Sleep is known to be a major factor that reduces the quality of life in the human body.This study aimed to investigate the relationship between sleep and sleep in a group that has been diagnosed with diabetes. This study used raw data from the Community Health Survey and targeted a total of 227,754 people. Data analysis used cross-tabulation and logistic regression analysis. As a result of the analysis, gender, age, diabetes experience, subjective health status, sleep time, smoking, drinking, and blood sugar awareness were different from the diabetes experience group. In the group with diabetes diagnosis experience, the odds ratio for sleep time was 1.4 times higher in the group with 5 hours of sleep than in the group with 6 to 7 hours of sleep. In the future, empirical research may be needed to determine the relationship with sleep time, and this study showed the importance of sleep time. Based on the importance of sleep time, there is a need to develop a health management program that considers the importance of sleep for those who have been diagnosed with diabetes.
Central sleep apnea (CSA) is a highly prevalent comorbidity in patients with heart failure and may present in 25 to 40 percent of heart failure patients. Continuous positive airway pressure (CPAP) is the primary therapeutic option and effective in treatment of obstructive sleep apnea (OSA). In heart failure patients with CSA, several trials of CPAP showed a number of positive effects in heart failure treatment. A 58-year-old male visited the hospital because of dyspnea and he was diagnosed as heart failure with ischemic heart disease. He underwent coronary angiography and received percutaneous coronary intervention due to stenosis at the middle of left anterior descending coronary artery. However, dyspnea was not completely improved after treatment with percutaneous coronary intervention. The patient also experienced snoring and sleep apnea which worsened with symptom of dyspnea in the recent year. We suspected CSA and the patient underwent polysomnography to confirm whether sleep apnea was present. During the polysomnography, CSA with Cheyne-Stokes respiration (CSR) was observed and apnea-hypopnea index was 45.9/hr. The patient was treated with CPAP. After CPAP treatment, hypoxemia and CSA were resolved and dyspnea was improved with reducing NYHA class. We report a case successfully treated with clinical improvement by presuming CSA in a patient with heart failure.

The purpose of this study was to examine the relevance of mental health and nutritional intake according to the sleep duration of Korean adolescents. This study was based on data from the 2007-2016 Korea National Health and Nutrition Examination Survey(KNHNES), including 5,489 total subjects (2,795 middle school students, 2,694 high school students). The association between sleep duration and mental health was analyzed using a logistic regression analysis, and the link between sleep duration and nutritional intake was analyzed via a generalized linear model. An analysis of sleep duration showed that middle school students had a higher average sleep duration than high school students (P<0.0001). An analysis of the relationship between sleep duration and mental health showed that middle school students had lower rates of stress perception (P<0.0001) and suicidal ideation (P=0.0005) as their sleep duration increased. High school students had 53% less suicidal ideation in the group getting 6-7 hours compared to the group getting less than 6 hours, and 37% less suicidal ideation than the group getting 7-8 hours. The link between sleep duration and stress perception was statistically significant among both middle and high school students (P for interaction=0.02). An analysis of the daily intake of major nutrients according to sleep hours found high intake of vitamin C in groups where high school students slept more than nine hours (P=0.003). The state of nutritional intake according to higher sleep duration showed statistically significant differences between higher intake of phosphorus, riboflavin, niacin, and vitamin C in Nutrient Adequacy Ratio for high school students. In conclusion, adolescents' sleep duration is associated with stress perception, suicidal ideation and nutritional intake. Therefore, this study emphasizes the mental importance of adolescent sleep and can be used as a basis for nutrition education.
Objectives: Obstructive sleep apnea syndrome(OSAS) is known to be associated with the changes of autonomic nervous system (ANS). Nasal continuous positive airway pressure(nCPAP) treatment was found to correct abnormal ANS changes in OSAS but it remains to be further clarified. We aimed to assess the effects of nCPAP on ANS manifested on electrocardiogram, using spectrum analysis in the subjects with OSAS. Methods: Digital polysomnography was performed in 18 patients with OSAS(mean age $43.7{\pm}16.6$ years ; 17 males, 1 female ; mean respiratory disturbance index (RDI) $48.6{\pm}20.9$) for one baseline and another CPAP nights. From each night, 300 continuous beats of ECGs without artifact were chosen from both stage 2 sleep and REM sleep and they were used for power spectrum analysis. We compared between baseline and CPAP nights the heart rate variability including VLF(very low frequency power), LF (low frequency power), HF(high frequency power), R-R means, R-R variance, and LF/HF ratio, using Wilcoxon signed ranks test. Results: In all patients, nCPAP proved to be effective in relieving apneas and snoring. During nCPAP night compared with baseline night, decreases in VLF(p<0.05), LF(p<0.01), and R-R variance(p<0.05) were found in stage 2 sleep, and decreased LF(p<0.05) was found in REM sleep. No significant differences in each sleep stage were found in other variables between the two nights. Conclusion: Our findings suggest that OSAS increases the activity of sympathetic nervous system and nCPAP application effectively decreases the activity. And nCPAP does not appear to influence the parasympathetic nervous activity in OSAS.
The present study compared the actigraphic indices between both wrist actigraphies (WATGs), and the sleep estimates between each WATG and nocturnal polysomnography (NPSG) to assess their differences and consistencies. We studied 22 right-handed subjects (mean age $43.9{\pm}13.3\;years$, M:F=14:8) with untreated primary sleep disorders (primary insomnia=8, simple snorer=2, obstructive sleep apnea=12) undergone by overnight both WATGs and NPSG, simultaneously. Comparison and correlation were analyzed between right and left wrist actigraphic data. In the sleep estimates of both WATGs and NPSG, each WATG was compared and correlated with NPSG in sleep period time (SPT), total sleep time (TST), sleep latency (SL), sleep efficiency (SE) and wake time (WT). Sleep indices between both WATGs showed significant positive correlations with no correlations in SL and fragmentation index (FI). There were no differences in sleep indices between both WATGs. SPTs of both WATGs, SL of left WATG, and TST of right WATG showed positively significant correlations, and SE of right WATG did negatively significant correlation in sleep indices between each WATG and NPSG. As each WATG was compared to PSG, SPTs of both WATGs and WT of right WATG were decreased, and TST and SE of right WATG and SL of left WATG were increased. Inconsistent SL and FI between both WATGs indicate that the activities between both WATGs can differentially happen during wake or arousal. Inconsistent sleep estimates between each WATG and NPSG may indicate the limited usefulness in measuring and analyzing one-night sleep by using WATG.

저수지나 하천 사면에서 발생하는 산사태와 토석류는 저수지와 하천 수체에 충격을 가한다. 이로 인해 발생하는 수면 충격파는 전파되어 반대편 제방으로 파의 처오름 또는 댐 제체위로의 물넘이로 큰 피해를 줄 수 있다. 최근 외국에서는 2차원 충격파 생성 및 전파의 기본 과정을 구명하기 위한 실험적 연구가 이뤄지고 있으며, 이들 연구들은 충격파의 발생과 전파, 사면활동 물질과 수체의 상호작용 그리고 자유 수면과 유속분표의 발달에 대한 자세한 관측 자료를 제시하고 있다. 아울러 충격파에 영향을 주는 지배 매개변수를 제시하고 있다. 하지만, 이러한 실험적 연구의 최근 진보에도 불구하고, 이들 지배 매개변수를 고려한 충격파 지배공식들은 대상 지역의 복잡한 바닥 지형이나, 평면적 지형 변화를 단순한 추정치로만 고려하게 된다. 따라서 복잡한 지형조건에서 토석류와 수체의 상호작용과 수면 충격파의 전파를 합리적으로 해석하는 데는 한계가 있다. 이 경우 수치모델링 기법을 대안으로 적용할 수 있으나, 수치모델링은 수면에서 충격파의 전파와 수중에서 토석류의 전파를 동시에 모의해야 하고, 뉴턴 유체와 비뉴턴 유체의 특성을 동시에 고려해야하므로 수치해석 연구자들에게는 하나의 큰 도전사항이다. 이 연구는 경계면 포착기법을 이용한 계산유체동력학 기법을 이용하여 사면활동과 이로 인한 정지 수역에서의 충격파의 발생 및 전파를 재현하기 위한 수치 모델링 기법을 개발하는 것이 목적이다. 사면활동과 수면의 경계면을 포착하고 위치를 정립하기 위해서 VOF (volume of fluid) 경계면 재구축 기법을 이용한다. 지배 방정식은 비압축성(incompressible) 질량 보존방정식과 나비어-스톡스(Navier-Stokes) 방정식이며, 서로 다른 유체의 상(phase)애 대한 체적분할이송방정식을 이용한다. 큰와 모의 계열의 난류 모델링 기법을 적용하여 충격파의 전파와 붕괴에 대한 난류의 영향을 고려하였다. 토석류는 비뉴턴 흐름저항 관계식을 적용하여 그 흐름특성을 재현하였다. 이들 지배방정식은 2차 정확도의 유한체적법(finite volume method)을 이용하여 해석한다. 외국의 연구자들이 관측하여 제시한 길이 11 m 그리고 폭 0.5 m의 수로에서 발생한 충격파를 수치적으로 재현하여 개발된 모형의 실제 문제에 대한 적용성을 보여준다.

Objectives: To find evidence that sleep is necessary for normal brain function, thus indicating that declines in both sleep quality and quantity are related to worse performance of many daily tasks and deteriorated physical functions. The present study investigates the relationships of balance control and reaction time with sleep quality. Methods: 58 healthy (male 20, female 38) volunteers with informed consent participated in this study. The Self-reported Pittsburgh Sleep Quality Index (PSQI) was used to evaluate sleep quality and relevant factors, and the subjects were divided into groups A (PSQI < 5) and B ($PSQI{\geq}5$) based on this index. Static balance control and reaction time to visual stimuli were conducted to assess their relationship with sleep quality. Results: Group B exhibited excessive daytime sleepiness significantly more often compared to group A. Static balance control did not markedly change relative to sleep quality, but reaction time and error to visual stimuli were significantly increased in group B compared to group A. Conclusion: These findings indicate that a decline in sleep quality can result in delayed reactions, as well as decreased accuracy of these reactions. They also suggest that low sleep quality may be associated with changes in physical functions, including balance control through reduced selective attention.
The data collected to date indicate that sleep-related breathing disorders, including sleep-disordered breathing(sleep apnea) and underlying respiratory system diseases, are one of the important risk factors for cardiovascular dysfunction. Sleep-disordered breathing(sleep apnea) is now recognized as one of the leading causes of systemic hypertension, cardiac arrhythmias, coronary heart disease, pulmonary hypertension, right heart failure, and stroke. Sleep may exert a profound effect on breathing in patients with underlying respiratory system disease including bronchopumonary diseases, chest wall abnormalities, central alveolar hypoventilation syndromes or respiratory neuromuscular disorders. Chronic hypoxia and hypercapnia in these patients may accelerate the development of long term cardiovascular complications such as cardiac arrhythmias, pulmonary hypertension, and right heart failure(cor pulmonale). Several recent studies reported that sleep-related breathing disorders are associated with long-term cardiovascular morbidity and mortality. Careful assessment of respiratory and cardiovascular function in these patients is critical. Aggressive and highly effective treatment of sleep-related breathing disorders using tracheostomy, mechanical ventilation, nasal continuous positive airway pressure therapy(nCPAP), intercurrent oxygen therapy or other interventions can reduce the prevalence of cardiovascular dysfunction and the long-term mortality.

Objective: Stress is known to be a common cause of short-term insomnia and insomniacs often complain that stress induces sleep problems. However, previous studies on the correlation between stress and sleep do not show consistent results. We aimed to investigate the effects of minor stressful events on sleep among college students. Method: Physically and mentally healthy college student volunteers filled out a self-assessment questionnaire to evaluate their stress and sleep. To find out the status of average stress and sleep, the volunteers filled out K-DSI and daily sleep assessments on three consecutive days. In addition, we surveyed the amount of caffeine beverage intake and assessed the degree of depression and anxiety. Results: The total number of students participating in this study was 202, 101 men and 101 women. Minor stress turned out to significantly affect non-restorative sleep and secondary symptoms of insomnia (awakening difficulty, displeasure, feeling of dissatisfaction with sleep, physical uneasiness or pain at awakening, daytime sleepiness, depressive moods, tiredness and concentration difficulty). However, global PSQI score, self-reported sleeping hours, sleep latency, awakening frequency, frequency and duration of napping, were not explained by stress scores. Conclusion: In this study, minor stresses seemed to affect sleep, especially secondary symptoms caused by non-restorative sleep. We can thus infer that minor stresses impair the restorative effects of sleep by inducing arousal, and the direct relationship the two can be confirmed by polysomnogram.
