A Patellar Surgery Haptic Simulator for Veterinary Training (수의학 훈련을 위한 슬개골 수술 햅틱 시뮬레이터)

  • Patella surgery of small animal is an important veterinary surgery that the veterinarian should saw and drill the dislocated patella in order to fix the corrected position. However, the animal protection laws restrict the veterinarian students' chances for the practice and training of the patella surgery. This paper proposed a haptic based patella surgery simulator for veterinarian students. We modelled force feedback methods in order to provide best similar haptic feedbacks to the real drilling feedbacks in the patella surgery. The proposed patella drilling simulator provides haptic interface as a drill and a workbench in order to provide best surgery experiences. We conducted the performance evaluations in order to prove usability of the proposed patella surgery interface.

  • 효과적인 수술 훈련 시뮬레이터를 구축하기 위해서는 사실적인 훈련 환경을 제공하는 것뿐만 아니라 훈련 결과를 바탕으로 사용자의 숙련도를 객관적으로 측정하고 평가하는 과정 또한 중요하다. 이를 위해 본 연구는 수술 훈련 중 큰 비중을 차지하는 절개 동작에 대한 정량적 평가 척도를 제공하는 것을 목표로 한다. 사용자가 가상 장기 모델에 대해 절개를 수행하는 동안 평가 시스템은 절개 경로와 깊이를 일정 간격으로 샘플링하여 저장하고, 이를 두 곡선 간의 유사성 측정 알고리즘을 통해 훈련 시나리오 상에 정의된 표준 절개 경로와 깊이, 속도를 각각 비교한다. 이렇게 계산된 두 경로 사이의 거리가 가까울수록 유사성이 높은 것으로 간주하며, 사전에 설정된 기준치 이상의 유사성을 기록할 경우 훈련 목표를 충족한 것으로 판단할 수 있다. 본 연구에서는 단순 거리 측정에 의존한 일반적인 경로의 유사성 판단 알고리즘의 문제점을 제시하고, 전체 절개 경로의 길이 대비 현재까지 진행된 정도를 매개변수로 하는 방법을 이용하여 절개 경로의 방향을 고려한 유사성 측정 알고리즘을 제안하였다. 이와 같이 정량적이며 자동화된 절개 숙련도 평가 기법을 제안함으로써 사용자의 훈련 결과에 대해 보다 객관적인 피드백을 제공 할 수 있다.

  • 수술 시뮬레이션은 의학 교육, 수술 훈련, 수술 계획과 정확한 수술을 위해 그 중요성이 더해지고 있다. 본 논문은 물리적 모델을 기반으로 3차원 객체의 변형을 인터액티브하게 수행할 수 있는 인체 내장 기관에 대한 수술 시뮬레이터의 개발을 목적으로 한다. 물리적 모델은 컴퓨터 애니매이션과 컴퓨터 그래픽 모델에 대한 새로운 시도로써 많이 연구되고 있으며, 본 논문에서는 심장에 대한 3차원 자료를 매스-스프링 모델로 구현 및 변형을 수행하였다. 본 연구의 결과는 일차적으로는 심장 수술 시뮬레이션에 적용 가능하며 나아가 다른 내장 기관의 수술 시뮬레이션 및 non-rigid한 객체에 대한 변형에 적용 가능하다.

Lower Limbs Muscle Comparative Research for Verification Effect of Rehabilitation Training Program of Total Hip Arthroplasty (재활운동 프로그램에 참가한 엉덩인공관절 수술자의 하지근력 변화에 대한 비교연구)

  • The purpose of this study was to examine the differences in kinetics between 6 months of rehabilitation training and 12 months of rehabilitation training after total hip arthroplasty. 10 unilateral THA participants performed kinetic tests. Three dimensional kinematics and hip flexors and abductors electromyography (EMG) were collected during each trial. T-test was used for statistical analysis (p<0.05). There was no significant difference in EMG data between the two groups, but the mean comparison EMG data was higher in the 12 months rehabilitation training group than the 6 months rehabilitation training group. The moment value was found with motion-dependent interaction analyzing method which was used by Feltner and Dapena. There was no significant difference between moment values of the two groups. There was no significant difference between ground reaction forces of the two groups; however, there were some differences shown in Fz (vertical reaction force) between the two groups ($892{\pm}104\;N$, $820{\pm}87\;N$). The first peak impact force was about 9% lower in the 12 months group compared to the 6 months group. The second peak active force was nearly equal between the two groups. More research is necessary to determine exactly what constitutes optimal rehabilitation training biomechanics for patients with total hip arthroplasty.

증모판막부전증 환자를 중심한 심장 수술 전후의 간호

  • mitral insufficiency환자를 일렬로 하여 open heart surgery에 있어서의 수술 전, 수술 후의 nursing care에 대해 적어보았다. 수술 전 nursing care에 있어서는 이 환자의 심장질환을 정확하게 진단 후 대수술을 받기에 알맞도록 전신상태나 심장질환 그 자체를 호전시키는 데에 의사와 함께 적극적으로 협조 노력하여야 한다. 수술 후에 있어서는 회복기에 들어갈 때까지 한시도 쉴새없이 환자를 감시하고 조금이라도 이상이 있으면 즉시로 그것을 교정하는 방법을 강구하여야 한다. 심장수술은 다른 수술과는 달리 특별히 훈련된 nurse에 의해 care되어야 한다는 점을 강조하고 싶다.

  • In general, dizziness is caused by various situations, but among them, symptoms due to dysfunction of the motor system belonging to the nervous system are the most severe, accompanied by nausea and vomiting. Treatment of these dizziness includes drug therapy, surgical therapy, and rehabilitation. Drug therapy and surgery are generally performed in vest rehabilitation training, which is a rehabilitation therapy because of the risk of aftereffects. The vestibular rehabilitation training includes eye training, posture stabilization training, and walking training. Among them, walking training is performed in a certain space under the supervision of a doctor or a professional therapist, so that the time and space burden is increased. In order to solve this problem, we implemented gait training contents which can be used for rehabilitation training by using the augmented reality technology. It is expected that it can be utilized as dizziness rehabilitative contents which can be used in medical environment through clinical tests for patients with dizziness.

Dental Surgery Simulation Using Haptic Feedback Device (햅틱 피드백 장치를 이용한 치과 수술 시뮬레이션)

  • Virtual reality simulations are used for education and training in various fields, and are especially widely used in the medical field recently. The education/training simulator consists of tactile/force feedback generation and image/sound output hardware that provides a sense similar to a doctor's treatment of a real patient using real surgical tools, and software that produces realistic images and tactile feedback. Existing simulators are complicated and expensive because they have to use various types of hardware to simulate various surgical instruments used during surgery. In this paper, we propose a dental surgical simulation system using a force feedback device and a morphable haptic controller. Haptic hardware determines whether the surgical tool collides with the surgical site and provides a sense of resistance and vibration. In particular, haptic controllers that can be deformed, such as length changes and bending, can express various senses felt depending on the shape of various surgical tools. When the user manipulates the haptic feedback device, events such as movement of the haptic feedback device or button clicks are delivered to the simulation system, resulting in interaction between dental surgical tools and oral internal models, and thus haptic feedback is delivered to the haptic feedback device. Using these basic techniques, we provide a realistic training experience of impacted wisdom tooth extraction surgery, a representative dental surgery technique, in a virtual environment represented by sophisticated three-dimensional models.

Case Study of Auditory Training for the Acquired Hearing loss Adult with Cochlear Implant (후천성 인공와우 이식 성인의 청능훈련 사례 연구)

  • Recently, the number of those who were transplanted cochlear implants increased as health insurance increases has expanded. Last six years between 2005 to 2009, patients who received a cochlear implant surgery were about 3,300 and number of cochlear implants in adults of them have shown growing aspects. In the case of young children, they actively participated auditory training program after cochlear implant surgery and the studies related to auditory training in child are many, but the studies related to auditory training in adults is insufficient. In this study, we perform the auditory training for the female adult (age 54) received cochlear implant after language acquisition used Ling 6 sounds test, standardized consonants, vowels and sentences listening test and word recognition and confirmation test. As a result after auditory training for 10 weeks, she identified all phonemes in Ling 6 sound test and performed close to 100% in standardized consonants, vowels and sentences listening tests. Also, she improved the ability of real-world environmental sound and real-world words identifications by 57-95%. The results of this study showed the need of auditory training program with systematic and effective planning and considering the characteristics of the individual for adults.

The Change in Binocular Function and Sensory Anomaly After vision Training on Intermittent Exotropia and Recurrent Intermittent Exotropia Patients After Surgical Correction (간헐성 외사시 및 외사시 재발환자에 대한 시기능 훈련 후 양안시 기능 및 감각이상 변화)

  • Purpose: This was a study of the effects of the vision training on binocular function and sensory anomaly for intermittent exotropia and recurrent intermittent exotropia patients. Mehtods: The change was examined weekly at the clinic during the 8 week vision training. The sample group of 17 patients, intermittent exotropia and recurrent intermittent exotropia patients without amblyopia, were chosen from the patients with the short and the long-distance deviation angle p=0.558 and p=0.338, and also greater than 10${\Delta}$, respectively. Results: The longdistance deviation angle was reduced by 67.9(${\pm}$32.7)%, and the short-distance deviation angle by 75.4(${\pm}$32.9)% after the vision training. There is an improvement on the binocular function and sensory anomaly according to the fusion reverse and Worth 4 dot tests. Conclusions: This study shows that the vision training is effective for phoria, function and sensory anomalyon of the intermittent extropia and recurrent intermittent exotropia patients.
