A Clinical Study on Relationship of Surgical Glove and Back Pain after Lumbar Puncture in Children (소아에서 요추천자 후 발생하는 요통과 수술용 장갑과의 관계에 대한 임상적 연구)

  • Propose : In this study, we evaluated whether powder on surgical gloves is a cause of postpuncture backpain in children. Mothods : In 164 children with meningitis between July and September 1997, we did not remove powder from surgical gloves. However, in 149 children with menigitis between May and October 2001 the powder was removed from the surgical gloves. Results : Out of the 164 patients in 1997, 41 cases(25.00%) were found to have postdural puncture backpain. On the other hand, out of 149 patients in 2001, with whom we used gloves from which the powder was removed, we found only 8 patients(5.36%) with postdural puncture backpain. Conclusion : We conclude that the powder on surgical gloves is one of the main causes of postdural puncture backpain in children.

Incidence and Risk Factors for Surgical Glove Perforation (수술용 장갑의 천공률에 대한 연구)

  • Purpose: This study was conducted to evaluate the incidence and risk factors for surgical glove perforation during operation. Methods: During the month of december 2008, a total of 1,400 pairs of surgical gloves used in major operations was collected in a tertiary hospital. All gloves were examined immediately after operation using the standardized water-leak method to detect any perforation. Incidence of the glove perforation was counted according to the type of operation, operation time, the number of involved personnel, perforation sites, and the manufacturing companies. Results: Out of 2,800 gloves examined, 312 perforations were detected comprising 11.1% of samples. In terms of the type of operation, the perforation incidence varied from 5% to 20%, and the perforation rates in CS (20%) and NS (18%) (p<.001) were significantly higher than those in other departments. The 1st assistant or scrub nurse got glove perforation more frequently than the 2nd assistant or operator (p<.001). Longer operation time was associated with higher incidence of perforation evidently (p<.001). In terms of the sites, the thumb and index finger were more frequently perforated than other sites (4.1% and 3.4% respectively) without any differences between left and right side. Conclusion: Risk factors for glove perforation including the department of operation, operation time, participating personnel, and location of perforation should be taken into account to improve surgical safety.

Character Analysis that influences to Surgical Gloves Punctures (수술장갑의 천공에 영향을 미치는 특성 분석)

  • Purpose: The purpose of this study was to examine the puncture incidence of surgical gloves in an operational setting. Methods: The 277 surgical gloves were used collecting at one general hospital located in Seoul from February 1, 2011 to April 3. The data were analyzed by $x^2$ test, Fisher's exact test, and multiple logistic regression. Results: In the dental and orthopedic department, the puncture incidence were significantly higher than others. The time of wearing gloves demonstrated significant difference: The group 181-240 minutes had a significantly higher than those with under 90, 91-180, or over 240 minutes. The influencing factors were as follows. Compared to that of the 2nd assistants, the puncture incidence rates of the surgeons were 9.91 times, scrub nurses were 8.39 times higher respectively. The participants in work experience under 1 year showed a 4.58 times higher than those with over 7 years. In addition, compared to the puncture incidence rate of neurosurgery department, the 17.41 times in cardio-thoracic surgery, 13.89 times in dental surgery, 4.93 times in gynecology, and 4.97 times in orthopedics higher respectively. Conclusion: There is a need for training operational room personnels to occasionally exchange the gloves even during the procedure and to use double surgical gloves.

The Effect of Glove Changing during Surgery for Colon Cancer (대장암수술 시 장갑교환의 효과)

  • Purpose: The purpose of this study was to explore the effect of glove changing during surgery for colon cancer on reducing the degree of contamination of surgical gloves. Methods: The randomized posttest control group design was used. Total 72 surgeries of colon cancer in the K University Hospital in Seoul performed by the team of A-Surgeon and B-Scrub nurse were randomly assigned to one of the three groups. Glove changing with single-gloved state was implemented in the Experimental Group I and outer glove changing with double-gloved state was executed in the Experimental Group II. Single-gloved state was carried in the Control Group. Following the surgical procedure, specimens for bacterial culture were collected the from scrub nurse's gloves to compare the degree of contamination among the three groups. Data were analyzed with One-way ANOVA and the Scheff$\acute{e}$'s multiple comparison test. Results: The degrees of contamination of the Experimental Group I and II were significantly lower than that of the Control Group. There was no significant difference between Experimental Group I and II. Conclusion: Glove changing during surgery with either single or double-gloved state is effective in reducing the degree of contamination of surgical gloves, therefore these methods should be utilized in clinical practice.

  • This study was aimed to investigate the practice on the infection control and dental waste disposal in dental clinic. The data used in this study had been collected in 100 dental clinics located in Seoul city from August to October, 2001. It was shown that examination instruments, oral surgery instruments, and periodontal instruments were almost sterilization in dental clinic. It was high in the recognition of individual protection but it was low in the degree of practice. Therefore, the practice in infection control is very important to dental team for safety of patients and operators.

비폐색 부위에 따른 비강자음의 음향학적 특성과 비음비율의 변화

  • 비폐색이 있는 경우 음성이 변하는 것을 쉽게 느낄 수 있지만, 비폐색 때의 음향학적인 특성에 대하여 알려진 바를 찾기는 쉽지 않다. 저자들은 인위적으로 비폐색을 유발하여 비폐색 부위에 따른 비강자음의 음향학적 변화특성을 파악하고 비음도의 변화 정도를 비교하고자 하였다. 정상비음도를 보이는 성인남녀 각 10명을 대상으로, 2ml의 부피를 갖도록 수술용장갑에 Merocel$^{\circledR}$을 넣은 뒤, 이를 이용하여 ostiomeatal unit(OMU)을 중심으로 전후상하 4부위의 인위적인 비폐색을 유발하여, 비폐색 전과 후의 부위에 빠른 차이틀 비교하였다. /나나/의 발성을 각 조건에서 3회 실시하여, 모음중간의 /ㄴ/중 (CVCV) 안정된 spectorgram소견을 보이는 부위를 선택하여, 해당구간의 제1, 제2, 제3 음형대와 각각의 bandwidth 평균값을 남녀별로 비교하였고, 표준비음비율이 알려진 rabbit, baby, mama 문장을 이용하여 비음비율을 비교하였다. 남녀모두 비폐색전에 비하여 OMU의 앞쪽부위를 막은 경우에 제1음형대가 가장 뚜렷하게 감소되었으며, 비음비율의 유의한 감소를 보였다. 비폐색이 있는 경우, 비강자음 /ㄴ/은 제1음형대를 중심으로 주요변화가 일어남을 알 수 있었으며, 비폐색 부위에 따라 비음비율이나 제1음형대 감소의 정도가 다름을 알 수 있었다.

Simple Molding Method for Post-distant Flap Stated Finger by Using Surgical Rubber Gloves (원위피판술을 시행한 환자의 손가락에 수술용 고무장갑을 이용한 간단한 조형법)

  • In both cosmetic and functional aspects, loss of digital pulp is a common problem. Compound or composite defects of the hand and fingers with exposed denuded tendon, bone, joint, or neurovascular structures may require flap coverage. Most often these lesions can be repaired by using simple local flap, neurovascular flap, thenar flap, and cross-finger flap. But microvascular reconstruction is sometimes needed for large defects. But Authors do not recommend these procedures in case of severe crushing injuries involving multiple finger pulp losses because they have possibility of damage of the vascular network and infection. So we applied distant flaps such as chest flaps, groin flaps, abdominal flaps and etc. And then we applied surgical rubber gloves for remodeling the flap after cutaneous healing. We have acquired satisfactory results, after the simple molding method for distant flap finger by using surgical rubber gloves treatment.

Verification of the Protective Effect of Functional Shielding Cream for the Prevention of X-ray Low-dose Exposure (X-ray 저선량 피폭방지를 위한 기능성 차폐크림의 방어 효과 검증)

  • In the case of radiation workers in medical institutions, radiation exposure is made for patient protection and accurate procedures, so they have a problem of low dose exposure. Low-dose radiation exposure occurs mainly in parts of the body other than the Apron area, and the most frequent place is the skin of the back of the hand. In particular, since the medical personnel's hands require senses and fine movements during the procedure, they are defenseless in the radiation exposure area and are at risk of exposure. It can solve the problem of shielding such as lead gloves, and it is difficult to use by suggesting the activity of the hand during the procedure. To solve this problem, a shielding cream capable of obtaining a functional radiation protection effect was developed and its shielding performance was compared with lead equivalent of 0.1 mmPb. In the process of manufacturing shielding cream, the shielding performance was improved by adding a defoaming process to reduce air holes to increase the density of the cream. Therefore, the shielding cream using barium sulfate as the main material has a lower shielding rate than the lead plate, and in the realm of effective energy, it is 59%, At high effective energy, a difference of about 37% was shown, indicating that there is a functional radiation protection effect. The advantage is that it can be used directly on the skin, and it is considered that it can be used before wearing surgical gloves and has a permanent protective effect.
