Development of Task Guidelines for Hospice Team Members (호스피스 팀 구성원의 직무지침 개발)

  • Purpose : The purpose of this study was to develop task guidelines for hospice team members. The task range of all personnel who work for hospice institutions was identified, and a tool describing roles and tasks of the hospice team members was developed based on review of related literature, both domestic and international. Methods : The content validity of the tool was assured by an expert panel through two phases of discussion. The first phase of the study included a survey conducted from December 1999 to January 2000 for a total of 126 hospice experts and practitioners affiliated with domestic hospice institutions. The second phase of the study included 35 subjects. The data were collected using a survey when the investigators visited each hospice institution. The data were analyzed using descriptive statistics. Results : 1) In the first phase of the study, all items scored over 80 points in CVI were selected as the roles and tasks of hospice coordinator, nurse, pastor, social worker, pharmacist, nutritionist, therapist, volunteer, and nurse aide. However, two items were excluded because they scored below 80 points: an item describing eligibility of a physician, a person who has a license for managing anesthetic agents was scored as 78.6 points, and an item describing the eligibility of the team leader of volunteers, a person educated at the graduate level was scored as 74.7 points. 2) In the second phase of the study, all items scored over 80 points in CVI were selected as the roles and tasks of hospice nurse, pastor, social worker, pharmacist, nutritionist, therapist, volunteer, and nurse aide. Of the roles of the hospice coordinator, however, the item scored as 77.9 points, assess and plan a patients physical, social, emotional, and spiritual status, and, of the roles of the team leaders of volunteers, the item scored as 78.6, attend a team meeting once a week and participate in building an standard nursing plan for patients were included in the tool since they scored over 80 points in the first phase of the study. Conclusion : The developed task guideline should be further modified and revised based on the findings of a preliminary application in the actual field. There is also a need of continuous research for developing more culturally-appropriate task guidelines for hospice team members.

Safety Evaluation of Microbiological and Aflatoxin of Traditional Dried Persimmon (곶감의 미생물 및 aflatoxin에 대한 안전성 평가)

  • To evaluate microbiological and aflatoxin safety on traditional dried persimmon, a total of 315 samples were collected from 105 farms. The collected samples were assessed on aflatoxin and microorganisms (Aerobic plate count, coliform count, Escherichia coli, Escherichia coli O157:H7, Salmonella spp., Staphylococcus aureus, Listeria monocytogenes, Bacillus cereus). The the APC of sliced dried persimmon, dried persimmon, and semi dried persimmon were $3.93{\pm}0.96$, $2.12{\pm}0.93$, and $1.50{\pm}1.08{\log}\;CFU/g$, respectively. S. aureus was detected in 40.0% of sliced dried persimmon, 29.5% of dried persimmon, and 23.5% of semi dried persimmon. E. coli recovered from dried persimmon and semi dried persimmon was 6.6%, and 2.9%, respectively. However, E. coli O157:H7, Salmonella spp., and L. monocytogenes were not detected. According to the result of aflatoxin by ELISA and UPLC, aflatoxin was not detected in any sample. These data suggested that safety management system should be introduce to the farms producing traditional dried persimmon to enhance the safety of traditional dried persimmon.

Effects of the 26-weeks Obesity Management Program on Obesity Index, Self-esteem, Self-efficacy, and Body Image among Obese Elementary School Children (초등학교 비만아동을 위한 26주-비만관리프로그램이 비만지수와 자아존중감, 자기효능, 및 신체상에 미치는 효과에 미치는 효과)

  • This study was conducted to develop a 26-week obesity management program and examine the effects of this program on obesity index (body mass index, fat mass percentage, and triglyceride) and psychological variables, self-esteem, dietary self-efficacy, physical activity self-efficacy, and body image among 46 4th-6th obese elementary school children with a BMI above $20kg/m^2$ using a nonequivalent control group quasi-experimental design (experimental group=23, control group=23). This program included a 12-week intensive course (12 times for group education on obesity, exercise, and nutrition, 36 times for practicing diet and physical exercise, and 5 times for psychological counseling) and a 14-week follow-up course (3 times for group education, 3 times for checking diet and physical exercise, and 1 time for psychological counseling. The collected data were analyzed by the Kolmogorov-Smirnov test, chi-squared test, Fisher's exact probability, t-test, and ANCOVA. There were above normal values in body mass index ($20kg/m^2$), fat mass percentage (15-20%), and triglyceride tolerance ($90mg/d{\ell}$) in experimental group and control group. The body mass index (F=94.869, p<.001), fat mass percentage (F=37.361, p<.001), and triglyceride tolerance (F=7.907, p= .007) were significantly lower, while self-esteem (F=19.661, p<.001), dietary self-efficacy (F=16.942, p<.001), physical activity self-efficacy (F=9.363, p= .004), and body image (F=9.782, p= .003) were significantly higher in the experimental group than the control group after 26 weeks. This study provides evidence that the 26-weeks obesity management program is an effective nursing intervention for managing obesity and psychological variables of obese elementary school children.

A Study on Radiation Management Status and Exposure Anxiety Awareness of Dental Hygienist (치과위생사의 방사선 안전 관리 실태 및 피폭 불안감 인식)

  • This study intends to improve the radiation safety management and the recognition for handling radiation using structured questionnaires to dental hygienists working at Jeollabuk-do from September 1 to October 31 in 2014. As a result, 63% of respondents have not received education for radiation safety management. Moreover, the practical degree for radiation safety management was $2.58{\pm}1.11$, while the degree of knowledge was $3.74{\pm}0.83$ of total 5.0. The results of insecurity for radiation danger were high as $3.88{\pm}0.92$, and insecurity for fetus during pregnancy shows the highest value as $4.43{\pm}0.71$. From the results of statistical significance level, the knowledge degree of radiation safety management is affected by total numbers of radiograpy for a day (p<0.05), and the practical degree of radiation safety management is affected by age group, academic background, monthly income, continuous service year, practice area, present position, and status of radiography in present (p<0.05). In addition, the knowledge degree of radiation safety management have a negative correlation (r=-0.232) with the practical degree, but have a positive correlation (r=0.262) with the insecurity for radiation danger. The high knowledge degree of radiation safety management (${\beta}=0.252$, p<0.001) and the short radiography work period (${\beta}=-0.341$, p<0.05) were the influential factors to the insecurity for radiation danger. Consequently, countermeasures are necessary to encourage dental hygienists to put their radiation safety management knowledge into the practice and to reduce the insecurity degree for radiation danger. Furthermore, it is important to prevent psychological and physical risks by radiation exposure through the improvement of radiation safety management level and recognition for handling radiation to improve medical environment.

Dental Service System and Oral Health Providers in Correctional Institutions (구금시설의 치과진료체계 및 구강보건의료인력 현황 조사)

  • The purpose of this study is to analyze the dental service system in correctional institutions and 10 find the factors for improving inmates' oral health. This study is comprised of document review, telephone and questionnaire survey. The subjects of questionnaire survey are public health dentists and doctors in correctional institutions. They responded to questionnaire and the survey was collected from previous research and selected information about the dental service system. The findings of the study were as follow : Documentary survey 1. According to 2004's study, there are 42 dental offices in 46 all correctional institutions. 2. Criminals who took an health examination occupied 69.0% when committed to a jail in 2002's study. Majorities of them(81.5%) responded that they didn't take any oral examination. Telephone & Questionnaire survey 1. Full-time public health dentists are 26 in 2009. There is no correctional institution having oral health providers in 26 correctional institutions surveyed. 2 About 10 patients use the dental services in a day. Part-time dentists visit 4 times a month as average in 80% of institutions. 40% of institutions responded dental treatments can't be progressed conveniently because of the lack of oral health providers. 3. 80% of respondents answered that it is hard to cure prisoners, and that's because they are forbidden to get out of the institutions. 4. Only 20% of correctional institutions offered the oral hygiene instructions. There is no regular oral hygiene education for all inmates. 5. They need to increase the number of oral health providers.

The Oral Health Behaviors of Workers by Dental Caries (근로자의 치아우식수에 따른 구강보건형태)

  • The purpose of this study is to investigate into company workers' general characteristics and their awareness or oral health examinations. Answer sheets for questionnaire for 267 industrial workers at Changwon city, Korea, were collected and analyzed using SPSS 12.0. Among workers who haven't visited a dental clinic for last one year, 75.6% of them didn't have dental caries. 74.0% of workers who haven't received dental scaling didn't have dental caries. 84.4% of workers who thought of themselves as having good oral health had good oral health and none of them had dental caries. 9.1% of people who had dental caries of 4 or more had bad breath. 73.4% of workers didn't need to treat dental caries, while scaling in 57.7% of them was required. To reform and improve of the system, incremental dental health care system for industrial workers is needed. Oral health education is needed to increase the motivation of industrial workers to control their basic disease.

Atmospheric Aerosol Monitoring Over Northeast Asia During 2001 from MODIS and TOMS data (MODIS와 TOMS자료를 이용한 2001년 동북아시아 지역의 대기 에어로졸 모니터링)

  • The spatial and temporal variations of aerosol optical depth (AOD) over Northeast Asia regions have special importance in the aerosol research for estimation of aerosol radiative forcing parameters and climate change. Aerosol optical and physical properties (AOD and ${\AA}$ngstrom parameter) have been investigated by using Moderate Resolution Imaging Spectroradiometer (MODIS) and Total Ozone Mapping Spectrometer (TOMS) Aerosol Index (AI) to estimate aerosol characteristics over the study region during 2001. Additionally, aerosol characteristics over the Korean peninsular during Aerosol Characteristic Experiment in Asia (ACE-Asia) Intensive Observation Period (IOP) have been investigated by using satellite observations. The results showed that the daily-observed aerosol data indicate seasonal variations with relatively higher aerosol loading in the spring and very low during the winter. The typical Asian dust case showed higher AOD (>0.7) with lower Angstrom exponent (<0.5) and higher AI (>0.5) that is mainly due to the composition of coarse particles in the springtime. Mean AOD for 2001 at 4 different places showed 0.65$\pm$0.37 at Beijing, 0.31$\pm$0.19 at Gosan, 0.54$\pm$0.26 at Seoul, and 0.38$\pm$0.19 at Kwangju, respectively. An interesting result was found in the present study that polluted aerosol events with small size dominated-aerosol loading around the Korean peninsular are sometimes observed. The origin of these polluted aerosols was thought to East China. Aerosol distribution from satellite images and trajectory results shows the proof of aerosol transport. Therefore, aerosol monitoring using satellite data is very useful.

Prevalence of Pathogenic Bacteria in Livestock Manure Compost and Organic Fertilizer (가축분퇴비와 유기질비료에서 병원성박테리아의 분포도 분석)

  • In recent years, there has been an increasing public concern about fecal contamination of water, air and agricultural produce by pathogens residing in organic fertilizers such as manure, compost and agricultural by-products. Efforts are now being made to control or eliminate the pathogen populations at on-farm level. Development of efficient on-farm strategies to mitigate the potential risk posed by the pathogens requires data about how the pathogens prevail in livestock manure composts and organic fertilizers. Microbiological analysis of livestock manure composts and organic fertilizers obtained from 32 and 28 companies, respectively, were conducted to determine the total aerobic bacteria count, coliforms, Escherichia coli count and the prevalence of Staphylococcus aureus, Bacillus cereus, Salmonella spp., Escherichia coli O157:H7, Listeria monocytogenes, and Cronobacter sakazakii. The total aerobic bacteria counts in the livestock manure composts and organic fertilizers were in the range of 7 to $9log\;CFU\;g^{-1}$ and 4 to $6log\;CFU\;g^{-1}$, respectively. In the livestock manure composts, coliforms and E. coli were detected in samples obtained from 4 and 2 companies, respectively, in the range of 2 to $5log\;CFU\;g^{-1}$ and $2log\;CFU\;g^{-1}$. In the organic fertilizers, coliforms and E. coli were detected in samples obtained from 4 and 1 companies, respectively, in the range of 1 to $3log\;CFU\;g^{-1}$ and $2log\;CFU\;g^{-1}$. In 3 out 32 compost samples, B. cereus was detected, while other pathogens were not detected. In 28 organic fertilizers, no pathogens were detected. The complete composting process can result in the elimination of pathogens in livestock manure compost and organic fertilizer. The results of this study could help to formulate microbiological guidelines for the use of compost in environmental-friendly agriculture. This research provides information regarding microbiological quality of livestock manure compost and organic fertilizer.

Estimation of Carbon Emission and LCA (Life Cycle Assessment) From Sweetpotato (Ipomoea batatas L.) Production System (고구마의 생산과정에서 발생하는 탄소배출량 산정 및 전과정평가)

  • LCA (Life Cycle assessment) was carried out to estimate on carbon footprint and to establish of LCI (Life Cycle Inventory) database of sweetpotato production system. Based on collecting the data for operating LCI, it was shown that input of organic fertilizer was value of 3.26E-01 kg $kg^{-1}$ and it of mineral fertilizer was 1.02E-01 kg $kg^{-1}$ for sweetpotato production. It was the highest value among input for sweetpotato production. And direct field emission was 2.47E-02 kg $kg^{-1}$ during sweetpotato cropping. The result of LCI analysis focussed on greenhouse gas (GHG) was showed that carbon footprint was 4.05E-01 kg $CO_2$-eq. $kg^{-1}$ sweetpotato. Especially $CO_2$ for 71% of the GHG emission and the value was 2.88E-01 kg $CO_2$-eq. $kg^{-1}$ sweetpotato. Of the GHG emission $CH_4$, and $N_2O$ were estimated to be 18% and 11%, respectively. It might be due to emit from mainly fertilizer production (32%) and sweetpotato cultivation (28%) for sweetpotato production system. $N_2O$ emitted from sweetpotato cultivation for 90% of the GHG emission. With LCIA (Life Cycle Impact Assessment) for sweetpotato production system, it was observed that the process of fertilizer production might be contributed to approximately 90% of GWP (global warming potential). Characterization value of GWP and POCP were 4.05E-01 $CO_2$-eq. $kg^{-1}$ and 5.08E-05 kg $C_2H_4$-eq. $kg^{-1}$, respectively.

Detection of Arctic Summer Melt Ponds Using ICESat-2 Altimetry Data (ICESat-2 고도계 자료를 활용한 여름철 북극 융빙호 탐지)

  • As the Arctic melt ponds play an important role in determining the interannual variation of the sea ice extent and changes in the Arctic environment, it is crucial to monitor the Arctic melt ponds with high accuracy. Ice, Cloud, and Land Elevation Satellite-2 (ICESat-2), which is the NASA's latest altimeter satellite based on the green laser (532 nm), observes the global surface elevation. When compared to the CryoSat-2 altimetry satellite whose along-track resolution is 250 m, ICESat-2 is highly expected to provide much more detailed information about Arctic melt ponds thanks to its high along-track resolution of 70 cm. The basic products of ICESat-2 are the surface height and the number of reflected photons. To aggregate the neighboring information of a specific ICESat-2 photon, the segments of photons with 10 m length were used. The standard deviation of the height and the total number of photons were calculated for each segment. As the melt ponds have the smoother surface than the sea ice, the lower variation of the height over melt ponds can make the melt ponds distinguished from the sea ice. When the melt ponds were extracted, the number of photons per segment was used to classify the melt ponds covered with open-water and specular ice. As photons are much more absorbed in the water-covered melt pondsthan the melt ponds with the specular ice, the number of photons persegment can distinguish the water- and ice-covered ponds. As a result, the suggested melt pond detection method was able to classify the sea ice, water-covered melt ponds, and ice-covered melt ponds. A qualitative analysis was conducted using the Sentinel-2 optical imagery. The suggested method successfully classified the water- and ice-covered ponds which were difficult to distinguish with Sentinel-2 optical images. Lastly, the pros and cons of the melt pond detection using satellite altimetry and optical images were discussed.
