  • 영화 <자전차왕 엄복동>은 평범한 인물 엄복동(정지훈 분)이 자전차(자전거) 한 대로 조선의 희망이 된 이야기를 그린다. 우연한 기회를 통해 자전거를 접한 엄복동은 타고난 재능과 끝없는 노력으로 각종 대회에서 우승을 휩쓴다. 그러나 엄복동처럼 고강도 훈련을 지속하다보면 건강에 해를 끼칠 수 있다. 특히 운동 후 근육통이 오래 지속되거나 소변색이 짙어질 경우 '횡문근융해증'을 의심해봐야 한다.

A Study on the Preparation of Polyurethane Diagnostic Membrane for Urine Glucose Test (요당 시험을 위한 폴리우레탄 진단막의 제조에 관한 연구)

  • The preparation procedure and optimal composition of polyurethane diagnostic membranes were described to measure the glucose concentration in a urine. Vessel size, blade size and the ratio of solvent mixtures were found to be critical factors to get the better sensitivity and the stability of the color which appeared on polyurethane membranes after the reaction with the urine glucose. These urine strips made of polyurethane membranes made it possible to measure the urine glucose quantitatively because they showed a good color separation at glucose concentration from 30mg/dL to 500mg/dL.

The Effect of Fluid Intake Enhancing Program for Institutionalized Elderly (시설노인의 수분 섭취 강화 프로그램의 효과분석)

  • The purposes of this study were to 1) develop fluid intake enhancing program for the institutionalized elderly and 2) examine the effect of fluid intake enhancing program on amount of daily fluid consumed, urine specific gravity, and urine color. Data were collected from 39 nursing home residents in a nursing home located in urban Chung-chung providence. With a convenient sample of 39 nursing home residents, consecutive three days of 24 hour fluid intake, were measured and recorded. Urine samples were obtained and urine specific gravity, urine color were analyzed at pre-intervention, 4 weeks, and 6 weeks following the intervention. When compared to pre-intervention, the average amount of daily fluid intake was significantly increased at 4 weeks and 6 weeks following the intervention. The proportion of subjects who consumed less than Adequate Intake(AI) was 35.9% at pre-intervention and was decreased to 10.3%, 7.7%. In conclusion, inadequate fluid intake among institutionalized elderly is prevalent. From careful employment of the fluid intake enhancing program, increase in fluid consumption among institutionalized elderly can be expected.

  • Idiopathic renal hypouricemia is a disorder characterized by impaired urate handling in the renal tubules. Most patients with hypouricemia are asymptomatic and are found incidentally, but the condition is known to be at high risk for exercise-induced acute renal failure or urolithiasis. URAT1 protein encoded by SLC22A12 gene has been identified recently as a urate/anion exchanger in the human kidney. Inactivation mutations in SLC22A12 gene have been shown to cause renal idiopathic hypouricemia. We experienced a 3-year-old boy who presented with persistent orange-colored urine since infancy. His urine contained many uric acid crystals, while the serum showed hypouricemia(0.7 mg/dL). The fractional excretion of uric acid was increased to 41.7%. SLC22a12 gene analysis revealed W258X homozygote alleles. Renal hypouricemia must be included in the differential diagnosis of red-urine and SLC22A12 gene analysis is recommended in idiopathic renal hypouricemia.

The Effects of a Fluid Intake Intervention for Elders in Long-term Care Hospitals

  • Purpose: This study was done to evaluate the effects of a fluid intake intervention on increasing fluid intake and ameliorating dehydration status in elders admitted to long-term care hospitals. Methods: A nonequivalent control group, pretest and posttest design was used. The experimental group of 39 participants received the 4-week intervention while the control group of 38 participants received routine care. Outcome variables were daily fluid intake and physiological indexes such as blood urea nitrogen and creatinine ratio (BUN/Cr), urine specific gravity (USG), and urine color. Results: After the intervention to increase fluid intake, there were statistically significant increases in daily fluid intake, normal BUN/Cr, and USG in the experimental group. However, a statistically significant improvement in normal urine color was not found for either group. Conclusion: The findings of this study demonstrated that the fluid intake intervention improved hydration status of the experimental group participants. Consequently, it was confirmed that the intervention is considered to be effective in preventing dehydration which occurs frequently in older adults in long-term care facilities and, thus this intervention may contribute to preventing various health issues resulting from dehydration.

  • Alkaptonuria is a rare metabolic disease in which homogentisic acid cannot be metabolized due to a lack of the enzyme homogentisic acid oxidase. The disease often manifests itself in childhood by darkening of the urine upon standing. The disease leads to such serious consequences as ochronosis of cartilage and connective tissues with arthritis. It is expected that treatment with ascorbic acid and a dietary restriction of protein may decrease the late and serious consequences by diminishing the serum concentration of the metabolite benzoquinone acetic acid. A thirteen month-old girl was recently diagnosed with alkaptonuria by urine organic acid analysis. She excreted pinkish urine on a diaper and as time went by the urine color changed to a light brown. In laboratory findings, urine examination and culture results were normal. But urine organic acid analysis detected abnormal findings a prominent and massive elevation of homogentisic acid. The other physical findings were normal. This is the first case diagnosed in Korea.

  • Recently, the miniaturization and digitalization for the inspection devices of point-of-care testing (POCT) are rapidly evolving. In the urine test, a lot of researches on index paper technology are being conducted because people can be self-diagnosed through visual color comparison using a urine test paper, Dipsick. The purpose of this study is to analyze the RGB values from the color changes on Dipstick Pad, which isused for urine test, using a smartphone camera. To this end, the primary, analytes in urine wasdiabetes-related parameters such as glucose, ketone body and pH, which is the most frequently tested elements, and we pursuited to quantify the changes in dipstick color caused from artificial urine containing different ranges of sugar, ketone body, and pH. In this experiment, changes in RGB values under bright and dark illuminances were compared, and changes in RGB value were monitored as a function of concentration of analytes under the ambient illumination of laboratory. As a result, color separation at the bright luminance region was good, but it did not appearat the low luminance region, and the changed profiles in RGB value under different illuminances was suggested to correct the problem of the color separation algorithm.

  • Human urine is a process of excreting waste products in the blood, and it is easy to collect and contains various substances. Urinalysis is used to check for diseases, health conditions, and urinary tract infections. There are three methods of urinalysis: physical property test, chemical test, and microscopic test, and chemical test results can be easily confirmed using urine test strips. A variety of items can be tested on the urine test strip, through which various diseases can be identified. Recently, with the spread of smart phones, research on reading urine test strips using smart phones is being conducted. There is a method of detecting and reading the color change of a urine test strip using a smartphone. This method uses the RGB values and the color difference formula to discriminate. However, there is a problem in that accuracy is lowered due to various environmental factors. This paper applies a deep learning model to solve this problem. In particular, color discrimination of a urine test strip is improved in a smartphone using a lightweight CNN (Convolutional Neural Networks) model. CNN is a useful model for image recognition and pattern finding, and a lightweight version is also available. Through this, it is possible to operate a deep learning model on a smartphone and extract accurate urine test results. Urine test strips were taken in various environments to prepare deep learning model training images, and a urine test service application was designed using MobileNet V3.

Isolation Frequency of Candida species from Clinical Specimens (임상검체에서 Candida spp.에 대한 분리빈도)

  • A total of 167,955 microorganisms were isolated from 366,661 clinical specimens. Among them, 6,517 strains of the Candida spp. were isolated from the department of laboratory medicine in "C" hospital from Jan. 1, 2005 to Dec. 31, 2009. All clinical specimens were reviewed by the medical records of patients with Candida by the method of retrospectiveness. From this, we got the some isolated pure cultured yeasts. We identified these yeast by the identification kit system of VITEKII and VITEKII-ID-YST card. The isolation frequencies of Candida spp. were as follows. 56.4%,of C. albicans, 17.7% C tropicalis, 10.7% C glabrata and 9.5% C parapsilosis. The isolated frequency of Candida spp. in 2009 was 1.9 times higher than that in 2005. The clinical materials showing over 10.0% isolation rate were in sputum (30.1%), random urine (25.0%), 15.8% blood (15.8%) and catheterized urine (13.5%) in Candida spp.. The clinical department of showing over 7.0% isolation rate were in pulmonary medicine (20.5%), renal medicine (11.0%), infection disease medicine (10.4%), critical care medicine (10.0%), hematooncology (9.6%), general surgery (7.5%) and gastrointestinal medicine (7.4%) in Candida spp.. In monthly analysis, Candida spp. were most friquency isolated in July (10.6%), but lowest one in February (6.1%). Candida spp. were most frequently isolated in patient of over 50 years old (16.7-40.1%) than those isolated from the patients under the age of 0-49 (1.3-7.5%).

Partial HPRT Deficiency Due to a Missense Mutation in the HPRT Gene (HPRT 유전자 돌연변이에 의한 HPRT 부분결핍증 1례)

  • An 8-month-old male infant presented with persistent, gross, orange-colored crystals in his urine. His physical and neurological development was normal. Laboratory study showed hyperuricemia, hyperuricosuria and urate crystaluria. He was determined to have partial hypoxanthine-guanine phosphoribosyl transferase(HPRT) deficiency. The molecular genetic analysis revealed a missense mutation in the patient's HPRT gene. By sequencing the patient's cDNA, we identified an A-to-G transition at nucleotide 239, resulting in the replacement of Aspartate with Glycine at amino acid 80 in the HPRT. To our knowledge, this mutation has not previously been reported. Our patient is now being placed on allopurinol therapy, and has had no problem since. Partial HPRT deficiency has been known to cause recurrent acute renal failure without the phenotypic features of Lesch-Nyhan syndrome. Therefore, we think that early diagnosis and treatment are very crucial in preventing acute renal failure.
