Human Lung Cancer Cell Xenografts Implanted under the Capsule of Kidney, Spleen and Liver (폐암 세포주를 사용한 신, 비장 및 간 피막하 분식법의 비교)

  • Bakground : Complete resection by the surgery has been selected as the treatment of choice in lung cancer patients, but in cases of recurrence after excision or inoperable cases, the importance of anticancer chemotherapy has been emphasized. If one can select a set of the sensitive chemotherapeutic agents before anticancer chemotherapy, it will give more favourable results. Subrenal capsular assay has been recognized as a useful in-vivo chemosensitivity test of thoracic and abdominal tumors and it can be done in a short time for a rapid interpretation of tumor responsiveness to anticancer chemotherapeutic drugs. It has been reported that various kinds of cancer cells can be implantable to the kidney, but so far there is no comparative study of xenogeneic cell implantation on liver, spleen and kidney. The author implanted the human lung cancer cells under the capsule of S.D rat's liver, spleen and kidney respectively and compared the pattern of growth and histology. Material and Method: After incubation of human lung cancer cell line (SW-900 G IV) in RPMI 1640 (Leibovitz L-15 medium) culture media, 3${\times}$3${\times}$3 mm size fibrin clots which contain 108 cancer cells were made. Thereafter the fibrin clots were implanted at subcapsule area of liver, spleen and kidney of S.D. female rat. For immune suppression, cyclosporin-A (80 mg/Kg) was injected subcutaneously daily from post-implantation first day to sixth day. The body weight was measured at pre and post implantation periods. The growth pattern and the size of tumor mass were observed and the pathologic examination and serum tumor marker tests were performed. Result: Body weight increased in both of control and experimental groups. Serum Cyfra 21-1 was not detected. Serum levels of CEA and NSE revealed no significant change. The SCC-Ag increased significantly in implanted group. The growth rate of human lung cancer cells which was implanted on spleen was higher than on liver or kidney. The surface area, thickness, and volume of tumor mass were predominant at spleen. The success rates of implantation were 80% on kidney, 76.7% on spleen and 43.3% on liver. Pathologic examination of implanted tumors showed characteristic findings according to different organs. Tumors that were implanted on kidney grew in a round shape, small and regular pattern. In the spleen, tumors grew well and microscopic neovascularization and tumor thrombi were also found, but the growth pattern was irregular representing frequent daughter mass. Human lung cancer cells that were implanted in the liver, invaded to the liver parenchyme, and had low success rate of implantation. Microscopically, coagulation necrosis and myxoid fibrous lesion were observed. Conclusion: The success rate of implantation was highest in the kidney. And the mass revealed regular growth that could be measured easily. The SCC-Ag was presented earlier than CEA or Cyfra21-1. The Cyfra21-1 was not detected at early time after implantation. The best model for tumor implantation experiment for chemosensitivity test was subrenal capsular analysis than liver and spleen and the useful serum tumor marker in early period of implantation was the SCC-Ag.

  • Recently, the Personal Health Device(PHD) that measures various biometric data easily are highlighted for ensuring portability, scalability and interoperability among the device as well as needs for a standardization of managing information measured by. In this paper, we'd like to propose u-healthcare monitoring system that measure biometric data(Oxygen saturation, Body weight, ECG and Blood pressure) by PHD featured with transferring data into devices such as smartphone using Bluetooth Health Device Profile(HDP) based on the ISO/IEEE 11073.

Biocompatibility of Tissue-Engineered Heart Valve Leaflets Based on Acellular Xenografts (세포를 제거한 이종 심장 판막 이식편을 사용한 조직공학 심장 판막첨의 생체 적합성에 대한 연구)

  • Current artificial heart valves have several disadvantages, such as thromboembolism, limited durability, infection, and inability to grow. The solution to these problems would be to develop a tissue-engineered heart valves containing autologous cells. The aim of this study was to optimize the protocol to obtain a porcine acellular matrix and seed goat autologous endothelial cells on it, and to evaluate the biological responses of xenograft and xeno-autograft heart valves in goats. Material and Method: Fresh porcine pulmonic valves were treated with one method among 3 representative decellularization protocols (Triton-X, freeze-thawing, and NaCl-SDS). Goat venous endothelial cells were isolated and seeded onto the acellularized xenograft leaflets. Microscopic examinations were done to select the most effective method of decellularizing xenogeneic cells and seeding autologous endothelial cells. Two pulmonic valve leaflets of. 6 goats were replaced by acellularized porcine leaflets with or without seeding autologous endothelial cells while on cardiopulmonary bypass. Goats were sacrificed electively at 6 hours, 1 day, 1 week, 1 month, 3 months, and 6. months after operation. Morphologic examinations were done to see the biological responses of replaced valve leaflets. Result: The microscopic examinations showed that porcine cells were almost completely removed in the leaflets treated with NaCl-SDS. The seeded endothelial cells were more evenly preserved in NaCl-SDS treatment. All 6 goats survived the operation without complications. The xeno- autografts and xenografts showed the appearance, the remodeling process, and the cellular functions of myofibroblasts, 1 day, 1 month, and 3 months after operation, respectively. They were compatible with the native pulmonary leaflet (control group) except for the increased cellularity at 6 months. The xenografts revealed the new endothelial cell lining at that time. Conclusion: Treatment with NaCl-SDS was most effective in obtaining decellularized xenografts and facilitate seeding autologous endothelial cells. The xenografts and xeno-autografts were repopulated with myofibroblasts and endothelial cells in situ serially. Both of grafts served as a matrix for a tissue engineered heart valve and developed into autologous tissue for 6 months.

신경외과 수술에서의 인공 생체재료

  • 신경외과는 뇌수술과 척추수술로 나눈다. 이중뇌수술 분야에서는 두개강내압 항진시 감압술이라고 '머리의 뚜껑을 연다'라는 표현과 마찬가지로 두개골의 일부분(손바닥크기)을 절제하는 수술이 있다. 이때 과거에는 떼어낸 뼈를 복부의 피하지방밑에 심어서 보관을 하다가 3-6개월 후 환자의 뇌상태가 안정이 되면 다시 꺼내어 제자리에 놓았는데, 골편이 피하지방에서 녹는 경우가 다소 있고, 복부에 이식한 장소가염증이 생기는 경우도 있어서 요즘은 냉동고에서 -70도를 유지하여 보관 후 나중이 복원 수술을 할 때녹여서 사용한다. 이를 '자가골 두개성형술' 이라 한다. 하지만골편의 오염이나 소실 혹은 1차 두개성형술 후 감염 등 어쩔 수 없이 자가골을 사용 못하는 경우에는인공으로 두개골편 모형을 제작하여 '뚜껑'으로 사용해야 한다. 현재PMMA를 이용하여 수술 시 모형을 제작하는 방법이 많이 사용되며 최근 단단한 스펀지 형태의 인공제품이 사용되고 있으나 가격이 매우비싸지만 스펀지 기공내로 자가골이 자란다는 장점이 있다. 척추수술 분야에서는 뼈 대치품을 비교적 많이필요로 한다. 즉 척추 후방고정을 하는 경우 원래 수술의 목적인 감압술이나 교정술 등을 한 후 척추분절간 쇠(티타늄사용)고정을 한 뒤에 뼈조직(자가골이나 동종이식골, DBM, HA stick) 등을 충분히 사용하여덮어줌으로써 분절간 골유합을 유도할 때 많은 양이 필요하며, 척추 전방으로 수술을 하는 경우에도 디스크공간이 빈 상태에서 Cage(추체와 추체를 지지하는 작은 상자모양) 내에뼈조직을 넣어서 척추분절간 골유합이 일어나도록 한다. 최근 HA를 cage 대용품으로 사용한 경우에서 HA가 부서지는 경우가 기사화된바 있다. 또한 경추의 추체 내에 작은 구멍만 뚫고 디스크 수술을 한 후에 그 구멍에 HA+B-TCP 소재를 넣어 천연뼈의 성장 유도를 위해 사용되는 등 신경외과의 다양한 분야에서 생체소재들이 사용되고 있다.

Development and Application of Wireless Power Transmission Systems for Wireless ECG Sensors (지속적인 심장질환 모니터링을 위한 인체 삽입형 생체 센서의 무선전력전송 시스템)

  • We investigated the variations in the magnetic-field distribution and power transmission efficiency, resulting from changes in the relative positions of the transmitting and receiving coils, for electromagnetic-induction-type wireless power transmission using an elliptical receive coil. Results of simulations using a high-frequency structure simulator were compared to actual measurement results. The simulations showed that the transmission efficiency could be maintained relatively stable even if the alignment between the transmitting and receiving coils was changed to some extent. When the centre of the receiving coil was perfectly aligned with the centre of the transmitting coil, the transmission efficiency was the maximum; however, the degree of decrease in the transmission efficiency was small even if the centre of the receiving coil moved by ± 10mm from the centre of the transmitting coil. Therefore, it is expected that the performance of the wireless power transmission system will not be degraded significantly even if perfect alignment is not maintained. The results suggested a standardized application method of wireless transmission in the utilization of wireless power for implantable sensors.

  • Various biomaterials have been widely used for biomedical applications, including bio-organs, medical devices, and clinical devices like vessel, blood pumps, artificial kidneys and hearts, even in contact with blood. The issue of blood compatibility has been studied intensively to prevent negative effects such as thrombosis due to the implanted devices. The use of lotus-leaf-like structured surfaces has been extended to an increasing number of applications such as contamination prevention and anticorrosion applications. Various methods such as template, sol-gel transition, layer-by-layer, and other methods, developed for the fabrication of lotus-leaf-like surfaces have been reported for major industrial applications. Recently, the non-wettable character of these surfaces has been shown to be useful for biomedical applications ranging from blood-vessel replacement to antibacterial surface treatment. In this review, we provide a summary of current and future research efforts and opportunities in the development and medical applications of lotus-leaf-like structure surfaces.

Hepatic Vessel Segmentation using Edge Detection (Edge Detection을 이용한 간 혈관 추출)

  • Hepatic vessel tree is the key structure for hepatic disease diagnosis and liver surgery planning. Especially, it is used to evaluate the donors' and recipients' liver for the LDLT(Living Donors Liver Transplantation) and estimate the volumes of left and right hepatic lobes for securing their life in the LDLT. In this study, we propose a method to apply canny edge detection that is not affected by noise to the liver images for automatic segmentation of hepatic vessels tree in contrast abdominal MDCT image. Using histograms and average pixel values of the various liver CT images, optimized parameters of the Canny algorithm are determined. It is more time-efficient to use the common parameters than to change parameters manually according to CT images. Candidates of hepatic vessels are extracted by threshold filtering around the detected the vessel edge. Finally, using a system which detects the true-negatives and the false-positives in horizontal and vertical direction, the true-negatives are added in candidate of hepatic vessels and the false-positives are removed. As a result of the process, the various hepatic vessel trees of patients are accurately reconstructed in 3D.

  • 산한지통(散寒止痛)의 효능(效能)이 있는 고량강(高良薑)이 체내 면역활성에 미치는 영향을 검토하기 위하여 인삼과 함께 실험을 행하여 아래와 같은 결과를 얻었다. 고량강(高良薑) 및 인삼 수침액을 Sarcoma-180 고형 종양 세포를 이식한 생쥐에 투여하여 생존기간을 연장시켰다. 이를 바탕으로 면역에 대한 효과를 검토한 결과, 정상 ICR 생쥐에서 고량강(高良薑) 수침액은 백혈구수, 체중에 대한 상대적 비장의 무게를 약간 감소시켰으나 통계적으로는 유의성이 없었고, 간에는 영향을 주지 않았다. in vitro 실험에서, splenic IgM PFCs을 유의성있게 증가시켰으며, BSA를 주사한 생쥐에서의 Arthus반응도 증가시켰다. 그러나, 지연형 과민반응에는 영향을 주지 않았으며, 비특이성 면역에 관여하는 Macrophage 등의 탐식 세포의 기능에도 영향을 주지 않았다. 이상의 결과, 고양강은 체액성 면역을 강화할 것으로 보여진다.

Serum Lmmunoglobulin G and M Level after Xenograft Valve or Valved Conduit Implantation (이종 조직판막 도관 혹은 판막 이식술 직후 면역 글로불린 항체 G, M의 혈중 농도 변화)

  • Background: The aim of this study is to confirm that peripheral blood sampling for measuring of serum immunoglobulin can predict immunological changes after xenograft implantation. Material and Method: Between March 2006 and January 2007, 19 patients were enrolled (10 xenograft implantation group, 9 control group). Through 3 peripheral blood samples, we measured changes in serum immunoglobulin G and M levels preoperatively, and 2 and 10 days postoperatively. Result: In both groups, serum immunoglobulin levels showed similar changes-they decreased 2 days postoperatively, then increased up to the baseline levels 10 days postoperatively. However, this postoperative change of immunoglobulin G and M was not significantly different in absolute value or pattern between the 2 groups (Ig G; p-value=0.393, Ig M; p-value=0.193). Conclusion: We could not predict immunological changes after xenograft implantation by measuring serum immunoglobulin levels by simple blood sampling. Direct checking of ${\alpha}$-Galactose antibody may confirm an immunological reaction after xenograft implantation.

Tissue Engineered Bone Formation Using Porous Chitosan and Chitosan/Tricalcium Phosphate Matrices (Chitosan 및 Chitosan/Tricalcium Phosphate 다공성 기질을 이용한 조직공학적 골형성에 관한 연구)

  • chitosan은 골치유증진 및 골세포의 분화를 촉진하는 것으로 알려진 천연의 생분해성 고분자이다. 이연구에서는 chitosan 및 chitosan/tricalcium phosphate(TCP) 다공성 기질을 제조하여 골이식재 및 조직공학적 골형성을 위한 3차원적 세포배양 지지체로서의 가능성을 평가하고자 하였다. chitosan 용액 및 TCP가 포함된 chitosan 용액을 동결건조함으로써 소공의 크기가 $100-200{\mu}m$인 스폰지형태의 chitosan 및 chitosan/TCP 다공성 기질을 제작하였다. 골이식재로서의 효과를 평가하기 위하여 백서의 두개골 결손부에 제작된 chiosan 및 chitosan/TCP 다공성 기질을 각각 이식하고 2주 및 4주 후에 동물을 희생하여 조직학적으로 치유양상을 관찰하였다. 조직공학적 골형성을 위한 세포배양 지지체로서의 가능성을 평가하기 위하여 백서 태자의 두개골에서 분리된 골아세포를 chitosan 및 chitosan/TCP 다공성 기질에 각각 접종하고 56일간 배양하면서 각 기간 별로 세포수, 염기성 인산효소 활성, 축적된 calcium의 양을 측정하였고 배양된 세포-기질 혼합체를 광학현미경 및 주사전자현 미경하에서 조직학적 관찰을 시행하였다. 백서 두개골결손부에 이식된 chitosan 및 chiosan/TCP 다공성 기질은 별다른 이물반응 없이 자연 분해되면서 신생골조직 내에 매립되었으며 이식하지 않은 대조군에 비해 유의하게 높은 신생골형성 효과를 나타내어 우수한 골전도성이 있음이 확인되었다. 신생골형성 양상이나 형성된 양에 있어서 두 가지 기질간의 유의한 차이는 없었다. 골아세포-기질 혼합체의 배양결과, 접종후 배양 28일 경과 시까지 골아세포수는 지속적으로 증가하다가 이후에는 5 8일까지 성장정도가 둔화되었다. 염기성 인산효소의 활성 및 calcium 축적량은 접종후 배양시간경과에 따라 56일까지 지속적으로 증가하였다. 세포수 및 염기성 인산효소의 활성에서 두 기질간의 유의한 차이는 없었고, calcium 축적량에 있어서는 chitosan/TCP 기질에서 유의하게 높았고 증가속도도 컸다. 배양된 골아세포가 접종된 다공성 기질의 조직학적 관찰결과, 골아세포는 다공성 기질에 잘 부착하여 중층의 형태로 성장하면서 광화된 골기질을 형성함이 관찰되었다. 배양 14일부터 작은 골편형태의 골형성이 기질 표면에 부착되어 관찰되었고, 배양기간이 길어짐에 따라 성장하여 배양 56일째에는 상당한 양의 광화된 골질이 형성됨이 관찰되었다. 배양 56일 경과후의 광화된 골질의 양은 chitosan/TCP 기질에서 더 많았다. 이 연구의 결과, chitosan 및 chitosan/TCP 다공성 기질이 골이식재로서 뿐만 아니라, 조직공학적 골형성에 적용되는 골아세포의 배양을 위한 3차원구조의 세포지지체로 이용되어 골재생술식에 유용한 생체재료로 활용될 수 있음이 확인되었다.
