Common Scope of Practice for Advanced Practice Nurses in Korea Derived from Expert Agreement (전문가 합의로 도출된 한국 전문간호사의 공통 업무범위)

  • Purpose : This study aimed to propose a common scope of practice (SOP) for 13 specialties of Advanced Practice Nurses (APNs) in Korea. Methods : The first draft of a common SOP was extracted from domestic and international laws with a literature review by 17 experts from the Korean Association of Advanced Practice Nurses (KAAPN). Then, the common SOP was finalized after comparing the activities of APNs in clinical settings. Results : A total of 70 duties were identified and six categories were suggested for the common SOP. The SOP proposed by the KAAPN featured the following: 1) identification of and discrimination between health problems; 2) prescription and implementation of diagnostic tests; 3) treatment of injuries and diseases while implementing measures to prevent exacerbation; 4) prescription of medicinal products in line with 1) to 3); 5) referral and consultation; and 6) education and counseling. It was then confirmed that the proposed six categories in the common SOP reflected all the duties performed by APNs in clinical practice, including all 40 activities. Conclusion : The results of this study can be used as evidence for the legalization of a common SOP for APNs. Given the increasing multidisciplinary team approach adopted in Korean hospitals, it may be desirable to establish a broader SOP to reflect the diverse duties of APNs.

A Study on Health Information and Medical Consulting via Internet Focusing on the Age Group of 20s (인터넷을 활용한 건강정보 및 의료상담에 관한 연구 (20대를 중심으로))

  • High Internet usage and the public's keen interest on health have influenced the health care system, and a potential value of the online health information and medical consultation market is immense. This study reveals results from data collected from Seoul residents in the age group of 20s in 2011. Out of 499 respondents, 75.2% answered that they used online health information; however, only 7.2% answered that they have used online medical consultation services. Findings on the purposes of using online medical consultation included treatments of symptoms(33.6%) and self-disciplines of one's health(19.5%). Mostly used Websites for health information search included search engines and blogs, but respondents preferred to use government sites and hospital sites in the future. When choosing a medical consultation, respondents preferred a certain website for different reasons including creditability of the consultant(23.7%), creditability of the organization(23.7%), rapid responses(21.2%), and more. Overall, although health information search via web is being highly utilized among people in their 20s, utilization of online medical consulting is not. Thus, promotion efforts to increase awareness and utilization of online medical consulting based on the site selection criteria, type of personal information disclosure, and other preferences are essential. Also, creating websites meeting these criteria is important.

Effect of Health Behavior and Obesity Indices on Blood Pressure in 20s Man (20대 성인 남성에서 건강행태와 비만지표가 혈압에 미치는 영향)

  • This study was conducted to explore the effect of health behavior and obesity indices on blood pressure(BP) in 20s men. This was a retrospective exploratory study and the subjects were 214 men who participated in screening test for clinical trial at a tertiary hospital between October 2006 and April 2011. The proportion of normal SBP and DBP was 28.5% and 50.0%, respectively. BP was significantly higher in individuals who were hypertriglyceridemia and hypercholesterolemia than BP in subjects of opposing categories. DBP was significantly higher in individuals who were 25~29 years and smokers than DBP in subjects of opposing categories. The significant factors of increasing SBP were BMI and TG, and those for DBP were TC, TG, age, and smoking amount. In conclusion, these results demonstrate hypertention prevention program to include decreasing intake of TC and TG, maintaining normal BMI and quitting smoking needs to the young men in 20s.

Determinants of Profitability of Regional Public Hospitals in Korea - Focusing on the COVID-19 Pandemic Period - (지역거점 공공병원의 수익성 결정요인 - COVID-19 유행기간을 중심으로 -)

  • Purposes: We analyzed the profitability determinants of regional public hospitals during the entire period between 2010 and 2020 and the period before and after COVID-19. We intended to provide fundamental data for developing publicness evaluation index and task of establishing and expanding regional public hospitals. Methodology: The financial and non-financial information of the regional public hospitals were used as the main analysis data; The financial data was established by the Center for Public Healthcare Policy of National Medical Center, and the non-financial data by the Health Insurance Review and Assessment Service. T-test and regression analysis were used. Findings: The results can be summarized in two. First, the main determinants of profitability of the regional public hospitals were appeared to be the total asset turnover rate and the labor cost rate. Second, during the COVID-19 pandemic in the regional public hospitals, the number of sickbeds, the number of isolation rooms, the total asset turnover rate and the labor cost rate appeared to be the factor worsening the profitability. Practical Implication: The results of this study suggests that the management of the regional public hospitals is not aiming for the profit making, but it performs the functions as the community healthcare safety net such as controlling infectious diseases.

Study on the Infection Control and Dental waste Disposal in Dental Clinic Located in Seoul City (서울시 치과의원의 진료실 감염관리 및 폐기물 처리 실태 조사연구)

  • This study was aimed to investigate the practice on the infection control and dental waste disposal in dental clinic. The data used in this study had been collected in 100 dental clinics located in Seoul city from August to October, 2001. It was shown that examination instruments, oral surgery instruments, and periodontal instruments were almost sterilization in dental clinic. It was high in the recognition of individual protection but it was low in the degree of practice. Therefore, the practice in infection control is very important to dental team for safety of patients and operators.

Lecture Note of a Thoracic Surgeon, Fifty More Years Ago (50여년 전 한 흉부외과 의사의 강의록)

  • The late Professor Lee CB (1915~1967) was one of the pioneers in the early stages of thoracic surgery in Korea while he was in charge of the Department of Thoracic and Cardiovascular Surgery at Seoul National University hospital as the first director from 1957 until he died of liver cancer in 1967. He was a graduate from the old Seoul National University, and he dedicated himself to the field of lung surgery after he joined the army during the Korean War, which broke out in 1950. Among his many contributions to pulmonary surgery, he performed the first partial lung resection in Korea in 1953. His lecture notes were recently found. These lecture notes for medical students were written by hand in late 1950s, Considering the content and form of the lecture notes, they are thought to have very significant historical value. The lecture notes were a total of 277 pages on general thoracic surgery and cardiovascular surgery with 75 self-drawn figures. This study was intended to facilitate a better understanding of the history of thoracic surgery in Korea, and especially for young thoracic surgeons. These notes may well contribute to the field of Thoracic and Cardiovascular Surgery in Korea.

Long-Term Impact of Stigma on Coping Strategies and Depressive and Posttraumatic Stress Symptoms in MERS Survivors (메르스 대유행 생존자들이 인식한 낙인이 대처전략 및 우울 및 외상후 스트레스 증상에 끼치는 장기적 영향)

  • Objectives : This study intends to assess the associations among perceived stigma at the time of infection, coping strategies adopted 12 months later and depressive and posttraumatic symptoms 24 months later in Middle Eastern Respiratory Syndrome (MERS) survivors. Methods : A nationwide cohort study was conducted on 63 survivors of 2015 MERS outbreak. Demographic data, illness severity of MERS, depression and posttraumatic stress symptoms, coping strategies and MERS-related stigma were collected 12 and 24 months after the MERS outbreak, respectively. Results : Higher levels of perceived stigma at the time of outbreak were associated with higher levels of dysfunctional coping strategies after 12 months (p=0.003) and more severe depressive (p=0.058) and posttraumatic stress symptoms (p=0.011) after 24 months. Moreover, higher levels of dysfunctional coping strategies after 12 months were significantly associated with more severe depressive (p=0.002) and posttraumatic stress symptoms (p<0.001) after 24 months. Conclusions : Social stigma against people who have contracted an emerging infectious disease can leave a negative impact on the mental health of the survivors in the long term. In case of novel pandemics in the future, prompt rectification of stigma and promotion of adaptive coping strategies in survivors are needed.

Identification of Viral Pathogens for Lower Respiratory Tract Infection in Children at Seoul During Autumn and Winter Seasons of the Year of 2008-2009 (서울의 2008-2009년 가을, 겨울철에 유행한 소아의 하기도 호흡기 바이러스 병원체의 규명)

  • Purpose : The Purposes of this study are to identify the circulating etiologic viruses of acute lower respiratory tract infection in children and to understand the relation with clinical diagnosis. Methods : We obtained a total of 418 nasopharyngeal aspirates from children admitted for their acute lower respiratory tract infections at three tertiary hospitals in Seoul from September 2008 to March 2009. We performed multiplex RT-PCR to identify 14 etiologic viruses and analyzed their emerging patterns and clinical features. Results : Average age of patients was 16.4 months old and the ratio of male to female was 1.36. Viruses were detected in 56.2% of a total of 418 samples. Respiratory syncytial virus (35%) was the most frequently detected and followed by human rhinovirus (22%), human bocavirus, adenovirus, human metapneumovirus, parainfluenza virus, influenza virus and human coronavirus. Co-infection reached 21.9 % of positive patients. Conclusion : When we manage the patients with acute lower respiratory infectious diseases, we should remind the role of various viral pathogens, which might be circulating by seasons and by local areas.

Evaluation of Radiation Exposure to Medical Staff except Nuclear Medicine Department (핵의학 검사 시행하는 환자에 의한 병원 종사자 피폭선량 평가)

  • Purpose The goal for this study is to figure out that medical staff except Nuclear Medicine Department could be exposed to radiation from the patients who take Nuclear Medicine examination. Materials and Methods Total 250 patients (Bone scan 100, Myocardial SPECT 100, PET/CT 50) were involved from July to October in 2015, and we measured patient dose rate two times for every patients. First, we checked radiation dose rate right after injecting an isotope (radiopharmaceutical). Secondly, we measured radiation dose rate after each examination. Results In the case of Bone scan, dose rate were $0.0278{\pm}0.0036mSv/h$ after injection and $0.0060{\pm}0.0018mSv/h$ after examination (3 hrs 52 minutes after injection on average). For Myocardial SPECT, dose rate were $0.0245{\pm}0.0027mSv/h$ after injection and $0.0123{\pm}0.0041mSv/h$ after examination (2 hrs 09 minutes after injection on average). Lastly, for PET/CT, dose rate were $0.0439{\pm}0.0087mSv/h$ after examination (68 minutes after injection on average). Conclusion Compared to Nuclear Safety Commission Act, there was no significant harmful effect of the exposure from patients who have been administered radiopharmaceuticals. However, we should strive to keep ALARA(as low as reasonably achievable) principle for radiation protection.

Comparison Analysis of The results of IRMA Test among Different Equipment According to Algorithm change. (IRMA 검사법 중 알고리즘 변경에 따른 장비 간 결과값 비교분석)

  • Purpose The principle of nuclear medicine test is divided into two main categories: competition(radioimmunoassay, RIA) and noncompetitive reaction(Immunoradiometric assay, IRMA). It is known that the curve fitting method, which is commonly used in inspection field, uses Spline interpolation in RIA method and Linear interpolation method in IRMA method. Among them, the insulin test using the IRMA test showed a significant difference, especially at low concentrations, despite the same algorithm of linear interpolation between fully automated radio immunoassay analyzers. In this study, we aim to obtain results from applying two different of algorithm using fully automated radio immunoassay analyzers including Gamma pro, Gamma 10, Cobra, and SR300. Materials and Methods A total of 30 test samples were selected for the test of TSH, ferritin, C-peptide, and insulin serum levels. Test was performed by IRMA method. We compared the difference in the results of applying the linear interpolation method and the spline interpolation method to Gamma Pro, Gamma 10, Cobra, and SR300 equipment. Results Two-way ANOVA was used for statistical analysis. The significance level was applied as P <0.05. The results of TSH, ferritin, C-peptide, and insulin tests were compared between the fully automated radio immunoassay analyzers. There was a significant difference between ferritin, C-peptide, and insulin serum levels(P<0.001). TSH didn't show any significant different between the devices(P=0.29). In the difference between linear and spline interpolation, there was no significant difference between insulin test(P=0.08), TSH test(P=0.81), and Ferritin test(P=0.06). However, C-peptide test showed a significant difference(P=0.03). Especially, the insulin test showed significant difference in lower ranges. As a result of comparing and analyzing the difference between the two interpolation methods, the devices in the low concentration group showed significant difference(P<0.001). Conclusion In case of new equipment in the laboratory it is necessary to recognize that there is a difference in the curve fitting method for each automated radio immunoassay analyzers in the low concentration area when the principle of inspection is IRMA method.
