  • Obstructive sleep apnea syndrom(OSAS) is defined by total or partial collapse of the upper airway during sleep. In the presence of specific anatomic features, OSAS is potentially amenable to surgical treatment. Initially, the only treatment available for these patients was a tracheotomy that bypassed the obstruction and resulted in a 100% cure. However, this was not readily accepted by most patients, and surgical methods other than tracheotomy were developed to successfully maintain adequate upper airway patency during sleep by comparing to postoperative polysomnography(AHI,RDI etc). In this paper, I would like to provide an overview of some of the multilevel surgical techniques available for treating OSAS as well as the necessary preoperative considerations.

  • Background: Recent advance in video technology, endoscopic equipments, and surgical techniques have expanded the use of thoracoscopy from diagnosis of the pleural diseases to treatment of the various intrathoracic diseases. Video Assisted Thoracoscopic Surgery(VATS) is a pretty new and fascinating thoracic surgical modality, and so we present our early VATS resuls. Methods: Using Video Thoracoscopic techniques in 30 patients for 10 months from July 1992 to April 1993, we had performed a variety of procedures. These incuded (1) bleb resections in 18 patients (19 cases), (2) mediastinal tumor excision in 4, (3) lung biopsies for parenchymal pulmonary disease in 3, (4) pleural biopasies in 3, (5) pleural tumor excision in 1, (6) and pleuropericardial window in 1. Results: There were no mortality associated with the procedures. We had minor 8 complications; prolonged air leak in 3 patients, prolonged serous drainage in 2, recurrence of pneumothorax in 1, Honer's syndrome in 1, and hoarseness in 1 patient. None of the 30 patients had reverted to the conventional full thoracotomies. Mean postoperative hospital stay of non-complicated pneumothoraces was about 5 days, which was a little shorter than conventional thoracotomy group. Conclusion: Though we had somewhat higher postoperative complication rate due to lack of experiences in the begining, we were able to convince that VATS had benifical value for patients; lesser postoperative pain, shorter hospitalization, quicker recovery time, and cosmetically superior scar. The role of VATS can be expanded to the diagnosis and treatment of various thoracic diseases, even to the cardiovascular diseases, with satisfactory outcome and less postoperative morbidity.

  • 부인두강(Parapharyngeal space)는 비인두강과 구인두강에 인접하여 위로는 두개저, 아래로는 설골사이에 위치하는 해부학적 잠재공간으로 이곳에 발생하는 종양은 매우 드무나 다양한 병리조직학적 소견을 보이며 인접한 중요장기들로 인한 수술적 어려움으로 두경부 외과의사의 관심이 되어 왔다. 최근 CT나 MRI의 도입으로 이학적 검사가 어려웠던 이부위의 진단에 많은 도움을 얻게 되었고, 발달된 수술방법의 사용으로 크기가 매우 큰 종양도 절제가 가능하게 되었다. 저자들은 1990년 9월부터 1993년 8월까지 한림대학교 이비인후과학교실에서 술전 CT나 MRI를 시행한 후 조직검사로 확진된 부인두강 종물 22례의 후향적 임상분석을 시행하여 다음의 결과를 얻었다. 1) 양성종양이 11례(30.0%), 악성종양이 11례(50.0%)로 빈도에 차이는 없었다. 2) 종양의 조직학적 기원은 타액선종양이 10례(45.5%)로 가장 많았으며 신경종양이 4례(18.2%)였고 기타 종양이 8례(36.4%)로 다양하였다. 3) 방사선학적 진단방법으로는 18례(81.8%)에서 CT를 시행하였고 필요한 경우에 MRI를 5례(22.7%), 혈관조영술을 3례(13.6%)에서 시행하였다. 4) 치료방법으로는 악성종양은 수술 및 방사선치료 또는 항암약물요법을 병리조직검사 결과에 따라 단독 또는 병합하여 시행하였고, 양성종양의 경우에는 모두 수술을 시행하였는데 수술방법은 경이하선 접근법이 가장 많이 사용되었다. 5) 추적조사는 평균 20개월로 양성종양의 경우는 모두 종양의 재발이 없이 치료되었으나 악성종양 중 전이암 1례와 악성 임파종 1례가 사망하였다.

  • Difficulty in exposing anastomotic sites is a frequently encountered problem during surgical repair of a distal aortic arch aneurysm via median sternotomy or lateral thoracotomy. Endovascular repair has th limitation that it usually requires surgical rerouting of some of the brachiocephalic branches in order to get sufficient length for proximal fixation of the stent-graft. To take advantage of each approach, we fixed the distal end of the prosthetic graft by means of a pre-mounted metallic stent instead of performing conventional surgical anastomosis during the repair of distal arch aneurysms with using median sternotomy and hypothermic circulatory arrest. We report here on our experience with such 3 patients.

  • Stereotactic radiosurgery (SRS) is a technique that delivers a high dose to a target legion and a low dose to a critical organ through only one or a few irradiations. For this purpose, many mathematical methods for optimization have been proposed. There are some limitations to using these methods: the long calculation time and difficulty in finding a unique solution due to different tumor shapes. In this study, many clinical target shapes were examined to find a typical pattern of tumor shapes from which some possible ideal geometrical shapes, such as spheres, cylinders, cones or a combination, are assumed to approximate real tumor shapes. Using the arrangement of multiple isocenters, optimum variables, such as isocenter positions or collimator size, were determined. A database was formed from these results. The optimization procedure consisted of the following steps: Any shape of tumor was first assumed to an ideal model through a geometry comparison algorithm, then optimum variables for ideal geometry chosen from the predetermined database, followed by a final adjustment of the optimum parameters using the real tumor shape. Although the result of applying the database to other patients was not superior to the result of optimization in each case, it can be acceptable as a plan starling point.

  • Stereotactic radiosurgery for intracranial lesion is well established since the Lars Leksell first introduced radiosurgery concept in 1951 Its use in the treatment of spinal lesion has been limited by the availability of effective immobilization devices. The first clinical experience of the spinal stereotactic radiosurgery technique was reported by Hamilton AJ. in 1995. Recently, Optic-guided patient positioning technique for extracranial stereotactic radiosurgery was developed and reported. This study is for assess the target positioning accuracy of the optic guided patient positioning system Exactrac (BrainLab., Inc, Germany). We have designed phantom for assess the accuracy of spinal stereotactic radiosurgery The infrared reflective body markers attached to the relatively immobile part of the body and a series of 2 mm CT images was taken. The image sets were transferred to the planning computer. During the radiosurgery treatment, we measure the real-time display showing the positioning values from Exactrac computer. And we compare the isocenter deviation from irradiated center point of the film which was mounted on the lesion site of the phantom and pin hole site of that film. The accuracy of the ExacTrac system in positioning a target point shows enough for the clinical applications.

  • 부식성 물질에 의한 양성 식도 협착 환자에서 식도 확장술이 일반적으로 사용되는 술식이나 협착부위가 잔존하여 연하곤란이 발생하므로 식도 재건술이 이용된다. 식도재건술은 대용 식도로 위관, 대장관, 소장관이 이용하여 광범위한 박리, 여러 부위의 절개, 문합부 대용 식도 위치에 따른 문제점 및 협착된 식도를 잔존시킴으로 식도암 발생가능성이 있으며 식도 재건술후 식도 기능의 문제점이 많다. 이에 시고 기능에 이상을 초래하지 않으면서 합병증이 적고 수술하기 쉬운 방법이 요구된다. 저자는 식도 근층만을 절개하고 점막하층을 박리한 후 협착부 점막만 절제하고 점막 단단 문합함으로 좋은 결과를 얻었기에 증례와 더불어 수술 방법을 소개하고자 한다.

Surgical Results for Myasthenia Gravis (중증근무력증의 수술적 치료 결과)

  • Myasthenia gravis is relatively rare disease which is related autoimmune response. There are various methods of management for myasthenia gravis, but nowaday radical thymectomy is the treatment of choice in the aspect of bringing out complete remission and clinical improvement. Sixteen patients of myasthenia gratis underwent radical thymectomy during last eight years, and its result was analysed. Complete rem ssion was achieved in five patients (31 %) and pharmacological or symptomatic improvement in seven patients (44%), thus giving a total remission in 12 patients (75%). Postoperative result was not correlated with age, sex, degree of preoperative symptom, surgical approach, pathologic diagnosis.

  • Since 1968, we have performed intact canal wall tympanoplasty with mastoidectomy and reported its methods and results on several occasions. We also reported ossicular reconstruction for hearing improvement after intact canal wall tympanoplasty with mastoidectomy. Many problems were noticed, so several variable operation methods were performed. This paper deals with the following: A checked up of hearing progress after ossicular reconstruction with a 93 cases out of a total 153 cases who had undergone this operation. This covered a period of January, 1973 to February, 1979 in our Department of Otolaryngology.

Is Video-assisted Thoracoscopic Resection for Treating Apical Neurogenic Tumors Always Safe? (흉강 첨부 양성 신경종의 흉강경을 이용한 절제술: 언제나 안전하게 시행할 수 있나?)

  • Background: Mediastinal neurogenic tumors are generally benign lesions and they are ideal candidates for performing resection via video-assisted thoracoscopic surgery (VATS). However, benign neurogenic tumors at the thoracic apex present technical problems for the surgeon because of the limited exposure of the neurovascular structures, and the optimal way to surgically access these tumors is still a matter of debate. This study aims to clarify the feasibility and safety of the VATS approach for performing surgical resection of benign apical neurogenic tumors (ANT). Material and Method: From January 1996 to September 2008, 31 patients with benign ANT (15 males/16 females, mean age: 45 years, range: 8~73), were operated on by various surgical methods: 14 VATS, 10 lateral thoracotomies, 6 cervical or cervicothoracic incisions and 1 median sternotomy. 3 patients had associated von Recklinhausen's disease. The perioperative variables and complications were retrospectively reviewed according to the surgical approaches, and the surgical results of VATS were compared with those of the other invasive surgeries. Result: In the VATS group, the histologic diagnosis was schwannoma in 9 cases, neurofibroma in 4 cases and ganglioneuroma in 1 case, and the median tumor size was 4.3 cm (range: 1.2~7.0 cm). The operation time, amount of chest tube drainage and the postoperative stay in the VATS group were significantly less than that in the other invasive surgical group (p<0.05). No conversion thoracotomy was required. There were 2 cases of Hornor's syndrome and 2 brachial plexus neuropathies in the VATS group; there was 1 case of Honor's syndrome, 1 brachial plexus neuropathy, 1 vocal cord palsy and 2 non-neurologic complications in the invasive surgical group, and all the complications developed postoperatively. The operative method was an independent predictor for postoperative neuropathies in the VATS group (that is, non-enucleation of the tumor) (p=0.029). Conclusion: The VATS approach for treating benign ANT is a less invasive, safe and feasible method. Enucleation of the tumor during the VATS procedure may be an important technique to decrease the postoperative neurological complications.
