Establishment of Standards for Architecture & Operation Planning of Public Health Services (서울시 종합의료시설 도시계획지원을 위한 공공필요의료시설 설치 및 운영 기준 정립)

  • 병원은 다양한 의료서비스를 제공하기 위해 의료기관 별 운영 전략에 따라 각기 다른 방식으로 운영된다. 특히 종합의료시설은 중증도가 높은 환자를 대상으로 수술이나 입원 등의 의료서비스를 효율적으로 제공하고자 그에 따른 운영 전략이 상이하다. 이러한 병원의 운영방식에 의해 결정되는 건축 역시, 건립 시기, 유형, 중증도에 따른 병원의 규모 등에 따라 시설 수준의 차이가 나타나며 이는 최근 정부에서 요구되는 제도 및 정책의 수용 여부를 결정짓는 중요한 요인이 되기도 한다. 최근 의료법은 기존 의료시설뿐만 아니라 신규 의료시설에 대한 설치 및 운영기준이 강화 추세에 있고, 서울시 또한 공공의료 확충을 위해 감염관리시설이나 필수의료시설 설치를 위한 용적률 인센티브 정책을 추진 중이다. 병원 운영 환경이 상이함에 따라 종합의료시설 설치 및 운영에 대한 인센티브제도의 적용 기준을 일괄적으로 적용하기에는 어렵다. 그러나 공익을 위해서 종합의료시설 인센티브제도를 지속 가능하게 운영하기 위한 객관적이고 합리적인 기준은 반드시 필요하다. 공공의료 기능 확충을 위한 서울시 종합의료시설 지구단위계획 수립·운영 기준은 공공과 민간이 모두 만족하는 의료환경 구축을 위해 매우 깊은 고민이 필요했다. 본 논설은 서울시 공공필요의료시설 확충 제도의 주요 내용과 공공필요의료시설 설치 및 운영 기준에 관해 소개하고자 한다. 특히 기준의 주요 내용을 정립하게 된 배경과 이유 등을 설명하여 본 제도의 의의를 정확히 전달하고 향후 보완해야 할 부분들을 검토하고자 한다.

Non-osseous Coalition of the Third Metatarsal-Lateral Cuneiform Joint: A Case Report (제 3중족-외측 설상 관절에서 발생한 비골성 족근골 결합: 증례 보고)

  • This paper reports a rare case of the symptomatic third metatarsal (MT3) - lateral cuneiform (LC) in a 55-year-old male who presented with complaints of severe intermittent pain in his right foot. Plain radiographs and computed tomography scans revealed sclerosis and irregularity at this joint. The intraoperative findings demonstrated a fibrocartilaginous coalition. The pain had improved one year after removing the MT3-LC joint by en bloc and arthrodesis.

  • Common carotid artery occlusion (CCAO) is a very rare disorder that has rarely been studied. CCAO causes several neurological symptoms but can sometimes be asymptomatic due to the development of various anastomoses. Herein, we report the case of a 70-year-old male patient diagnosed with asymptomatic CCAO due to anastomotic flow. The patient underwent transfemoral cerebral angiography (TFCA) and was found to have CCAO with two collateral pathways, including an occipital artery-vertebral artery anastomosis. We emphasize the importance of TFCA when CCAO is suspected and review the types and anastomotic pathways of CCAO.

  • Bertolotti's syndrome is a congenital abnormality in which the expanded lower lumbar transverse process articulates with the ilium or sacrum. It is an important cause of low back pain in children and adolescents that is frequently misdiagnosed. We describe the case of a 17-year-old girl with low back pain who had a 4-year history of juvenile idiopathic arthritis. She subsequently underwent plain radiography and magnetic resonance imaging and was eventually diagnosed with Bertolotti's syndrome. She was managed conservatively with 6 weeks of physical therapy and an exercise program, and her pain subsided. This case demonstrates the importance of considering anatomical and structural variants when evaluating low back pain in adolescents.

Medical Accessibility Analysis by Optical Store and Ophthalmic Clinic Distribution (Centering on Special and Metropolitan Cities) (안경원과 안과의원 분포에 따른 의료접근도 분석 (특별시와 광역시 중심으로))

  • Purpose: This study investigated medical accessibility on optical stores and ophthalmic clinics of Seoul Special City and six other metropolitan cities. Methods: By using a number of households, population, optician stores, ophthalmic hospitals, and real estate (apartments) standard market price of Seoul Special City and six other metropolitan cities provided by the Commercial District Information System (2016.6) of Small Enterprise & Market Authority, we analyzed the level of healthcare accessibility and business area zones, Statistical analyses were performed with SPSS 18.0. Results: Inchon (household 2,227/population 5,723) had the highest household and population rate per optical store, and Gwangju (1,146/2,979) had the lowest. Gwangju (24,612/63,987) had the highest household and population rate per optical store, and Seoul (10,021/24,432) had the lowest. From the consumer and patient's point of view, lower household rate per optical store is a city with good accessibility to healthcare, but from an optical store and ophthalmic clinic's view, it will have great difficulty due to issues of competition. Conclusions: Consumers and patients should be the center of healthcare. A healthcare system that can provide smooth service anywhere anytime should be constructed. However, most metropolitan cities, including Seoul, have optical stores and ophthalmic clinics densely populated where profitability and liquidity are ensured and causing unbalanced distribution of healthcare. To solve such problems, we need proper distribution of optician stores according to the population proportion and industrial-educational research to find balance point of local healthcare.

A Study on the Change of Physical Environment in Seoul Medical Center by Providing Comprehensive Nursing Service (간호간병통합서비스 도입에 따른 서울의료원 병동부 시설환경변화 연구)

  • Purpose: The research looks at differences between the 'General ward' and the 'Comprehensive nursing ward' in Seoul Medical Center, regarding the facility improvements and changes in nursing services. It investigates and analyzes spatial problems and improvement needs through a survey and conduct investigation of staffs. It is to propose the primary data for the architectural planning of the future ward with the comprehensive nursing service. Methods: Targeting the comprehensive nursing ward, changes of the physical environment and spatial problems are analyzed through a field survey, behavior investigation, present-condition investigation, and floor plan analysis. Results: The workforce is increased by approximately twice the amount of the nursing staffs in the comprehensive nursing ward, compared to the pre-general ward. When utilizing the general ward, various spatial problems arise due to the restrictions of the facility condition. Because Sub N.S is an important facility as a part of the nursing work function in the ward of the comprehensive nursing service, the opinions of staffs must be considered when selecting a location and composing a space. Implications: It can be used as a primary data for the comprehensive nursing ward when architectural planning of a new hospital.

Trends in Deep Learning-based Medical Optical Character Recognition (딥러닝 기반의 의료 OCR 기술 동향)

  • Optical Character Recognition is the technology that recognizes text in images and converts them into digital format. Deep learning-based OCR is being used in many industries with large quantities of recorded data due to its high recognition performance. To improve medical services, deep learning-based OCR was actively introduced by the medical industry. In this paper, we discussed trends in OCR engines and medical OCR and provided a roadmap for development of medical OCR. By using natural language processing on detected text data, current medical OCR has improved its recognition performance. However, there are limits to the recognition performance, especially for non-standard handwriting and modified text. To develop advanced medical OCR, databaseization of medical data, image pre-processing, and natural language processing are necessary.

Chest CT Findings of COVID-19 Patients with Mild Clinical Symptoms at a Single Hospital in Korea (경증의 임상 소견을 보이는 COVID-19 환자들의 흉부 CT 소견)

  • Purpose To retrospectively evaluate the chest computed tomography (CT) findings of coronavirus disease 2019 (COVID-19) in patients with mild clinical symptoms at a single hospital in South Korea. Materials and Methods CT scans of 87 COVID-19 patients [43 men and 44 women; median age: 41 years (interquartile range: 26.1-51.0 years)] with mild clinical symptoms (fever < 38℃ and no dyspnea) were evaluated. Results CT findings were normal in 39 (44.8%) and abnormal in 48 (55.2%) patients. Among the 48 patients with lung opacities, 17 (35.4%) had unilateral disease and 31 (64.6%) had bilateral disease. One (2.1%) patient showed subpleural distribution, 9 (18.8%) showed peribronchovascular distribution, and 38 (79.2%) showed subpleural and peribronchovascular distributions. Twenty-two (45.8%) patients had pure ground-glass opacities (GGOs) with no consolidation, 17 (35.4%) had mixed opacities dominated by GGOs, and 9 (18.8%) had mixed opacities dominated by consolidation. No patients demonstrated consolidation without GGOs. Conclusion The most common CT finding of COVID-19 in patients with mild clinical symptoms was bilateral multiple GGO-dominant lesions with subpleural and peribronchovascular distribution and lower lung predilection. The initial chest CT of almost half of COVID-19 patients with mild clinical symptoms showed no lung parenchymal lesions. Compared to relatively severe cases, mild cases were more likely to manifest as unilateral disease with pure GGOs or GGO-dominant mixed opacities and less likely to show air bronchogram.

Two Cases of Herpes Zoster Following Varicella Vaccination in Immunocompetent Young Children: One Case Caused by Vaccine-Strain (건강한 어린 소아에서 수두 백신 접종 후 발생한 대상포진 2예: 백신주에 의한 1예)

  • Herpes zoster (HZ) has been reported in immunocompetent children who received the varicella vaccine. In vaccinated children, HZ can be caused by vaccine-strain or by wild-type varicella-zoster virus (VZV). Like wild-type VZV, varicella vaccine virus can establish latency and reactivate as HZ. We report two cases of HZ in otherwise healthy 16- and 14-month-old boys who received varicella vaccine at 12 months of age. They presented with a vesicular rash on their upper extremities three to four months after varicella vaccination. In one case, a swab was obtained by abrading skin vesicles and VZV was detected in skin specimens by polymerase chain reaction. The VZV open-reading frame 62 was sequenced and single nucleotide polymorphism analysis confirmed that the virus from skin specimen was vaccine-strain. This is the first HZ case following varicella vaccination confirmed to be caused by vaccine-strain VZV in the immunocompetent children in Korea. Pediatricians should be aware of the potential for varicella vaccine virus reactivation in vaccinated young children.
