Impact of symptoms of Work-related Musculoskeletal Disorders on health related Quality of Life in firefighter under the IT environment (IT 환경에서 소방공무원의 근골격계 증상이 건강관련 삶의 질에 미치는 영향)

  • This study was conducted to assess the impact of work-related musculoskeletal disorders (WMSDs) on health related quality of life (QoL) in the firefighter under the recent IT environment. The data were collected by face to face interview using a structured questionnaire in the 366 respondents. WMSDs symptoms were measured by a self-assessed questionnaire on symptom table of NIOSH and health-related QoL was measured by SF-36. The prevalence of WMSDs was 38.0% in upper limbs, 35.5% in the low back, 21.6% in lower limbs, and 59.3% in two or more parts of the body. Subjects with symptoms of WMSDs had significantly lower scores in 7 dimensions of QoL except 'emotional role limitation' than those without symptoms of WMSDs at the area of upper extremities (neck, shoulder, arm/wrist, and hand/wrist/fingers). On the other hand, subjects with symptoms of WMSDs had significantly lower scores on all QoL dimensions than those without symptoms of WMSDs at the area of lower back or lower extremities. These results suggest that WMSDs had a negative effect on QoL. Therefore, prevention of WMSDs should be considered intervention strategies for improvement of QoL, especially in firefighters.

  • 최근 산업안전/보건분야에서 누적외상성질환(Cumulative Trauma Disorders: CTDs)과 직업성요통(Low Back Injury)과 같은 근골격계질환은 산업현장에서 어렵지 않게 발견할 수 있는 신종 산업재해의 일종이다. 특히 미국의 경우, 발생건수와 그로 인한 인적/물적 손실이 급격하게 증가되고 있는 것으로 나타나고 있다. 그러나 미국과 같이 근골격계질환의 폐해를 일찌감치 인식하고 그 해결방안을 연구해온 나라들에서는 현재 환자관리 및 인간공학적인 기준 등을 포함하는 예방지침들이 잘 정착되어 있는 실정이다. 우리나라의 경우에는 작업환경 등의 조건과 상황이 미국과 같은 구미 선전국과 비숫하고, 특히 제조업의 생산체계가 그런 나라들과 매우 유사하여 근골격계질환의 발생과 그로 인한 인적/물적 손실이 적지 않을 것이라고 예상할 수 있지만 아직까지 CTDs에 대한 전반적인 이해가 부족하고, 정확한 실태가 파악되지 못하였으며, 따라서 체계적인 관리 대책 또한 마련되어있지 않은 실정이다. 본 조사에서는 서울 근교의 수송기계관련 사업장의 작업들에 대한 작업자세에 중심을 둔 인간공학평가를 통하여 조사 작업의 근골격계질환에 대한 상대적인 위험도, 위험요인 등을 파악하고, 그에 따른 개선 대책의 방향을 제시하고자 하였다. 특히, 각 부위별 위험점수와 부위별 증상호소율의 상환분석결과, 작업자 개인적 수준에서는 통계적으로 유의한 차는 없는 것으로 분석되었으나, 부서별로 분석한 결과, 작업시 주로 사용하는 오른쪽 상지의 경우, 오른쪽 어깨, 팔꿈치, 손목 순으로 통계적으로 유의하게 관련이 있는 것으로 나타났다. 즉 증상호소율과 해당 신체부위의 위험도 점수는 서로 정적(positive)한 관계에 있는 것으로 나타났고, 상지 중 주로 사용하는 오른쪽 부담의 위험도 자세 점수와 증상호소는 비교적 높은 상관관계에 있는 것으로 나타났다. 따라서 이와 같은 인간공학적 분석결과는 향후 인간공학적인 개선대책의 개발 뿐만 아니라 의학쪽에서 근골격계질환을 접근하는 초기단계에서 매우 유용한 역할을 할 수 있으리라 생각된다.

A Correlation between the Perceived Symptom of Musculoskeletal Diseases and Psychosocial Factors of Dental Professionals in C Region (C지역 치과종사자의 근골격계 질환 자각증상과 사회심리적 특성과의 관계)

  • To investigate the correlation between the perceived symptom of musculoskeletal diseases and psychosocial factors in dental professionals in Chungcheong province, a self-administered survey was conducted on 206 dental professionals in Chungcheong province from March 9th, 2009 to March 28th, 2009. SPSS WIN 12.0 software was used to analyze the data and the following results were obtained. 1. As for the perceived symptom degree of musculoskeletal diseases based on measured body parts, the frequency of 'high' was the highest in all the parts of the body. 2. As for gender, females showed higher interest in musculoskeletal diseases than males (p=.000). As for age, subjects ranged 26~30 years showed the highest interest in musculoskeletal diseases, which was statistically significant (p=.000). 3. Work satisfaction showed a positive correlation with the perceived symptom of musculoskeletal diseases in lumbar group and lower extremity group. Work stress showed a weak negative correlation with the perceived symptom of musculoskeletal diseases in lumbar group and lower extremity group. Coworker satisfaction showed a positive correlations with the perceived symptom in upper extremity group and lower extremity group. Hospital satisfaction showed a positive correlations with the perceived symptom in upper extremity group, lumbar group and lower extremity group. Working environment satisfaction showed a positive correlations with the perceived symptom in neck group, shoulder group, upper extremity group and lower extremity group. This study showed a correlation between psychosocial factors and the perceived symptom of musculoskeletal diseases based on body parts. Therefore, psychosocial factors should be considered when the prevention and management program of musculoskeletal diseases are developed.

The Relation Between Work-Related Musculoskeletal Symptoms and Rapid Upper Limb Assessment(RULA) among Vehicle Assembly Workers (자동차 조립 작업자들에서 상지 근골격계의 인간공학적 작업평가(Rapid Upper Limb Assessment) 결과와 자각증상과의 연관성)

  • Objectives. This study was conducted to evaluate the association between upper extremity musculoskeletal symptoms and Rapid Upper Limb Assessment(RULA) in vehicle assembly line workers. The goal of this study is to show the feasibility of RULA as a checklist for work related musculoskeletal symptoms (WMSDs) in Korean workers. Methods. The total number of 199 people from the department of assembly and 115 people from the department of Quality Control(QC) in automotive plant were subjects for this cross sectional study. A standard symptom questionnaire survey has been used for the individual characteristics, work history, musculosketal symptoms and non-occupational covariates. The data were obtained by applying one-on-one interview for the all subjects. RULA has been applied for ergonomic work posture analysis and the primary ergonomic risk sure was computed by RULA method. Association between upper extremity musculoskeletal symptoms and RULA were assessed by multiple logistic regression analysis. Results. A total of 314 workers was examined. The prevalence of musculoskeletal symptoms by NIOSH case definition was 62.4%. The distribution of musculoskeletal symptoms by the part of the body turned out to be following; back:41.4%, neck: 32.8%, shoulder: 26.4%, arm: 10.5% and hand:29.3%. The relationship of the individual RULA scores were statistically significant for the prevalence of musculoskeletal symptoms. As the result of the multiple logistic regressioin analysis, grand final score (OR=2.250 95% CI: 1.402-3.612) was associated with musculoskeletal symptoms in any part of the body.; upper arm score(OR=1.786 95% CI: 1.036-3.079) and posture score A(OR=1.634 95% CI: 1.016-2.626) in neck; muscel use score(OR=3.076 95% CI:1.782-5.310) and posture score A(OR=1.798 95% CI: 1.072-3.017) in shoulder; upper arm score(OR=1.715 95% CI: 1.083-2.715) and muscel use score(OR=2.057 95% CI:1.303-3.248) in neck & shoulder; muscle use score(OR=10.662 95% CI: 3.180-35.742) in arm; writst/wist score(OR=2.068 95% CI: 1.130-3.786) and muscle use score(OR=2.215 95% CI: 1.284-3.819) in hand & wrist.; muscle use score of trunk (OR=2.601 95% CI: 1.147-5.901) in back. Conclusions. Musculoskeletal symptoms of the extremities were strongly associated with individual RULA body score. These results show that RULA can be used as a useful assessment tool for the evaluation of musculoskeletal loading which is known to contribute to work-related musculoskeletal disorders. RULA also can be used as a screening tool or incorporated into a wider ergonomic assessment of epidemiological, physical, mental, environmental and organizational factors. As shown in this study, complement of the analysis system for the other risk factors and characterizing between the upper limb and back part will be needed for future work.

The Effect of Some Dental Clinic Workers' Working Environment and Health Habit Upon Their Musculoskeletal Disorder Subjective Symptom (일부 치과종사자들의 근무환경과 건강습관이 근골격계 자각증상에 미치는 영향)

  • This study examined the musculoskeletal symptom experience rate of some dental clinic workers and the factors influencing musculoskeletal symptoms according to their working environment and health habit. To that end, between March 9 and March 28, 2009, 200 workers at dental clinics in Chungcheong-do were surveyed using the self-administered questionnaire method. The data were analyzed using the SPSS WIN 12.0 program. The following conclusion was reached. 1. One-year musculoskeletal symptom experience rates of the subjects were categorized according to pans of the body: shoulders - 60.5%, lower limbs (legs, knees, and feet) - 57.0%, the lumbar region (back and waist) - 56.0%, the neck - 51.0% and the upper limbs (arms, wrists, and fingers) - 32.0%. 2. In general characteristics, a statistically significant difference was found according to gender, age, education, position, and oral health state. In working environments, a statistically significant difference was found according to work experience, the average daily number of patients handled, the average daily working hours, the posture for diagnosis, and satisfaction over work. In health habit, a statistically significant difference was found according to regular exercise, regular examination, and regular eating habit. 3. Factors influencing musculoskeletal symptoms were examined. As a result, influencing the musculoskeletal symptom experience were satisfaction over work among working environment characteristics, and regular exercise among health habit characteristics.

Risk Assessment and Intervention of Ergonomic Risk Factor in Beekeeping (치과위생사의 직무스트레스 그리고 작업자세와 상지의 증상여부 및 기능과의 관련성)

  • Objectives: This study investigated musculoskeletal symptoms in upper limbs according to the working environment (job stress) of dental hygienists and examine their relationship with upper limb functions. Methods: The subjects include 198 dental hygienists in dental hospitals and dental clinics in Pusan and Gyeongnam. The questionnaire was consisted of general characteristics of the subjects, job stress, musculoskeletal symptoms in upper limbs and function (Disability Measurement Tool for Upper Extremity Disorders-11, DASH-11). Results: The study was analyzed their musculoskeletal symptoms in upper limbs according to their general characteristics and found that the symptoms occurred in the neck (39.4%), the shoulders (54.6%), elbows (14.7%), and the hands (50.0%). Job stress was associated with upper limb functions (DASH-11) (model 3, B=5.210, p=0.012) and repeated elbow bending and spreading posture was associated with DASH-11 (model 3, B=6.561, p=0.029). Elbow symptoms were associated with DASH-11 in the upper limbs (B=10.679, p=0.003). Conclusion: Dental hygienists are experiencing limitations of upper limb function due to job stress. In particular, even if the correction of their uncomfortable posture is significantly related to the job stress and upper limb function, in order to improve the upper limb function of the dental hygienist, efforts to reduce the job stress as well as the uncomfortable posture are necessary.

The Effect of the Health Promotion Program for the Fruit Farm Workers (과수재배 농업인을 위한 건강증진 프로그램의 효과)

  • The present study aims to determine the effect of the prevention exercise program of musculoskeletal disorders developed for apple and pear farmers. To achieve this goal, 25 farmers who have subjective symptoms in the musculoskeletal system (Criteria 1 in the National Institute for Occupational Safety and Health) have conducted the prevention exercise program for 12 weeks from April 7, 2011 to July 1, 2011. As a result, the prevention exercise program of the musculoskeletal disorders for farmers in the fruit growing industry have improved physical capability of them (upper extremity endurance, lower extremity endurance, balance and agility) as well as a quality of life effectively. In addition, a quality of life showed a significant correlation with Oswestry Disability Index, and Psychosocial Well-being Index. For the future study, various studies are expected to develop and apply exercise programs in consideration of the characteristics of farming work.

Effects of the Ergonomic Lap Board for Computer Mouse Padding on Upper Limb Movements, Myoelectric Activities, Task Performance, and Subjective Discomfort (인체공학적 랩보드를 이용한 컴퓨터 마우스 사용이 상지의 움직임 및 근활성도, 작업수행도, 주관적 불편도에 미치는 영향)

  • This study aimed to investigate the effect of a lap board that was developed to prevent musculoskeletal disorders caused by the use of a computer mouse. Study participants were 10 college students in their 20s who did not have any musculoskeletal symptoms. We analyzed the difference in upper limb movements, electromyographic activity, task performance, and subjective discomfort between 2 conditions: use of a mouse with the lap board and use of a mouse placed on a desk. Results of behavior analysis showed that there was a significant difference between the 2 conditions in terms of the average angle of shoulder flexion, shoulder internal rotation, and forearm pronation(p< 0.05). However, electromyographic activity, task performance, and subjective discomfort showed no significant differences between the 2 conditions. When subjects used the mouse with the lap board, their upper extremity was located much closer to the torso than when they used the mouse placed on the desk. Six of 10 participating students preferred the lap board. We expect that advanced research on prevention of musculoskeletal disorders due to the use of the computer mouse will be conducted in the near future.
