  • Charles Lindbergh has experienced the uncontrolled situation of the aircraft while he was asleep with open eyes during his Atlantic crossing flight around 20 hours after departure from the New York in 1927 because of the fatigue. According to the NTSB(National Transportation Safety Board) reports, about 40 incidents are related fatigue in each year. The sleep loss is the most wanted item for the NTSB and may cause the similar effect as the alcohol does. The fatigue may affect the management of the threats and errors especially during take-off and landing. In this study, threats and errors of major accidents that NTSB analysed fatigue as a causal factor will be identified in order to understand the seriousness of fatigue.

  • The aim of this study was to investigate the effects of death education on the attitude toward DNR, fatigue, and quality of sleep for generic care worker. This study followed a nonequivalent control group and a non-synchronized design. This study was performed in a visiting generic care service institution in J city, Chung-Cheong province, Korea between February 15 to May 30, 2016. The study included 43 participants who agreed to participate in this study. They were randomly divided into two groups: 21 participants in the experimental group and 22 in the control group. Participants in the experimental group received death education 2 hours per day, once a week, for a duration of greater than 8 weeks. The effect of treatment was measured using a structured questionnaire on the attitude toward DNR, fatigue, and quality of sleep before and after 8 weeks of intervention. Data were analyzed using t-, chi-square, Fisher Exact- and paired t-tests. The experimental group showed a significantly increased attitude toward DNR (p=0.001) and quality of sleep (P<0.001), whereas significantly decreased attitude toward fatigue (p=0.030) than the control group after 8 weeks of intervention. The death education program was shown to be an effective nursing intervention for generic care workers. Therefore, we can consider the possibility of incorporating death education in the nursing program.

  • Recently, about 600 cases of marine accidents occur annually in Korea. According to many studies and analyses on occurrence of marine accidents, 70~80% of marine accidents were caused by human factors. Among the human factors, navigation officers' fatigue is very important factor. Although navigation officers' fatigue serves as an important role in marine accidents, there is no method to exactly examine the degree of officers' fatigue. Accordingly, this study analyzed human factors according to types of marine accidents and extracted important five factors affecting navigation officers' fatigue through the questionnaire survey by means of literatures and 5-point scale. In addition, evaluation factors of marine accident risks caused by fatigue factors were divided and structured by using ISM. Lastly, it found out the importance of each fatigue factor drawn by AHP and decided marine accidents that were most highly caused by navigation officers' fatigue in order. At the result, weights were high as sleep time 0.385, stress 0.302, health condition 0.139, rest time 0.099, alcohol and drug 0.074 in fatigue factors, and death and injury 0.328, collision 0.308, grounding 0.195, sinking 0.094, fire accident 0.075 in evaluation factors of marine accident risks. Therefore, the control plan to lower marine accident risks should be prepared on the basis of high weight factors.

  • Central sleep apnea (CSA) is a highly prevalent comorbidity in patients with heart failure and may present in 25 to 40 percent of heart failure patients. Continuous positive airway pressure (CPAP) is the primary therapeutic option and effective in treatment of obstructive sleep apnea (OSA). In heart failure patients with CSA, several trials of CPAP showed a number of positive effects in heart failure treatment. A 58-year-old male visited the hospital because of dyspnea and he was diagnosed as heart failure with ischemic heart disease. He underwent coronary angiography and received percutaneous coronary intervention due to stenosis at the middle of left anterior descending coronary artery. However, dyspnea was not completely improved after treatment with percutaneous coronary intervention. The patient also experienced snoring and sleep apnea which worsened with symptom of dyspnea in the recent year. We suspected CSA and the patient underwent polysomnography to confirm whether sleep apnea was present. During the polysomnography, CSA with Cheyne-Stokes respiration (CSR) was observed and apnea-hypopnea index was 45.9/hr. The patient was treated with CPAP. After CPAP treatment, hypoxemia and CSA were resolved and dyspnea was improved with reducing NYHA class. We report a case successfully treated with clinical improvement by presuming CSA in a patient with heart failure.
