The Result of Conservative Treatment in Displaced Radial Head Fracture (전위된 요골두 골절의 보존적 치료의 결과)

  • Purpose: The purpose of this study is to assess the result of conservative treatment in displaced radial head fracture. Materials and Methods: We analyzed 15 cases of displaced radial head fracture who were treated conservatively from April 1992 to May 2002. Follow-up period was more than 1 year. They were all males and the average age of patients was 20.2($11{\sim}24$) years. The average follow-up period was 13.4($12{\sim}15$) months. They were able to do active pronation and supination more than 70 degrees initially, who were applied long arm splint less than 1 week. The functional rating index modified after B.F. Morrey and no further displacement and union state of fragment in simple X-ray were used in follow-up assessment. Results: On the basis of the functional rating index modified after B.F. Morrey that included elements of pain, motion, strength, and stability, the results were 8 excellent, 7 good in patients who were treated conservatively, who had union and no further displacement of fragment in follow-up. Conclusion: We conclude that if they are able to do active pronation and supination more than 70 degrees initially, we take them to do early range of motion exercise after subsiding swelling, which can restrict limitation of range of motion exercise and return to the living early.

  • The improvements of berth productivity is closely related to berth/port systems and its relationship extends to the overall pattern of port development/logistics, transport and trade. Since ports in general and berths in specific function as a bridge transferring seaborne cargoes from seaside to landside and vice versa, berth productivity improvements should be considered in terms of the total system in which the cargoes are transported from origin to destination via berths/ports. In that respect, the objective of this study is to improve berth productivity which is a vital factor in the strategic planning in port development and operation and logistics. According to a literature survey on the subject, the contents of the studies have been limited to either a macro-perspective or a micro-perspective which is only a partial solution to the problem. Hence, this limitation requires an integrated approach to find and ultimate solution, which should be considered not separately, but as a whole. The contents and scope of this study cover the improvements of berth productivity in conjunction with strategic planning in port development. efficient operation and logistics from two different viewpoints, macro- and micro-perspective. After that, a combination of these two perspectives is applied to the integrated approach due to lacking a comprehensive perspective from either macro-approach or micro-approach. In reality, a port itself is a complex and dynamic organization. Therefore, a comprehensive insight needs an integrated approach for the long-term national economic development. In this approach, the utmost importance is how to find the bottlenecks and to solve the problems effectively from the beginning stage in port development. In conclusion, derived from the macro- and micro-perspective. the attempt to establish an integrated approach is the merit of this study, which is required as a comprehensive measure since none of the two can be a solution independently. In terms of reinforcing this study, it needs further empirical research due to the restriction of the theoretical method of this paper. Consequently, the reinforced research will be able to enhance the integrated approach on the basis of the conceptual perspective.

  • 2010년 천리안위성의 성공적인 발사에 따라 인공위성의 활용에 대한 기대가 커지고 있다. 천리안 해양관측위성(GOCI)이외에 아리랑 2호가 현재 운용중인 우리나라 위성들이다. 가까운 시기에 아리랑 5호(2011년 말), 아리랑 3호(2012년), 아리랑 3A호(2013년)가 발사될 예정이다. 즉, 해양적용을 위한 위성환경은 이제부터 준비되고 있다고 볼 수 있다. 대외적으로 보면, 인공위성 자원은 아주 많다. 문제는 이와 같은 자원을 어떻게 활용할 것인가 인데 이의 활용 기술 개발적 측면에서는 많이 소홀한 것이 사실이다. 전세계적으로 이 시스템 개발을 위한 치열한 경쟁이 진행 중에 있다. 이미 소말리아 주변 감시체계는 많은 부분을 위성에 의지하고 있다. 우리나라에서 최초로 위성활용 가능성을 보여준 사건이 허베이스피리트호 원유유출 사고이다. 이 사고는 2007년 12월7일 아침 7시6분경 서해안 만리포 북서쪽 10km 해상에서 크레인을 적재한 1만1800t급 바지선이 정박 중인 홍콩 선적 유조선 허베이 스피리트호(14만6000t급)와 부딪치면서 발생했다. 이와 같은 기름 유출 사고의 경우, 유출 범위를 정확하게 이해하는 것이 중요하다. 거의 준비된 상태가 아님에도 불구하고 12월 8일 아침 최초로 유출된 기름을 모습을 보여주는 위성이미지(광학위성)가 얻어졌다. 하지만 이와 같은 자료가 관련 전문가가 이용할 수 있기까지 많은 시간이 소용되었고, 이 정보를 전달할 수 있는 방법도 없었다. 사실 단순한 이미지가 아니라 지리정보체계를 가진 오염정보를 제공할 방법도 준비도 되어 있지 못한 상황이었다. 본 발표를 통하여, 허베이스피리트호 사고뿐만 아니라, 2011년 6월부터 수개월간 지속된 발해만 오염사고 적용 등 다양한 사례 소개를 하고, 이를 기반으로 해양경찰청에서 업무활용을 위한 방안을 제시한다. 먼저, 해경청의 주요 임무인, 경비, 수색구조, 오염대응 분야별로 현황 분석을 수행하였다. 또한 국외사례에 대한 조사를 한 후, 최종 인공위성 원격탐사기술의 해경청 도입방안에 대한 설계를 실시하였다. 국제적으로 인공위성을 이용한 해양 경비, 수색구조, 오염 모니터링기술 개발이 이루어지고 있으며, 유럽 국가는 시범도입을 진행 중에 있다. 유럽해사안전국(EMSA)은 해양경비 및 수색구조를 위한 선박통항 및 보고 서비스와 오염대비대응(Pollution Preparedness and Response, PPR) 위성 서비스를 회원국에 제공하고 있다. 해양경찰청 임무 수행뿐만 아니라, 해양영토 관리적 측면에서 첨단 위성장비 활용, 선진국형 해상경비 패러다임의 전환 필요성이 크다고 할 수 있다.

Radiographic Estimation of Renal Dimension in Korean Domestic Cats (한국 재래 고양이 신장크기에 대한 방사선적 평가)

  • Renal length, width, and ratio to the length of the second lumbar vertebrae body were assessed on survey radiographs and excretory urograms in 50 Korean domestic cats. Renal dimensions on plain radiographs were: right kidney length $4.31{\pm}0.50cm\;(mean{\pm}SD)$, right kidney width $3.10{\pm}0.36cm$, left kidney length $4.17{\pm}0.48cm$, left kidney width $3.03{\pm}0.34cm$, right kidney length ratio $2.37{\pm}0.21$, right kidney width ratio $1.71{\pm}0.15$, left kidney length ratio $2.30{\pm}0.20$, and left kidney width ratio $1.38{\pm}0.08$, respectively. Renal dimensions on excretory urograms were: right kidney length $4.56{\pm}0.53cm$, right kidney width $3.29{\pm}0.37cm$, left kidney length $4.43{\pm}0.47cm$, left kidney width $3.22{\pm}0.33cm$, right kidney length ratio $2.51{\pm}0.22$, right kidney width ratio $1.81{\pm}0.17$, left kidney length ratio $2.45{\pm}0.20$, and left kidney width ratio $1.78{\pm}0.16$, respectively. There were significant difference in renal size and renal dimension ratio measured from contrast studies compared with survey radiographs (P<0.05). There were also significant difference in renal size and renal length ratio between right and left kidneys (p<0.05).

A Study on Evaluation of Ecological Function before and after River Improvement (하천개수 전·후 하천공간의 생태기능 평가방법에 관한 연구)

  • In this study, the assessment scheme has been developed to evaluate the ecological function of rivers which were changed by the river improvement project. The evaluating factors are composed of physical structure in river channels and ecological connectivity in river areas, and each value of the factors is quantified based on 4 and 2 variables, respectably. This scheme was applied to past (1918, before artificial river improvement) and present Mankyung River area. A GIS model was adopted for calculating, analysing, and presenting river ecological conditions using the 204 grids and 7 reaches in study area. Comparison results show that the evaluation grade was decreased in both factors after river improvement. The main causes of lower grade (from II to IV) for physical structure are the river straightening and crossing structures. The reduction (from II to III) in ecological connectivity grade effected by linear fragmentation due to roads and rails is found to be greater than areal fragmentation due to land-use. In particular, it is also found that a high degree of ecological connectivity in 1918 was distributed along the river, but that one in 2007 showed a tendency to scatter because of the construction of levee and increase of urbanized area.

Stability Characteristics based on Crane Weight of Small Fishing Vessels Under Standard Loading Conditions: Investigation Report of the Capsize Accident at Goseong Port (크레인 교체에 따른 표준재화 상태에서의 소형 어선의 복원성 특성 - 고성항 전복 사고 재결서 중심 -)

  • In March 2016, a 6.67-ton fishing boat capsized owing to the loss of stability during crane operations. Capsizing occurs when a boat or ship is flipped over (or turned upside down) for reason other than accidents caused by collisions, contact, stranding, fire or explosion. Over the past nine years (2010-2018), capsize accidents have accounted for 2.34 % of all marine accidents and are gradually increasing. The loss of stability from improper shipping is the main cause of most capsizes, especially for small fishing vessels weighing 10 tons. According to the Fishing Vessel Act, small fishing vessels weighing less than a ton are exempted from inspections on stability and load cranes. This study analyzes the issue cited as the reason for the capsizing of the small fishing boat in Goseong, namely, the reduction of restoring moment due to increased weight of the crane. Fishing boats with similar loading conditions were modeled on the basis of re-determination, and their stability before and after the accident was assumed. The fishing boats with heavier cranes were found to be at higher risk of capsizing owing to the reduction of the restoring moment and the angle of deck immersion. Under standard loading conditions, the stability moments of fishing vessels are lesser during fishing, compared to when they depart from or arrive at the port.

  • As a potential approach for non-destructive testing of concrete structures, we evaluate the time-of-flight (TOF) ultrasound tomography technique In conventional X ray tomography, the reconstructed Image corresponds to the internal attenuation coefficient However, in TOF ultrasound tomography, the reconstructed Image is proportional to the retractive index of the medium Because refractive effects are minimal for X-rays, conventional reconstruction techniques are applied to reconstruct the Image in X-ray tomography However, since ultrasound travels in curved path, due to the spatial variations in the refractive index of the medium, the path must be known to correctly reconstruct the Image. Algorithm for determining the ultrasound path is developed from a Geometrical Optics point view and the image reconstruction algorithm, since the paths are curved It requires the algebraic approach, namely the ART or the SIRT Here, the difference between the computed and the measured TOP data is used as a basis, for the iteration process First the initial image is reconstructed assuming straight paths. It then updates the path based on the recently reconstructed image This process of reconstruction and path determination repeats until convergence The proposed algorithm is evaluated by computer simulations, and in addition is applied to a real concrete structure.

  • Lymphangiomas are relatively rare benign tumors of the lymphatic system, characterized by congenital lymphatic malformation such as anastomosis or obstuction of the lymphatic channels. There are most frequently diagnosed in children younger than 3years. In contrast to that of the hemangioma, the sex distribution of the lymphangioma is nearly evenly divided. The head and neck lymphangioma represents about $70{\sim}75%$ of all lymphangiomas, and they are difficult to manage. Spontaneous regression is rare, and rapid intermittent enlargement occurs secondary to infection or trauma. Enlargement may cause serious sequelae such as airway obstruction, feeding difficulties, and cosmetic problems. Treatments previously used for lymphangiomas include surgical excision and intralesional injection of sclerosants. Problems associated with surgical excision include the risk of cosmetically unacceptable scarring and the risk of damage to surrounding vital stricture and the high risk of incomplete excision. The sclerosants previously used have numerous other local and systemic side effects. This report describes a case that was successfully treated using OK-432 as a new sclerosant drug and secondary surgical excision for congential cavenous lymphangioma extensively enlarged to tongue, mouth floor and submandibular region.

  • It is well known that oral appliance could improve respiratory difficulty in patients with obstructive sleep apnea (OSA). To evaluate the effect of oral appliance, polysomnography and cephalometry were performed in OSA patients before and after oral appliance was used. Twenty four OSA patients were included in this study. Respiratory difficulty index (RDI) was obtained from polysomnography and the movement of soft palate, tongue, posterior pharygeal wall, hyoid bone, and mandible and the variables of the facial pattern were measured on the cephalogram. The changes of cephalometric parameters were compared to the RDI changes and the correlation was tested. After oral appliance was used, RDI decreased from 46.8 to 13.3. There was significant relationship between RDI improvement and the anterior movement of the mandible and superior movement of the hyoid bone. In 8 patients whose RDI was most improved, RDI improvement rate was correlated with the anterior movement of the tongue and anterior-posterior (AP) diameter of the airway at the levels between the lower portion of the soft palate and epiglottis. These results could conclude that anterior movement of the tongue and superior movement of the hyoid bone would be favorable cephalometric parameters for the improvement of OSA.

Study on Decay Characteristics Change of Spent Fuel Materials by DUPIC Fuel Cycle (DUPIC핵연료주기에 의한 사용 후 경수로핵연료의 방사선적 특성변화 분석)

  • The change in spent fuel characteristics by DUPIC fuel cycle(burnup of spent PWR fuel again in CANDU) is examined with time elapse since discharge. Major characteristics examined include isotopic concentration, radioactivity, decay heat radiotoxicity and radiation source-term of spent fuel material, which is existing in a type of spent PWR and DUPIC fuel. Behaviors of major nuclides contributing to such changes are also analyzed in terms of radionuclide concentration. From the analysis, the change in radionuclide concentration by DUPIC shows approximately 2% decrease in actinides concentration and 20% increase in fission products concentration. Radioactivity and decay heat of spent DUPIC fuel does not depend upon radionuclides concentrations, which is a unique in sence of general characteristics of spent fuel. In terms of gamma spectrum, spent DUPIC fuel shows lower values than that of spent PWR fuel by 40 to 50% in the range of $0.01{\sim}0.575$ MeV but much higher over 3.5MeV. Neutron Intensities of both spent fuels are mainly determined by $({\alpha},\;n)$ reaction and spontaneous fission reaction of actinides. Of them, especially, the spontaneous fission reaction Is a major neutron source-term, which causes that neutron intensities of spent DUPIC fuel $having{\sim}3.3$ times higher Cm-244 concentration are ${\sim}4$ times higher than that of spent PWR fuel.
