Evaluating efficiency of Coaxial MLC VMAT plan for spine SBRT (Spine SBRT 치료시 Coaxial MLC VMAT plan의 유용성 평가)

  • Purpose : The purpose of the study is to evaluate the efficiency of Coaxial MLC VMAT plan (Using $273^{\circ}$ and $350^{\circ}$ collimator angle) That the leaf motion direction aligned with axis of OAR (Organ at risk, It means spinal cord or cauda equine in this study.) compare to Universal MLC VMAT plan (using $30^{\circ}$ and $330^{\circ}$ collimator angle) for spine SBRT. Materials and Methods : The 10 cases of spine SBRT that treated with VMAT planned by Coaxial MLC and Varian TBX were enrolled. Those cases were planned by Eclipse (Ver. 10.0.42, Varian, USA), PRO3 (Progressive Resolution Optimizer 10.0.28) and AAA (Anisotropic Analytic Algorithm Ver. 10.0.28) with coplanar $360^{\circ}$ arcs and 10MV FFF (Flattening filter free). Each arc has $273^{\circ}$ and $350^{\circ}$ collimator angle, respectively. The Universal MLC VMAT plans are based on existing treatment plans. Those plans have the same parameters of existing treatment plans but collimator angle. To minimize the dose difference that shows up randomly on optimizing, all plans were optimized and calculated twice respectively. The calculation grid is 0.2 cm and all plans were normalized to the target V100%=90%. The indexes of evaluation are V10Gy, D0.03cc, Dmean of OAR (Organ at risk, It means spinal cord or cauda equine in this study.), H.I (Homogeneity index) of the target and total MU. All Coaxial VMAT plans were verified by gamma test with Mapcheck2 (Sun Nuclear Co., USA), Mapphan (Sun Nuclear Co., USA) and SNC patient (Sun Nuclear Co., USA Ver 6.1.2.18513). Results : The difference between the coaxial and the universal VMAT plans are follow. The coaxial VMAT plan is better in the V10Gy of OAR, Up to 4.1%, at least 0.4%, the average difference was 1.9% and In the D0.03cc of OAR, Up to 83.6 cGy, at least 2.2 cGy, the average difference was 33.3 cGy. In Dmean, Up to 34.8 cGy, at least -13.0 cGy, the average difference was 9.6 cGy that say the coaxial VMAT plans are better except few cases. H.I difference Up to 0.04, at least 0.01, the average difference was 0.02 and the difference of average total MU is 74.1 MU. The coaxial MLC VMAT plan is average 74.1 MU lesser then another. All IMRT verification gamma test results for the coaxial MLC VMAT plan passed over 90.0% at 1mm / 2%. Conclusion : Coaxial MLC VMAT treatment plan appeared to be favorable in most cases than the Universal MLC VMAT treatment planning. It is efficient in lowering the dose of the OAR V10Gy especially. As a result, the Coaxial MLC VMAT plan could be better than the Universal MLC VMAT plan in same MU.

A Study of the Registration of Simulator Images and Portal Images Using Landmarks in Radiation Treatment (랜드마크 (Landmark)를 이용한 방사선 치료 X선 시뮬레이터 영상과 포탈영상의 비교법 연구)

  • The goal of radiation treatment is to deliver a prescribed radiation dose to the target volume accurately while minimizing dose to normal tissues. Due to inaccurate placement of field and shielding block and patient's movement, there could be displacement errors between the planed and treatment regions. In order to verify the location of radiation treatment, we in this study developed the registration algorithm of the x-ray simulator images and portal images and quantified the inaccuracy in terms of shift, scale and rotation. The algorithm for registration of pairs of radiation fields consists of the alignment of pairs of radiation images by points matching and field displacement analysis by field boundary matching. In the first step, paired surface landmarks are matched to calculate the transformation parameters (scale, rotation and shift) using the corresponding line pairs which are created by connecting two landmarks of each image. In the next step, portal field boundary is extracted and then the two field boundaries are matched by the $\rho$-$\theta$ technique. Applying the phantom portal images, detection errors were calculated to be less than 2mm in translation, 1$^{\circ}$ in rotation and 1% in scale. In conclusion, we quantitatively analyzed the displacement error of x-ray simulator images and portal images. The present results could contribute to the study of the radiation treatment verification.

Performance Evaluation of the Tumor Tracking Method Using Beam on/off Interface for the Treatment of Irregular Breathing (호흡이 불규칙한 환자의 치료를 위한 Beam on/off Interface를 이용한 종양 추적 치료 방법의 성능 평가)

  • Dose rate regulated tracking is known to be an efficient method which adaptively delivers tracking treatments when patient breathing is irregular. The Motion Management Interface (MMI, Varian Medical System, CA), which provides beam on/off switching during treatment is available for clinic. Study is to test if delivering the adaptive tumor tracking is feasible for irregular breathing using beam switching with MMI. 55 free breathing RPM traces acquired from lung cancer patients are used. The first day RPM traces of the patients are utilized to design preprogrammed tracking MLC patterns, of which periods are intentionally reduced by 20% in order to catch up the variation of patient breathing irregularity in the treatment day. Eligibility criteria for this technique are the variation of amplitude and period less than 20%. An algorithm which determines beam on/off every 100 ms by considering the preprogrammed (MLC) positions and current breathing positions is developed. Tracking error and delivery efficacy are calculated by simulating the beam-switching adaptive tracking from the RPM traces. Breathing patterns of 38 patients (70%) met the eligibility criteria. Tracking errors of all of the cases who meet the criteria are less than 2 mm (average 1.4 mm) and the average delivery efficacy was 71%. Those of rest of the cases are 1.9 mm and 48%. Adaptive tracking with beam switching is feasible if patient selection is based on the eligibility criteria.

A Definition of Korean Heat Waves and Their Spatio-temporal Patterns (우리나라에 적합한 열파의 정의와 그 시.공간적 발생패턴)

  • This study provides a definition of heat waves, which indicate the conditions of strong sultriness in summer, appropriate to Korea and intends to clarify long term(1973-2006) averaged spatial and temporal patterns of annual frequency of heat waves with respect to their intensity. Based on examination of the Korean mortality rate changes due to increase of apparent temperature under hot and humid summer conditions, three consecutive days with at least $32.5^{\circ}C,\;35.5^{\circ}C,\;38.5^{\circ}C,\;and\;41.5^{\circ}C$ of daily maximum Heat Index are defined as the Hot Spell(HS), the Heat Wave(HW), the Strong Heat Wave(SHW), and the Extreme Heat Wave(EHW), respectively. The annual frequency of all categories of heat waves is relatively low in high-elevated regions or on islands adjacent to seas. In contrast, the maximum annual frequency of heat waves during the study period as well as annual average frequency are highest in interior, low-elevated regions along major rivers in South Korea, particularly during the Changma Break period(between late July and mid-August). There is no obvious increasing or decreasing trend in the annual total frequency of all categories of heat waves for the study period However, the maximum annual frequencies of HS days at each weather station were recorded mainly in the 1970s, while most of maximum frequency records of both the HW and the SHW at individual weather stations were observed in the 1990s. It is also revealed that when heat waves occur in South Korea high humidity as well as high temperature contributes to increasing the heat wave intensity by $4.3-9.5^{\circ}C$. These results provide a useful basis to help develop a heat wave warning system appropriate to Korea.
