병원감염관리에 관한 인지도와 수행도 - 중소병원 초보간호사를 중심으로 - (Awareness and performance about nosocomial infection management; a early stage nurse in small and medium hospitals)

  • 중소병원에서 근무하는 초보간호사의 병원감염관리에 대한 인지도와 수행 정도를 확인하고, 병원감염에 대한 기초자료를 제공하고자 실시된 서술적 조사연구이다. 2017년 9월15일부터 2017년 9월 30일까지 A시와 B시에 300병상 규모의 중소병원 2곳의 초보간호사 150명중 무응답 및 미흡한 응답자 11명을 제외한 139명을 대상으로 하였다. 본 연구에서 수집된 자료는 SPSS Win 22.0 프로그램을 이용하여 실수, 백분율, 평균, 표준편차, t-test, ANOVA, 사후검정은 $Scheff{\bar{e}}$ test로 분석하였다. 대상자의 일반적 특성에 따라 병원감염관리에 대한 인지도를 비교해 본 결과 요로 감염관리, 호흡기 감염 관리 그리고 소독 오염물품 관리 부분은 연령에서 유의한 차이가 있었고, 수행 정도는 손 씻기 영역에서는 근무부서와 직위, 수액요법영역은 감염관리 교육 필요성 여부(p<.001)에서, 요로감염관리 영역 및 호흡기 감염관리 영역(p=<.001)에서는 직위에 따라 차이가 있는 것으로 나타났다. 또한 병원감염관리에 대한 인지도와 수행 정도에 대한 차이를 살펴본 결과 손 씻기, 수액요법, 요로감염관리, 호흡기 감염관리, 감염성 폐기물 관리, 소독오염 물품관리영역의 모든 영역에서 인지도가 수행도 보다 모두 높게 나타났으며 모두 유의한 차이를 보였다. 경험과 지식이 부족한 초보간호사들이 병원감염관리 간호를 원활하게 수행하기 위해 이론적 지식을 바탕으로 수행도도 높일 수 있는 체계적인 병원감염관리 관련 교육프로그램이 구체적으로 개발 운영되어야 할 것이다.

작업치료실의 감염 실태와 감염예방 관리 실태 : 부산지역을 중심으로 (The Status of Infection for Infectious Diseases and Status of Infection Control in Occupational Therapy - Focusing on Busan)

  • 목적 : 부산 지역 내 작업치료사를 대상으로 작업치료실의 감염성 질환 감염 실태와 감염예방 관리 실태에 관하여 알아보고자 하였다. 연구방법 : 2016년 6월 한 달 동안 감염성 질환 감염 실태, 감염예방 관리 및 감염관리 인식도의 네 파트로 구성된 설문지를 부산 지역 내 작업치료실 23 곳, 작업치료사 111명을 대상으로 실시하였다. 결과 : 34.8%의 의료기관에서 작업치료사들이 옴, 결핵, C-diffficile, MRSA, HFM 등의 감염성 질환을 경험하였다. 작업치료사는 대부분의 감염예방 관리 항목에서 수행도가 높으나, '15초 이상 손 씻기'와 '장갑 착용 전 후에 손 씻기', '치료 시 마스크 착용'과 '사용한 마스크 즉각 버리기', '화장실 이용 시 시트 클리너 사용하기' 에서 낮은 수행도를 보였다. 그 외 '피, 침, 고름 및 분비물로 오염된 세탁물을 일반 세탁물과 분리 세탁하기'항목에서 낮은 수행도를 보였다. 또한 대부분의 작업치료사는 작업치료 임상에서 감염관리 교육은 중요하고 교육이 필요하다고 인식하고 있지만 현재 감염교육은 대부분 근무처에서만 이루어지고 있는 것으로 나타났다. 결론 : 감염예방 관리 수행 향상을 위해서는 치료사 개인의 노력이 필요할 뿐만 아니라, 제도적인 뒷받침이 이루어져야 한다. 또한 본 연구는 차후, 작업치료사들을 대상으로 하는 감염관리 교육 시, 활용할 수 있는 기초자료가 될 수 있을 것이다.

어린이집 원장의 감염병 예방 교육요구도 및 자기효능감 (Educational Needs and Self-efficacy for Prevention of Infectious Diseases of Day Care Center Administrators)

  • 본 연구는 어린이집 원장을 대상으로 감염병 예방 교육요구도, 자기효능감을 파악하고 감염병 예방 자기효능감에 영향을 미치는 요인을 파악하고자 시행되었다. 연구는 G, J, S시 소재 어린이집 원장 172명을 대상으로 설문지를 이용하여 진행하였다. 수집된 자료는 SPSS 24.0 프로그램을 이용하여 기술통계, t-test, ANOVA, Pearson's correlation coefficient, Multiple regression analysis로 분석하였다. 연구 결과, 감염병 예방 교육요구도는 손 씻기 방법 및 시기, 세척과 소독 방법이 높았고, 감염병 예방 자기효능감은 영유아 손 씻기 교육과 시기가 가장 높았다. 감염병 예방 교육요구도와 자기효능감(r=.528, p<.001) 간에는 양의 상관관계가 있는 것으로 나타났다. 감염병 예방 자기효능감에 영향을 미치는 유의한 요인은 감염병 예방 교육요구도(β=.53, p<.001), 감염병 예방 교육의 경험(β=.25, p<.001)로 나타났다. 이러한 연구 결과는 어린이집 원장에 대한 감염병 예방에 대한 교육 요구를 충족시킬 수 있는 프로그램 개발이 필요하다는 것을 보여준다. 이러한 교육을 통해 어린이집 원장의 감염병 예방에 대한 자기효능감이 향상될 것이다.

중환자실 간호사의 간호업무내용에 따른 손씻기에 관한 연구 (A Study of Handwashing by Intensive Care Unit Nurses according to the Content of Nursing Faculty Practice)

  • Purpose: This study was done to determine the rate for handwashing by intensive care unit nurses according to the content of nursing tasks, to investigate the relationship between hand washing practice evaluated by nurses themselves and their actual practice observed, and finality to provide basic materials for strategy for hand washing education. Method: Data were collected by observing 27 nurses working in intensive care units of a hospital in Uijeongbu, Gyeonggi-do and by using observation and a structured self-assessment tool. Collected data were analyzed with SPSS and SAS. Results: The handwashing rate for the nurses was 4.3%. The handwashing rate was high in proportion to the risk of cross infection. In addition, the handwashing rate was highest in nurses working in the neurosurgery intensive care unit. The average score for self-assessment of handwashing was $49.42{\pm}3.78$ points and it was higher than their actual practice of handwashing. Conclusion: In order to improve handwashing by nurses, it is necessary to educate them on the importance of handwashing. In addition, there should be strategies for standardizing knowledge and attitudes to handwashing and inducing nurse:3 to practice hand washing in compliance with the policies and working conditions of the institution.

중환자실에서 적용한 반코마이신(Vancomycin) 내성 장구균의 감염관리 전략 효과 (Effects of Infection Control Strategies for Vancomycin Resistant Enterococci in Intensive Care Units)

  • Purpose: This study was to examine whether VRE infection control strategies have an effect on the decrease in incidence rates for VRE acquisition and VRE nosocomial infection in ICU. Methods: All the patients were examined for VRE carriers on ICU admission. Among them, patients hospitalized for over 48 hours were investigated for VRE acquisition rates and VRE nosocomial infection rate using VRE infection control strategies in ICU for the experimental group from September 2007 to April 2008. Before that, incidence of VRE acquisition and VRE nosocomial infection for the control group without Intervention were investigated from May to August 2007 retrospectively. Results: VRE acquisition rate in clinical specimens was 0.6% in the experimental group, that was significantly lower when compared to the control group. VRE carrier rate at admission to ICU was 15.4%. Out of 182 VRE carriers, 180 patients were identified by the active surveillance culture. Conclusion: These results suggested that active surveillance culture at admission was considered to be an essential measure for detection of VRE carrier. But without strict isolation and adherence rating after each intervention, hand washing and contact isolation alone did not significantly decrease VRE nosocomial infection, although it did significantly decrease incidence of VRE acquired from clinical specimen.

여자 청소년의 비뇨생식기 관련 개인위생과 월경기 위생 관리 (Personal Hygiene Practices related to Genito-urinary Tract and Menstrual Hygiene Management in Female Adolescents)

  • Purpose: The study was to survey personal hygiene practice related to genito-urinary tract and menstrual hygiene management in female adolescents in order to obtain basic information for health education. Methods: With a descriptive survey design, 389 adolescents were recruited via convenience sampling in Korea. Survey instrument was the feminine and menstrual hygiene practice and perception of vaginal douching. Data were collected from a self-administered structured questionnaire. Results: Mean age of adolescents was 16.09 and menarche was at 13.21. While washing hands after urination/defecation was highly performed, wipe front to back and wash with soap and water were reported as being not well done. Twenty-eight percent reported douching habit. More positive beliefs about douching were reported by adolescents who practiced douching. Menstrual hygiene management was very appropriate with changing sanitary pads regularly with hand washing; but less performed for limiting bathing activity during menstrual periods and washing hands after activity of genito-urinary area. Conclusion: Some adolescents practiced inadequate hygiene practices especially for body cleansing during menstrual period and vaginal douching. It is important to develop and implement school health education programs on feminine and personal hygiene for adolescents to help them perform adequate health behaviors.

중년여성의 여성생식기 관련 위생과 질 세척 행위 (Feminine Hygiene and Vaginal Douche Practices in Middle-aged Women)

  • Purpose: The study was done to survey feminine hygiene and vaginal douche practices in middle-aged women to obtain basic information for public health education. Methods: With a descriptive survey design, 134 middle-aged women who have menstrual period were recruited via convenience sampling. Survey contents were from the study by Czerwinski (2000) regarding feminine hygiene and vaginal douching practices. After obtaining IRB approval, a self-administered structured questionnaire was distributed to study participants. Results: Mean age of middle-aged women was 46.88 years old. Fifty-six percent of the women practiced vaginal douching. Women performed douching with water (68%), commercial products (13%), soap (12%), and vinegar-mixed water (6%) for clean and fresh feeling, removal of unpleasant odor, and removal of vaginal discharge. Vaginal douche practice was significantly related to a history of vaginitis. Conclusion: The study results indicate that most of the women practiced inadequate feminine hygiene especially in douching, suggesting that is important to develop education programs on feminine hygiene practices for women of all ages, especially regarding douching and hand washing before and after changing pads or tampons.

초등학생의 코로나19에 대한 인식과 손 씻기 지식, 태도 및 실천 (Perception of COVID-19, and Knowledge, Attitude and Practice of Hand washing in Elementary School Students)

  • Purpose: This study aimed to investigate factors affecting handwashing practice among elementary school students by assessing the perception of coronavirus disease-19 (COVID-19) and characteristics related to knowledge, attitude, and practice of handwashing. Methods: The cross-sectional study design and an online self-reported survey were used in this study. The subjects were 211 elementary students at a school located in G province, Korea. Data were collected from January 4 to 8, 2021. The data were analyzed using SPSS version 25.0. Results: Mean scores for knowledge, attitude, and practice of handwashing were 17.09 (out of 20), 21.15 (out of 24), and 27.51 (out of 33), respectively. There were significant positive correlations between the perception of COVID-19 and knowledge of handwashing (r=.23, p=.001), between knowledge and attitude toward handwashing (r=.45, p<.001), between knowledge and practice of handwashing (r=.18, p=.010), and between attitude and practice of handwashing (r=.28, p<.001). The factors influencing handwashing practice were handwashing frequency at home (β=.39, p<.001), handwashing frequency at school (β=.18, p=.006), and attitude toward handwashing (β=.15, p=.026), which explained 34.2% of the variance. Conclusion: These results suggest a need to raise awareness of the importance of handwashing and develop an education program for infectious disease prevention.

  • 본 논문은 유아기 아이들에게 올바른 위생관리 습관 확립과 화장실 내 안전사고 예방을 도와주는 "Kids Self Care Mirror" 시스템을 제안한다. 제안하는 주요 기능은 다음과 같다. 첫째, 적외선/온습도 센서 등을 통해 실내 LED 및 환풍기 제어와 습도로 인한 미끄러움을 경고등과 경고음을 사용하여 방지한다. 둘째, 카메라 모듈을 사용하여 사용자를 인식하고 디스플레이를 통해 아이들을 위한 양치 교육 동영상을 재생시킬뿐만 아니라 치아 상태에 따른 치약을 추천한다. 또한 액체비누 디스펜서 버튼을 누르면 손 씻기 교육 동영상이 재생된다. 셋째, 액추에이터를 사용하여 사용자의 키에 맞게 거울의 높낮이를 자동으로 조절한다. IoT 기술과 OpenCV, 치아 관리 알고리즘을 활용한 "Kids Self Care Mirror" 시스템을 통해 아이들에게 발생할 수 있는 각종 질병과 사고를 방지할 수 있을 것으로 기대된다.

조리종사자의 손 씻기 의식과 실천 및 손의 지표미생물 오염도에 관한 연구 (A Study on the Hand Washing Awareness and Practices of Food-service Employees and the Load of Index Microorganisms on the Hands)

  • Hand-washing is one of the major factors in personal hygiene and public health. This study was undertaken to investigate the hygienic behavior of food-service employees, focusing on awareness of hand washing, hand washing practices, and the load of index microorganisms (aerobic plate count, total and fecal coliforms, Escherichia coli, and Staphylococcus aureus) on the hands of food-service employees. A questionnaire survey completed by direct interview, direct observation of restrooms by the researcher and trained observers, and microbiological examination according to the Food Code of Korea were carried out. In the survey, a positive attitude toward hand washing compliance was reported; however, improper hand washing and poor hand hygiene of the food-service employees were seen under direct observation. Significant differences (p<0.05) were found between the questionnaire survey and the direct observations in hand washing compliance after using the toilet, duration of hand washing, use of hand washing agents, use of hand washing tools, washing of different parts of the hands, hand-drying method, temperature of water, and method of turning off the water. Samples taken from employees' hands before washing showed higher levels of bacteria than those taken during work and/or after washing (p<0.05). Poor hand washing practices were indicated by the positive results for total and fecal coliforms, E. coli, and S. aureus on the hands of some food-service employees. This study showed that there is a marked difference between the food-service employees' awareness of hand-washing and their actual hand-washing practices. The poor hand hygiene of and improper hand washing by the food-service employees should be addressed for improved food safety.
