  • Histopathologic evidence of "full-house" immune complex deposits is a pathognomonic feature of lupus nephritis. This report presents the case of a 12-year-old boy with persistent microscopic hematuria and proteinuria. He was diagnosed with "full-house" nephropathy based on a renal biopsy. However, there was no other clinical or biological evidence of systemic lupus erythematosus (SLE). Although the potential for isolated "full-house" nephropathy preceding SLE is unclear, such patients should be followed for clinical signs and autoantibodies of SLE. In most cases, microscopic hematuria has a good prognosis, and follow-up usually requires only regular urinalysis. However, we should be aware of isolated "full-house" nephropathy that remains asymptomatic for a long time, as few patients with no clinical signs and negative serology ultimately develop SLE.

  • The author assessed the availability of urine reference material for proficiency test provided for laboratories in occupational health. N-methylacetamide is the biomarker of exposure to N,N-dimethyl acetamide, which was used as the substitute for hepatotoxic N,N-dimethylformamide (DMF). N-methylacetamide (NMAC) urine samples of 3 different levels covering the 0.2~2 times of the exposure limit were tested. Stability test up to 180 days (0, 7, 30, 60, 180 days) at 4 different temperatures (-60, -20, 5, $25^{\circ}C$) and homogeneity test were performed for these samples. New analytical condition by GC/MSD using SIM mode (m/z 58, 59) and DB-624 column was investigated for better selectivity, sensitivity and resolution. Urinary NMAC samples showed good homogeneity for 3 levels. These samples also showed good stability up to 180 days. The data of stability and homogeneity of urinary DMAC confirmed the basis of including this item into Korean proficiency test for occupational health laboratories since 2008.

  • In this paper, we propose to develop a Bluetooth Health Device Profile (HDP)-based smartphone system to utilize it for early detection of urinary tracts diseases that occur a lot in cats. Therefore, based on Bluetooth HDP, we developed a device and mobile application system (Mycatner®) that can monitor cat activity, toilet usage, urinary disease, and health status, and evaluated its availability to monitor cat health status. The specific feature of this system is that it can measure the number of cat urination frequencies to identify abnormal conditions suspected of urinary tract diseases early, and second, it can be tested with urine test paper and shared with animal hospitals, reducing time and cost. As a result, it is evaluated that the developed device capable of wireless monitoring the urinary system health status of cats is the first in our knowledge.

  • Purpose: The plcr of spot urine has been uised to predict the timed urine protein excretion. Although this method reduces errors caused by variations in urine volume, it is relatively thconvenient and expensive. Recently, a more rapid and less expensive screening method with specific gravity(SG) has been reported, and we have examined whether estimated-creatinine(Cr-est) with urine 5G could be used in place of urine creatinine to predict 24-hour collected urine protein excretion in children. Methods: We had retrospectively analyzed protein, creatinine and urine SG in randomized spot urine samples of 147 patients from March 1998 till June 1998 in Korea university Guro hospital and compared the urinary protein creatinine ratio(P/Cr) with the protein estimated-creatinine ratio(P/Cr-est). We compared the correlation of urinary creatinine vs-urine 5G with the timed urine pretein excretion. Results: 1) urine SG accurately estimated urine creatinine concentration (r=0.407, P<0.001, Cr=SG x 4485.82-4482.87). 2) P/Cr correlated with urine protein excretion measured in a 24-hour urine collection (r=0.771, P<0.001, 24-hour collected urine protein : 0.338 x (P/Cr) 4+667.885). 3) P/Cr-est correlated with a 24-hour collected urine protein (r=0.723, P<0.001, 24-hour collected urine protein =0.354 x (P/Cr-est)+726.044), Conclusions: These results suggest that P/Cr-est with urine SG could be useful method for screening proteinuria in children.

  • Purpose : To evaluate the significance of clinical signs and laboratory findings as predictors of renal parenchymal lesions and vesicoureteral reflux (VUR) in childhood urinary tract infection (UTI). Methods : From July 2005 to July 2008, 180 patients admitted with a first febrile UTI at the Pediatric Department of Konkuk University Hospital were included in this study. The following were the clinical variables: leukocytosis, elevated C-reactive protein (CRP), positive urine nitrite, positive urine culture, and fever duration both before and after treatment. We evaluated the relationships between clinical variables and dimercaptosuccinic acid (DMSA) scan and voiding cystourethrography (VCUG) results. Results : VCUG was performed in 148 patients; of them, 37 (25.0%) had VUR: 18 (12.2%) had low-grade (I-II) VUR, and 19 (10.5%) had high-grade (III-V) VUR. Of the 95 patients who underwent DMSA scanning, 29 (30.5%) had cortical defects, of which 21 (63.6%) had VUR: 10 (30.3%), low-grade (I-II) VUR; and 11 (33.3%), high-grade VUR. Of the 57 patients who were normal on DMSA scan, 8 (14.0%) had low-grade VUR and 6 (10.5%) had high-grade VUR. The sensitivity, specificity, and positive and negative predictive values of the DMSA scan in predicting high-grade VUR were 64.7%, 69.9%, 33.3%, and 89.5%, respectively. Leukocytosis, elevated CRP, and prolonged fever ($36{\geq}$ hours) after treatment were significantly correlated with the cortical defects on DMSA scans and high-grade VUR. Conclusion : Clinical signs, including prolonged fever after treatment, elevated CRP, and leukocytosis, are positive predictors of acute pyelonephritis and high-grade VUR.

  • Purpose : The neonatal screening test for homocystinuria primarily measures methionine by using a dried blood specimen. We investigated the incidence and clinical manifestations of homocystinuria, isolated hypermethioninemia, and transient hypermethioninemia among patients with hypermethioninemia on a neonatal screening test. Methods : We performed a retrospective study of 58 patients transferred to Shoonchunhyang Hospital because of hypermethioninemia on a neonatal screening test between January 1996 and August 2009. We analyzed the level of amino acid from plasma and urine, as well as blood homocysteine. Results : Almost half of the 58 patients were identified as normal. Whereas only 3 (5.1%) patients were identified as having homocystinuria, about 20.7% (12 cases) of the patients had isolated hypermethioninemia. The ages of these two groups at initial detection of hypermethioninemia on plasma amino acid analysis were $50.0{\pm}22.5$ days and $34.9{\pm}13.5$ days, respectively. Both groups were put on diets, and they showed a normal developmental course as a result of early diagnosis and treatment. Conclusion : Hypermethioninemia without homocystinuria, referred to as isolated hypermethioninemia, was also detected. Thus, the impact of hypermethioninemia on a neonatal screening test should be carefully evaluated through analysis of amino acid levels from blood and urine, and we need to detect and treat an early stage of isolated hypermethioninemia as well as homocystinuria.

  • Acute flank pain from urolithiasis is the most common condition in people visiting emergency rooms. This study is to evaluate the usefulness of color Doppler in detecting ureteral stones in patients without hydronephrosis. We performed ultrasonography and retrospective analysis on 161 patients who were suspected of urinary stones through plain radiography of the kidney, ureter, and bladder examination or urine tests that showed positive signs of hematuria. In ultrasonography, a total of 154 (95.6%) cases from the 161 patients were diagnosed with ureteral stones. In color Doppler, ureteral stones with twinkling artifact (TA) in the absence of hydronephrosis was shown in 18 (85.7%) cases of the 21 patients(p<0.001). The use of color Doppler has a high diagnostic efficacy for the detection of indistinguishable stones, non-dilatational ureteral stones, and middle ureter stones.

  • The genus Lactococcus is a gram-positive, catalase-negative, non-motile bacterium. It is a facultative anaerobe and can be cultured at 10~40℃. The genus Lactococcus consists of 16 species, of which Lactococcus garvieae and Lactococcus lactis are known to cause disease in humans. This study reports the first case in which L. garvieae was identified in the urine culture of a 74-year-old woman. The patient confirmed the findings of acute urinary tract infection through blood tests, microbial identification tests, antibiotic susceptibility tests, and computed tomography performed at a hospital. The patient was admitted to the nephrology ward and was treated with IV fluids and erythromycin antibiotics and discharged 5 days later. This is the first case in Korea in which L. garvieae was isolated from the urine of a patient with a urinary tract infection and is expected to be useful in treating patients with L. garvieae infection in the future.

  • Purpose : To evaluate the diagnostic significance of simultaneous examination of hematuria and proteinuria in the urinary mass screening for early detection ot incipient renal diseases. Method and Object : During the period of 4 months from August to December in 1997, we did urinary mass screening on students of first grade of high school (16 years aged group) nationwide together with Korean Association of Health(KAH). In the first screening test, Comber-10 $N^{(R)}$ M dipsticks were used to detect proteinuria, hematuria, pyuria and nitrite simultaneously. Total 26,508 students (16 years aged group) from 33 high schools in every province in Korea, respectively, complied to the urinary mass screening. After then, one high school in Seoul was selected to reveal the true incidence of incipient renal diseases among students who showed hematuria in the initial screening through intensive examinations. Those who had hematuria and/or proteinuria visited the Paik hospital, and underwent blood tests and ultrasonographic examinations. The results were evaluated. Results 1) The initial screening revealed that the prevalence of proteinuria, hematuria, pyuria and positive nitrite urine, were $0.73\%,\;2.69\%,\;0.23\%\;and\;0.03\%$, respectively. 2) The first urinary screening among 875 students from one high school in Seoul selected fir the second test showed proteinuria, hematuria, pyuria and positive nitrite urine, were $0.91\%,\;4.68\%,\;0.34\%\;and\;0\%$, respectively. a) Total 8 among 875 students showed proteinuria, but one of them had orthostatic proteinuria and those remaining 7 students had transient proteinuria. b) There were 41 students who had hematuria in the initial screening. Among 33 who complied the second test, only one student showed asymptomatic isolated hematuria and those remaining students were normal. Conclusion : 1) Because of high false positive hematuria rate in the urinary mass screening, it dosen't seem to be appropriate that hematuria screening using dipsticks be included in the urinary mass screening. 2) A unified organization is needed from the fret that so various results of urinary mass screening came out. 3) Positive rates of pyuria and nitrite were so low that validity of urinary mass screening for urinary tract infection needs more studies.
