motor function of hemiplegic patient must maintain interest and demand a device for a quantitative evaluation of rehabilitation training. In this paper, we developed the device that is composed of arm cradle, handle, and balance ball for rehabilitation exercise. We have performed experiment for validity as to whether to use the rehabilitation device when tilting the upper extremity training device developed to measure changes in the EMG signal to the main upper limb muscles for 7 healthy volunteers. We have analyzed muscle activation signals on agonist and antagonist as a reference in the muscle contraction and relaxation in the upper limb extension and flexion when the balance-handle device is tilted to front-rear and left-right. The experimental results showed that a tendency of muscle activation of biceps, triceps, and deltoid used in upper limb motor function of hemiplegic patients from extension and flexion evaluation items of Fugl-Meyer Assessment(FMA). These results may be helpful for rehabilitation training for upper limb motor function of hemiplegic patients by utilizing a developed unit.

The Effects of 12-Week Training for the Physical Fitness and Cardiovascular Factors to Examine Physical Fitness on Firefighters Test-Taker (소방공무원 수험생의 체력검정을 위한 12주간 훈련이 체력요인, 심혈관계요인에 미치는 영향)

  • The purpose of this study was to investigate the effects of 12-week training on changes in physical fitness and cardiovascular factors for firefighters. For this purpose, 40 men in their 20s and 30s who agreed to participate voluntarily were recruited. They were divided into four groups: the firefighters' physical fitness test training group (hereinafter referred to as PT group), firefighters' physical fitness test and aerobic training group (hereinafter referred to as PT+AR group), firefighters' physical fitness test and both aerobic and anaerobic training group (hereinafter referred to as PT+CO group). Physical fitness factors (grip strength, back muscle strength, seated forward bend, standing long jump, sit-ups, 20-meter shuttle run), cardiovascular factors (total cholesterol, triglycerides, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, glucose, waist circumference, systolic blood pressure, diastolic blood pressure) and the relationship between Framingham Heart Risk Score and physical/cardiovascular factors were compared and analyzed, and the following conclusions were obtained. Aerobic training, anaerobic training, and combined training, including 12 weeks of firefighter physical examinations, all had positive effects on fitness and cardiovascular factors, which would be an appropriate way for firefighter examinees to improve physical strength and reduce the risk of cardiovascular disease.
