편측 성대마비에 의한 부전의 수술적 치료로는 그 동안 테플론주입에 의한성대내측전위술, 제1형 갑상연골성형술, 또는 피열연골내전술 등이 이용되었으며, 성대부전이 심하거나 성대높이에 차이가 있을 때는 제1형 갑상연골성형술 등에 비해 피열연골내전술이 좋은 결과를 보이는 것으로 보고되고 있다 그러나, 성대의 움직임은 있으면서 뒷쪽에 심한 성대부전(posterior glottic chink)을 보이는 경우에는 아직은 특별한 수술적인 방법이 없는 바, 저자들은 갑상선 부분절제술 후에 생긴 양쪽성대의 움직임은 있으면서 성문 뒷쪽에 심한 부전을 보인 발성장애 환자 1례에서 제1형 갑상연골성형술과 동시에 피열연골내전술을 시행하여 좋은 결과를 얻었기에 보고하는 바이다. 성문폐쇄부전의 개선을 위해 후두내에 주입하여온 여러 재료 중 현재에는 teflon만이 가장 흔히 사용되어지고 있다. 그러나 teflon은 점막하와 가동성 성대 내에 주입할 수 없어 경도 성문 폐쇄부전의 미세 조정과 성대의 작은 결손에는 부적합하다. 성문폐쇄부전을 개선하기 위하여 9명의 환자에서 17회 cross-linked collagen을 주입한 결과 이 방법은 환자의 음성을 개선하고 성문폐쇄부전을 교정하는데 성공적이었다. (중략)

배경 : 성문폐쇄부전을 나타내는 질환 중 성대구증은 음성개선을 위한 치료면에서 가장 어려운 질환중에 하나다. 성대구증의 음성개선을 위해서 현재 다양한 수술적 방법과 보형물의 주입술이 이용되고는 있으나 만족할 만한 음성개선을 얻기는 힘들었다. 목적 : 진행된 성대구증 환자의 성문폐쇄부전을 치료하기 위한 새로운 방법으로 본 교실에서 시도하고 있는 경피적 근전도유도하 교원질주입술의 치료성적 및 음성 및 음향학적 추적결과와 술기의 유용성에 관하여 알아보고자 하였다. (중략)

성문부폐쇄부전(glottic insufficiency)를 수술적으로 치료하기 위한 노력은 여러 형태(silicone, hydrogen gel, teflon, etc.)의 성대이물주입술, 갑상성형술등 다양하게 시도되어 왔다. 그러나, 이러한 노력에도 불구하고 성문부폐쇄부전은 여전히 해결하기 어려운 문제로 남아있다. 여기에 1995년 Ford 등은 성문부폐쇄부전에 이은 음성장애치료의 새로운 접근방법으로 자가 콜라겐주입술(autologous collagen injection)을 소개하였다. (중략)

Amyotrophic lateral sclerosis Is an unusual pattern of distal muscular atrophy with permanent bulbar sign. Vocal fold paralysis and velopharyngeal insufficiency(VPI) due to soft palate paralysis Is occasionally associated with distal muscular atrophy. Recently we experienced a case of amyotrophic lateral sclerosis whose symptom was initially expressed with hoarseness and VPI. So we report a case with review of literature.

Glottal incompetence is one of the causative condition of hoarseness. There have been various methods to improve the glottal conditions. Vocal fold augmentation by injection under direct visual control is a quick and simple operation. Various vocal fold augmentation procedures have developed in recent years. Phonosurgical vocal fold injection can be classified as superficial or deep, by the location of the injection. The choice of material fir vocal fold injection is critical and should be determined by the injection location, technique, and the pathologic condition. There are multiple approaches or methods for vocal f31d injection ; the most widely used are presented in detail in this article.

성대구는 성대점막 고유층의 일부 결손 또는 위축성 함입으로 유리연이 내함되어 성대궁형(bowing cord)이 형성되기 때문에 발성시 정상점막 파동이 소실되고 성문폐쇄부전을 보이며 그 결과 애성을 초래하게 된다. 이러한 성대의 내함을 교정하기 위하여 구절제술, 성대 상하순(upper and lower lip) 점막의 위치조절 둥이 시행되고 있으나 성대궁형이 교정되지 않아 음성개선에 실패하는 경우가 많다. 저자들은 구절제술 후 성대궁형을 교정할 목적으로 연골막을 포함한 연조직을 성대근 내측으로 삽입하여 음성을 개선하는데 도움을 얻고자 하였다. 이러한 방법으로 음성수술을 시행한 5례의 경험과 그 효과 및 문제점을 문헌고찰과 더불어 보고하고자 한다.

Unilateral vocal cord paralysis is induced by various causes and its effective treatment has been diversely searched out until now. Currently used treatment modalities are intracordal injection of exogenous materials such as Teflon or Silicone, and thyroplasty and so forth. But, with the above mentioned modalities, it has been not satisfactory to obtain a good postoperative results especially in cases when the glottal incompetence is very severe or the level difference between the vocal cords is large. In such cases, vocal cord adduction can be accomplished by anteromedial traction of the muscular process of paralyzed vocal cord via surgical exposure resulting improvement of voice quality. Recently, authors performed arytenoid adduction in 3 cases of unilateral vocal cord paralysis to obtain a better improvement of voice quality, and experienced satisfiable postoperative results.

Voice is a physical phenomenon, generated by vocal fold and expiratory airflow. Dysphonia should come from abnormal vocal fold and airflow. Occassionally larynx looks normal in show, but it is actually not. There should be undetected structural or functional abnormalities. So when ENT doctors face dysphonia patients who looks normal larynx, should make a diagnosis through close observation. In this review article we present some dysphonia diseases which looks normal larynx. For example vocal fatigue, vocal fold paresis, posterior glottic diastasis, muscle tension dysphonia and psychogenic dysphonia.

In small children with left ventricular outflow tract obstruction, a few methods of surgical treatment could be considrred. The pulmonary autogrart provides a promising options for aortic valve replacement as part of the aortoventriculoplasty procedure in children. We report a successfully treated congenital aortic stcnoinsufricicncy with severe left ventricular dysfunction in an early infant with the aortoventriculoplasty using thc pulmonary autograft (the Ross-Konno procedure).
