A Study on Wave Responses of Vertical Tension-Leg Circular Floating Bodies (연직인장계류된 원형부유체의 파랑응답에 관한 연구)

  • In the present study, we proposed a new numerical wave tank model to analyze the vertical tension-leg circular floating bodies, using a 2-D Navier-Stokes solver. An IBM(Immersed Boundary Method) capable of handling interactions between waves and moving structures with complex geometry on a standard regular Cartesian grid system is coupled to the VOF(Volume of Fluid) method for tracking the free surface. Present numerical results for the motions of the floating body were compared with existing experimental data as well as numerical results based on FAVOR(Fractional Area Volume Obstacle Representation) algorithm. For detailed examinations of the present model, the additional hydraulic experiments for floating motions and free surface transformations were conducted. Further, the versatility of the proposed numerical model was verified via the numerical and physical experiments for the general rectangular floating bodies. Numerical results were compared with experiments and good agreement was archived.

  • Bilateral diaphragmatic paralysis is a rare disease. It is caused by trauma, cardiothoracic surgery, neuromuscular disorders, corvical spondylosis, and infection. A 60 year-old male patient developed bilateral diaphragmatic paralysis after an on-bloc resection of thymic carcinoma which invaded the right upper lobe, pericardium, superior vena cava and innominate vein. Severe respiratory difficulty developed and ventilator weaning was impossible. We performed bilateral diaphragmatic plication. After the operation, satisfactorily ventilator weaning and sleeping in supine position were possible; therefore, we report this case.

  • 입자상 물질에 대한 건성침적속도를 구하는 두 가지 모형 중 Slinn의 모수화로 구축된 건성침적모형이 ADOM 건성침적모형에 비해 좀 더 신뢰성있는 결과를 산출하였고 부산광역시에서의 입자상 물질에 대한 건성침적속도 수치모의 결과도 전반적으로 거칠기 길이가 큰 산림지역에서 높은 값이 나타남을 보여주었다. 입자상 물질의 건성침적속도 계산에 있어 가장 중요한 것은 대상 입자의 크기이므로 무엇보다도 그 물질에 대한 정확한 크기분포에 대한 규명이 있어야 하고 또한 본 연구에서는 다루지 못한 수면의 높은 풍속에서의 물결 파괴, 물보라 형성 그리고 물과 공기경계면 근처의 습한 지역에서 입자 성장의 영향 등을 고려하는 좀더 자세한 연구가 추후에 이루어져야 할 것이다.

Changes of the Pharyngeal Space by Various Oral Appliances for Snoring (수종의 코골이장치 장착에 따른 인두공간의 변화)

  • The purpose of this study was to investigate the changes of the pharyngeal space when the following appliances were inserted: the mandibular advancement appliance (MAA), tongue retaining appliance (TRA), and mandibular advancement-tongue retaining appliance (MATRA). Nine male dental students exhibiting Class I occlusion, normal body mass index (BMI), and no signs and symptoms of snoring were selected for this study. The three kinds of snoring appliances (MAA, TRA and MATRA) were fabricated for each subject. The mandibular advancement of the MAA and MATRA was set at a distance of 5 mm, and the TRA and MATRA were made to hold the tongue in front of the maxillary incisors by 10 to 20 mm. Lateral cephalometric radiographs of the following four states - with no appliance, MAA, TRA, and MATRA - were taken to examine any anatomical changes resulting from the application of the appliances. All four radiographs were traced and analyzed for twenty selected variables related to the pharyngeal space, cranio-cervical posture, and position of the soft palate and hyoid bone. According to the results of this study, there were significant increases in both the upper and lower oropharyngeal spaces when the mandible and tongue were protruded simultaneously, although there was a significant increase only in upper oropharyngeal space when the mandible or tongue was advanced separately. In conclusion, it is suggested that the MATRA may result in more positive effect on the control of snoring and OSA compared to a single use of the MAA or TRA, especially for the patients whose upper airway obstruction occurs in the lower oropharynx.

A Study on the Speed Performance of a Medium Patrol Boat using CFD (CFD를 이용한 중형 경비정의 속도성능 평가)

  • The primary objective of the current work is to predict speed performance of the medium patrol boat over $F_N=0.5$ employing experimental materials based on the CFD before model tests. In other words, the predicted brake powers according to ship speeds are assessed satisfying the main engine capacity. The subject ships are selected the two different stern hull forms. The flow computation are conducted considering free surface and dynamic trim using a commercial CFD code(STAR-CCM+). The resistances of the bare-hull are obtained from CFD. Wave patterns, pressures and limiting streamlines on the hull and velocity distribution in the propeller plane for the two hull forms are compared using CFD. The effective powers of the object ships are assessed based on CFD. Resistance increase according to the attached appendages and quasi-propulsive efficiency are employed the experimental datas. Speed performance prediction method concerning high speed vessels like a medium patrol boat is developed employing CFD and experimental data.

  • The image information acquired by a model submarine is transmitted through the repeater. The control signal of a position for submarine and its speed is also controlled by the repeater. Shooting images of underwater circumstances are transmitted to the repeater where the received signal controls a position and speed of underwater submarine. This repeater is combined by a buoy that is floating on the surface to relay the signal of image as well as control between a control unit and a submarine whereas the repeater communicates wirelessly with a control unit. Due to wire communication between the repeater and the submarine, the underwater exploration can be smoothly carried out without a risk of loss of a model submarine. Also, connecting to the repeater and control unit wirelessly makes it possible to conduct easily the underwater exploration. The convergence technology that combines a wireless communication and a control as well as a model submarine is designed.

The Association between Excessive Daytime Sleepiness and Blood Pressure in Patients with Obstructive Sleep Apnea-Hypopnea Syndrome (폐쇄성 수면 무호흡-저호흡증후군 환자에서 과도한 주간졸음증과 혈압 사이의 연관성)

  • The purpose of this study was to investigate the relationship between excessive daytime sleepiness (EDS) and blood pressure (BP) in patients with obstructive sleep apnea-hypopnea (OSAH). Patients were classified into four groups based on their severity of polysomnographic data: the snoring group (n=108)-characterized by Apnea-Hypopnea Index (AHI<5); the mild OSA group (n=186)-AHI $5{\leq}AHI$<15; the moderate OSA group (n=179)- AHI $15{\leq}AHI$<30; and the severe OSA group (n=233)-$AHI{\geq}30$. On the same night of polysomnography (PSG), BP levels were measured before sleeping (bedtime BP) and immediately after waking up on the following morning (morning BP). EDS was recognized as ESS (epworth sleepiness scale)${\geq}9$. The differences and correlations between BP and PSG parameters in the EDS and non-EDS groups of OSAH patients were analyzed. MAP was positively correlated with BMI, AHI, and total arousal (r=0.099, r=0.142, r=0.135, p<0.01, p<0.01, p<0.01), while negatively correlated with mean $SaO_2$ (r=-0.258, p<0.01). The EDS group had overall younger population ($47.2{\pm}11.3$ vs $50.3{\pm}11.4$, p=0.023), higher DBP (both bedtime and morning, $83.1{\pm}9.7$ vs $81.4{\pm}8.8$ and $86.4{\pm}9.2$ vs $83.6{\pm}9.7$)(p=0.031, p=0.047), and higher SBP (both bedtime and morning, $126.7{\pm}11.2$ vs $123.4{\pm}12.4$, $128.9{\pm}12.4$ vs $125.3{\pm}12.9$)(p=0.021, p=0.021) than compared with the non-EDS group. In hypertensive OSAH patients, patients with EDS were also younger and had higher total arousal number, as well as higher morning and bedtime DBP and SBP than compared with the non-EDS group (p<0.005, p=0.008, p<0.001 and p<0.001). EDS in OSAHS patients is a special phenotype characterized by younger age, higher DBP, more severe desaturation, and hypertension.

  • The safety and success of dental sedation for children depend mainly on respiratory status of patients. A special condition, that is, nasal breathing in supine position with their oral airway blocked by rubber dam, should be considered. Therefore, irrespective of medical consultation, pediatric dentists themselves should do respiratory assessment especially adenotonsillar hypertrophy, nasal obstruction, posterior nasal drainage and airway hypersensitivity. Patients with sinusitis, allergic rhinitis, asthma, snoring and OSAS(obstructive sleep apnea syndrome) can induce the sedation failure and complete management of these can improve the safety of dental sedation.

  • 최근 치수계획 분야에서 원격탐사(RS) 및 지형정보시스템(GIS)의 비중이 높아짐에 따라 이를 이용하여 유역 치수계획에서의 홍수범람분석 향상을 위한 활용이 증가하고 있다. 홍수범람분석을 할 때 대상유역의 지형학적 특성을 제대로 판단하고 범람 가능지역에 대한 사전 추출 및 홍수에 의한 피해 상황을 미연에 방지하는 연구가 필요하다. 그러나 수리해석에서 사용하는 대표적인 지형자료인 수치표고모델(Digital Elevation Model ; DEM) 제작시 제외지 수면부에 대한 지형정보취득 불가로 인해 자세한 표고자료와 제방 및 제외지의 형태를 구분할 수 있는 정보가 없으므로 홍수범람분석에는 한계가 있으며, 이에 따라 하천지형 보완에 대한 검토가 이루어져야 할 것이다. 본 연구에서는 하천지형 보완에 따른 홍수범람 비교연구를 위하여 섬강 유역의 DEM 및 하천측량자료를 이용하여 수행하였다. 국토지리정보원의 DEM과 하천지형 보완을 수행한 DEM(하천측량자료)을 이용하여 대상유역의 지형구축을 하였으며, HEC-GeoRAS/HEC-RAS 및 CCHE-2D를 이용하여 RS/GIS 프로그램과 연계한 홍수범람분석을 수행하였다. 또한 이상홍수시에 홍수범람 양상을 산출하고 하천지형 보완 유무에 따른 비교를 함으로서 RS/GIS를 이용한 수리분석시 지형자료의 활용성에 대해 알아보고자 한다.

A Method for Detecting Movement and Posture During Sleep Using an Acceleration Sensor of a Wearable Device (웨어러블 단말의 가속도 센서를 이용한 수면 중 움직임 및 자세를 감지하는 방법)

  • The number of patients with many complications grows with the increase of aging population. As the elders and severely ill patients spend most of their time in bed, it leads to Pressure Injuries (PI) such as bedsores. Unfortunately, there is no method to automatically detect changes in patient's posture which leads to the need for a caregiver every set of times when the patient needs to be moved. Many studies are conducted to solve this inefficient problem. Yet, these studies require costly devices or use methods that disturb patient's sleeping environment. Those methods are mostly hard to implement in practice due to these reasons. We propose a method to detect posture using a three-axis acceleration sensor from the wrist band. We developed a wearable watch that measures sleep-related data. We analyzed 40 people's sleep data with a wearable module and watch to measure their postures such as supine, left-side, and right-side. Then, we compared the classified posture from the watch with the wearable module and achieved 90% accuracy. Therefore, we concluded that only by using the wearable watch, we can detect the sleeping position without any new equipment or system to diagnose the patients without discomfort during their daily lives.
