The etiologic factor of senile cataract and mechanism of occurance of diabetic cataract (노인성 백내장의 원인과 당이 백내장 발생에 미치는 영향에 대한 고찰)

  • The most common cause of blindness is cataract. The possible causes, radiation, sugar, drugs, trauma, nutrition, congenital, secondary to eye disease etc. Generally, the opacification of the lens are the result fa oxidation of the lens fibers. The basis for lens transparency is the structural integrity of lens fibers. Opacities were occur if there is a significant amount of high molecular weight protein aggregates. If Ca ions combined with lens ${\alpha}$-crystallin proteins, the lens fibers were aggregated by high molecular weight proteins and the lens were opacified. If Ca ions detached from lens ${\alpha}$-crystallin proteins, the lens fibers were aggregated by low molecular weight proteins and the lens were re-cleared. We need to find out the variety of factors can initiated the process of age-related cataract. And to understanding the mechanism how the various kind, of diabetic cataracts occur.

Prevalence of Cataract with Different Type of Lens Opacity in the Korean Population (한국 성인의 수정체 혼탁 유형에 따른 백내장의 유병률)

  • Purpose: This study calculated the prevalence of cataract in Korean and examined corrected eyesight with different types of opacification of crystalline lens. Methods: Using the data of the national health and nutrition examination survey (NHANES), population-based and cross-sectional research were designed in this study. After dividing total 4,977 persons into different groups by age, from 40 to 95 and by sex, the prevalence of cataract with different types of opacification was investigated. The average age of the total subjects was $58.28{\pm}12.07$, and the number of men and women were 2,142(43.0%) and 2,835(57.0%), repectively. Eyesight was calculated when it was less than 0.63. Results: Among total 4,977 subjects, 40.6% (men 17.5%, women 23.1%) had cataract. Different prevalence rates were found with different types of cataract; nuclear cataract (56.3%), cortical cataract (23.4%), mixed cataract (16.2%), anterior subcapsular(3.0%), and posterior subcapsular (1.1%) in order. There was no difference among men and women by the types. And those in their 40's showed a higher prevalence of cortical cataract than those in the other age groups. Most of subjects whose corrected eyesight is less than 0.63 had anterior subcapsular cataract. Conclusions: The type of cataract having the highest prevalence was nuclear opacity. Moreover, epidemiologic investigation and prevention policy with different type of lens opacity seems to be required.

Endophthalmitis after Removal of an Intralenticular Foreign Body in Place without Symptoms for 20 Years (20년 동안 증상없이 잔존한 수정체 내 이물제거 후 발생한 안내염)

  • Purpose: We report a case of postoperative endophthalmitis in the cataract patient, associated with removal of an intralenticular foreign body that had remained in place without symptoms for 20 years. Case summary: A 45-year-old male visited our outpatient clinic complaining of gradual visual loss in his right eye over the past 3 months. In slit-lamp examinations, anterior capsular opacification, nuclear sclerosis, and posterior subcapsular opacity were observed in the right eye. Twenty years before, a tiny metallic projectile had hit his right eye, but slit-lamp examination at the time of injury did not reveal any intraocular foreign body. We decided to undergo cataract surgery. During phacoemulsification, a metallic foreign body was found in the lens and safely removed; then an intraocular lens was implanted. As hypopyon was evident 3 days later, we injected intravitreal antibiotics and applied fortified antibiotic eye drops to the right eye. The anterior chamber inflammation improved and the best-corrected visual acuity recovered to 1.0. Conclusions: Surgeon should be aware of that endophthalmitis could develop after cataract surgery with removal of an intralenticular foreign body that had been in place for 20 years. But did not trigger inflammation or cause any symptoms as the cataract progressed.

Screening of Inhibitory Effect on Aldose Reductase of Korean Herbal Medicines and Preventive Effect of Catalpa bignonioides against Xylose-induced Lens Opacity (I) (한국산 한약재 (생약) 추출물의 알도즈 환원 효소 억제 효능 검색과 꽃개오동의 수정체 혼탁 억제 (I))

  • Aldose reductase (AR), the principal enzyme of the polyol pathway, has been shown to play an important role in the diabetic complications, including diabetic cataract. The inhibitors of AR, therefore, would be potential agents for prevention of diabetic complications. As part of our ongoing project directed toward the discovery of preventive and/or treatment for diabetic complications, 48 Korean herbal medicines have been investigated with an in vitro evaluation system using aldose reductase inhibitory activities. Of these, 12 herbal medicines exhibited a significant inhibitory activity against aldose reductase. Particularly, seven herbal medicines, i.e., Eurya japonica, Chrysanthemum indicum, Akebia quinata, Saururus chinensis, Catalpa bignonioides, Lonicera japonica, Vitex rotundifolia showed two times more potent inhibitory activity than the positive control 3.3-tetramethyleneglutaric acid (TMG). In addition, Catalpa bignonioides showed the retardation of cataract-opacification of the lens of the eye under diabetic condition by xylose. Therefore, this result may provide a potential therapeutic approach for preventing and treating diabetic cataracts.

Risk Factors Associated with Cataract by in Middle-aged and Older Korean Adults (한국인의 장년층과 노년층의 백내장 위험 요인)

  • Purpose: This study was found the risk factors of cataract by gender in elder and older Korean adults. Methods: We investigated the data for 5,024 (men 2,163 people, women 2,861 people) people, aged 40-95 years, from the Korean National Health and Nutrition Examination Survey. We divided into two groups (group 1: 40-64 years, group 2: 65-94 years). We divided into two groups (group 1: 40-64 years, group 2: 65-94 years). Cataract was decided in case of at least one eye with lens opacity. We presented significant odds ratio (OR) increase of cataract according to the obesity and socioeconomic information including house income and education level after adjusting for diabetes, hypertension, high triglyceride, outside active, smoking and drinking habits in two groups with distinction of sex. Results: House income (or education) was strongly related to the prevalence of agerelated cataract in all groups. In models adjusting for all other related factors, cataract patients with lower income and education had a higher prevalence of cataract (group1: OR, 1.84[1.17-2.91], OR, 3.00[1.90-4.74], group 2: OR, 3.47[2.53-4.74], OR, 7.44[5.41-10.23] for men. In comparison, for women, cataract patients with lower income, lower education and obesity had a higher prevalence of cataract (group1: OR, 1.72[1.14-2.60], OR, 4.48[2.72-7.38], OR, 1.40[1.10-1.82], group 2: OR, 2.32[1.70-3.16], OR, 29.99[20.31-44.28], OR, 1.27[1.04-1.54]). Conclusions: Factors of low socioeconomic status were associated with age-related cataract in Korean. Obesity-cataract association was stronger in women.
