독성 표피괴사 증후군(Toxic epidermal necrosis)은 매우 드문 수포성 피부 질환으로 표피 전층의 괴사와 박리를 특징으로 하는 질환이며, 적절한 치료에도 불구하고 높은 사망률을 보이는 질환이다. 대부분의 독성 표피괴사 증후군의 약물과 연관이 있으며, 대표적인 약물로는 설폰아마이드(Sulfonamide), 항경련제(Phenytoin, Phenobarbital), 페니실린계 항생제, 비스테로이드성 소염제 등이며, 드물게 부신 피질 호르몬제에 의한 발병도 보고되고 있다. 최근 독성 표피괴사 증후군에서 각질 세포의 세포고사(cell apoptosis)에 Fas 수용체가 중요한 역할을 한다고 알려진 후 치료에 면역 글로불린 사용이 점차 늘어나고 있으며, 생존율의 증가와 치료기간의 단축이 보고되고 있다. 국내에서는 아직 독성 표피괴사 증후군에서 면역 글로불린정주가 치료방법으로 확립되지 않았지만 몇몇의 경우에서 면역 글로불린 정주로 치료한 경험들이 보고되고 있다. 저자들은 막성 신염 환아에서 부신 피질 호르몬제와 안지오텐신 수용체 길항제 사용 후 발생한 독성 표피괴사 증후군에서 면역 글로불린을 사용하여 치료한 1례를 경험하였기에 문헌 고찰과 함께 보고하자 한다.

Quinoline, pyrazolo-[5, 4-b]-pyridine, isoxazolo-[5, 4-b]-pyridine, pyrazolo-[4, 3-c]-quinoline, isoxazolo-[5, 4-e]-thiazine, and isothiazolo-[5, 4-e]-thiazine derivatives were prepared as possible antiinflammatory agents. Some of the synthesized compounds showed antiinflammatory activities comparable to Aspirin and Naproxen.

Effects of Allolobophora caliginosatrapezoides (Ac) polysaccharide fractions on the inflammation and hypersensitivity were studied in vivo. It showed that Ac polysaccharide fractions have the significant inhibitory activities of inflammation and hypersensitivity; They inhibited significantly the carrageenin-induced paw edema and acetic acid-induced writhing syndrome. They also inhibited significantly the Arthus reaction and delayed hypersensitivity in the sheep red blood cell-sensitized mice in accordance with the inhibition of haemaglutinin titer, haemolysin titer, plaque-forming cells and rosette-forming cells. They also improved markedly the oxazolone-induced dermatitis in rats dose-dependently. As the above results, it exhibited that Ac polysaccharide fraction inhibited not only humoral immune response, but also cell-mediated immune response. It seemed that methanol and ether extracts have also another physiological active agents.

투구운동은 견관절에 반복적인 압박력 및 신장력을 가하게 되며 이는 견관절의 정적 안정력의 약화를 유발하게 된다. 초기의 경도의 불안정성은 근육 등 동적 안정력의 작용으로 안정적인 운동상태를 유지할 수 있으나 반복적인 운동이 지속될 경우 근육이 피로해 져서 동적 안정력의 보상작용을 통한 안정성을 얻을 수 없게 된다. 이는 일차적으로 견관절의 불안정성을 유발할 뿐만 아니라, 이차적으로 견봉하 충돌증후 및 내적 충돌증후 등의 증상을 나타내기도 한다. 과 투구로 인한 대부분의 운동손상은 보존적 치료로 좋은 결과를 볼 수 있으며, 이는 일차적으로 투구를 중지하여 관절을 안정시키며 진통 소염제나 물리치료 등을 통한 항 염증치료, 그리고 회전근개 및 견갑골 주위근육의 강화운동 등의 단계로 진행된다. 적절한 보존적 치료에도 불구하고 증상이 지속될 경우 정확한 진단에 근거한 수술적 치료 및 조절된 재활운동 프로그램을 통해 조기 운동 복귀를 기대할 수 있다.

New prednisolone derivatives including methyl $20{\xi}-acetoxy-prednisolonate$ have been synthesized and evaluated for local anti-inflammatory agents with reduced systemic side effect. Methyl $20{\beta}-dihydroprednisolonate$ and methyl $20{\xi}-acetoxyprednisolonate$ showed comparable anti-inflammtory activity in vivo with significantly reduced systemic side effects.

Salicylic acids as anti-inflammatory agents were analyzed by ab initio, quantum chemical methods to study the possible modes of binding to the receptor. As the result of multiple regression analysis of reactivity indices and interpretation of normalized frontier orbital charges of drugs, potency seems to be related to energy of HOMO and LUMO at the 5 position of benzene ring, and in the 5-phenyl substituted case, the para position of substituting ring is important. The binding occurs first at the positive site of its receptor. The charge density exhibited by the frontier orbitals suggests that charge moves from receptor site to carboxyl group. The electrostatic orientation effect makes an important contribution to the binding of the active molecules to their receptors. Also the electrostatic potential model may be able to rationalize the source of activity or inactivity of the drugs under investigation.

Gastric cancer is one of the leading causes of cancer-related deaths worldwide. The increased expression of cyclooxygenase (COX)-2 has been implicated in the development and progression of gastric cancers. A number of recent studies have been published evaluating the chemopreventive effect of aspirin and non steroidal anti inflammatory drungs (NSAIDs) against gastric cancer. Aspirin and NSAIDs use may reduce the risk of gastric cancer incidence and death, whereas other studies have reported contradictory results. Therefore, further study should be needed to clarify the role of aspirin and NSAIDs in the chemoprevention of gastric cancer.

소염제 piroxicam (PX, 4-hydroxy-2-methyl-N-2-pridyl-2H-1, 2-benzothiazine-3-carboxadiamide-1,1-dioxide)의 착물형성을 전이금속이온, Co(II), Ni(II), Cu(II), Zn(II)과 함께 메탄올(MeOH)/물 이성분 혼합물에서 25$^{\circ}C$, 일정한 pH=5.0와 I=0.1 M에서 분광광도법으로 연구하였다. 컴퓨터 프로그램 SQUAD를 스팩트라 데이터로부터 원하는 정보를 얻는데 사용하였다. Fitting 과정의 output은 안정도 상수, 평가한 안정도 상수의 표준편차, 농도분포 다아그램, 모든 종의 스팩트럼 프로파일이다. Co(II), Ni(II), Cu(II), Zn(II)의 PX 착체의 안정도 순서는 Cu(II) > Co(II) > Ni(II) ${\approx}$ Zn(II) 순서이다. 이것은 이들 금속이 온들이 기하학적 경향이 다른 이유일 것이다. PX의 산성도 상수는 다른 pH 값에서 흡수 스펙트럼으로 부터 위의 조건에서 역시 결정하였다. 컴퓨터 프로그램 DATAN을 PX의 산성도 상수의 결정하는 데 사용하였다. 산성도 상수의 validity는 잘 알려진 컴퓨터 프로그램 SPECFIT/32을 사용하였다. 안정 및 산성도 상수의 용매성질, 음이온과 같은 다른 인자의 효과에 관하여 자세하게 논의하였다.
