ROC Analysis of Acid Demineralized Artificial Caries (인공치아 우식병소 진단의 ROC 분석)

  • 조직학적으로 유용성이 입증된 산탈회법을 이용한 인접면 비교적 초기 치아 우식의 병소를 형성하여 진단율을 조사하였다. 산 용액을 이용하여 20개 인접면 치아우식을 20개 소구치에 형성하였고, 37개 인접면 치아우식을 30개 대구치에 형성하였다. 건전한 소구치 20개, 대구치 30개를 포함하여 총 96개 치아를 4개씩 나누어 24개의 블록을 형성하였고, 각각 2개 블록의 교합면을 교합시켜서, 교익촬영을 하였다. 촬영 결과를 36명의 치과의사들이인접면 치아우식의 유무를 기록하고, 동시에 및 ROC 분석을 위한 5 개 범주의 판독 기준으로 판독하여 기록하였다. 인접면 치아우식증 유, 무만으로 판독한 결과 진단의 sensitivity는 0.71, specificity는 0.78 이였다. ROC 분석 한 결과의 곡선도표 아래부분의 평균 면적은 약 0.806 이였다. 치아우식증 유무만으로 진단한 결과는 특정한 sensitivity와 specificity 만을 나타내지만, ROC 분석 결과는 주관적 진단 기준과 구별되는 고유의 진단 능력을 표시하는 1-specificity(False Positive)의 변화에 따른 sensitivity(True Positive)의 변화를 연속적으로 나타내어 주었다.

3-D Finite element stress analysis in screw-type, cement-type, and combined-type implant fixed partial denture designs (임플란트 상부보철물의 유지형태에 따른 3차원 유한요소 응력분석)

  • Statement of problems: Stress analysis on implant components of the combined screw- and cement-retained implant prosthesis has not investigated yet. Purpose: The purpose of this study was to assess the load distribution characteristics of implant prostheses with the different prosthodontic retention types, such as cement-type, screw-type and combined type by using 3-dimensional finite element analysis. Material and methods: A 3-dimensional finite element model was created in which two SS II implants (Osstem Co. Ltd.) were placed in the areas of the first premolar and the first molar in the mandible, and three-unit fixed partial dentures with four different retention types were fabricated on the two SS II implants. Model 1 was a cement-retained implant restoration made on two cement-retained type abutments (Comocta abutment; Osstem Co. Ltd.), and Model 2 was a screw-retained implant restoration made on the screw-retained type abutments (Octa abutment; Osstem Co. Ltd.). Model 3 was a combined type implant restoration made on the cement-retained type abutment (Comocta abutment) for the first molar and the screw-retained type abutment (Octa abutment) for the first premolar. Lastly, Model 4 was a combined type implant restoration made on the screw-retained type abutment (Octa abutment) for the first molar and the cement-retained type abutment (Comocta abutment) for the first premolar. Average masticatory force was applied on the central fossa in a vertical direction, and on the buccal cusp in a vertical and oblique direction for each model. Von-Mises stress patterns on alveolar bone, implant body, abutment, abutment screw, and prosthetic screw around implant prostheses were evaluated through 3-dimensional finite element analysis. Results: Model 2 showed the lowest von Mises stress. In all models, the von Mises stress distribution of cortical bone, cancellous bone and implant body showed the similar pattern. Regardless of loading conditions and type of abutment system, the stress of bone was concentrated on the cortical bone. The von-Mises stress on abutment, abutment screw, and prosthetic screw showed the lower values for the screw-retained type abutment than for the cement-retained type abutment regardless of the model type. There was little reciprocal effect of the abutment system between the molar and the premolar position. For all models, buccal cusp oblique loading caused the largest stress, followed by buccal cusp vertical loading and center vertical loading. Conclusion: Within the limitation of the FEA study, the combined type implant prosthesis did not demonstrate more stress around implant components than the cement type implant prosthesis. Under the assumption of ideal passive fit, the screw-type implant prosthesis showed the east stress around implant components.

CORRECTION OF ECTOPIC ERUPTION WITH BILATERAL ANCHORAGE : REPORT OF CASES (양측성 고정원을 이용한 이소 맹출의 치료에 대한 증례 보고)

  • Ectopic eruption of the first permanent molar means the first permanent molar assumes an atypical path of eruption resulting in premature atypical resorption of the second primary molar. If the reversible eruption does not occur, early loss of the second primary molars results in space loss, mesial tipping of the first permanent molar, impaction of the second premolar, buccal segment crowding and overeruption of opposing tooth. The main objectives of treatment are (1) to prevent loss of the second deciduous molars so it can continue to serve as a space maintainer and (2) to regain lost arch length, allowing the second premolar to erupt into normal position. The optimal treatment approach depends on a number of factors including the clinical eruption status of /6/, the change in position of /6/, the amount of enamel ledge of /E/ entrapping /6/, the mobility of /E/, and the presence of pain or infection. Unilateral appliance to correct the mesial angulation of ectopic permanent first molars, as in the majority of the appliance designs, would produce a resultant force that would further enhance the space loss. A bilateral support similar to the holding arch design is recommended to maximize the anchorage. These case reports present the successful result of preserving space for the second premolar in treatment of ectopic eruption of the first permanent molar using Halterman appliance with bilateral anchorage on patients visiting department of pediatric dentistry in Samsung Medical Center.

IMPACTED PREMOLARS AND MOLARS ASSOCIATED WITH DENTIGEROUS CYSTS IN CHILDREN (어린이에서 함치성 낭과 연관된 매복 소구치와 대구치의 치료)

  • Tooth impaction is a frequently observed eruption anomaly in pediatric dental practice. Young patients with impacted or unerupted teeth have more prediction for dentigerous cyst formation. Dentigerous cyst presents radiographic features, unilocular or multilocular radioluscency. Cysts occur most frequently in the premolar region except third molar. Dentigerous cysts can grow to a considerable size, and large cysts may be associated with a painless expansion of the bone in the involved area. Extensive lesions may result in facial asymmetry, osseous destruction, root resorption of proximal teeth and displacement of associated tooth. The nature of the causative tooth influences the type of surgical treatment required for the dentigerous cyst. If the cyst is associated with a supernumerary or wisdom tooth, complete enucleation of the cyst along with extraction of tooth may be the first treatment choice. Otherwise, preservation of the associated teeth should be considered to prevent a young patient from psychological and mental trauma because of the loss of tooth. We should consider the degree of tooth displacement, osseous destruction and growth pattern of oromaxillofacial area when planning treatment. Thus a proper and logical treatment planning can help a proper growth and development of oromaxillofacial area and can save the patient from a psychological and mental trauma. This report describes 4 cases of the management of impacted premolars and molars associated with dentigerous cysts in children.

  • Supernumerary teeth (SNTs) refer to extra teeth over normal numbers. SNTs can influence adjacent teeth and structure. Therefore, it is important to find SNTs before they cause problems and to decide when to extract them. Etiology of SNTs has not exactly been found, but it has been proposed that genetic and environmental factors are associated. SNT appears as a single unit in most cases and multiple SNTs usually occur as a part of a syndrome. Non-syndromic multiple SNTs are rarely seen. This report is to document the occurrence of multiple supernumerary premolars in a father and his three sons. Heritage is an important factor in SNT development so familial history is important for early detection of SNT.

Consideration of maxillary sinus bone thickness when installing miniscrews (미니스크류 식립 시 상악동의 골두께에 대한 고려)

  • Objective: Miniscrews are widely used in orthodontic treatment for the purpose of anchorage control. Maximum anchorage can be acquired by the use of miniscrews. Maxillary miniscrew has many clinical advantage for orthodontic treatment. Maxillary sinus, tooth root can be an obstacle for maxillary miniscrew installation. The purpose of this study was to find the safest area and direction of miniscrew insertion in consideration of the maxillary sinus. Methods: The maxillary sinus area of 40 patients (20 male, 20 female) was measured using 3D computed tomography and 3D reconstruction program. Results: The maxillary sinus floor was located most inferiorly between the 1st molar and 2nd molar and located most superiorly between the 1st premolar and 2nd premolar. Buccal bone thickness from the maxillary sinus is significantly thicker between the 1st molar and 2nd molar and significantly thinner between the 1st premolar and 2nd premolar. The area between the 1st premolar and 2nd premolar has a significantly longer vertical distance from CEJ to sinus in consideration of buccal bone thickness. Conclusions: Considering maxillary bone thickness, the posterior area has advantages over the anterior area for installing miniscrews safely and preventing perforation.

Miniscrew Installation Area and Condition on Maxillary Palatal Side (상악구개측 미니스크류 식립위치 및 조건)

  • Anchorage control is important in orthodontic treatment. Recently miniscrew is widly used as maximum anchorage in orthodontic treatment, and then it is important to install miniscrew safely without damaging adjacent anatomic structure. In a view of Miniscrew's stability, maxilla is unfavorable than mandible, and moreover maxillary soft buccal bone has disadvantage on stability. so palatal area comes into notice for installation area. We measured distance between palatal roots and bone thickness at midpalatal area using 3D computed tomography, and have found following results. 1. On the comparison of distance between palatal roots, the distance between 2nd premolar and 1st molar was significantly longest and the distance between premolars was significantly shortest. 2. Going toward lateral area from midpalatal suture and posterior area from zero point, bone thickness significantly became shorter and shorter. And 5.0mm palatal sagittal plane has more significance decrease of bone thickness than 2.5mm palatal sagittal plane. According to these results, we can conclude that the palatal installation of miniscrew between 2nd premolar and 1st molar is safest. And it is more safe that comes closer to midpalatal suture and to anterior area in regard to incisive canal.

  • 치주질환 발생시의 대식세포의 형태적 변화 및 기능적인 상관관계를 연구하고저 성견 5마리를 사용 실험하였다. 실험동물을 전신마취한 후 상하악 우측 제 1 및 제 2 소구치의 치경부에 1.3mm굵기의 wire를 4주간 결찰하여 인위적으로 치주염을 유발시켜 실험적으로 하였고, 동일동물의 좌측 제 1 및 제 2 소구치를 대조군으로 하였다. 실험군 및 대조군의 치아협측치은을 0.5 0.5 1mm의 형태로 절제하여 전자현미경적 관찰을 위한 통법의 조직처리를 한 다음 관찰하였다. 그 관찰 결과를 보면 다음과 같았다. 1. 실첨군의 치은의 고유층에 탐식성 돌기를 가진 많은 대식세포들이 출현하였다. 2. 변성된 교원 섬유군 인접부위에 phagocytic vesicle을 가진 대식세포들의 세포질내에 지방과립 및 잔존물을 볼 수 있었다. 한편 RER 및 Golgi complex는 매우 적었다. 3. 세포막은 그 기능상으로 많은 ruffles or folds를 나타냈으며 이 물질을 포식하는 양식을 뛰기도 하였다. pinocytotic activity를 띄는 대식세포들도 보였다. 4. 신생섬유아세포 주위에 밀접한 관계를 띄고 대식세포가 나타났으며 상호 유사하다. 대식세포에는 많은 residual body가 나타났고 RER은 매우 적었다.

AUTOTRANSPLANTATION OF A MALPOSITIONED MANDIBULAR SECOND PREMOLAR : A CASE REPORT (이소매복된 하악 제2소구치의 자가치아이식을 이용한 치험례)

  • In the case of the impacted teeth, the clinician has to consider development of tooth, site of impaction, eruption path, and cooperation of patient. Treatment options for the management of impacted teeth are separated into four categories: observation, intervention, orthodontic or surgical relocation and extraction. Autotransplantation may be defined as the transplantation of embedded, impacted or erupted teeth, from one site to another in the same individual into extraction site or surgically prepared sockets. Autotransplantation ensures preservation of natural tooth, induction of alveolar bone growth and root development, offers one of the fastest and most economically feasible means in the replacement of young patients' missing teeth. This case presents a malpositioned impacted mandibular premolar of an 11-year-old girl. It was thought that orthodontic traction was difficult because of its unfavorable impacted position. Therefore the tooth was treated by autotransplantation, we can observe good healing pattern during 12 months.
