A Study of Effects of Psychosocial Factors and Quality of Life on Functional Dyspepsia in Firefighters (소방관에서 기능성 소화불량에 대한 심리사회적 요인의 영향 및 삶의 질에 관한 연구)

  • Objectives : The purpose of this study was to investigate the characteristics of psychosocial factors related to functional dyspepsia(FD) and their effects on quality of life(QOL) in firefighters. Methods : This study examined data collected from 1,217 firefighters. We measured psychological symptoms by Patient Health Questionnaire-9(PHQ-9), Generalized Anxiety Disorder questionnaire(GAD-7), Korean Occupational Stress Scale(KOSS), Ways of Coping checklist(WCCL), Rosenberg's Self-Esteem Scale(RSES) and World Health Organization Quality of Life Scale abbreviated version(WHOQOL-BREF). Chi-square test, independent t-test, Pearson's correlation test, logistic regression analysis, and hierarchical regression analysis were used as statistical analysis methods. Results : For the group with FD, the male participants showed significantly higher frequency(p=0.006) compared to the female participants. The group with FD had higher scores for depressive symptoms(p<.001), anxiety (p<.001), and occupational stress(p<.001), and did lower scores for self-esteem(p=.008), quality of life(p<.001) than those without FD. The FD risk was higher in the following KOSS subcategories: job demand(OR 1.94, 95% CI : 1.29-2.93), lack of reward(OR 2.47, 95% CI : 1.61-3.81), and occupational climate(OR 1.51, 95% CI : 1.01-2.24). In the hierarchical regression analysis, QOL was best predicted by depressive symptoms, self-esteem, and occupational stress. Three predictive variables above accounts for 42.0% variance explained of total variance. Conclusions : The psychosocial factors showed significant effects on FD, and predictive variables for QOL were identified based on regression analysis. The results suggest that the psychiatric approach should be accompanied with medical approach in future FD assessment.

Musculoskeletal Diseases in 119 Rescuers (119구급대원 구급활동에 따른 근골격계 질환 연구)

  • The purpose of this study is to investigate the prevalence and to describe the correlation between the working period and musculoskeletal diseases of 119 rescuers. We conducted 485 surveys from 547 rescuers working in Gyeonggi-do from November 5 to December 20, 2011. The collected data were analyzed by cross tabulation analysis, chi-square test, and correlation analysis using PASW 18.0. The p value was set at p<.05. The prevalence of musculoskeletal diseases, 328(67.6%) rescuers suffering from musculoskeletal diseases. General characteristics included gender(p<.01), age(p<.01), marriage(p<.001),household chores(p<.05), working period(p<.001). The correlation between physical loading of subjects and musculoskeletal diseases was significant(p<.001). The rescuers felt the pain at lower back(36.4%), shoulders(19.2%), neck(14.9%), hip/ankles(13.2%), hand/wrist/fingers(12.0%), elbow(4.3%). This study showed that working period and physical loading affect musculoskeletal diseases.

The Effect of Two Fingers-Chest Compression Using the Supporting Instrument During Infant Cardiopulmonary Resuscitation (영아 심폐소생술시 지지대를 사용한 두 손가락 가슴 압박의 효과)

  • This paper aims to compare the effectiveness of the chest compression when a person pushes on the infant's chest by using two fingers with the support during infant cardiopulmonary resuscitation, with the effectiveness of it without the support, and to find which one is better. For the study, 50 college woman students were tested during the simulation and the result of the test has been analyzed by chi-square test, Fisher's exact test. In case of the chest compression by using the support, the depth of the chest compression comes to $3.73{\pm}0.33cm$. On the other hand, in case of the chest compression without the support, $2.50{\pm}0.59cm$. It is founded that the method of pushing on the chest by using the support is more effective than that without the support (p < 0.001). It is concluded that the way that a person pushes on the infant's chest by using two fingers with the support during infant cardiopulmonary resuscitation has turned out to be more effective and useful. It is thought that in the future, the further study for it should be conducted.

Job Satisfaction and Work Stress of EMT-paramedic's According to type of Occupation (1급 응급구조사의 직종에 따른 직무만족도 및 업무스트레스)

  • This study provides basic data needed to improve the working environment of the first class emergency medical technicians, in order to decrease their job stress and increase their job satisfaction. Job stress and satisfaction were compared and analysed in emergency medical technicians who were working for 119 rescue and hospitals at the time when twenty-one years had passed after the initial training of emergency medical technicians in order to identify differences between 119 rescue and hospitals, and their advantages and disadvantages. The subjects of the study were 182 first-class emergency medical technicians who were working in twenty-eight 119 rescue centers and fourteen national and local emergency medical centers in Gwangju, Jeollanamdo and Jeollabukdo. The study subjects were interviewed by questionnaire. For statistical analysis, SPSS/WIN 20.0 was used, and the results were analysed with chi-square, t-test, ANOVA, and ANCOVA testing. There was no significant difference in job stress between the subjects working in 119 rescue centers (2.92) and those working in hospitals(3.01). The satisfaction score of fire-fighting workers was 2.98 and that of hospital workers was 2.54. Hospital workers' satisfaction was significantly lower (p<0.001). The score of hospital workers' pay satisfaction was the lowest($2.07{\pm}0.62$). The job stress decreased and job satisfaction increased with decreasing monthly average number of patients(p<0.001), increasing monthly income(p=0.047), and more frequent holidays they had(p<0.001), the less. For less job stress and higher job satisfaction, welfare conditions both at 119 rescue and hospitals should be improved. 119 rescue centers should develop violence prevention measures and offer resting places for emergency medical technicians. Hospitals should pay more attention to the yearly pay increase rate and stability in employment types.

Model for the Structural Relationships of Behavioral Attitude, Subjective Norm and Perceived Behavioral Control to Safety Intention and Safety Behavior in 119 Emergency Medical Technicians (119구급대원의 행위태도, 주관적 규범 및 지각된 행위 통제가 안전의도, 안전행동에 미치는 영향에 관한 구조적 관계 모형)

  • The purpose of the this study was to verify a model for the structural relationships of the behavioral attitude, subjective norm and perceived behavioral control of 119 emergency medical technicians to their safety attitude and safety behavior. A survey was conducted on 373 male and female 119 emergency medical technicians who were working as fire-fighting officers in Seoul, Inheon, Gangwon Province, Daegu, South Chungcheong Province, North Gyeongsang Province and Busan. The collected data were statistically analyzed by SPSS 20.0 and AMOS 16.0. The findings of the study were as follows: First, the behavioral attitude of the 119 emergency medical technicians was found to have affected and safety behavior(t=.32, p<.001). Second, the subjective norm of the 119 emergency medical technicians was found to have exerted an influence on their safety attitude and safety behavior(t=.27, p<.001). Third the perceived behavioral control of the 119 emergency medical technicians was found to have influenced their safety attitude and safety behavior(t=.28, p<.001). Fourth, the safety attitude of the 119 emergency medical technicians was found to have had an impact on their safety behavior(t=.39, p<.001).

Structure and Understory Species Diversity of Pinus parviflora - Tsuga sieboldii Forest in Ulleung Island (울릉도 섬잣나무-솔송나무림의 구조 및 하층식생의 종 다양성)

  • Vegetation structure, composition and diversity were quantified for 10 samples ($10m{\times}10m$) representing woody vegetation and for 30 samples ($1m {\times}3m$) representing understory vegetation in Pinus parviflora and Tsuga sieboldii forest of Taeharyeong, Ulleung-gun (Gyeongsangbuk-do). P. parviflora was limitedly advanced to sapling layer from seedling stage, and based on Mantel tests, composition of canopy layer was not established in ground woody vegetation. Non-metric multidimensional scaling revealed influence of biotic and abiotic factors in species composition of woody and understory vegetation. In the result of multiple regression model, abundance of P. parviflora (density and breast height area) and percent cover of woody debris were significant predict variables for understory diversity. These results suggest that relatively large disturbance is required for regeneration of P. parviflora and T. sieboldii forest, and control of expansion of monocultural understory species that monopolize resources such as Carex blepharicarpa and Maianthemum dilatatum, is necessary for maintenance of diversity.

Perception Difference Analysis between Manager and Field Worker about the Form work Collapse Accident (거푸집 붕괴재해에 대한 관리자와 작업자의 인식차이 분석)

  • In the past five years, form construction, which accounts for approximately 25% of the total construction period, has continued to occur without a significant decrease in the number of deaths, with the largest proportion of collapse accidents. Accordingly, this study analyzed the difference in perception between the manager and worker by a questionnaire survey on the degree of risk and safety management level regarding a mold collapse accident. The survey was conducted in three groups: safety manager, field worker, and field manager. The results of the survey were analyzed by an independent sample T-Test using the SPSS program. As a result, there was almost no difference in recognition between managers, but a significant difference in recognition between managers and workers. In addition, there was a difference in management perception between the administrator and worker, which clearly shows the difference in the position between the administrator who manages and supervises hazardous disaster factors and the worker who works directly in the field. Such differences in perception can be a factor that cannot be mitigated. Based on this study, more developed research can narrow the perception gap between managers and workers and be used as a basic material for disaster research.

The effects of out of hospital ACLS simulation training on the paramedic's duty ability (구급대원의 전문심장소생술 시뮬레이션훈련이 직무수행융합능력에 미치는 영향)

  • The purpose of this study is to analyze the effects of the simulation-based professional cardiac resuscitation training on the performance of professional cardiac resuscitation performed by paramedics in the pre-hospital stage and to provide basic data for effective cardiac resuscitation. This study is an experimental study of the design before and after the control of non-equality. The subjects of this study were 16 newly recruited paramedics from K firefighting school. The simulation training program and evaluation papers used as the evaluation tool were reviewed and commented by 6 ACLS simulation experts (2 emergency medical doctor, 2 emergency medical professors, 2 KALS instructors)Respectively. The training consisted of 30 minutes of theory and 150 minutes of practical training. The lecturer first demonstrated for 5 minutes, and after training by individual debriefing after individual training, individual and team education was conducted The evaluation scale was given a 5 - point Likert scale. The SPSS 22.0 program for Windows was used. The general characteristics of the subjects were analyzed for frequency, the examination of homogeneity between the experimental group and the control group wasfulfilled by t test, and the difference test between the groups of the two groups was performed using the paired t-test. The homogeneity test was able to confirm the homogeneity between experimental group and control group. In the evaluation of six ACLS techniques, it was proven that the experimental group that received the simulation training had better performance in all aspects than the non - training control group. The following are the technical items to be performed. 1. Electrocardiogram 2. Specialized instrument 3. Treatment of fluid 4. Leadership and teamwork 5. Medical guidance 6. Evaluation during transfer. It was proved that paramedics who received simulation training were improved on their job performance ability than general lecture and training group. Therefore, if simulation training and education are applied to a student in the synthetic course or an emergency resident who is engaged in clinical practice, he / she will be able to perform his / her duties more proficiently. It is expected that emergency services provided to patients with cardiac arrest will be improved.
