  • 1. Background and Purpose: I intended to understand the medical and pharmacological theory of Taeumin, through a study of the process through which Lee Je-ma discovered the Interior-Overheated-Disease of Taeumin and created the prescriptions for it. 2. Methods: I studied and analized the change in the medical and pharmacological theory, through a historical study on the quotations and prescriptions of "DongYi Soose Bowon". 3. Results: Through a literature study I could find that in the existing Oriental Medicine before Lee Je-ma, the difference of the Liver Febrile Disease of Taeumin and the Stomach Febrile Disease of Soyangin were recognized, and the prescriptions of the two diseases were a little distinguished, but the medical theories of those were not distinguished at all. And I found that the Liver Febrile Disease of Taeumin shows the pain in the eyes and the throat, and drying of the nose due to interior dry and heat, so it is different from the Stomach Febrile Disease of Soyangin that shows irritation of fever, headache, and the trouble in passing feces and urine. Also I could know that Radix Puerariae, Rhizoma Cimicifugae, Rhizoma Ligustici, Radix Angelicae Dahuricae, and Radix et Rhizoma Rhei are Taeumin's medicines because these medicines had been used for Taeumin's syndromes, and that Radix Scutellariae, Radix Platycodi, Semen Raphani, and Fructus Gleditsiae are Tae-Eum-In's medicines because these medicines are known as acting for lung.

  • Background: Pruritus is the most frequent undesirable symptom associated with epidural morphine. It is unpleasant and often difficult to treat. Naloxone is presently the drug of first choice for treating this symptom. Naloxone however decrease the pain threshold in some cases. Recently it was reported subhypnotic doses of propofol were efficient in relieving epidural-morphine-induced pruritus(EMIP). In a prospective. randomized, double-blinded clinical trial, we compared the efficacy of propofol and naloxone for treatment of EMIP. Methods: Forty patients with EMIP were allocated to receive either 20 mg propofol, or 1.5 ${\mu}g/kg$ naloxone intravenously. Pruritus and level of postoperative pain were assessed after 5 min, using pruritus rating scale and visual analogue scale. Results: The overall success rate in treating pruritus was similar in both groups (propofol 70% vs naloxone 65%). Twenty-five percent of the patients in the naloxone group had an increase in the level of postoperative pain versus none in the propofol group(P=0.018). Conclusions: These results suggest propofol and naloxone are equally effective in treating EMIP. However, the level of postoperative pain is significantly reduced when treated with propofol.

One patient took Chijasi-tang for 30 days and the other took it for 15 days. We compared patients' symptoms before and after treatment using self-made standards. Results : Patients who complained of pruritus on the elbow and thigh, pruritus decreased from NRS 8 to 1, and skin symptoms improved from 16 to 2. The other patient who complained of pruritus in the neck and chest, pruritus improved from NRS 9 to 2, and skin symptoms at the lesion site improved from 12 to 2. Also, accompanied symptoms such as heat of the feet and heartburn almost disappeared. • Purpose: The purpose of this study was to compare characteristics of two groups (pruritus and non pruritus group), in hemodialysis patients. Method: The degree of ichting in 130 patients treated at the D Medical Center Hemodialysis Unit was examined by itching severity & sites tool, itching observation tool and itching NRS tool. The data were collected from October 1 to 12, 2007 and analyzed by student t-test and Mann-Whitney test. Result: Sixty three patients (48.3%) of hemodialysis patients had pruritus. The severity and number rating scale score of pruritus in one day before hemodialysis was higher than during and after hemodialysis. The frequency of dialysis and usage of anti-histamine drug were significantly lower in nonpruritis group than in pruritus group (F=5.209, p=.022, F=6.549, p=.010). The other general characteristics, hemodialysis-related characteristics and laboratory data were not significantly different between the two groups. The depression score was significantly higher in pruritus than in nonpruritus group (t=-2.505, p=.017). But there were no significant differences in hostility and anxiety between the two groups. Conclusion: Residual renal function and depression were associated with pruritus in hemodialysis patients. Efforts for preservation of residual renal function and emotional support may decreased severity and frequency of pruritus in hemodialysis patients.

  • A 6-month-old, male, American cocker spaniel was presented with severe pruritus, alopecia and head shaking. In physical examination generalized erythema, papules, scales and crust were observed. Otic discharge was dark-brown colored in both ear canals. After extensive dermatologic diagnostic evaluations, the infestation of scabies, ear mites, Malassezia spp, cocci were proved. Therefore, he was treated with selamectin 6 ㎎/㎏ which was applied to the skin in a single spot twice a week for 2 weeks in order to investigate the effect of selamectin on canine scabies and ear mites by this treatment protocol. When he was rechecked at 14th day after fourth being administered, scabies and ear mites wasn't seen. The dermatological signs, pruritus, papules were remarkably decreased and only slight scales were observed. After treatment of scabies and ear mites, the clavamox and koto-conazole was administered for cocci and Malassezia spp, respectively, for 1 week and topical therapy was initiated. with sebolytic and chlorhexidine shampoo once a week for 3 weeks. Two months later skin lesions were normal dermatologically and the hair was fully recovered. Despite of the expensiveness the selamectin is very effective in the dogs that infected by scabies and ear mites as this treatment protocol.
