COMBINED THERAPY OF ADVANCED MALIGNANT PARANASAL SINUS TUMOR (진행된 악성 부비동 종양의 병용치료법)

  • 부비동에 발생하는 악성종양은 자각증상이 비교적 늦게 나타나 병변이 많이 진행된 후에 발견되는 경우가 대부분이다. 진행된 부비동압의 치료는 수술, 화학요법, 방사선요법의 병용치료법이 종래부터 시도되어왔으며 대부분의 경우 광범위한 수술적 절제가 필요하여 이에 따른 후유증이 크고 치료결과는 아직 많은 논란이 있다. 저자들은 진행된 부비동암 (T4)환자 10명에서 유도화학요법 (Cisplatin, 5-FU)후 수술 (상악절제술 4예, 두개안면 절제술 1예 )과 방사선 치료 (약 7000 cGy)를 받은 5명과 유도화학요법후 방사선치료만 받은 5명을 비교하여 다음과 같은 결과를 얻었다. 1. 수술을 포함한 병용요법을 받은 환자 6명중 3명은 평균추적기간 28개월 동안 재발의 증거가 없었으나 2명에서 국소재발이 있었고 이들은 국소 절제술 및 방사선치료후 현재 1년 이상 무병상태이다. 2. 수술을 포함한 병용요법을 받은 환자 5명중 4명에서 초진시 안와내 종양침윤의 소견이 있었으나 수술시 안와를 보존하였고 그 후 안와 부위의 국소 재발은 없었다. 3. 유도화학요법과 방사선치료만을 받은 5명 모두 6개월 이내에 국소재발하였다. 4. 진행된 악성 부비동암은 수술을 포함한 병용요법으로 좀 더 좋은 치료성적을 얻을 수 있을 것으로 기대된다.

The Results and Prognostic Factors of Mitomycin C Trabeculectomy in Neovascular Glaucoma (신생혈관녹내장에 대한 Mitomycin C 섬유주절제술의 성적과 예후인자)

  • Background: Neovascular glaucoma is common secondary glaucoma at high risk for failure of glaucoma filtering surgery. Recently, trabeculectomy with adjunctive mitomycin C trabeculectomy has been tried to improve the surgical success rate of conventional trabeculectomy. But, the long-term effects of mitomycin C trabeculectomy for neovascular glaucoma are unknown. Thus, we evaluated the long-term effects of mitomycin C trabeculectomy and its prognostic factors influencing the outcome. Materials and Methods: Medical records of 62 eyes of 55 neovascular glaucoma who had undergone mitomycin C trabeculectomy were retrospectively reviewed. Surgical success was defined as intraocular pressures of 21 mmHg or less with or without glaucoma medications and no loss of light perception. Surgical failure was defined as postoperative loss of light perception in patients with preoperative vision better than light perception, additional glaucoma surgery, or phthisis bulbi in patients with preoperative vision of no light perception. Results: Postoperative success was obtained in 37 (60%) out of 62 eyes after mean followup period of $23.9{\pm}16.2$ months. Using Kaplan-Meier survival analysis, cumulative success rate at the 6-, 12-, 24- and 36-month intervals were 85%, 71%, 57% and 52%, respectively. Success rate was greater in eyes with diabetic retinopathy than other causes(p=0.005) and in eyes with preoperative panretinal photocoagulation(PRP) than without PRP(p=0.015). However, Cox proportional hazard regression analysis revealed that preoperative PRP was not a significant risk factor for surgical failure. Conclusion: Prognosis of neovascular glaucoma caused by diabetic retinopathy was better than that caused by the other disorders following mitomycin C trabeculectomy. The author would suggest that mitomycin C trabeculectomy could be effective and relatively safe as the first procedure of choice before performing glaucoma drainage device implantation or cyclodestructive procedure.

Comparison of Ovariectomy and Ovariohysterectomy in Terms of Postoperative Pain Behavior and Surgical Stress in Dogs (개에서 난소자궁절제술과 난소절제술 후 나타나는 통증 행동과 스트레스 반응의 비교)

  • This study was conducted to evaluate the clinical relevance and effectiveness of ovariectomy (OVE) and ovariohysterectomy (OVH) in terms of postoperative pain behavior and surgical stress. Thirteen healthy intact mongrel purpose-bred female dogs were used in this study. OVE was performed in six dogs whereas OVH was performed in seven dogs. Prior to, 1, 2, 4, 6, 12, and 24 h after surgery, pain assessment was performed by using the Short form of Composite Measure Pain Scale (CMPS-SF) and blood analysis for the determination of glucose, creatine kinase (CK), and cortisol were done. Also, surgical time, duration of anesthesia, and incision length were compared between two groups. As a result, OVH group as opposed to OVE group showed significantly longer in surgical time, duration of anesthesia, and incision length. Also, based on the two-way ANOVA test, the CMPS-SF had significant differences (p < 0.05) between two groups, with the OVE dogs having lower values at 1, 2, 4, and 6 h postoperatively. In addition, in terms of CK, the value at 1, 2, 4, 6, and 12 h were significantly (p < 0.05) increased from the baseline value for the OVH group. However, as for CK in OVE group, the values at 4, 6, and 12 h were significantly increased from the baseline value. As for blood glucose, a significant (p < 0.05) increase from the baseline was shown at 1 h postoperatively in OVH group and no significant increase was shown in OVE group. In terms of serum cortisol level, the values at the 1 and 2 h were significantly (p < 0.05) increased from the baseline value for both groups. In conclusion, our study has shown that OVE can be considered as a superior choice in terms of sterilization technique in female dogs, considering shorter incision length, surgical time, duration of anesthesia along with lower pain and surgical stress response than OVH.

  • Cardiac surgery in patients with a prior pneumonectomy has a high prevalence of postoperative complications and mortality. We have successfully performed a mitral valve replacement and tricuspid annuloplasty in a 71-year-old man who had a left pneumonectomy 33 years previously due to pulmonary tuberculosis. We report this case with a literature review on the issue of the prevention perioperative pulmonary impairment, the technique of mitral valve exposure during surgery and postoperative rhythm disturbance.

Thoracic Sympathetic Ganglionectomy for Primary Hyperhidrosis (본태성 다한증의 흉부 교감신경절 절제술)

  • From January 1996 to December 1996, we performed 137 thoracic sympathetic ganglionectomies with VATs for primary hyperhidrosis in the department of thoracic and cardiovascular surgery at Anam hospital, Korea university medical center. There were 83 men and 54 women whose ages ranged from 13 to 63 years old(mean age: 25years). Of these patients, 128 patients had complained of palmar hyperhidrosis and 26 of facial hyperhidrosis. Thoracoscopic sympathetic ganglionectomies procedures included lower 1/3 Stellate ganglionectomies in 4 patients; lower 1/3 Stellate ganglionectomies and T2-sympathetic ganglionectomies in 18 patients; T2-sympathetic ganglionectomies in 86 patients; T2 and T3 sympathetic ganglionectomies in 10 patients; and T2, T3 and T4 sympathetic ganglionectomies in 19 patients. The mean operating time was 59 minutes(range: 25 to 162 minutes), and the mean hospital stay was 3.8 days(range: 2 to 8 days). Common complications were compensatory hyperhidrosis(109 patients) and causalgia(18 patients). At the end of the follow-up period(mean: 8 months) ninety-seven percent of the patients reported satisfactory results. Thoracic sympathetic ganglionectomy with VATs is an efficient, safe, and minimally invasive surgical procedure for primary hyperhidrosis.

Partial Left Ventriculectomy in the Pediatric Patient with Dilated Cardiomyopathy (확장성 심근증 환아에서의 부분 심실 절제술의 적용 -1례 보고-)

  • Heart transplantation was planned for a 10-year old boy who had dilated cardiomyopathy with severe congestive heart failure and had been on dopamine for 1month. However, partial left ventriculectomy and mitral annuloplasty were performed instead, because there was no donor heart of the adequate size and the symptoms were aggravated. The clinical symptoms were markedly improved after the surgery. Comparing the postoperative echocardiographic results with the preoperative results, there were remarkable changes in the left ventricular ejection fraction(preoperative LV EF 17% to postoperative 3 months 29%, 6 months 35%, 1 year 36%) and the left ventricular end-diastolic dimension(preoperative 72 mm to postoperative 3 months 59 mm, 6 months 61 mm, 1 year 61 mm). Partial left ventriculectomy and mitral annuloplasty reduced the cardiac loading in the dilated cardiomyopathy. Partial left ventriculectomy and mitral annuloplasty may be considered as one of the alternative surgical metho s to carry over until a heart transplantation can be performed, especially for children.

The Surgical Treatment of Malignant Bowel Obstruction Caused by Recurrent Gastric Cancer (재발된 위암 환자에서 발생한 악성 장폐쇄증의 수술적 치료)

  • Purpose: Malignant bowel obstruction caused by recurrent gastric cancer must be treated appropriately to improve the effects of treatment and to prolong survival. We reviewed the surgical treatments for malignant bowel obstruction caused by recurrent gastric cancer. Materials and Methods: The subjects were patients with malignant bowel obstruction caused by recurrent gastric cancer and these patients were treated by surgical procedures at our hospital from 1998 to 2008. The patients were treated by resection, ostomy or bypass. The success of treatment was decided when the patients were able to tolerate more than a liquid diet. Results: 42 patients were treated 46 times by surgical procedures. Resection was done12 times, ostomy was done 24 times and bypass was done 10 times. The hospital stay and the period to liquid diet after the operation were shorter in the ostomy group. The post operative morbidity rate was 21.7% and the post operative death rate was 8.7%. There was no significant difference in survival according to the type of surgery. Conclusion: Ostomy is good choice for selected patients because it has a shorter hospital stay and period to liquid diet. There was no significant difference in survival according to the type of surgery because curative resection is difficult to perform in patients with malignant bowel obstruction.

Surgical Treatment of Myasthenia Gravis (중증 근무력증의 수술적 치료)

  • Thymectomy is an accepted therapeutic modality for patients with myasthenia gravis. The selection of patients for operation, the timing of operation and the surgical approach are still controversial. We reviewed 82 patientsraged 13 to 66 years; mean age, 37.7 years treated with transsternal thymectomy between January 1983 and December 1994. Patients were symptomatically staged according to the modified Osserman's classification. There was one hospital death and postoperative follow-up was obtained on 75 patients. During a mean follow up of 56.9 months, 64 patients (85.3%) benefited from the operation with complete remis ion achieved in 28(37.3%). The thyroid disease was present in 8 patients, of whom 7(87. 5%) achieved complete remission in contrast to 21 (31.3%) of the 67 patients without thyroid disease. The disease duration less than 2 years in 32 patients was associated with complete remission in 16 (50%) in contrast to remission in 12(27.4%) of the 43 patients whose disease duration was more than 2 years. In conclusion, the complete remission rate after transsternal thymectomy was affected by the presence of thyroid disease and disease duration. Myasthenia gravis with late onset(>40 years), thymoma pathology, old age and male gender appear to decrease the complete remission rate after transsternal thymectomy, although it was not statistically significant. There was no difference of complete'remission rate between normal and hyperplasia of thymus. Transsternal thymectomy was found to be beneficial in most patients with myasthenia gravis, but the majority of patients with ocular disease did not b nefit from the operation.

Surgical Treatment of the Wolff-Parkinson-White Syndrome (Wolff-Parkinson-White 증후군의 외과적 치료)

  • From October 1993 to February 1996, 9 patients with Wolfr-Parkinson-White syndrome underwent surgical ablation of the accessory atrioventricular conduction pathways. The indications for surgical ablation we e radiofrequency ablation failure in 6 cases, multiple accessory pathways in 1 case, catheter tip fracture ducting catheter ablation in 1 case and additional procedure(redo mitral valve replacement due to valve thrombosis) in 1 case. There was no operative mortality. The postoperative complications were noted In 2 cases pericardial effusion and wound Infection. All patients had accessory atrioventricular connections ablated which were proven by surface ECG and follow-up electrophysiologic study and have remained free of symptomatic tachycardia. The indications for surgical treatment of Wolff-Parkinson-White syndrome are radiofrequency ablation failure, multiple pathways, or when additional procedures are required The present results were satisfactory.

Surgical Treatment of Pulmonary Actinomycosis Mimicking Pulmonary tuberculosis (폐결핵으로 오인된 폐방선균증의 수술적 치험 1례)

  • Pulmonary actinomycosis is a rare disease entity to undergo thoracic surgery. We experienced a 49-year-old man with pulmonary actinomycosis who was admitted due to recurrent hemoptysis. Prior to admission, he was diagnosed as pulmonary tuberculosis on the basis of his clinical manifestations and chest radiological findings. The plain chest x-ray and chest computed tomography(CT) showed a cavitary lesion in left upper lobe and was given anti-tuberculous medication, but the x-ray revealed no imprcovement. He underwent left upper lobe lobectomy with segmentectomy of lower lobe and the surgical specimen showed no evidence of mycobacterial infection, but revealed sulfur granules which is a typical pathological finding of actinomycosis. He was discharged uneventfully and is scheduled to receive 6 months of antibiotic treatment.
